Abstract

ステープラーは、
(i)トリガーと、
(ii)ステープラーの長手軸と略平行に配置されたアンビルドッキングピンと、

本発明は以下の実験例を参照してさらに説明され、それらの実験例は単に説明目的のために提示され、本発明の範囲を限定することを意図していない。
14人の患者が登録され、うち7人は腫瘍を有し、うち5人は潰瘍性大腸炎を有し、うち2人は外傷性直腸損傷を有する。患者は手術前に評価され、そして、手術後の2年間の中央値で評価された。
傍ストーマヘルニア(PH)は、高い疾病率を伴う。3つのランダム化比較試験によって、メッシュ補強がそれらの発生率を大幅に減少させることが示された。しかしながら、上記の技術は時間がかかり、腹腔鏡下で実行するのが困難であり、トレフィンの手動による伸張に依存する。SMARTは、これらの問題を未然に防ぐ。

- 前記アンビルヘッドは、湾曲しており、円錐形または円錐台形である、請求項1〜6のいずれか1つのステープラー装置。
- 前記アンビルは、前記アンビルシャフトから延伸し前記アンビルヘッドの厚み全体を通る1つ以上の中空チャネルを有する請求項1〜7のいずれか1つのステープラー装置。
- ステープラー装置に用いられるアンビルであって、アンビルヘッドと、アンビルシャフトとを有し、前記アンビルシャフトの長さは少なくとも4cmであることを特徴とするアンビル。
- 前記ステープラー装置の使用、又は、吻合の形成若しくはストーマトレフィンの形成における請求項1〜9のいずれかのステープラー装置へのアンビルの使用。
- 請求項1〜10のいずれかの前記ステープラー装置を使用して2つの面の間に吻合を形成する方法であって、
ステープル留めされる第1の面を前記ステープラー装置の前記アンビルに接触させ、
前記アンビルドッキングピンに前記アンビルシャフトを係合させ、
前記ステープル手段にステープル留めされる第2の面を係合するために、前記ステープラーを配置し、そして、
前記第1の面と前記第2の面をステープルによって接続するために前記トリガーを作動させる、ことを有する吻合を形成する方法。 - 前記アンビルドッキングピンと、係合されたアンビルを前記ステープラー内に格納するために格納手段を操作するステップをさらに有する請求項11の方法。
- ステープル留めされる前記第1の面は、巾着縫合の手段によって前記アンビルに接触される、請求項11または請求項12の方法。
- 前記アンビルドッキングピン、またはアンビルシャフトにメッシュを適用することをさらに有する、請求項11〜13のいずれか1つの方法。
- 請求項1〜14のいずれかの前記ステープラー装置を使用して生体にストーマトレフィンを形成する方法であって、
ストーマが形成される組織に切開部を形成し、
前記生体の内部に前記アンビルヘッドが配置され、かつ、前記アンビルシャフトが前記切開部を通って突出するように、前記ステープラー装置の前記アンビルを配置し、
前記ステープラーの前記アンビルドッキングピン上に前記アンビルシャフトをドッキングし、そして、
ステープル留めされる前記組織内へ一連のステープルを発射するためにトリガーを作動させる、ことを有するストーマトレフィンを形成する方法。 - 前記切開部を通して前記アンビルシャフトを回収するステップをさらに有する請求項15の方法。
- 前記アンビルドッキングピン、またはアンビルシャフトにメッシュを適用することをさらに有する、請求項15または請求項16の方法。
- 前記アンビルドッキングピンと、係合されたアンビルを前記ステープラー内に格納するために格納手段を操作することをさらに有する、請求項15〜17のいずれか1つの方法。
- 前記アンビルは、前記アンビルシャフトから延伸し前記アンビルヘッドの厚み全体を通る1つ以上の中空チャネルを有し、
前記方法は、前記アンビルを介して吸引を適用するステップをさらに有する、請求項11〜18のいずれか1つの方法。 - 請求項1〜9のいずれか1つの前記ステープラー装置と、セーフティガードとを有するパーツのキット。
- 請求項1〜9のいずれか1つの前記ステープラー装置と、メッシュとを有するパーツのキット。
- トングをさらに有する、請求項20または請求項21のパーツのキット。
- 前記トングは、ヒンジまたはフレキシブルな関節によって一端が互いに接続される一組の対称な長手部材を有し、
前記一組の長手部材は他端側が湾曲しており、かつ、1つ以上の凹部を向かい合う長手部材に有する、請求項22のパーツのキット。
