Description
本願は国際出願であり、2010年11月16日に出願された米国仮特許出願第61/414,318号明細書、及び2011年8月31日に出願された米国仮特許出願第61/529,614号明細書に対する優先権を主張するものであり、これらの仮特許出願の各々は、その全体が参照により援用される。
用語「IGF−I」は、分子インスリン様成長因子Iを指し、及び用語「IGF−II」は、分子インスリン様成長因子IIを指す。用語「IGF−I/II」は、インスリン様成長因子I及びIIの両方を指す。
ヒト抗体では、マウス又はラットの可変領域及び/又は定常領域を有する抗体に関連する問題のいくつかが回避される。かかるマウス又はラット由来のタンパク質の存在は抗体の急速なクリアランスをもたらすことがあり、又は患者による抗体に対する免疫応答の発生をもたらすことがある。マウス又はラット由来の抗体の利用を避けるため、機能的ヒト抗体遺伝子座をげっ歯類、他の哺乳動物又は動物に導入して、そのげっ歯類、他の哺乳動物又は動物に完全ヒト抗体を産生させることにより、完全ヒト抗体を生成することができる。
本明細書に記載される治療方法での使用に好適な抗IGF抗体は、Goya et al.,Cancer Res.64:6252−6258(2004);Miyamoto et al.,Clin.Cancer Res.11:3494−3502(2005);国際公開第2005/018671号パンフレット、国際公開第2005/028515号パンフレット、及び国際公開第2003/093317号パンフレット;及び米国特許第7,939,637号明細書に記載されている。これらに開示される抗IGF抗体についての記載は、参照によって本明細書に援用される。
抗IGF−I/II抗体の無菌医薬製剤が、本明細書に開示される方法において有用である。無菌製剤は、例えば、抗体を凍結乾燥及び再構成する前又はその後に無菌ろ過膜でろ過することにより作成することができる。抗体は通常、凍結乾燥形態で、又は溶液中に保存され得る。治療用抗体組成物は、概して、無菌アクセスポートを有する容器、例えば、皮下注射針によって穿刺可能な栓などの、製剤の取り出しを可能にするアダプターを有する静脈内注射液バッグ又はバイアルに入れられる。
1.患者における癌の治療方法であって、IGF−I及びIGF−IIを結合する抗体、又はその抗原結合断片の少なくとも2用量を患者に投与するステップを含む方法において、用量が約1週間隔てられ、且つ各用量が約1.5mg/kg体重〜約15mg/mg/kg体重である、方法。
2.投与するステップが、前記用量の少なくとも3用量を約3週間にわたり投与するステップを含む、実施形態1に記載の方法。
3.患者における癌の治療方法であって、IGF−I及びIGF−IIを結合する抗体、又はその抗原結合断片の少なくとも2用量を患者に投与するステップを含む方法において、用量が約3週間隔てられ、且つ各用量が約30mg/kg体重〜約45mg/mg/kg体重である、方法。

MEDI−573は、Xenomouse(登録商標)技術によって生成される完全ヒト免疫グロブリンG2λ(IgG2)抗体で、チャイニーズハムスター卵巣(CHO)細胞で製造される。MEDI−573はヒトインスリン様成長因子hIGF−I及びhIGF−IIに選択的に結合し、腫瘍細胞においてインスリン様成長因子IGF−I及びIGF−II媒介性のシグナル伝達を阻害して、それにより腫瘍成長を阻害する。米国特許第7,939,637号明細書に記載されるとおり、キーホールリンペットヘモシアニン(KLH)と結合した可溶性組換えヒトhIGF−I及びhIGF−IIで交互に免疫したマウスから抗体を単離した。MEDI−573は2本の軽鎖と2本の重鎖とから構成され、総分子量は約151キロダルトンである。
IGF−1R及びIGF−I又はIGF−IIのいずれかのリガンドを発現する細胞のインビボ成長に対するMEDI−573の効果を決定するため、抗腫瘍効力試験を実施した。P12(ヒトIGF−I及びヒトIGF−1Rを異所性に過剰発現するNIH3T3細胞)又はC32(ヒトIGF−II及びヒトIGF−1Rを異所的に過剰発現するNIH3T3細胞)を、雌性無胸腺ヌードマウスの右側腹部に移植した。移植14日後(P12)又は17日後(C32)、腫瘍の容積がそれぞれ110mm3及び175mm3に達したとき、マウスを無作為化した。これらの試験について、MEDI−573は、0、3、10、30、及び60mg/kgの用量で週2回、合計4用量を腹腔内(IP)投与した。MEDI−573はP12腫瘍の成長を有意に阻害した。3〜30mg/kgの用量レベルで明らかな用量反応が観察され、3、10、及び30mg/kgでそれぞれ20%、66%、及び86%の腫瘍成長阻害(TGI)が生じた。より高用量の60mg/kgを投与したとき、生じたTGIは30mg/kgと同様であった。同様に、MEDI−573はC32モデルに対しても高度に有効であり、単剤として投与したとき投与した最も高い用量で最大91%のTGIであった。3〜30mg/kgの用量レベルで18%TGI〜86%TGIの範囲の抗腫瘍性用量反応が観察された。
このGLP試験の目的は、カニクイザルにおいてMEDI−573を13週間にわたり週1回IV投与した後の毒性及びトキシコキネティクスを評価すること、及び8週間の無治療期間中の任意の潜在的な毒性作用の回復を追跡することであった。この試験では、試験1日目〜8日目における抗体投与後の遊離MEDI−573及び全MEDI−573の双方のPK特性を評価した。加えて、30分間の静脈内注入によって週1回、13週間にわたり抗体を投与した後、定常状態の血清MEDI−573濃度を評価した。この試験で観察された結果に基づけば、複数用量投与後のMEDI−573(全抗体)のPK特性は、線形的(用量非依存的)であるとともに定常的(時間非依存的)であった。
この非GLP試験は、雄のカニクイザルにおいて1日目及び8日目にmAbを静脈内投与した後のMEDI−573のPK及びPD特性を評価するように設計した。4匹の動物を4つの治療群に無作為に割り当てた:群1(n=1、1mg/kg)、群2(n=1、3mg/kg)、群3(n=1、10mg/kg)及び群4(n=1、30mg/kg)。試験全体を通じて様々な予め定められた時間点で試料(PK用に血清及びバイオマーカー分析用に血漿)を採取した。血清中のMEDI−573濃度の測定は、遊離抗体の評価用のMeso Scale Discovery(登録商標)(MSD)検出プラットフォームに基づく認定された抗原捕捉アッセイを用いて実施した。実施例3に記載されるものと同じ方法論を用いてMEDI−573濃度を計測した。
カニクイザルにおいて用量容積6mL/kg/用量で1、10、又は60mg/kg/用量のMEDI−573を使用して、GLP、13週間、反復用量、静脈内注入の毒性試験を実施した。対照動物には6mL/kg/用量の媒体(生理食塩水、注射用0.9%塩化ナトリウム)を投与した。MEDI−573は、合計13用量を1週間に1回、30分間の連続静脈内注入により投与した。
第1相多施設非盲検単群用量漸増及び用量拡張(dose−expansion)試験を実施し、標準的治療に抵抗性の、又はそれに対する標準的治療が存在しない進行固形腫瘍を有する成人ヒト対象におけるMEDI−573の安全性、耐容性、及び抗腫瘍活性を評価した。複数の地点の評価可能対象のコホートの各々に対し、MEDI−573の5つの投薬量レベル(0.5、1.5、5、10又は15mg/kg)のうちの1つを7日毎に投与した。投与コホートは表5に示される。
Claims (84)
- 患者における癌の治療方法であって、IGF−I及びIGF−IIを結合する抗体、又はその抗原結合断片の少なくとも2用量を前記患者に投与するステップを含み、用量が約1週間で隔てられ、且つ各用量が約1.5mg/kg体重〜約15mg/kg体重である、方法。
- 前記投与するステップが、前記用量の少なくとも3用量を約3週間にわたり投与するステップを含む、請求項1に記載の方法。
- 患者における癌の治療方法であって、IGF−I及びIGF−IIを結合する抗体、又はその抗原結合断片の少なくとも2用量を前記患者に投与するステップを含み、用量が約3週間隔てられ、且つ各用量が約30mg/kg体重〜約45mg/kg体重である方法。
- 各用量が、IGF−I及びIGF−IIを少なくとも1日間中和するのに十分である、請求項1〜3のいずれか一項に記載の方法。
- 各用量が、IGF−I及びIGF−IIを少なくとも約1週間中和するのに十分である、請求項1〜3のいずれか一項に記載の方法。
- 各用量が、IGF−I及びIGF−IIを少なくとも約3週間中和するのに十分である、請求項3に記載の方法。
- 各用量が、IGF−Iを約40%超中和し、且つIGF−IIを約29%超中和するのに十分である、請求項1〜6のいずれか一項に記載の方法。
