物質および方法:PSA30および11B6抗体は、Iodogen法を用いて125I(PerkinElmer、米国)で標識した。簡潔にいえば、200μgのPSA30を標識するため、1,3,4,6−テトラクロロ−3α,6α−ジフェニルグリコールウリル150μgでコーティングされた試験管を用いた。混合物を室温で15分間インキュベートした後、低分子量成分をゲルろ過(PD−10カラム、GE Healthcare、英国)によって除去した。放射化学的純度は、ゲルろ過後に95%であった。
その後、注射後72時間によって示される腫瘍取り込みにおいて急激に減少した。重要なことに、この同じ時点で、甲状腺取り込みには急激な増加がある。この逆の相関性は、脱ハロゲン効果が起こった有望な指標である。結論として、125I−PSA30は、LNCaPベースの異種移植片マウス中でfPSAを効果的に標的にすることができる。
物質および方法:動物実験は、実験動物の保護における国内の法律に従って実施した。動物実験は、当施設の動物研究の倫理委員会(local Ethics Committee for Animal Research)によって承認されている。Taconic Europe(Bomholt,デンマーク)から購入した雄免疫不全ヌードマウス(6〜8週齢)を本研究に用いた。hK2を発現するLnCAP前立腺がん細胞の異種移植片を、右側腹部に皮下移植した。異種移植のため、細胞培地100μLおよびマトリゲル(BD Sciences、ベッドフォード、米国)100μL中のLNCaP細胞、5.2×106細胞/マウス。DU145細胞(hK2陰性細胞系統)、1〜2×106を右側腹部にsc移植して陰性対照とした。LNCaP細胞の注射後3〜6週、LNCaP異種移植片を有するマウス5群(4〜5匹の動物/群)およびDU145異種移植片を有する1群(3匹の動物/群)に、それぞれ111In−DTPA−11B6(100μlで約200kBqおよびタンパク質22.5μg)100μlをiv注射した。動物は、異なる時点、4時間、24時間、48時間、72時間または168時間p.i.で、そして対照群は48時間p.i.で殺した。興味の器官(表を参照)を、シンチレーション計数用の20mlバイアル(Zinsser Analytic、フランクフルト、ドイツ)中に置き、計量し、そして3インチのNaI(Tl)検出器を備えた自動化ガンマ計数器(1480 Wizard OY, Wallac, Turku, フィンランド)中で測定した。すべての器官について、切開後および最終時点の後、2回測定した。放射活性の測定は、下の標準プロトコールとして実施した。

物質および方法:DARは、125I−PSAおよび18F−コリンを注射した動物で実施した。代謝性プローブの注射後1時間に動物を安楽死させ、腫瘍を直ちに除去し、Cryomount(HistoLab products AB,スウェーデン)中に固定し、液体窒素中で急速に凍結し、そしてDAR用に100μm切片または組織病理学および免疫組織化学(IHC)分析用に20μm切片に切断した。シリコンストリップ検出器ベースのシステム(Biomolex 700 Imager; Bimolex AS,オスロ)を用いてより厚い切片内の放射活性の分布を造影した。複数の放射性核種、この場合、125Iおよび18Fを注射した動物において、放出スペクトルおよび崩壊速度の両方における違いを用いて各放射性核種の個々の画像を作成した。
直接ヨウ素化(125I/124I/131I/):クロラミンT(CAT, Sigma St. Louis, MO,米国)法を用いて、タンパク質(10μl、PBS中1mg/ml)を、NaI溶液(4MBq)として125Iと混合した。PBS中のCAT(10μl、2mg/ml)を加えて反応を開始し、激しく振盪している間に1分間インキュベートし、次いで、メタ重亜硫酸ナトリウム(20μl、2mg/ml)を加えることによって終結した。PBSで予め平衡にしたサイズ排除クロマトグラフィーによってNAP−5カラムにおいて(セファデックスG−25、GE Healthcare)によって標識タンパク質を未反応125Iおよび低分子量反応成分から分離した。
放射標識複合体のin vivo安定性を分析するため、正常マウスに放射標識をi.v.注射し、異なる時点の後に安楽死させた。次いで、血液を集め、5,000gで遠心分離した。次いで、PBSで平衡にしたNAP−5カラム(カットオフ、5kDa)上で血液の試料を分離し、放射活性を示す高分子画分(high-molecular-fraction)の相対量を決定する。
細胞をペトリ皿(直径6cm、約2×105細胞/皿)中に播種する。48時間後、放射標識タンパク質(57ng/皿または287ng/皿(抗原当たり約1:1および5:1の抗体に対応する)を細胞に加える。培地中の125/131I/177Luの効果を決定するため、いくつかの皿を過剰量の非標識タンパク質(29μg/皿)でプレインキュベートする。余分の皿は、細胞(DPC)当たりの崩壊(decay)数の評価に用いる。これらの皿では、放射標識タンパク質の細胞取り込みを、24時間インキュベーション中に6回の時点で測定する。次いで、細胞を、冷たい無血清培地で6回洗浄し、新たな培地中でインキュベーションを続ける。細胞を、1週につき約1回計数し、2週毎に再播種する。DPCは、2つの抗体濃度について、およびブロックした皿について、取り込み曲線下で面積を算出することによって算定される。最低の放射標識タンパク質濃度では、細胞は約56DPCを受け、最高では約150DPCであるが、ブロックした細胞は約2DPCを受ける。得られた結果は、荷重係数として1/Y2を用いて非線形回帰(指数増殖)によって分析する。
以下の異種移植片モデル:LnCAP、DU145、PC−3腫瘍モデルを用いる。PSAは、全3つの細胞系統によって発現されるが、hK2はLnCAPによって発現され、かつDU145またはPC−3中では発現されない。
異種移植のため、0.02%トリプシン/PBS中で収集したLNCaP、DU145またはPC−3細胞(200万〜1000万の細胞)を培地中に再懸濁し、等量のMatrigen(BD Biosciences, Bedford, MA)を含む細胞懸濁液200μLを右側腹部にsc注射した。腫瘍形成は視覚的にまたは触診によってモニターした。
スキャッチャードの方法
産生された抗体変異体の結合親和性(Kd)は、Scatchard, Ann N Y Acad Sci 51:660-72 (1949)によるスキャッチャードの方法を用いることによって決定した。
簡潔にいえば、一定濃度の抗体(または、この場合、Fab抗体フラグメント)および様々な濃度のEu3+標識PSAトレーサーを用いた。
また、抗体変異体の結合動態および親和性は、Biacore機器におけるリアルタイム生物特異性相互作用分析によって決定してもよい。簡潔にいえば、PSAまたはhK2をアミンカップリングによってCM5センサチップ上に固定化し、固定化レベルを1000〜2000反応単位に到達させる。異なる抗PSAまたは抗hK2抗体誘導体(mAbおよびFabの両方)をHBS−EP緩衝液中の0.1〜10nMの範囲の濃度に希釈する。結合動態は、50μL/分の流速で結合相中5分および解離相30分、続いて再生(regeneration)で研究する。速度定数は、1:1のラングミュア結合モデルを用いて物質移動を補正して算出する。
放射標識mAbまたはFabの結合品質に関するモノクローナル抗体ベースの免疫放射線測定法(IRMA)は、インキュベーション間の洗浄ステップによる4ステップサンドイッチアッセイとして3通り行った(洗浄緩衝液:10mM Tris−HCl pH8.0、0.15M NaCl、0.05%Tween20)。アッセイは、確立された勧告に従って組み立て、最適化した。BreakapartマイクロタイタープレートをH117(0.2μg/ウェル)、遊離または完全PSAおよび同じ親和性を有するヒトカリクレイン2(hK2)を認識するモノクローナル抗体、30でコートし、コーティング緩衝液(75mM炭酸ナトリウムpH9.6)中で希釈し、そして4℃で一夜インキュベートした。次いで、クエンチング緩衝液(洗浄緩衝液中3%のフィッシュゼラチン)0.2μg/ウェルを用いてウェルを室温で2時間インキュベートした。次に、3ng/μL fPSAを含有する200μl血漿(雌)でウェルをコーティングし、室温で2時間インキュベートした。次いで、放射標識および非標識PSA30を、下行性の濃度でアッセイ緩衝液(50mM Tris−HCl pH7.5、0.1M NaCl、5mM EDTA、0.25%のBSAおよび0.05%Tween 20)中で一緒に混合し、そしてウェルに加えた(全体積:50μL/ウェル)。ウェル当たりの標識抗体のパーセンテージは、以下のとおりであった:100、92、84、68、50、30および0パーセント。プレートを室温で2時間インキュベートし、洗浄し、そしてNaIー(Tl)ウェルカウンター(1282 Compugamma CS; LKB Wallac, Turku,フィンランド)中で測定した。さらなる適用では、理論的な逸脱に関して<25%の検出能力における違いが容認された。標識後の検出品質の評価は、放射標識抗体が非標識0抗体の親和性/結合能力の70〜90%を維持していることを示した。
前立腺がんは、西欧諸国において男性で最も一般的に診断されるがんであり、がんのすべての新たな症例の25%を占め、がんによる死亡の14%を占める(22700443)。現在の治癒的な治療戦略(手術および照射)は、悪性腫瘍が前立腺に限局化されたときに成功しているだけである。播種性疾患の場合の治療戦略は、去勢に限定され、これは、しばしば、難治性になる前に12〜18ヵ月間増殖を抑制するだけである、(Scher HI et al, Cancer of the prostate. In: DeVita VT Jr, Hellman S, Rosenberg SA, eds. 7th ed. Philadelphia, PA: Lippincott Williams & Wilkins; 2005)。一過性反応を超える効果を有することが立証された療法がないため、新規な分子標的療法が緊急に必要である。前立腺がんが放射線感受性であるため、それは放射免疫療法に対する理想的な標的を呈している。また、放射免疫療法は、がんが典型的に広がる部位である骨髄およびリンパ節に高レベルの循環抗体を典型的に送達する。さらに、放射免疫療法は、「交差照射効果(cross-fire effect)」を利用しており、これは、選択された放射性同位体の放出された粒子範囲に応じて、抗体の直接結合なしで、周囲の抗原陰性バイスタンダー細胞を殺すことができる(16029058)。
物質
177Luは、Mallinkrodt Medical BV, Petten,オランダから購入した。標識動態および放射化学的純度を決定するため、CycloneTM Storage Phosphor SystemおよびOptiQuantTM画像分析ソフトウェア(Perkin Elmer, Wellesley, MA,米国)を用いてITLC(即時薄層クロマトグラフィー)細片(Biodex, 米国)上で放射活性を測定した。すべての化学物質は、Sigma Alchrichから入手し、緩衝液は、特に明記しない限り分析グレードの水を用いて内部で調製した。mAb 11B6は、ヒトカリクレイン2に特異的な抗体であり、約1.2nMのこの抗原に対する親和性を有する;上の配列番号:4および5を参照のこと(University of Turku,フィンランドから入手した)。in vivo研究では、hK2を発現している前立腺がん細胞系統LNCaP(ATCC, Manassas, VA,米国)を用いた。細胞を、10%のウシ胎児血清およびPEST(ペニシリン100IU/mlおよび100μg/mlストレプトマイシン)を補充したRPMI 1640培地で培養した。細胞を、5%CO2の加湿インキュベーター中37℃で維持し、トリプシン−EDTA溶液(0.25%トリプシン、緩衝液中0.02%EDTA、Thermo Scientific)で分離した。
11B6によるCHX−A''−DTPAの結合:PBS中のmAb 11B6の溶液を、0.07Mホウ酸ナトリウム緩衝液を用いてpH9.2に調整した。試料を、Amicon Ultra-2遠心ろ過機(2ml、100K)において濃縮した。キレート剤対抗体3:1のモル比で、40℃でタンパク質溶液をキレート剤CHX−A''−DTPA(Macrocyclics, 米国)と結合させた。反応を4時間後に終結し、以後DTPA−11B6と呼ぶCHX−A''−DTPA−11B6を、0.2M酢酸アンモニウム緩衝液(pH5.5)20mlで平衡にしたNAP−5カラム(GE Healthcare)上のサイズ排除クロマトグラフィーによって遊離キレートから分離した。結合した11B6および5A10を酢酸アンモニウム緩衝液1mlで溶離した。
10mM NaAc緩衝液(pH4.0)中のタンパク質hk2(Department of Biote
chnology, Turku,フィンランド)をBiacore 2000システムにおいてN−ヒドロキシスクシンイミド(NHS)、1−エチル−3−(3−ジメチルアミノプロピル)カルボジイミド塩酸塩(EDC)および1Mエタノールアミン塩酸塩−NaOH pH8.5を用いてアミノカップリングによってBiacoreから購入したCM4研究グレードチップ上に固定化した。mAb 11B6、その複合体DTPA−11B6および非特異的参照mAbとしてハーセプチン、すべてをBiacore緩衝液中で5つの異なる濃度(0.5nM、5nm、10nM、50nMおよび100nM)で2つのフローセル上に送り、最終的な結合を検出した。2つのフローセルの一方は、固定化hK2を含み、もう一方はブランク参照であった。チップは、25mMグリシン緩衝液(pH2.7)を用いて再生した。
標識複合体の安定性をPBS中、および過剰のEDTA、500×を超えるEDTA中で試験し、次いで、DTPAをm11B6上に結合させ、1週間および2週間4℃でインキュベートし、ITLC細片を用いて分析した。上記参照。
すべての動物実験は、実験動物の保護における国内の法律に基づいて実施した。動物実験は、当施設の動物研究の倫理委員会によって承認されている。雄の免疫不全ヌードマウス(NMRI)(6〜8週齢)を、Taconic Europe(Bomholt, デンマーク)から購入し、本研究に用いた。
111In−m11B6の体内動態(biokinetic)研究のため、放射性核種で標識したm11B6抗体25μgをマウスの尾静脈に注射した。動物を一定時間の間隔で屠殺した。
さまざまな標的器官への吸収線量を決定するため、MIRD−スキームを、マウス特異的S因子と共に適用した。崩壊数(累積活性)は、111In−m11B6の動態データから誘導した。双指数関数的関数を、最小自乗アルゴリズムによってデータポイントに適合させ、これらの表示と崩壊因子を乗算した積分として崩壊数を計算した。骨髄の累積活性は、血液方法に基づいており、ここで、赤色骨髄中の活性濃度は、血中活性濃度に比例すると仮定している。この血液に対する赤色骨髄の比率(RMBLR)は、0.36であると提案されており、それを本研究でも用いた。
177Lu−DTPA−m11B6の放射標識
図16の標識動態の結果は、標識効率が非常に高く、2時間インキュベーション後、90%に達することを示している。これは、非結合177Luの効果が僅かであり、優れた治療有効性が見込めることを保障している。
図18中のSPECT画像は、異種移植したヌードマウス(8MBq注射)における177Lu−DTPA−m11B6の分布を示す。
図18の種々の画像は、表2に説明したとおりである。
111In−m11B6の体内動態研究の結果は、上の実施例2で議論する。腫瘍組織中で蓄積が見られ、48時間で最大の16%IA/gとなり;他のすべての器官は、唾液腺を除いて活性の低下を示す(図8を参照のこと)。したがって、正常器官に対する高い腫瘍比が得られ、これは高い治療有効性にとっての必要条件である。
図21は、線量測定計算値に用いた111In−m11B6の体内動態研究の結果を示す。複合図内の各グラフにおいて、上方の点線は、1つの標準偏差を用いた動態研究の結果を表しており、実線の曲線は、適合させた双指数関数的関数であり、そして下方の点線の曲線は、111Inの崩壊が考えられる場合である。下方の点線の曲線下の面積は、線量測定計算値に用いる累積活性である。
図22は、1匹のマウスの腫瘍において(動物の側腹部の皮膚下に見える)、治療後に体積がどのように減少したかを示す。

- 請求項1〜45のいずれか1項に定義された薬剤の治療有効量および薬学的に許容される希釈剤、担体または賦形剤を含む医薬組成物。
- 非経口、静脈内、皮下、筋肉内送達または注射による送達に適合させた、請求項46に記載の医薬組成物。
- 請求項1〜45のいずれか1項に定義された薬剤または請求項46または47に定義された医薬組成物を含むキット。
- 放射性物質を使用する手術のような手術の前または手術中に前立腺がん患者への投与によって薬剤に使用するための、カリクレインタンパク質に対する特異性を有する結合部分(例えば、請求項1〜18のいずれか1項によって上に定義されたとおり)および検出可能部分(例えば、請求項29〜36のいずれか1項によって上に記述されたとおり)を含む、または、これからなる薬剤。
- 放射性物質を使用する手術のような手術の前または手術中に前立腺がん患者へ投与する薬剤の製造における、カリクレインタンパク質に対する特異性を有する結合部分(例えば、請求項1〜18のいずれか1項によって上に定義されたとおり)および検出可能部分(例えば、請求項29〜36のいずれか1項によって上に記述されたとおり)を含む、または、これらからなる薬剤の使用。
- 前立腺がん患者において実施される放射性物質を使用する手術のような手術の方法であって、該方法が、カリクレインタンパク質に対する特異性を有する結合部分(例えば、請求項1〜18のいずれか1項によって上に定義されたとおり)および検出可能部分(例えば、請求項29〜36のいずれか1項によって上に記述されたとおり)を含む、または、これらからなる薬剤の有効量を、手術前または手術中に患者に投与するステップを含む方法。
