Abstract

米国において毎年約800,000人の人々が脳卒中を患っており、毎年の直接的および間接的な社会的費用は400億ドルを超えている。脳卒中は全死因のうち3位を占めている。現在のところ、虚血脳の再かん流を誘導する治療(たとえば、アルテプラーゼ(組織プラスミノゲン活性化因子つまりrt−PA)での血栓溶解)のみが、急性脳卒中の治療として広く認可されている。これらは、改善された全体的転帰と重篤な合併症の可能性とのバランスを取るが、十分に活用されてはいない。安全な、薬理学的神経防護作用、虚血に対する脳の回復力を高めることによる脳救済は、急性脳卒中治療から恩恵を得ることができた患者の数を劇的に増し得た。しかし、数十年にわたって、研究は、細胞および齧歯類における発見からの1000を超える神経防護治療をヒトにおける有用性に転換することができておらず、推定の神経防護作用薬の臨床試験は成功していない。この科学的危機は、薬理学的神経防護がヒトにおいては適しないまたは実施可能ではないという、支配的なパラダイムを生じさせた。したがって、神経防護がヒトにおいて可能であるか否かを決定するという、満たされていない差し迫った必要性がある。

I.概説

PSD−95阻害剤での虚血性脳卒中の治療は、再かん流療法と組み合わせることができる。そのような再かん流は、tPA、ストレプトキナーゼもしくはウロキナーゼなどの血栓溶解剤の静脈内もしくは動脈内投与を使用して、閉塞した動脈を再び開く機械的手段を用いて、または、虚血脳領域への側副循環を増大させる他の手段によって、達成することができる。PSD−95阻害剤を投与することによって、虚血性脳卒中の存在を決定するための脳スキャンの実行に利用可能な時間が多くなり、その後、適当であれば、tPAを投与しまたは他の再かん流療法を実施する。したがって、虚血性脳卒中を有するより多くの対象者が、tPA治療または脳再かん流を増大させる他の療法、および、同時に、PSD−95阻害剤での治療の恩恵を受けることができる。
脳卒中は、原因によらず、CNSにおける血流障害の結果の疾患である。原因となり得るものには、塞栓症、出血および血栓症がある。血流の障害の結果として、いくつかのニューロン細胞が直ちに死ぬ。これらの細胞は、グルタメートを含むそれらの成分分子を放出し、これはNMDA受容体を活性化し、NMDA受容体は、細胞内カルシウムレベルおよび細胞内酵素レベルを上昇させて、さらなるニューロン細胞の死を招く(興奮毒性カスケード)。CNS組織の死は、梗塞と呼ばれる。梗塞体積(つまり、脳において脳卒中の結果死んだニューロン細胞の体積)は、脳卒中に起因する病理学的損傷の程度の指標として使用することができる。いくつかの例において、脳卒中は、複数の塞栓によって、または、全身性動脈障害によって生じるかもしれない。前者においては、塞栓は、心内膜炎、心房細動または心臓弁膜症の場合のように、心臓から発生するかも知れない。後者においては、動脈障害は、動脈炎(感染性または自己免疫性の動脈の炎症)を含むかも知れない。そのような場合、時に多くの小さい塞栓によって、脳に複数の脳卒中が生じるかも知れない。そのような場合、脳卒中に起因する病理学的損傷の程度を測定する他の手段は、虚血病変の数を数えることである。これは、脳動脈瘤の血管内修復の後に発生する脳卒中などの、手術誘発性の脳卒中の場合に特に適用される。この場合、血管内操作が、脳において多くの脳卒中を生じさせる多くの塞栓を解放するかも知れない。症候性作用は、梗塞の体積、梗塞の数、および、それが脳の何処に位置するかの、双方に依存する。ランキンストロークアウトカムスケール(Rankin,Scott Med J;2:200−15(1957))およびバーセルインデックスなどの障害指数は、症候的損傷の測定に使用することができる。ランキンスケールは、以下のように、対象者の全体的状態を直接評価することに基づいている。
臨床試験は、PSD−95阻害剤が、未破裂のおよび破裂した動脈瘤の両方の血管内修復を受ける対象者における、梗塞および神経認知障害の低減に有効であることの証拠を提供する。未破裂の動脈瘤を有する対象者は、虚血性脳卒中またはくも膜下出血の危険性が圧倒的に高い。破裂した動脈瘤を有する対象者も、虚血性脳卒中の危険性が高いが、特にくも膜下出血に由来する、出血性脳卒中の危険性が加わる。出血性脳卒中の危険性が加わることで、破裂した動脈瘤を有する対象者は、動脈瘤の結果としての死または消耗性損傷の危険性が最大になる。驚いたことに、本発明のデータは、病理学的試験(梗塞の数および体積)および神経認識試験の双方によって、これらの患者が、PSD−95阻害剤から最大の恩恵を受けることを示している。これらの結果は、PSD−95阻害剤が、虚血性脳卒中または出血性脳卒中の治療に使用可能であることを示すのみならず、そのような阻害剤が、脳卒中の結果であるか否かによらず、CNSにおけるまたはそうでなければCNSに影響する出血を有する対象者の治療に使用可能であるという証拠を提供する。PSD−95阻害剤は、虚血の低減を通じて作用して、より良い転帰をもたらすと一般に考えられてきたので、このことは驚くべきである。SAHにおいて、Tat−NR2B9cなどのPSD−95阻害剤は、一般に虚血が存在しないときは、破裂の72時間以内に与えられたが、それでも恩恵を示した。SAH患者における虚血は、一般に血管攣縮の後に生じ、一般に破裂の5〜12日後に存在する。Tat−NR2M9cは、血漿中の半減期が短く(約20分)、脳での半減期も短く(約5時間)、これは、Tat−NR2M9cが、少なくとも部分的に、虚血を低減するのとは異なる機構で作用して、下記の実施例に見られる恩恵をもたらすことを示唆している。ただし、本発明の実施は、機構の理解に依存するものではない。
虚血の兆候については、治療に適する対象者に対して、PSD−95阻害剤および再かん流の一形態を施すことができる(2011年6月24日出願の米国特許出願第US 61/501117号を参照。この出願は、あらゆる目的のために、参照によって取り込まれる)。PSD−95阻害剤および再かん流は、順次または同時に施すことができる。通常、PSD−95阻害剤および再かん流は、同時に、または、重複したもしくは近接した時間(つまり、15分間隔以内)に施され、あるいは、PSD−95阻害剤が先に施される。
PSD−95阻害剤は、CNSの虚血または出血の1つ以上の損傷作用、および好ましくは、虚血もしくは出血またはその外科的治療に関連する疼痛を軽減し、阻害しまたは遅延させる量、頻度および投与経路で投与される。他に示さない限り、内在化ペプチドに連結した薬理活性剤を含む、キメラ薬剤である阻害剤の用量は、キメラ薬剤の薬理活性剤成分のみならず、薬剤全体についていう。有効量は、疾患を患い本発明の薬剤で治療される対象者(または動物モデル)の集団における虚血および出血ならびに好ましくは疼痛の1つ以上の損傷作用を、その疾患または症状を患いその薬剤で治療されない対象者(または動物モデル)の対照集団における損傷に比べて、軽減し、抑止しまたは発症を遅延させるに足る薬剤の量を意味する。対照集団は、プラシーボで同時に処理され得、または、歴史的対照であり得る。量は、また、個々の治療対象者が、本発明の方法により治療されない比較対象者の対照集団における平均の転帰よりも、好ましい転帰を示す場合に、有効であると考えることができる。有効な投与計画には、意図する目的を達成するために必要な、頻度、投与経路での有効な薬剤の投与が含まれる。
研究計画
2008年9月〜2011年3月にカナダまたは米国の14の病院にて患者を登録した、無作為化二重盲検プラシーボ制御試験を行った。試験は、地方および国家治験審査委員会によって承認され、患者から、または法的に許容可能な代理人から、インフォームドコンセントを得た。包含および除外基準を、表4に要約する。すべての患者はそれぞれの動脈瘤の治療を受け、破裂した動脈瘤、くも膜下出血の場合は、地方施設治療基準に従った。
すべの臨床評価およびMRI評価は、処理割当を知らない個人が行った。各登録患者は、血管内手術前2週間以内および血管内手術後24〜96時間に、1.5T(最小)スキャナーでのMRIスキャンを受けた。各スキャンには、次の最小シーケンスを含めた:軸位断(axial)FLAIR:3mm、ギャップなし、軸位断(axial)DWI:3mm、ギャップなし、および、3D T1強調傾斜エコーシーケンス(T1 weighted gradient echo sequence)2.0mm(たとえば、GE装置上のFSPGR)。
2破裂した動脈瘤を有する患者については登録および2〜4日目の評価を省略(30日目にのみ実施)。

8体重は、すべての患者について、登録時および2〜4日目に測定すべきである。登録時に(臨床的処置の後に)体重を評価しなければならない、破裂した動脈瘤患者の場合、実際の体重は2〜4日目に測定することを要した。
9投与前4〜6時間および投与後12時間の12誘導ECG。登録と1日目が同日の場合、12誘導ECGは、登録時と1日目投与前ECGの双方として作用するように行ってよい。(最小で)3回のECGモニターが、投与の開始から投与後少なくとも2時間まで、麻酔専門医および/または手術後回復室スタッフによって行われるであろう。
10投与後24時間の12誘導ECG。
11投与前および試験薬物注入の終了後10分に、2つの試料を採取した。
12試験薬物注入の開始後5〜10分に、2つの連続試料をPK分析のために、採取した。

15投与前。
コンピュータ生成コーディングシステムによって、患者を、TAT−NR2B9Cまたはプラシーボのいずれかの静脈内注入を受けるように、無作為に割り当てた。試験薬物は、100ccの生理食塩水への希釈により2.6mg/kgで投与され、10分にわたって静脈内注入される、20mg/ml TAT−NR2B9Cを含有する薬物バイアルとして提供した。注入は、処理割当を知らない個人によってなされ、処理する神経介入者が動脈瘤修復の終了したことを認め、かつ、麻酔が終わる前に、開始した。このタイミングの根拠は、ENACTの目的が、ヒトにおける神経保護が、処理前のパラダイムにおいてのみならず、脳卒中が発生した後に実現可能か否かを、試験することであったことである。
新たな虚血病変を、2〜4日目DWI MRI上の新たな高強度信号であると規定した(図1A〜図1C;黄色矢印;DWI病変という)。新たなDWI病変の体積は、各DWI病変(図1A〜図1C)の周りに見出される注目領域(ROI)の表面積に基づき、スライス厚さ(3mm)を乗じて決定した。FLAIR画像上の病変は、新たなDWI病変の場所の内部に入り、かつ、登録時MRI上に前から存在しなかった場合に、新たなものであると判断した(図1A〜図1C)。新たなFLAIR病変の体積は、DWI病変の体積と同様に決定した。すべての計算は、Osirixソフトウェア(v.3.9.2,32ビットバージョン)を用いて行った。
ENACTの主たる目的は、神経保護仮説を試験することであった。したがって、検出可能な新たな病変の数および体積に対するTAT−NR2B9Cの影響は、主要な関心事であった。しかし、ENACTパラダイムはこれまで実施されたことがなく、MRI評価は、同じ関心度の4つの測定(DWI病変数、DWI病変体積、FLAIR病変数、FLAIR病変体積)を提供した。1次有効性成果基準として、手術後24〜96時間のDWI MRIおよびFLAIR MRI造影で測定される塞栓性脳卒中の体積を減少させる、TAT−NR2B9Cの単回静脈内投与の能力を選択した。主たる関心事の他の目的は、現在の患者集団におけるTAT−NR2B9Cの安全性および認容性を決定することであった。2次成果には、TAT−NR2B9Cの、塞栓性脳卒中の数を減少させることにおける有効性、微小脳卒中(<10cc)を有する患者における有効性、30日目フォローアップで手術誘発性認識機能障害を減少させることにおける有用性、大きな脳卒中(>10cc)の頻度を減少させ、破裂したまたは未破裂の動脈瘤を有する患者のサブグループにおける(新たな脳卒中の、神経学的および神経認知の)転帰を改善することにおける有用性、を含めた。
変更包括解析(mITT)原理;ITT;および、パープロトコル(per protocol)集団に従ってデータを分析した。mITT原理下では、評価可能な試料には、無作為化されあらゆる量の試験薬物を受けるすべての対象者が含まれる。生存していることが知られている患者の中で臨床結果のデータが欠けている場合は、可能な最悪のスコアを割り当てた。P値は、調整することなく表し、明記した場合には、転帰判断に影響する変数について調整した。
試験患者
すべての試験患者の処分を図2に示す。2008年9月〜2011年3月に、カナダの11ヶ所および米国の3ヶ所からの212人の患者を、試験の包含/除外基準(表4)に従って、スクリーニングした。15人は、基準に合致せず、無作為化しなかった。12例の無作為化失敗があり(患者は無作為化されたが、試験薬物を受けなかった):5例は、血管内動脈瘤修復ができなかったため、3例は、手術前MRI(手術と同日に計画)を得ることができなかったため、2例は、手術前ECGがCTc間隔>45ms(除外基準)を示したため、1例は、手術中かつ薬物注入の前に致命的な動脈瘤破裂があったため、1例は、麻酔専門医が重篤な慢性閉塞性肺疾患を有する患者に試験薬物を投与することを拒んだため、である。185人のすべての患者は、無作為化されて試験薬物を受け(mITT集団)、92人の患者が、無作為にTAT−NR2B9Cを受けるよう割り当てられ、93人の患者が、プラシーボを受けるように割り当てられた(図2)。TAT−NR2B9C群において、1人の患者は、2〜4日目MRIスキャンの前に死亡し、2人は、30日目フォローアップに参加するのを拒絶した。プラシーボ群において、2人の対象者が、2〜4日目MRIスキャンの後に死亡し、第3の対象者は、30日目フォローアップに参加するのを拒絶した(図2)。
185人の無作為化した対象者のうち、184人が手術後(2〜4日目)MRIスキャンを完了した。血管内手術の後TAT−NR2B9Cで処理した患者は、DWI MRIで検出される新たな虚血性病変の数において、43%減少を示した(表8;p=0.005)。このことは、また、FLAIR MRIスキャンにも反映され、このスキャンにおいて、TAT−NR2B9Cでの処理は、新たな虚血性病変の数を39%減少させた(表8;p=0.026)。FLAIR MRIおよびDWI MRIで測定した脳卒中体積の中央値も、TAT−NR2B9Cで処理した患者において減少した(表8;FLAIRおよびDWI MRI体積[Stataにおいてランク順序ロジスティック回帰を使用]の双方についてp<0.001)。ただし、体積データの分布が歪んで広く分散しているため(表8)、平均梗塞体積は、大きな脳卒中(>10ccと規定)を示した患者によって、有意に歪められた。特に、TAT−NR2B9C群に割り当てられた2人の患者は、動脈瘤修復手術の間に、大きな脳卒中(10.7ccおよび49.2cc)を引き起こした合併症を患った。このことが、平均脳卒中体積において、処理群間の有意でない全体差をもたらした。
ENACTに登録された患者は、選択的動脈瘤修復を受けたか、WFNSスコアI〜III(グラスゴー・コーマ・スコア13〜15)のSAHを経験したかの、どちらかである。したがって、彼らは、血管内手術の時点で、低レベルの神経学的障害を示した
(NIHSSおよびmRSの中央値が0;表7)。手術の30日後に、TAT−NR2B9Cで処理された全患者の93.5%、および、プラシーボで処理された患者の98.2%が、好ましいNIHSSスコアを有した(NIHSS 0〜1,p=0.434;表9)。mRSスコアは、TAT−NR2B9C群およびプラシーボ群の各々の93.5%において、好ましかった(0〜2)(表9)。ただし、破裂した動脈瘤を有する登録対象者において、TAT−NR2B9Cで処理した対象者の100%が、手術の30日後に好ましいNIHSSを有したが、これは、プラシーボで処理した患者では68.4%であったことと好対照である(p=0.020;表4)。TAT−NR2B9Cで処理したSAH患者は、また、プラシーボで処理した患者と比べて、より好ましいmRSスコアを有する傾向にあった(TAT−NR2B9Cおよびプラシーボにおいて、それぞれ、94.4%対73.7%;p=0.180;表9)。手術合併症により大きい脳卒中(>10cc)を被った患者は、TAT−NR2B9Cに含めた。30日目に、1人は、NIHSSが2、mRSが2であり、他の患者は、NIHSSが2、mRSが1であった。
神経認識試験バッテリーからの転帰を、補助的表7に詳述する。TAT−NR2B9C処理した患者は、TAT−NR2B9Cでの処理後、神経精神医学的な質問一覧における有意な改善を含む、神経認識利益を示した。驚いたことに、破裂動脈瘤群において、患者の数は統計分析には少なすぎるが、TAT−NR2B9Cで処理した患者について、認識試験および神経精神医学的試験における改善の有意な傾向があった。
合計で3人の患者が死亡した。1人は、未破裂動脈瘤を有して登録され、TAT−NR2B9Cを受けたが、鼠径部穿孔によって誘導される後腹膜血腫に由来する出血性ショックにより3日目に死亡した。2人はプラシーボを受けた。1人は、先に未破裂であった動脈瘤の手術中破裂に続く神経学的合併症で、13日目に死亡した。他の1人は、破裂動脈瘤を有して登録され、SAHの神経学的合併症により、12日目に死亡した。
手術誘発脳卒中におけるTAT−NR2B9Cのこの試験は、破裂したまたは未破裂の頭蓋内動脈瘤を有する患者において、塞栓性脳卒中の数および中央値体積を減少させるTAT−NR2B9Cの効果を示した。あらかじめ特定したサブグループ分析において、最も恩恵を受けた患者は、破裂した動脈瘤によるSAHを有して登録された患者であり、TAT−NR2B9Cでの処理は、脳卒中の数を64%、体積を80%減少させた。さらに、このサブグループの患者は、NIHSSによる神経学的転帰の改善を示し、mRSによる神経学的転帰および認識転帰を改善する傾向があった。破裂した動脈瘤の対象者のこのサブグループの大きさが小さい(37患者)にもかかわらず、TAT−NR2B9C効果は、統計的に有意な結果をもたらすほど大きかった。このことは、TAT−NR2B9C処理の恩恵が、主として虚血の減少を通じて生じると考えられていたこと、および、SAHにおいて5日目よりも前に虚血はめったに見られないことに鑑みると、驚くべきである。
SAHを有する患者は、そのSAHの種々の異なる合併症を患う危険性にある。これらの合併症は、独立に、脳への障害を引き起こし、臨床転帰に影響するかも知れない。そのような合併症には、動脈瘤の破裂の結果としての頭蓋内圧(ICP)の突然の上昇によって被る元の脳障害に加えて、脳血管攣縮による脳水腫または脳虚血などの、より遅い合併症を含む。

- 前記薬剤は、中枢神経系に影響する出血の治療のための他の薬物での治療の前、途中または後に投与されることを特徴とする請求項34または35に記載の方法。
- NMDAR2サブユニットへのPSD−95の結合を阻害し、再かん流に起因して出血性脳卒中に変化する虚血性脳卒中の治療に使用される薬剤であって、前記虚血性脳卒中または出血性脳卒中の損傷作用を抑止することを特徴とする薬剤。
- 前記薬剤は、X1tSX2V(配列番号7)を含むまたはX1tSX2V(配列番号7)で構成されるアミノ酸配列を有するペプチドであって、tおよびSは代替アミノ酸であり、X1はE,QおよびAから選択され、X2はA,Q,D,N,(N−メチル)−A,(N−メチル)−Q,(N−メチル)−Dおよび(N−メチル)−Nから選択されるペプチドであることを特徴とする請求項1〜37のいずれか1項に記載の方法または薬剤。
- 前記ペプチドは、内在化ペプチドに連結されまたは脂質付加されており、それによって前記ペプチドの細胞膜または血液脳関門の通過が容易であることを特徴とする請求項38に記載の方法または薬剤。
- 前記薬剤は、YGRKKRRQRRRKLSSIESDV(配列番号6)もしくはYGRKKRRQRRRKLSSIETDV(配列番号37)を含むまたはYGRKKRRQRRRKLSSIESDV(配列番号6)もしくはYGRKKRRQRRRKLSSIETDV(配列番号37)で構成されるアミノ酸配列を有するペプチドであることを特徴とする請求項1〜39のいずれか1項に記載の方法または薬剤。
- 前記薬剤はミリストイル化ペプチドであることを特徴とする請求項1〜40のいずれか1項に記載の方法または薬剤。
- 前記ペプチドは、KLSSIESDVもしくはKLSSIETDVを含むまたはKLSSIESDVもしくはKLSSIETDVで構成されるアミノ酸配列を有することを特徴とする請求項41に記載の方法または薬剤。
- 前記ミリストイル化は前記ペプチドのN末端にてなされていることを特徴とする請求項41または42に記載の方法。
- 脳卒中もしくはCNSにおけるもしくはそうでなければCNSに影響する出血の治療または予防において有用な活性について化合物をスクリーニングする方法であって、
中枢神経系に影響する血管内修復処置を受けるヒトに前記化合物を投与すること、および
前記化合物がMRIによって観察される梗塞の数を、陰性対照に比べて、減少させるかを判断することを含むことを特徴とする方法。 - 前記MRI造影にはDWI MRIが含まれることを特徴とする請求項44に記載の方法。
- 前記MRI造影にはFLAIR MRIが含まれることを特徴とする請求項44に記載の方法。
- 前記MRI造影にはDWI MRIおよびFLAIR MRIが含まれ、前記血管内手術に起因する梗塞は、DWI MRIおよびFLAIR MRIの両者に存在する梗塞を同定することによって判断されることを特徴とする請求項44に記載の方法。
- 虚血の兆候および/または症状を呈する対象者集団を処理する方法であって、NMDAR2サブユニットまたはnNosへのPSD−95の結合を阻害する薬剤を前記対象者に投与することを含み、前記対象者は、再かん流療法の副作用の許容できない危険性について分析され、許容できない危険性を有しない対象者は再かん流療法を受け、許容できない危険性を有する対象者は再かん流療法を受けず、随意的に、前記薬剤は、NMDAR2サブユニットまたはnNosへのPSD−95の結合を阻害するペプチドであり、前記ペプチドは、内在化ペプチドに連結されまたは脂質付加されて、細胞膜または血液脳関門の通過が向上していることを特徴とする方法。
- X1tSX2V(配列番号7)で構成されまたはX1tSX2V(配列番号7)を含むアミノ酸配列を有する単離ペプチドであって、tおよびSは代替アミノ酸であり、X1はE,QおよびAから選択され、X2はA,Q,D,N,(N−メチル)−A,(N−メチル)−Q,(N−メチル)−Dおよび(N−メチル)−Nから選択され、当該ペプチドはN末端アミノ酸にて脂質付加されていることを特徴とする単離ペプチド。
- 前記ペプチドは、KLSSIESDVもしくはKLSSIETDVを含むまたはKLSSIESDVもしくはKLSSIETDVで構成されるアミノ酸配列を有することを特徴とする請求項49に記載の単離ペプチド。
- 対象者の集団におけるくも膜下出血の損傷作用を処理する方法であって、
NMDAR2サブユニットへのPSD−95の結合を阻害する薬剤を、くも膜下出血を有する対象者に投与することを含み、前記損傷作用は、前記投与された集団において、前記薬剤を受けない対照対象者に比べて、減少することを特徴とする方法。 - 減少する前記損傷作用は、ニューロン細胞死または認知障害であることを特徴とする請求項51に記載の方法。
