Description

椎骨動脈のサイフォン12aは、ウィリス輪への脊椎アプローチにおける頭蓋内脈管構造に見られる。内頸動脈により、前大脳動脈16及び中大脳動脈17、並びにウィルス輪も供給され、これには、後交通動脈18及び前交通動脈19が含まれる。内頸動脈5のサイフォン5aは、ウィリス輪内の頸動脈アプローチにおける、頭蓋内脈管構造に見られる。これらの動脈は、通常、約1mm〜5mm、最も一般的には2〜4mmの内径を有する。本明細書に開示される方法及びデバイスにより、上記動脈に接近し、上記動脈の分岐点に近接する動脈瘤を横断してブリッジングデバイスを配置することが可能になる。図1においては、挿入カテーテル2及びブリッジングデバイス1は、ブリッジングデバイスの一般的な接近経路である一般的な頸動脈4及び内内頸動脈5に通されて示され、ブリッジングデバイスは、脳底動脈先端部動脈瘤20の頸部に広がる脳底動脈13、及び後大脳動脈14内に配置される。
Claims (18)
- 血管の分岐点で使用するための動脈瘤ブリッジングデバイスであって、上記分岐点が、第一の直径を有する入口血管、第二の直径を有する第一の出口血管、及び第二の出口血管を有し、
遠位領域(21)、中央領域(23)及び近位領域(22)を特徴とする実質的に管状のワイヤフレーム構造(1)であって、送達カテーテル(2)を介して送達するための小口径形状と、拡張した大口径形状とを有するワイヤフレーム構造を含むデバイスであって、
上記遠位領域(21)及び近位領域(22)は、拡張した大口径形状で、それらが配置される血管の直径と一致するか、又はわずかに上回る、第一の直径を有する円筒状構造を規定し、上記中央領域は、拡張した大口径形状で、上記遠位領域及び近位領域の第一の直径を上回る、第二の直径を有する球状の輪郭を有し、
上記近位領域は、第一の近位ジグザグ形状部分(29)を有し、
上記遠位領域は、第一の遠位ジグザグ形状部分(25)を有し、
上記中央領域は、第一の中央ジグザグ形状部分(34)と、中央領域(23)のジグザグ形状部分(34)を遠位領域及び近位領域(25,29)のジグザグ形状部分と連結する、らせん状に配向した複数の支柱(39,42)とを有する、上記デバイス。 - 上記中央領域が第二の中央ジグザグ形状部分(35)を有し、上記第一の中央ジグザグ形状部分の中央に向いている頂点が上記第二の中央ジグザグ形状部分の中央に向いている頂点と直線状に配列するように、該第二の中央ジグザグ形状部分が上記第一の中央ジグザグ形状部分(34)の反対側に配置されている、請求項1記載のデバイス。
- 上記遠位領域が第二の遠位ジグザグ形状部分(24)を有し、上記第一の遠位ジグザグ形状部分の頂点(26)が上記第二の遠位ジグザグ形状部分の頂点(27)と直線状に配列するように、上記第二の遠位ジグザグ形状部分が上記第一の遠位ジグザグ形状部分(25)の反対側に配置されている、請求項1記載のデバイス。
- 上記近位領域が第二の近位ジグザグ形状部分(50)を有し、
上記第一の近位ジグザグ形状部分(29)の頂点(51)が上記第二の近位ジグザグ形状部分の頂点(51)と直線状に配列するように、上記第二の近位ジグザグ形状部分が上記第一の近位ジグザグ形状部分(29)の反対側に配置されている、請求項1記載のデバイス。 - 上記第一の中央ジグザグ形状部分(34)が、遠位に向いている頂点を特徴とし、上記第二の中央ジグザグ形状部分(35)が、近位に向いている頂点を特徴とし、上記第一の中央ジグザグ形状部分の遠位に向いている頂点が、上記第一の中央ジグザグ形状部分の最初の遠位に向いている頂点から伸長するらせん状に配向した支柱により、最初の頂点から周方向に移動する上記遠位ジグザグ形状部分の頂点と連結している、請求項4記載のデバイス。
- 上記第一の中央ジグザグ形状部分(34)が遠位に向いている頂点を特徴とし、上記第二の中央ジグザグ形状部分(35)が近位に向いている頂点(37)を特徴とし、上記第二の中央ジグザグ形状部分の頂点が、上記第二の中央ジグザグ形状部分の最初の近位に向いている頂点(37)から伸長するらせん状に配向した支柱(39)により、最初の近位に浮いている頂点から周方向に移動する上記近位ジグザグ形状部分(29)の頂点(38)と連結している、請求項4記載のデバイス。
- 上記中央領域が、第二の中央ジグザグ形状部分(35,67p)と、らせん状に配向された複数の支柱(69)とを有し、上記第二の中央ジグザグ形状部分が上記第一の中央ジグザグ形状部分(34,67d)の反対側に配置されており、上記らせん状に配向された支柱のそれぞれは、上記第一の中央ジグザグ形状の部分の頂点を、上記第二の中央ジグザグ形状の部分と連結している、請求項1記載のデバイス。
- 上記第一の中央ジグザグ形状部分の頂点が上記第二の中央ジグザグ形状部分の頂点と直線状に配列するように、上記第一の中央ジグザグ形状部分(34,67d)及び上記第二の中央ジグザグ形状部分(35,67p)が放射状に配列している、請求項7記載のデバイス。
- 上記第一の中央ジグザグ形状部分の第一の頂点(34,67d)がらせん状に配向した支柱(69)により、上記第一の頂点から周方向に移動する第二の中央ジグザグ形状部分の第二の頂点(35,67p)と連結している、請求項8記載のデバイス。
- 上記第一の中央ジグザグ形状部分(34,67d)の各頂点が、上記第一の中央ジグザグ形状部分の最初の頂点から伸長するらせん状に配向した支柱(69)により、最初の頂点から周方向に移動する第二の中央ジグザグ形状部分(35,67p)の頂点と連結している、請求項8記載のデバイス。
- 患者の血管系内に管腔内支持体を設置する工程;
上記血管内で上記管腔内支持体の遠位端を固定する工程;
上記管腔内支持体の近位端を操作して、上記管腔内支持体の中央領域を変形させる工程;
上記血管系内に上記管腔内支持体の近位端を固定し、上記管腔内支持体の中央領域を変形状態に保持する工程;
を含む、管腔内支持体の設置方法。 - 送達ワイヤに固定しながら、上記管腔内支持体を血管系内に送達する工程;及び
上記送達ワイヤから上記管腔内支持体を分離し、血管系から上記送達ワイヤを引き抜く工程を更に含む、請求項11記載の方法。 - 患者の血管系内の分岐点に近接する頸部が広い動脈瘤であって、上記分岐点が、血流を第一の出口血管及び第二の出口血管に供給する入口血管を特徴とする動脈瘤の治療方法であって、
動脈瘤をブリッジングするためのブリッジングデバイスであって、上記ブリッジングデバイスは、上記ブリッジングデバイスの第一端に入口固定部分及び上記ブリッジングデバイスの第二端に出口固定部分と、上記入口固定部分から上記出口固定部分まで伸長し、上記ブリッジングデバイスの中央部分から外側に向かって弓形に突出している1個以上の支柱とを有するブリッジングデバイスを供給する工程;
上記入口固定部分が上記入口血管内に配置され、上記出口固定部分が第一の出口血管内に配置され、上記支柱が上記動脈瘤及び第二の出口に向かって突出するように、患者の血管系内に、動脈瘤に接近させて上記ブリッジングデバイスを導入する工程;及び
上記ブリッジングデバイスの支柱が、頸部が広い動脈瘤内の塞栓塊を保持し、上記入口血管から塞栓塊によって実質的に閉塞されていない上記第二の出口血管流路を維持するように、動脈瘤内に塞栓塊を埋め込む工程;
を含む上記動脈瘤の治療方法。 - 動脈瘤が、頸部が広い脳底動脈先端部動脈瘤であり上記入口血管が脳底動脈であり、上記第一の出口血管が第一の後大脳動脈であり、上記第二の出口血管が第二の後大脳動脈である、請求項13記載の方法。
- 上記分岐点が、中大脳動脈、内頸動脈、前交通動脈、上小脳動脈、脳梁周囲動脈、脳底動脈、又は後下小脳動脈の分岐点である、請求項13記載の方法。
- 上記分岐点内の頸部の穴を特徴とする、頸部が広い動脈瘤を、第一及び第二の枝内の親動脈の分岐点で治療する方法であって、
第一のカテーテル内にブリッジングデバイスを供給し、上記ブリッジングデバイスを上記動脈瘤の頸部に送達する工程;
上記動脈瘤の頸部と交差するように第一のカテーテルの遠位部を配置する工程;
上記ブリッジングデバイスを送達ワイヤに固定し、連結を維持しながら、上記第一のカテーテルの遠位部から上記ブリッジングデバイスの遠位領域を展開する工程;
実質的に円筒状の断面の遠位領域の近位端が、動脈瘤の頸部を上記第一の分岐動脈の接点と直線上に配列するように、上記ブリッジングデバイスの位置を調整する工程;
上記ブリッジング部分が上記動脈瘤の頸部と隣接して配列され、上記動脈瘤の頸部を広げるように、上記カテーテルの遠位領域から上記ブリッジングデバイスのブリッジング部分を展開する工程;
上記ブリッジングデバイスの近位領域を押し込み、上記ブリッジング部分を長手方向に圧縮し、上記動脈瘤の頸部に接近して上記ブリッジング部分が放射状に膨脹した形態をとるようにする工程;
上記近位領域が親動脈内に配置されるように、上記カテーテルの遠位領域から、実質的に円筒状の断面の近位領域を取り除く工程;
閉塞装置又は閉塞材料を、第二のカテーテルから上記動脈瘤嚢に送達する工程;及び
上記第一のカテーテルから上記ブリッジングデバイスを解放する工程;
を含む上記親動脈の分岐点で治療する方法。 - 上記第一のカテーテルから上記ブリッジングデバイスを分離し、上記分岐点から上記第一のカテーテルを取り外し、上記分岐点内に上記ブリッジングデバイスを放置し、上記動脈瘤の頸部を広げる行程を更に含む、請求項16記載の方法。
- 上記閉塞デバイス又は閉塞材料を送達した後に、第一のカテーテルの遠位部分内の上記ブリッジングデバイスを引き抜き、上記分岐点から上記ブリッジングデバイスを取り外す工程を更に含む、請求項16記載の方法。
