図2は、GDC−0941のpH依存性の溶解度を示す。GDC−0941が、1.5および4.2のpKa値を有する弱塩基性化合物であることを考慮すると、この化合物はpH依存性の溶解度プロファイルを示す。溶解度データ(図2に示す)は、37℃で48時間平衡化した後に測定する。約0.8mg/mL〜8μg/mLという中程度の溶解度が、pH1.0〜3.0でそれぞれ観察される。pHが上昇するにつれて、溶解度が低下する。pH4.4を上回る試験したpH値のすべてにおいて、化合物は実質的に不溶性(USP定義)であり、測定される溶解度は約1μg/mLである。このように高pH値で溶解度が急勾配で低下することから、GDC−0941の吸収がpH依存性であり得ることが示唆される。したがって、適切なin vivo溶解および吸収には十分な胃酸が必要とされる可能性がある。最も急勾配の曲線部分は、PPIなどの酸抑制剤の影響を最も強く受けている領域にあたる。
GDC−0941のpH依存性の放出プロファイルを、USP溶解装置IIを用いてin vitroで評価した。様々な量のベタインHCl(100mg〜3000mg)を含有する2mM酢酸ナトリウム緩衝剤(pH5.0)900mlに溶かした個々のGDC−0941錠剤について、溶解試験を実施した。溶解媒体にベタインHClを添加していない対照も同様に試験した。緩衝能の低い媒体および初期pH5.0を選択して、無酸症(achlorhydridic)状態をシミュレートした。溶解データから、ベタインHClの添加により、媒体のpHが低下し、錠剤の溶解率が対応して向上していることが示される(図3)。(ベタインHClを100mg添加する場合および添加しない場合に実現する)pH4.5およびpH5.0において、初期の時点に放出される薬物のほぼすべては、溶液から沈殿していた。20分までに、溶解状態のままであったのは、活性薬物含有量の1%未満であった。500mg以上のベタインHClを媒体に添加した条件下では、同様の沈殿は観察されなかった。実際には、pHを2.4以下に低下させるのに十分なベタインHClを媒体が含有する場合には、薬物の90%超が急速に溶解し、溶解状態で維持することができた。このin vitro溶解データから、GDC−0941の薬物放出および溶解度を向上させるための再酸性化剤としてベタインHClを使用できることが実証される(実施例4)。
溶解度がpH依存性であるという理由から、ダサチニブをモデル、すなわちGDC−0941の代用物として試験した(図5)。すなわち、ダサチニブは、再酸性化化合物に反応して胃内でGDC−0941と同様の溶解作用を示す可能性が高い。
イヌにおけるGDC−0941の経口曝露を、40mg錠剤の初期投薬後のGDC−0941血漿中濃度の推移を用いて(図4)、かつ実施例6で説明するようにして測定した。グループ1のイヌはGDC−0941のみ、20mg錠剤2錠を与えられた。グループ2のイヌは、ファモチジン40mg錠剤で前処置して低酸症を誘導し、陰性対照とした。グループ3のイヌは、ペンタガストリン6mg/kgを筋肉内注射して前処置した。ペンタガストリンは合成ペプチドであり、ガストリン、ペプシン、および内因性因子の分泌を刺激し、酸分泌を生理的に刺激することによって消化管のpHを低下させる。グループ4のイヌは、再酸性化化合物であるベタインHCl(750mg錠剤2錠をGDC−0941 20mg錠剤2錠と同時に投与)で処置した。グループ5のイヌは、ファモチジンで前処置し、次いで、ベタインHCl 750mg錠剤2錠で処置した。
食物およびプロトンポンプ阻害剤であるラベプラゾールがGDC−0941の錠剤製剤のバイオアベイラビリティおよび薬物動態に与える影響を決定するために臨床試験を行った(実施例1)。PPIとGDC−0941の同時投与がGDC−0941の薬物動態に与える影響を評価する際、絶食状態時または高脂肪食の後のいずれかに、ラベプラゾール20mgを5日間連続で投与し、5日目にGDC−0941 40mgのプロトタイプ錠剤(2×20mg)を同時投与した。絶食状態におけるラベプラゾール+GDC−0941とGDC−0941単独の効果を比較したところ、GDC−0941のTmax中央値は約2時間(48.1% CV)であった。絶食状態において、CmaxとGDC−0941への曝露のどちらも、ラベプラゾールの存在下で有意に減少し(両方の変数に関してp<0.001)、CmaxおよびAUC0〜∞の推定の平均比(90%CI)はそれぞれ0.31(0.21、0.46)および0.46(0.35、0.61)であった。高脂肪食の存在下でラベプラゾールを投与した場合、AUC0〜∞に対する影響は、著しさが若干失われ(推定値0.60、90%CI[0.50、0.65]、p<0.001)、Cmaxについては不変であった(推定値0.43、90%CI[0.37、0.50]、p<0.001)。高脂肪食とラベプラゾールとの有意な相互作用は観察されなかった(CmaxおよびAUC0〜∞に関する二元配置ANOVAモデルにおいて相互作用の有意性を試験した際、それぞれp=0.16およびp=0.27)。全体的に見て、高脂肪食存在下でのGDC−0941投与後のラベプラゾール処置中のGDC−0941への曝露は、ラベプラゾールによって引き起こされるGDC−0941吸収の減少に打ち勝たなかった。そこで、ベタインHClを用いて提唱されている再酸性化法が、薬理学的に誘導された低酸症に起因するGDC−0941曝露のこの減少を和らげるであろうと仮説が立てられている。図6は、ラベプラゾール(PPI)が、絶食状態または摂食状態の健常志願者においてGDC−0941の吸収を有意に減少させることを示す。図7は、PPIで前処置されたI期癌の患者におけるGDC−0941吸収にラベプラゾールが与える影響を対照の1日目の薬物動態と比べて示す。
図8および実施例7に示したように、ラベプラゾールによってダサチニブAUC0〜22hrが18分の1に減少した。しかし、ラベプラゾール単独と比べて、ベタインHClを同時投与した場合は、ダサチニブAUCが約5倍増加し、それによって、DAS AUCを対照の80±32%まで回復させた。したがって、ベタインHCl(BHCl)によって、ラベプラゾール誘導性低酸症時のダサチニブ曝露が向上する。ダサチニブは、GDC−0941の代用分子であり、pH依存性の溶解性を示し、BCS2に分類される。現在の米国処方情報において、医師および患者は、H2受容体拮抗薬(H2RA)またはプロトンポンプ阻害剤(PPI)を同時投与しないことが推奨されており、この有意な曝露の減少がその理由である。ベタインHClが同時投与された場合に、低酸症状態におけるダサチニブの薬物動態プロファイルが大きく改善されたことに基づくと、曝露は対照のダサチニブレベル近くまで回復されるようである。
GDC−0941の薬物動態に対するpH改変物質の影響を説明および予測するための薬物動態学的/薬力学的(PK/PD)モデリングおよびシミュレーション
PK/PDモデリングおよびシミュレーションは、薬物濃度(PK)および薬物効果(PD)を定量的に説明するために、現在の薬物開発において広く使用されている。図10は、GDC−0941の薬物動態に対するpH改変物質の影響を説明および予測するために提唱されたモデルの概略図を示す。PK/PDモデルのパートIは、GI pHに対するPPI、H2受容体遮断薬、および/または再酸性化剤の影響、ならびに該当する場合には、バイオマーカーと胃内pHとの潜在的関係を定量的に説明し得る。PK/PDモデルのパートIIは、胃内pHとGDC−0941などpH依存性の溶解性/吸収性を有する基質のPKとの関係を説明し得る。臨床的データを用いて一度確立されると、PK/PDモデルは、様々な試験されていない投薬計画(scenario)のもとでのGI pHに対するPPI、H2受容体遮断薬、および/または再酸性化剤の影響、ならびに(GDC−0941など)pH依存性の溶解性/吸収性を有する試験された基質もしくは試験されていない基質のPKに対するその後の影響を予測するのに使用され得る。シミュレーションは、治験デザインおよび投薬推奨を最適化するのに使用され得る。また、モデルは、(GERDなど)GI pHが変動する患者における基質PKプロファイルを予測するのにも使用され得る。
健常志願者において2つのパートからなる非盲検無作為化薬物動態(PK)試験を行って、絶食状態でのGDC−0941のカプセル製剤と錠剤製剤の相対的バイオアベイラビリティを評価し(パート1)、また食物およびプロトンポンプ阻害剤(PPI)がGDC−0941の錠剤製剤の薬物動態に与える影響を決定した(パート2)。年齢18〜65歳の健常な成人男性および成人女性がパート1に好適であり、パート2には年齢18〜45歳が好適であった。高齢者では胃酸産生の減少がより頻繁に観察されるため、パート2の年齢の上限を下げた。18名の被験者をパート1に登録し、31名の被験者をパート2に登録した。
ステージ2の進行した固形腫瘍を有する患者約100名が、GDC−0941を用いた試験に登録されることが予期された。GDC−0941薬物動態に対するプロトンポンプ阻害剤(PPI)の影響を、PIK3CA変異陽性コホートに参加していない最高9名の患者において試験した。ステージ2の最高9名の患者(腫瘍PIK3CA変異陽性コホートに参加している患者を除く)が、GDC−0941薬物動態に対するPPIの影響の評価に参加する。サイクル1の4〜8日目に、ラベプラゾール20mg(朝が最適)を患者に投与する。8日目にGDC−0941をラベプラゾールと同時投与し、PK解析のために血液試料を採取する。GDC−0941のみによる処置を9日目に開始して続ける。この試験の結果を図7に示す。全体的に見て、GDC−0941の40%の減少が、ラベプラゾールで前処置した被験者において測定された。in vitro試験から、GDC−0941の溶解度はpH上昇と共に低下することが実証され、上部GI管のpHの変化が、GDC−0941の溶解性に、したがって、その経口吸収および曝露の速度および程度に、影響を及ぼし得ることが示唆される。これらのデータは、GDC−0941を摂取している間のPPIの使用に関する投薬指示を変更するか、かつ/またはGDC−0941と共にPPIを使用するタイミングに関して具体的な用量投与指示を提供するのに使用される。4〜7日目まで、患者はラベプラゾール20mgを毎日自分で投与する。8日目に、ラベプラゾールをGDC−0941と同時投与する。薬物動態学的(PK)アッセイのための一連の血液試料を1日目および8日目に得た。
GDC−0941のカプセル剤は、高密度ポリエチレン(HDPE)瓶中に入れられ、子供が開けられないキャップで閉められている。瓶はインダクションシーリングされており、臨床使用用にラベルが付けられている。また、GDC−0941は、フィルムコーティングされた即放性錠剤としても製造されている。この錠剤は、乾式造粒法によって製造され、経口投与向けの様々なサイズ/重量の錠剤に圧縮される。調製された製剤は、2種類の強さのフィルムコーティング錠剤(20mgおよび100mg)として提供され、これらは錠剤のサイズ、形状、および重量が異なる。錠剤は、乾燥剤と共にHDPE瓶中に入れられ、子供が開けられないキャップで閉められている。瓶はインダクションシーリングされており、臨床使用用にラベルが付けられている。
ラベプラゾールナトリウム20mg(ACIPHEX(登録商標))は、放出遅延腸溶コーティング錠剤として、経口投与用に入手可能である。
約50%の用量漸増、続いて約33%の用量漸増を、次のコホートで使用する。
pHの異なる媒体中でGDC−0941溶解度を測定した。溶解度試験は、フラスコ振盪法によって行った。pH値がpH1.0〜7.5の範囲である50mM USP緩衝液中でGDC−0941溶解度を試験した。各試料は体積約2mLで調製し、各試料中に過剰な固体を共存させた。溶解度試料はすべて、37℃で48時間、常に混ぜた。次いで、試料をろ過し、C18カラムおよびリン酸緩衝液(pH6.5)、アセトニトリル勾配による溶出を用いた逆相条件下のHPLCによって、濃度を決定した。注入体積は5μLであり、流速は1.2mL/分、検出は230nmで行った。GDC−0941の溶解度は、約0.8mg/mL(pH1.0の際)から1μg/mL未満(pH7.5の際)の範囲で変動した。GDC−0941のpH−溶解度プロファイルを図2に示す。
様々な量のベタインHCl(100mg〜3000mg)を含有する2mM酢酸ナトリウム緩衝液(900mL、pH5.0)ならびにベタインHClが添加されていない対照において、GDC−0941 85mg錠剤の溶解を行った。実験は、USP Apparatus II溶解装置を75RPMでパドルを回転させて用いて、37℃で実施した。各時点で放出されたGDC−0941の濃度を、接続されたUV−Vis分光光度計を用いて319nmで検出することにより決定した。図3は、得られた溶解プロファイルを示している。ベタインHClの添加により、媒体のpHが低下し、錠剤の溶解率がそれに対応して向上した。トリメチルグリシン塩酸塩としても知られるベタインHClは、式(CH3)3N+CH2CO2H Cl−を有している。
ファモチジン誘導性低酸症に罹患させたイヌにおいてダサチニブ薬物動態を評価した(図5A)。ダサチニブは、50mg SPRYCEL(登録商標)(Bristol−Myers Squibb)錠剤として投与した。ファモチジン40mg錠剤をダサチニブの3時間前に投与した。ダサチニブ投与より30分前に、ペンタガストリン注射を筋肉内に行った。ベタインHCl 750mg錠剤2錠を次の時点に投与した:ダサチニブ投与より20分前および5分前に1錠を経口投与。
グループ1:前処置なし(対照)
グループ2:ダサチニブより3時間前にファモチジン40mg錠剤を経口投与
グループ3:ダサチニブ投与より30分前にペンタガストリンを筋肉内注射(6mg/kg)
グループ4およびグループ5:ベタインHCl 750mg錠剤2錠をこれら2つのグループに次のように投与した:ダサチニブより5分前にベタインHCl錠剤を1錠、ダサチニブより20分前にもう1錠を経口投与。
グループ5:ダサチニブより3時間前にファモチジン40mg錠剤を経口投与。
GDC−0941処置前に、未処置の雄ビーグル犬の5グループ(1グループ当たりn=4)に異なる前処置をそれぞれ施した(図4)。
グループ1:前処置なし(対照)
グループ2:GDC−0941より3時間前にファモチジン40mg錠剤を経口投与
グループ3:GDC−0941投与より30分前にペンタガストリンを筋肉内注射(6mg/kg)
グループ4およびグループ5:ベタインHCl 750mg錠剤1錠をこれら2つのグループに次のように投与した:GDC−0941より5分前にベタインHCl錠剤を1錠経口投与。
グループ5:GDC−0941より3時間前にファモチジン40mg錠剤を経口投与。
経口用ラベプラゾール20mgを毎日2回、4日間投薬することによってラベプラゾール誘導性低酸症に罹患させた健常志願者である被験者において、ダサチニブ薬物動態を評価した。試験の5日目の午前中(a.m.)に、経口用ラベプラゾール20mgをさらに被験者に与え、ハイデルベルグカプセルpH診断システム(HC)を用いて胃内pHを連続的にモニターした。少なくとも15分間、胃内pHが4より大きい場合、用量1500mgのベタインHClを経口的に与え、胃内pHを2時間モニターした。この試験のパート2である3期クロスオーバー試験の間に、被験者10名の内の5名にそれぞれ以下を与えた:(A)ダサチニブ100mg(対照);(B)ラベプラゾール前処置後にダサチニブ100mg;(C)ラベプラゾール前処置後にダサチニブ100mg、ベタインHCl 1500mg。処置B+Cの場合、各試験日より前の3日間および各試験日の午前中に、ラベプラゾール20mgを1日2回与えた。22時間に渡って胃内pHをモニターし、DAS血漿中濃度を測定した(下記の試験概略を参照されたい)。ダサチニブ濃度対時間の結果を図8Aに、曝露(AUC)を図8Bに示す。
Claims (10)
- 再酸性化化合物と式:
を有するGDC−0941を、プロトンポンプ阻害剤、H2受容体拮抗薬、および制酸薬から選択される胃酸抑制治療薬を与えられている患者に投与することを含む、過剰増殖性障害を治療する方法。 - 再酸性化化合物が、ベタイン塩酸塩およびグルタミン酸塩酸塩から選択される、請求項1に記載の方法。
- プロトンポンプ阻害剤が、オメプラゾール、ランソプラゾール、ラベプラゾール、パントプラゾール、およびエソメプラゾールから選択される、請求項1に記載の方法。
- 患者に由来する試料のガストリンレベルまたはGDC−0941濃度を決定することをさらに含む、請求項1に記載の方法。
- 患者が過剰増殖性障害に罹患している、請求項1に記載の方法。
- 過剰増殖性障害が固形腫瘍または造血器腫瘍である、請求項1に記載の方法。
- 再酸性化化合物を用いずにGDC−0941と胃酸抑制治療薬を投与した場合より、患者におけるGDC−0941のバイオアベイラビリティが高い、請求項1に記載の方法。
- 再酸性化化合物を用いずにGDC−0941と胃酸抑制治療薬を投与した場合より、患者の胃液のpHが低い、請求項1に記載の方法。
- GDC−0941および再酸性化化合物が経口剤形に製剤化されている、請求項1に記載の方法。
- 式:
を有するGDC−0941のバイオアベイラビリティを高める方法であって、胃酸抑制治療剤を与えられている患者に、GDC−0941のバイオアベイラビリティを向上させるのに有効な有効量の再酸性化化合物を同時投与することを含む、方法。
