・薬物未投与のパーキンソン病ラット(処置を何ら受けていないパーキンソン病ラット)における、単独処置(NR2B/A2Acomboの単独投与):運動症状に対する有効性が、完全有効用量のL−ドーパによって達成されるものと同等である/それと本質的に類似しているが、L−ドーパとは異なり、慢性combo処置は、運動合併症を生じなかったことを実証することができた。
・L−ドーパでプライミングされたラット、すなわち以前のL−ドーパ処置によりジスキネジアになったラットにおける単独処置(NR2B/A2Acomboの単独投与)、異常運動の非存在下で運動活動性を完全に回復する能力。
・L−ドーパの非活性用量に対する追加処置:この組合せは、L−ドーパの非活性用量を有効にする;これは、異常運動の非存在下で起こる。L−ドーパの活性用量に対する追加処置:この組合せは、対側性回転に対するL−ドーパの効果を増強し、また慢性的処置におけるL−ドーパ誘発性のAIMの発症を低減する(すなわち、抗L−ドーパプライミング効果)。
パーキンソン病の前臨床モデル:6−OHDA片側損傷ラットモデル
対象:手術時に250〜275g、且つ薬物試験時に400〜450gの体重がある雄性Sprague−Dawleyラット(Janvier社、フランス)を使用する。
到着の1週間後の手術の日に、ラットは右側の内側前脳束に6−OHDAの片側注射を受けた。各ラットは、ノルエピネフリン作動性(NE)ニューロンを保護するための手術の15分前に、イミプラミンHCl(Sigma社)を投与された。ラットをケタミン(Ceva社、75mg/kg)及びキシラジン(Bayer社、10mg/kg)により麻酔をかけ、定位固定器(David Kopf Instrument社)に置いた。注射部位の上の頭蓋骨に小さな穴を空けた後、前頂及び硬膜の表面に対して、以下の座標(mm)にある右上行内側前脳束に、6−OHDAを注射した。AP=−3.5、ML=−1.5、DV=−8.7。各ラットは、6−OHDA(4μg/μl)の注射を5分間(0.5μl/分)で、ラットあたり合計10μgで1回受けた。毒素の逆流を回避するため、ニードルは4分後に引き抜いた。次に、動物を熱源下のクリーンケージ内に入れ、食物及び水を自由に与えて回復させた。各ラットは、実験者及び環境に対する完全回復並びに順化を確実にするため、3週間、規則的にモニターして取り扱った。
首尾よく損傷した動物を選択するため、手術後21日目に、ラットはすべて低用量のアポモルヒネ(Sigma社、0.05mg/kg、sc)による接種を行った。45分間の記録期間中に90を超える向反側回転(360°)を示すラットを本研究に含めた。これらの基準を満たすラットは、ドーパミン作動性ニューロンの片側喪失、及び95%を超える線条体ドーパミンの片側欠失を有することが以前に実証されている(内部データ)。
A2Aアンタゴニスト及びNR2Bアンタゴニスト参照化合物は、1kgあたりビヒクル体積5mlに溶解した。L−ドーパメチルエステル(Sigma社)及びベンセラジド(Sigma社)を5ml/kgの体積で、生理食塩水溶液に溶解した。参照化合物のビヒクル溶液は、ジメチルスルホキシド(DMSO)5%、及びメチルセルロース1%を含有している蒸留水95%から作製されている。損傷用の6−OHDA−HBrは、μlあたり6−OHDA4μgの濃度で、0.02%アスコルバート−蒸留水に溶解する。
回転活動(同側性/対側性回転)
回転行動は、自家製のコンピューターシステムを使用して記録した。PCに直接接続した機械センサーにリンクしたハーネスにラットを固定した。最高でも最大240分間、各360°の時計回り又は反時計回りを自動的に記録した。この実験全体を通じて、個々の試験用ケージにラットを割り当てた。水平な透明ガラス製底部に置き、丈夫な固定器により保持されている、8つの透明なアクリル製チャンバ(50×40cm)の各々を、25×22cmの4つの個別の四角型試験用ケージにさらに分類することができた。取り除きが可能なアクリル製プレートを蓋にする。これらのチャンバにより覆われる表面全体を観察するよう、各アリーナの真下に携帯用デジタルカメラ(Samsung社、SCB−3001PH)を直接設置した。この配置により、セッションあたり32匹のラットの中から合計8匹について、1台のカメラにより、1匹又は4匹のラットの同時撮影が可能であった。電照は、固定器の各脚に単独で固定したネオン管により供給した。ビデオシークエンスは、保存用ハードディスクに記録して、その後、解析及び保存用PCに転送した。
一般的活動は、PCに接続されており、水平及び垂直方向の運動を測定する広いオープンフィールド内で記録した。12個のチャンバ(LE8811IR、Panlab)中で、ラットを個別に試験し、各々は、底部を含まない、0.5cmの透明アクリルシートから作製されている正方形の囲いからなっている(表面45×45cm×高さ40cm)。各囲いは、床の役割をするステンレス鋼(inox社)の正方形プレート上に置いた。その板に、プラスチック被覆したシートを調節した。各装置は、高さ73.5cmの実験台に置いた。移動を検知して、高さ3cmにある囲いの各側面上に位置している16個の赤外光線センサーにより測定した。光線が表面全体に15×15の正方形マトリックスを形成するよう、センサーは互いに2.5cm、及び側面の各端から4.0cmの間隔を設けた。活動数はパーソナルコンピュータにより記録した。Acti−Trackソフトウェア(Actitrack°、Panlab)を使用して、取得セッション中に生成した生データを統計的に解析可能なパラメータに変換した。パラメータ「移動距離」及び「立ち上がり数」は、関連従属変数として保持された。
6−OHDA片側損傷ラットの特定の行動プロファイルは、ビデオ取得システムに接続した8台のカメラにより記録した。処置下のラットは、狭い環境(ロトメータ、25×22cm)又は広いオープンフィールド(50×40cm)内で自由に置き、その行動を55分間ビデオ記録した(合計で3300秒)。狭い(ロトメータ)又は広い(オープンフィールド)環境における配置は、試験に依存し得る。回転活動に関係する項目において記載されているとおり、ラットは下から撮影した。
− 「萎縮」=重症なAIM(閾値<30%)。このパラメータは、ラットが後脚で立ち(苛酷な身体のトランケーション(truncation)を伴う垂直方向の直立位)且つ非常に強いジストニア体位を示す姿勢で過ごした時間(秒)を指す。下からは、体形は円のように見える。
− 「正常」=中程度のAIM(閾値、30%<正常<50%)。このパラメータは、ラットが3本の脚で立ち且つ中程度のジストニア位置(若干の身体のトランケーションを伴う水平体位)を示す姿勢で過ごした時間(秒)を指す。
− 「伸展」=AIMがない(閾値>50%)。このパラメータは、ラットがより頻繁に4本の脚で立ち(水平位)且つ身体のトランケーションなしに若干の安定性を示す姿勢で過ごした時間を指す。
− 距離(=「距離」):動物が移動した合計距離(cm)
− 反時計方向(=「同側性ターン」):損傷の方向と同じ方向の、同側性回転の360°ターンの計数。
− 時計方向(=「対側性ターン」):損傷の方向とは逆の方向の、対側性回転の360°ターンの計数。
− 「伸展」:(=「適切な体位」):ラットが、屈曲した胴体の姿勢をとらずに、4本脚で立っている姿勢で過ごした時間。本プログラムでは、これは、適切な体位の強度及び頻度を区別するために使用されるパラメータ「身体の伸長」である。この閾値は70%に固定する。
− 「回転運動半径」(=「適切な軌道」):特定の時間間隔に対する、動物の位置と、この特定の時間間隔内での平均位置との間の平均距離。このパラメータは、アリーナの全空間を使用するラットの能力を反映しており、センチメートルで測定する。
一般的活動(アクトメータ)及び回転(ロトメータ):一般的活動(距離及び立ち上がり)、及び回転(対側性)に関する統計解析は、パラメトリック統計によって行った。

A.片側パーキンソン病ラットにおける、A2AアンタゴニストとNR2Bアンタゴニストとを一緒にした単独処置

6種の異なる実施形態間の活動量に対する、A2AアンタゴニストとNR2Bアンタゴニストとの組合せの有効性
パーキンソン病の6−OHDA片側損傷ラットモデルにおいて、表1に従って、A2AアンタゴニストとNR2Bアンタゴニストとの6種の異なる共投与(=COMBO)を試験する。
1つの具体例(トザデナント及びラジプロジル)により刺激された運動活動性の質に対する組合せ有効性の実証
NR2Bアンタゴニスト及びA2Aアンタゴニストの共投与は、6種の異なる交差comboにより、活動レベル(移動距離及び立ち上がり数)が予期せず向上することを示した。動物が活動チャンバ(アクトメータ)に置かれている間に目視観察された、この予期しない行動プロファイル(すなわち、優れた体位を伴って運動活動性を向上させる)は、以下の例において詳細に説明され、また様々な行動パラメータの定量的測定により例示される。
1)別々に行うこれらの化合物の試験、
2)部分活性用量のL−ドーパ(25mg/kg、ip)の試験、
3)完全活性用量のL−ドーパ/ベンセラジド(16/4mg/kg、ip)の試験
と比較する。
自動装置で試験した場合、comboは長期持続作用を示した一方、活性用量のL−ドーパ/ベンセラジドは約3時間続き、また部分活性用量は約1時間続いた(非常に短時間作用性効果である)。comboによる実験では、ラットは、化合物(すなわちトザデナント及び/又はラジプロジル)の経口投与の60分後に試験アリーナに入れた一方、L−ドーパ25mg/kgを投与したラットはL−ドーパ投与(ip)の10分後に、及びL−ドーパ/ベンセラジドによる処置ラットは腹腔内投与の15分後に、ロトメータに入れた。

Tukey事後検定(p<0.05):TOZ30/RAD3組合せにより処置されたラットは、VEH/VEH、TOZ30/VEH、及びVEH/RAD3の群の由来のものよりも高い立ち上がりスコアを有している。
処置効果は、有意な効果F(2,21)=43.68、p<0.001を示している。時間の有意な効果(F(17,357)=20.05、p<0.001)も観察され、また有意な相互作用(F(34,357)=11.74、p<0.001)も同様である。追加的な事後比較検定により、これはあくまで、ビヒクル処置ラット(Tukey、p<0.05)と比べて有意に活性であるL−ドーパ/ベンセラジドによる効果であることが示される。
完全な行動解析により、comboが片側パーキンソン病ラットの行動に対して予期せぬ効果を有することが示された。ラジプロジル及びトザデナントの組合せ(すなわち、「Combo」)は活性用量のL−ドーパ/ベンセラジドで観察されるものに匹敵する運動量を回復している(これは、個別に試験した化合物の場合ではない)。さらに、このcomboは、行動の質を強く改善する。方向、軌道、及び体位は顕著に改善され、また非損傷ラットにおいて観察された行動に相当している。
慢性処置後に任意の異常運動の発生がない運動症状に対する「トザデナント+ラジプロジル」の組合せ投与の有効性
comboによる10日間の慢性処置は、L−ドーパで観察されるような異常運動合併症の発症ももたらさない。これらのラットは、一部の馴化過程が試験アリーナにまで現れたとしても、活動的なままである。この正常現象があるにもかかわらず、活動レベル及び運動の質に対するcomboの有意な効果が依然として観察される。
L−ドーパによる以前の慢性処置により、以前に完全なジスキニアとなった片側パーキンソン病ラットに関する、運動症状に対するcomboの有効性
「トザデナント+ラジプロジル」の組合せ投与は、片側パーキンソン病ラットにおいて、向上した運動活動性を回復する能力を有する。しかし、この特定の共投与により、慢性L−ドーパ処置により完全なジスキニアにされており、L−ドーパによりプライミングされた片側パーキンソン病ラットにおいて、異常不随意運動なしに運動活動性が回復するので、この共投与は全く予期しない効果をやはり有している。
(1)L−ドーパ/ベンセラジドにより処置されたものに匹敵する移動距離レベルである一方、化合物単独により処置されたラットは活動レベルの低下を示したこと、
(2)L−ドーパ/ベンセラジドにより処置されたラットと比べて、L−ドーパ誘発性の対側性回転が減少し、且つ同側性回転が増加すること、
(3)L−ドーパ/ベンセラジドにより処置されたラットと比べて、適切な体位で過ごした時間が増加し、且つ軌道が改善されていること、
(4)L−ドーパ/ベンセラジドにより処置されたラットと比べて、異常不随意運動(AIM)を示して過ごしている時間がないこと
を示している。
(例5)
準活性用量又は部分活性用量のL−ドーパに対して急性追加処置を施した際の行動観察
NR2BアンタゴニストとA2Aアンタゴニストとを組合せ投与すると、個別に試験した化合物に比べて、準活性用量又は部分活性用量のL−ドーパの有効性が増強される。
活性以下の用量のL−ドーパ(15mg/kg)に対する追加処置として与えられる、1mg/kgのSCH−58261への1mg/kgのCo−101244の共投与により、ビヒクル処置ラット、及び化合物を単独で処置したラットと比べて(すべての群は、L−ドーパ15mg/kgにより処置されている)、対側性回転レベルが有意に向上している。
部分活性用量のL−ドーパ(25mg/kg)に対する追加処置として与えられる、30mg/kgのトザデナントへの3mg/kgのラジプロジルの共投与により、ビヒクル処置ラット、及び化合物を単独で処置したラット(L−ドーパ25mg/kgに加えて)と比べて、対側性回転レベルが有意に向上している。
活性用量のL−ドーパ及びベンセラジドに対する慢性追加処置において与えられる場合の行動観察
予期しない観察として、これらのデータにより、comboが、一部の運動欠陥の低下を伴って、L−ドーパのオンタイム効果を向上させる可能性を有することが示された。
図14:トザデナント+ラジプロジルと活性用量のL−ドーパ/ベンセラジドの組合せ投与に関する実験計画
1)6−OHDA片側損傷ラットにおいて、様々なA2A及びNR2Bアンタゴニストを組み合わせることにより作製される異なる6種の組合せ(combo)により、ビヒクル処置ラット、及びA2Aアンタゴニスト又はNR2Bアンタゴニストで単独処置されたラットと比べて、活動レベル(移動距離及び立ち上がり)が顕著に向上することが示される。
2)1種の具体的なA2A/NR2Bアンタゴニスト組合せ(トザデナント+ラジプロジル)について実施した完全な行動解析により、活性用量のL−ドーパ/ベンセラジドで観察されたものに匹敵する活動レベルの顕著な向上の他に、combo下でのラットの行動は、L−ドーパ処置下で観察されるものと比べて、顕著に改善されている(ステレオタイプで窮屈な対側性回転もジストニア体位もない)ことが示される。
3)A2AアンタゴニストとNR2Bアンタゴニストとの組合せによる慢性処置により、異常な運動合併症は何ら発生しない。
4)L−ドーパとcomboとの間の交差感作効果はない。L−ドーパ処置によりジスキニアにされたラットであって、L−ドーパにより慢性的に処置されたラットにおけるA2A/NR2B組合せによる急性処置により、いかなる異常不随意運動もなしに、高い質の運動活動性を回復する能力があることが示される。
5)A2AアンタゴニストとNR2Bアンタゴニストとの組合せによる急性処置が準活性用量又は部分活性用量のL−ドーパへの共投与で与えられた場合、ビヒクル処置ラット、又はA2Aアンタゴニスト若しくはNR2Bアンタゴニスト単独で処置された及びそれに上記のアンタゴニスト+combo群以外の投与を受けたものと等用量のL−ドーパを加えて処置されたラットと比べて、上記の急性処置は、L−ドーパ誘発性の対側性回転を有意に増加させることを示している。
6)活性用量のL−ドーパ/ベンセラジド(L−ドーパ誘発性の異常運動AIMを発症する原因となる用量)に共投与で与えられたA2AアンタゴニストとNR2Bアンタゴニストとの慢性処置は、A2Aアンタゴニストにより処置されたラットと比べて、対側性回転レベルに対するL−ドーパ効果の相乗作用、及び同時に、重症なAIMの低下をも示す。

Claims (11)
- 哺乳動物におけるパーキンソン病(PD)を処置する方法であって、第1の薬剤及び第2の薬剤を投与することを含み、第1の薬剤がアデノシン受容体2(A2A)のアンタゴニストであり、第2の薬剤が、N−メチル−D−アスパラギン酸(NMDA)受容体サブタイプNR2Bのアンタゴニストである、上記方法。
- A2Aアンタゴニストが、他のアデノシン受容体サブタイプA1、A2B、A3の各々よりも、少なくとも10倍選択性が高い、請求項1に記載の方法。
- NR2Bアンタゴニストが、他のNMDA受容体サブタイプNR2A、NR2C、及びNR2Dの各々よりも、少なくとも10倍選択性が高い、請求項1に記載の方法。
- A2Aアンタゴニスト対NR2Bアンタゴニストの比が、30:1〜1:30の間で変動する、請求項1から3までのいずれか一項に記載の方法。
- 両方の薬剤が、本質的に同時に投与される、請求項1から4までのいずれか一項に記載の方法。
- 両方の薬剤が、L−ドーパと一緒に投与される、請求項1から5までのいずれか一項に記載の方法。
- 両方の薬剤が1日1回、投与される、請求項1から6までのいずれか一項に記載の方法。
- A2Aアンタゴニストが、イストラデフィリン(KW−6002)、BIIB014、プレラデナント(SCH420814)、ST−1535、ASP5854、SYN−115を含む群から選択される、請求項1から7までのいずれか一項に記載の方法。
- NR2Bアンタゴニストが、MK−0657、トラキソプロジル(CP−101,606)、EVT−101、EVT−103、ラジプロジル(RGH896)を含む群から選択される、請求項1から8までのいずれか一項に記載の方法。
- アデノシンA2A受容体アンタゴニスト及びNR2Bアンタゴニストの組合せの治療有効量を、薬学的に許容される担体中に含む医薬組成物。
- (a)治療有効量のA2A受容体アンタゴニストを含む医薬製剤を収容している第1の収容容器と、
(b)治療有効量のNR2Bアンタゴニストを含む医薬製剤を収容している第2の収容容器と
を含むキットオブパーツの形態である、請求項10に記載の医薬組成物。
