第2のカテゴリには、36から42の構成要素を含み、ほとんど、あるいは全く活性部位を持たないが、1/12サブユニットと呼ばれるサブユニット又はプロトマーの集合を可能にする、いわゆる「構造」又は「リンカー」ポリペプチド鎖が属する。

好ましくは、3から5mMの間の量のMgCl2と、

本発明に係る組成物は、低体温条件下及び正常体温条件下(生理的温度に近い)の両方において機能可能である。
(a)上記死亡ドナーを、組成物を用いて灌流するステップ。当該組成物は、少なくとも、1種類のグロビン、1種類のグロビンプロトマー、又は、環形動物の1種類の細胞外ヘモグロビンと、安定化溶液及び/又は臓器保存液と、を含み、脳死ドナーにより提供、あるいは、心臓死ドナーにより提供される臓器を保存するために、0℃から37℃の間、好ましくは2℃から5℃の間、より好ましくは、4℃の温度を有する。または、上記死亡ドナーを、水溶液を用いて灌流するステップ。当該水溶液は、塩化ナトリウム、塩化カルシウム、塩化マグネシウム、塩化カリウム、グルコン酸ナトリウム、及び酢酸ナトリウムと、任意に1種類以上の抗酸化剤とを含み、6.5から7.6の間の、好ましくは、7.1±0.5、好ましくは、約7.35のpHを有する。その後、
(b)移植のため上記臓器を摘出するステップ。次いで、
(c)0℃から37℃の間、好ましくは2℃から25℃の間、より好ましくは約4℃の温度にて、上記臓器に応じた所定の時間、上記(a)のステップにて決定した組成物あるいは水溶液の中で、上記(b)にて得られた上記臓器の静的又は動的潅流保存を行うステップ。
ここで、「上記臓器に応じた所定の時間」とは、上記のように移植される臓器に依存する固有の保存時間を意味する。
正常体温ECMOシステム又は低体温ECMOシステムは、血液流体の体外循環のための技術である。
ダブルバルーンの三重管腔カテーテルは、部分的に、腹部臓器のin situでの冷却ために利用可能である。特に、それが大腿動脈回路に注入されると、腎循環を単離することが可能になる。本発明に係る組成物の迅速な灌流は、ダブルバルーン三重管腔カテーテルを介して行うことができる。排出経路が大腿静脈に注入されると、灌流された液状組成物の排出が可能になる。
<材料及び方法>
《保存液中のM101の製造と使用》
M101(HEMO2life(登録商標)、Hemarina SA、フランス)は、タマシキゴカイヘモグロビンの緩衝液であり、保健当局の仕様に合わせて生物学的生成物の抽出の標準的な手順を用いて作製した。精製されたタンパク質は、−80℃で凍結させ、その後、実験の前に4℃で解凍し、次の保存液で希釈した。保存液は、UW(ViaSpan(登録商標)、Bristol Myers Squibb、ベルギー)、HTK(Custodiol(登録商標)、ドイツ)、IGL(IGL-1(登録商標)、Georges Lopez、フランス)、Celsior(Celsior(登録商標)、Genzyme、フランス)、Ringer Lactate(RL、Aguettant、フランス)、又はPerfadex(Perfadex(登録商標)、Vitrolife、スウェーデン)である。
(酸素固定)
まず、M101を、UW溶液に添加した(1g/L)。上記溶液から酸素を除くためにN2ガスを使用した。もう一つは、上記溶液にLLC- PK1細胞を加えた。そして、このように準備した2つを密閉シールした。M101の機能は、分光測光法(参考文献[35]参照)によりモニターし、これにより、オキシヘモグロビン及びデオキシヘモグロビンの特徴付けが可能となる。吸光スペクトルは、390−650nmの範囲にわたって記録された(UVmc2、Safas、モナコ)。溶存O2(dO2)は、O2センサー(Metler、Toledo、フランス)を使用して確認した。
M101のSOD活性を、OberleyとSpitzにより修正されたニトロブルーテトラゾリウム(NBT)アッセイ(参考文献[36]参照)により評価した。簡単に言うと、スーパーオキシドは、カタラーゼ及びDETAPACの存在下で、キサンチン及びキサンチンオキシダーゼにより生成された。NBTの減少が、560nmの分光測光により検出された。Cu/Zn-SOD複合体を不活性化するために、実験開始前にKCNを1時間4℃で添加した。ウシ赤血球に由来するCu/Zn-SOD複合体を、コントロール(Calbiochem)として用いた。吸光度の減少は、捕捉活性の増加を示す。スーパーオキシドアニオン産生の阻害率は、以下の式、[(A0-A1) / A0x100]を用いて、算出した。ここで、A0は、コントロールの吸光度であり、A1は、試料の吸光度である。
M101を溶液に加え(1g/L)、その構造を、HPLCシステム(Dionex、フランス)及び1cm×30cmのSuperose 6Cカラム(フラクションレンジ5−5000kDa、GE Healthcare、フランス)を使用して、4℃にて固定した自動サンプラ―を用いた、定組成ゲル濾過により経時的にモニターした。流速は、0.5ml/分であり、溶出液は、250−700nmのレンジを超えてフォトダイオード検出器でモニターされた。溶出曲線を得て、Chromeleon(登録商標)ソフトウェア(Dionex)を使用して処理した。
解離曲線が、時間0(At0)でのM101ピークの面積を用いた時刻t(At)でのM101ピークの面積の正規化によって得られ、そして、時間の関数としてプロットした。Prism GraphPadソフトウェア(GraphPadソフトウェア、USA)を、線形プロファイル(f(At/At0) =- kd.t、 T 1/2= 1 / (2.kd))における曲線、又は、単一指数プロファイル(f(At/At0) = a.exp [-kd.t])における曲線を調整するために使用した。受容性を、ベストの相関係数を用いて判定した。解離定数(kd)及び半減期(T 1/2)を、ベストの調整曲線(線形の又は単一指数の)、から推定した。
LLC-PK1ブタ近位尿細管細胞(CL-101、ATCC, LGC規格、フランス)を、参考文献[37]に記載されているように、培養した。冷虚血性病変は、任意の方法でM101を補充した保存液(UW、HTK、IGL、Celsior、RL、又はPerfadex)中で、大気雰囲気下で、4℃で、単層の細胞を保存することにより、シミュレートした。
アッセイは以下の通りである。
・壊死:乳酸脱水素酵素(LDH)の放出を、in vitroでの毒性アッセイキットを使用してテストした。
・アポトーシス:カスパーゼ3活性を、蛍光カスパーゼ3アッセイキット(R & D Systems、フランス)を用いて測定した。
・生存率:代謝活性を、MTTテストを用いて測定した。
・エネルギー含量:細胞内ATPを、アデノシン5'三リン酸(ATP)生物発光用テストキットを用いて測定した。キットは、製造業者のガイドラインに従って使用した。反応は、マルチラベルリーダー(Victor 3、Perkin-Elmer、フランス)を用いて定量した。
各パラメータでは、結果は、病変(コントロール)の前に細胞にて測定された値に対する、冷却保存された細胞において測定された値の百分率として表される。
ラージホワイトタイプのオスのブタ(INRA/GEPA、Surgeres、フランス)を、人間と動物の研究のための生命倫理委員会のフランスの勧告に従って参考文献[4]の記載に基づき用意した。右腎を摘出し、冷虚血性病変を注入し移植前24時間維持した。この時間は、United Network for Organ Sharing for renal allograftsによって収集された冷虚血時間よりわずかに長いため(19.6±8.4h、2000年、参考文献[43]参照)、選択された。左腎は、移植状況における腎臓の質量を模倣するために、摘出した。外科チームは、プロトコルが使用されていることを知らなかった。血管吻合の時間は30±5分であり、失血は最小限であり、術後合併症は全く認められなかった。以下の4つのグループを観察した。
(1)UW:UWを用いた臓器保存
(2)UW+M101:UWにM101を5g/L補充したもの
(3)HTK:HTK
(4)HTK+M101:HTKにM101を5g/L補充したもの
コントロールとして、疑似手術を行った動物を用いた。
ブタは、利尿(ml/24h)の、血中クレアチン濃度(mol/L)の、排泄ナトリウム画分(%)の、及びタンパク(g/24h)の測定のために、代謝ケージに入れた(参考文献[3-5]参照)。
生検は、再灌流後の7日目、14日目、及び1ヶ月に行った。ボーダー損失(Border loss)と管腔内剥離とを、6点の半定量的スケールを使用して評価した。0は、異常なし、1は、腎臓サンプルの25%未満に影響するわずかな病変がある、2は、病変は腎臓サンプルの25−50%に影響を与える病変がある、3は、腎臓サンプルの51−75%に影響を与える病変がある、4は、75%より多くの腎臓サンプルに影響を与える病変がある、5は、広範な壊死病変及び腎病変がある、ことを示す(参考文献[38]参照)。
間質浸潤の定量には、Banffの分類を適用した(参考文献[39]参照)。0は、細管に、炎症単核細胞が無い、1は、細管断面積あたり、あるいは、10細管細胞あたり、1から4の単核細胞の病巣がある、2は、細管断面積あたり5から10の病巣がある、及び、3は、細管断面積あたり10以上の単核細胞の病巣がある、を示す。尿細管間質性線維症を、Picrosirius染色(参考文献[40]参照)を用いて測定した。
免疫組織化学を、 ED1+細胞及びCD3+細胞(SouthernBiotech、USA)の浸潤を測定するために使用した。定量的評価を、5−10の高電圧フィールド(200X)にて実行した。
平均±SEM又はSDを示す。in vitroのデータは、Dunnettテストを用いて比較した。2つのグループ間の比較には、インビボでのデータに対して、Mann-Whitney Uテストを用いた(UW+M101と比較したUWと、HTK+M101と比較したHTKとの比較を実行した)。Spearmanテストを用いて相関を測定し、及びANOVAテストを用いて、テストに使用された溶液におけるM101の効果の依存性を測定した。SPSSソフトウェア(IBM、USA)、及びGraphpadソフトウェア(Graphpad、USA)を、統計分析のために使用した。p<0.050で有意差ありとした。
《M101の機能》
(酸素結合)
UWを用いた溶液中のM101分子は、N2を用いたバブリングにより、又はLLC-PK1細胞を追加したことにより、酸素が除去(脱酸素化)されている。細胞を用いて得られたデータは、N2を用いたものと同じ結果を与える。周囲の大気条件の下では、UWに溶解されたM101は、HbO2構造(酸素正常状態)になっている。調製物は、次いで密封され、 O2は細胞(低酸素症)によって消費された。低酸素状態の75分後、M101のスペクトルは、ソレー帯のシフト、ベータ及びアルファ帯の減少、及び約555nmの吸光度の増加を伴い、デオキシヘモグロビンの配置へと徐々に変化する(データは示さず)。完全な脱酸素化は、低酸素状態の90分後に発生し、スペクトルは、最小428nmのソレー帯、及び最大555nmである頭打ちを示す。すなわち、これは、デオキシヘモグロビン誘導体のスペクトル特性である。試料中の酸素がM101の第1スペクトルへのリターンをもたらすため、この状態は、可逆的である。dO2の測定は、吸光スペクトルと相関がある(データは示さず)。
M101は、NBT形成の完全阻害剤(93.5±1.1%)を用いた、効果的な抗酸化剤である。Cu/Zn-SOD酵素活性の特異的阻害剤であるKCNが、完全にM101の捕捉能力を抑制するため、この活性は、Cu/Zn-SODに関連する。
M101の安定性を、UW、HTK 、IGL、Celsior、RL、及びPerfadex中にて分析した。M101の解離定数及び半減期により、それが長期間安定であることが示された。
標準溶液にて腎臓上皮細胞を保つに当たって、UWは、非常に有害であった。LDH放出による細胞生存率の評価により、CSの12時間後の細胞生存率の減少が示された。カスパーゼ3の有意な活性は検出されなかった。代謝活性及びATP含量は、LDH放出と共に減少した。
M101は、これらの事象を抑制する。0.312g/Lであるが、24時間後に構造及び代謝の完全性とエネルギー含量とを有意に向上させるためには、十分であった。総保護が1.25g/LのM101にて達成された(LDH放出:6±8%、MTTテスト:71±13%、ATP含量:78± 23%)。 細胞エネルギー含量は、2.5g/L以上の濃度にて増加する(ATP含量>コントロールに対して120% ) 。M101は、時間依存的に腎臓細胞を保護した。細胞の完全性は、24時間で1.25g/L、36時間で2.5g/L、48時間で5g/L、及び72時間で10g/L、完全に保たれた(LDH放出<20%)。
実験は、他の溶液、つまり、RL、Perfadex、HTK、IGL、及びCelsiorで再現された。 UWの場合、これらの溶液で細胞を保存することにより、構造的及び/又は機能的な細胞の損傷が引き起こされる。次の2種類の結果が得られた。(1)UWと同じ方法で、細胞をRLに冷却保存したもの(これは、より少ない範囲に)、及び、Perfadexに冷却保存したもの、これら両方は、構造的及び機能的な損傷を呈した。(2)細胞を、HTK、IGL、又はCelsiorに冷却保存したものは、機能的な病変のみ呈した。各溶液において、M101の補充は、細胞の完全性及び細胞の機能の両方を保護する(LDH放出<20%、すべての条件下及びMTTテストで50−100%)。
UW+M101の混合液を用いて保存された腎臓を移植したブタは、1日目に、尿産生を再開し(UWに対する2日目と比較した)、そして、早くも、4日目には安定した尿の生産レベルに回復した(データは示さず。p=0.016)。HTKのグループでは、同等に利尿が回復した。UWのグループでの血清クレアチニンのレベルは、3日目にピークに達する高レベルを示し、その後、低速減少した。UW+M101の動物では、1日目に、ピークに達する有意に低い値を示し、そして、7日目に移植前のレベルを回復した(全ての時点でp=0.009、データは示さず)。HTKの動物は、1日目に、高い血清クレアチニンのピークを示した後、プレ移植レベルを超える緩やかな回復が続いたが、他方、HTK+M101の動物は、1日目にはるかに低いピークであり(p=0.009)、速くも11日目に移植前のレベルに回復した。ナトリウム再吸収の測定により、溶液単独の場合と比較してM101を用いて保存された腎臓では、より高い性能レベルが示された。
刷子縁の損失及び細胞脱離、すなわち、典型的なIRIの細管病変、の評価により、7日目及び14日目のUWの移植においてかなりの損傷が明らかになった。月1で安定した。UW+M101中に保存された腎臓は、少ない範囲に病変を示した。HTKのグループはM101を使用して、組織学的病変の改善に類似した傾向を示した。
フォローアップを通じてUWの移植においてかなりの免疫反応の関与があることが示された。UW+M101に保存された腎臓は、初期から少しの免疫浸潤を示し、その後、炎症の減少の兆候を示した。2つのHTKのグループは、低レベルの浸潤を示した。3ヵ月の時点の、自然免疫細胞(ED1+)及び適応能力のある免疫細胞(CD3+)の浸潤により、溶液単独を用いた場合と比べてM101を用いて保存された腎臓における浸潤レベルの減少が明らかになる。
ブタを、3ケ月の時点で、すなわち本発明者らが、このモデル(ブタ)における慢性線維症の発症を確認した時点(参考文献[41、42]参照)で、屠殺した。UWの移植臓器における間質性線維症及び尿細管萎縮(IFTA)の進行は、広範囲に広がる(23%)一方で、M101の補充により、上記疾病は有意に減少した(11%、p=0.049)。HTKの腎臓はまた、かなりのIFTAの進行を示した(25%)。ここでも、M101の添加により、著しく疾病の進行が減少した(10%、p=0.038)。
他の統計分析により、M101の補充は、3日目のクレアチニンレベル(R2=0.75、P=0.0001)、及び、3日目のナトリウム排泄量(R2 =0.74、P=0.0001)と負の相関があることが示された。加えて、本発明者らは、M101の補充はまた、クレアチニン(R2 =0.75、P=0.0001)、蛋白尿(R2=0.55、P=0.013)、及び線維症(R2=0.78、p=0.0001)の長期(3ヶ月)の結果と負の相関があると結論付けた。
ANOVAは、M101と、3日目のクレアチニン血中レベル(p=0.001)及びナトリウム再吸収(p=0.04)の初期の結果に用いた溶液と、に相互関係があるが、しかし、3ヶ月後のM101の長期の影響は、使用される溶液に依存しない、ことを明らかにした(データは示さず)。

Claims (10)
- 少なくとも、1種類のグロビン、1種類のグロビンプロトマー、又は、環形動物の1種類の細胞外ヘモグロビンと、安定化溶液及び/又は臓器保存液と、を含む組成物の使用方法であり、上記組成物は、脳死ドナーにより提供、あるいは、心臓死ドナーにより提供される臓器を保存するために、0℃から37℃の間の温度を有する、使用方法。
- 上記環形動物の細胞外ヘモグロビンは、多毛環形動物の細胞外ヘモグロビンから、好ましくは、クロムシ科の細胞外ヘモグロビン及びゴカイ科の細胞外ヘモグロビンから、より好ましくは、タマシキゴカイの細胞外ヘモグロビン及びネイレス(Nereis)の細胞外ヘモグロビンから選択されることを特徴とする請求項1に記載の使用方法。
- 上記安定化溶液は、塩、好ましくは、塩化物イオン、ナトリウムイオン、カルシウムイオン、マグネシウムイオン、及びカリウムイオンを含む水溶液であり、かつ、本発明の上記組成物に6.5と7.6との間のpHを付与することを特徴とする請求項1又は2に記載の使用方法。
- 上記安定化溶液は、NaClを90mM、グルコン酸Naを23mM、CaCl2を2.5mM、酢酸Naを27mM、MgCl2を1.5mM,KClを12.5mM含む水溶液であり、かつ、pH7.1±0.5であることを特徴とする請求項1から3のいずれか1項に記載の使用方法。
- 上記臓器保存液は、6.5から7.5の間のpHを有する水溶液であり、
(a)塩(好ましくは、塩化物イオン、硫酸イオン、ナトリウムイオン、カルシウムイオン、マグネシウムイオン、及びカリウムイオン)、
(b)糖類(好ましくは、マンニトール、ラフィノース、スクロース、グルコース、フルクトース、ラクトビオン酸、又はグルコン酸)、
(c)抗酸化剤(好ましくは、グルタチオン)、
(d)活性剤(好ましくは、キサンチンオキシダーゼ阻害剤、例えば、アロプリノール、乳酸塩、あるいは、ヒスチジン、グルタミン酸(又はグルタミン酸)、又はトリプトファン等のアミノ酸類、等)、及び、
(e)任意に、コロイド(ヒドロキシエチルスターチ、ポリエチレングリコール、又はデキストラン等)、を含むことを特徴とする請求項1から4のいずれか1項に記載の使用方法。 - 上記環形動物の細胞外ヘモグロビン、そのグロビンプロトマー、および/またはそのグロビンは、上記組成物の最終体積に対して、0.001mg/mlから100mg/mlの間、好ましくは、0.005mg/mlから20mg/mlの間、より好ましくは、1mg/mlから5mg/mlの間の濃度で存在し、上記組成物は、250から350mOsm/Lの間、好ましくは、275から310mOsm/Lの間、より好ましくは、約302mOsm/Lの浸透圧を有することを特徴とする請求項1から5のいずれか1項に記載の使用方法。
- 塩化ナトリウム、塩化カルシウム、塩化マグネシウム、塩化カリウム、グルコン酸ナトリウム、及び酢酸ナトリウムと、任意に1種類以上の抗酸化剤とを含む水溶液の使用方法であり、脳死ドナーにより提供、あるいは、心臓死ドナーにより提供される少なくとも1つの臓器をin situで保存するために、上記水溶液は、6.5から7.6の間の、好ましくは、7.1±0.5、好ましくは、約7.35のpHを有することを特徴とする、水溶液の使用方法。
- 上記水溶液を臓器保存液と組み合わせて使用することを特徴とする請求項7に記載の水溶液の使用方法。
- 心臓死ドナーにより提供、又は、脳死ドナーにより提供される臓器をex situで保存する方法であって、
(a)請求項1から6のいずれか1項に記載の組成物、あるいは、請求項7及び8のいずれか1項に記載の水溶液を用いて、上記死亡ドナーを潅流するステップと、
(b)移植のため上記臓器を摘出するステップと、次いで、
(c)0℃から37℃の間、好ましくは2℃から25℃の間、より好ましくは約4℃の温度にて、上記臓器に応じた所定の時間、上記(a)のステップにて決定した組成物あるいは水溶液の中で、上記(b)にて得られた上記臓器の静的又は動的潅流保存を行うステップと、を含む方法。 - 6.5と7.6との間のpHを有する組成物であり、
少なくとも、1種類のグロビン、1種類のグロビンプロトマー、又は、環形動物の1種類の細胞外ヘモグロビンと、
好ましくは、0から0.5mMの間の量のカルシウムイオンと、
好ましくは、20から100mMの間の量のKOHと、
好ましくは、20から125mMの間の量のNaOHと、
好ましくは、20から25mMの間の量のKH2PO4と、
好ましくは、3から5mMの間の量のMgCl2と、
好ましくは、5から200mMの間の量の、ラフィノース及びグルコースからから選択される少なくとも1種類の糖類と、
好ましくは、3から5mMの間の量のアデノシンと、
好ましくは、2から4mMの間の量のグルタチオンと、
好ましくは、0から1mMの間の量のアロプリノールと、
好ましくは、1から50g/Lの間の量の、ヒドロキシエチルデンプン、種々の分子量のポリエチレングリコール、及びヒト血清アルブミン、から選択される少なくとも1種類の化合物と、を含む組成物。
