Abstract
Description
本明細書で用いられる「オブジェクト」とは、任意の所望の項目又は情報(名詞、動詞、形容詞、副詞、語句、文、記号、数字などを含め、一般に文字項目又は文字情報)を指す。したがって、オブジェクトとは、関係を形成しうる何かであり、情報源から得られ、同定され、且つ/又は検索される何かである。「オブジェクト」には、遺伝子、タンパク質、疾患、表現型、機構、薬物など、所望の実体が含まれるがこれらに限定されない。以下でさらに説明する通り、一部の態様では、オブジェクトが、データでありうる。
本明細書で用いられる「アレイ」という用語は、1又は2以上のペプチド又は核酸プローブを支持体に結合させた固体の支持体又は基質を指す。アレイは、1又は2以上の異なる核酸又はペプチドプローブを、基質表面の既知の異なる位置に結合させていることが典型的である。また、「マイクロアレイ」又は「遺伝子チップ」としても説明されるこれらのアレイは、既知のゲノム、例えば、ヒトゲノムに基づき、10,000、20,000、30,000、又は40,000個の同定可能な異なる遺伝子を有しうる。これらの汎用アレイを用いて、試料中で発現するか又は見出される遺伝子の全「トランスクリプトーム」又は転写物プール、例えば、RT及び/又はRT−PCRにかけて、相補的なDNAの単位複製配列セットを作製しうる、RNA、mRNAなどとして発現する核酸を検出する。アレイは、機械的合成法、リソグラフ以外の方法及び/又は光リソグラフ法と固相合成法との組合せを組み込む光指向の合成法などを用いて作製することができる。
結核菌感染に対する宿主反応について、総合的な不偏サーベイを得るために、Illumina HT12ビーズアレイを用いて、活動性TB患者、潜在性TB患者、及び健常対照の血液に由来する、ゲノム規模の転写物プロファイルを作成した。試料採取は、すべての患者から治療前に行った。結核菌についての陽性培養物により、活動性TBの診断を裏付けた。潜在性TB患者は、活動性TB患者に対する無症候性の家庭内接触者、又は流行国からの新規の入国者であり、ツベルクリン皮膚検査(TST)(ロンドン)の陽性、及びIGRA(ロンドン及び南アフリカ)の陽性により規定した。健常対照は、ロンドンで募集したものであり、上記の基準すべてについて陰性であった。3つのコホート:訓練集合(2007年1月〜9月にロンドンで募集した;13例の活動性肺TB患者;17例の潜在性TB患者;及び12例の健常対照);テスト集合(2007年10月〜2009年2月にロンドンで募集した;21例の活動性TB患者;21例の潜在性TB患者;12例の健常対照);及び検証集合(2008年5月〜2009年2月に南アフリカ(SA)、ケープタウン近郊、ケイエリチャタウンシップの高負荷流行地域で募集した;20例の活動性TB患者;31例の潜在性TB患者)(図16及び17;図7)を個別に募集し、そこから試料採取した。同様に、3つのコホートに由来する試料の処理及び解析も、すべて個別に実施した。知識の発見及び試料サイズの適合性評価には、訓練集合を用いた。「方法」で説明される通りに、全血試料からRNAを抽出し、処理した。結果として得られるデータにフィルターをかけて、検出されなかった転写物を除去したところ(α=0.01)、データセットを構成する試料の10%超において、標準化した表現で、全試料の中央値からの偏差が2倍未満であった。この教師なしのフィルター処理により、1836転写物のリストが得られ、これにより、活動性TB群において顕著な特徴が明らかとなった(図8a)。次いで、この1836転写物のリストを用いて、群間で有意に(クルスカル−ワリスによるANOVA;BH(Benjamini-Hochberg)多重検定補正を用いて、偽発見率を0.01に等しくした)示差的発現した特徴遺伝子を同定した。これにより、393転写物のリストが得られ、これらを、平均リンケージを2つのクラスター間の距離の尺度とするピアソン補正により階層クラスタリングにかけ、相対量が同様である転写物の遺伝子系統樹を創出した。これを、ヒートマップの左の系統図として示し、各個体に由来するデータを、臨床診断に基づき群分けされて示される、固有の転写物プロファイルに構成した(図1a)。これにより、潜在性TB患者又は健常対照に由来する試料の大半では見られなかった、活動性TBに顕著な特徴が明らかとなった。
本発明者らの結果(図1a〜1c)からは、活動性TB患者の転写サインについて、分子的異質性が見られることが明らかであった。大半の患者は、393遺伝子について同じ発現プロファイルを示したが、異なる転写物プロファイル又はよりあいまいな転写物プロファイルを示す少数の異常例が明らかであった。例えば、活動性TB群のテスト集合における21例の患者のうち、4例は、他の活動性TB患者と共にクラスタリングされることがなく、健常対照又は潜在性TB患者のプロファイルとより強く符合した(図1bでは、●、#、■、◆で表わす)。上記で論じた通り、これらが、K近傍アルゴリズムにより誤分類された4例の活動性患者であった。
これらの知見は、活動性TBの転写サインが、X線写真による疾患程度と相関することを示す。この転写サインが、TBの治療中に減殺され、治療効果を反映するかどうかを決定することは、所望であった。これはまた、この特徴が、TB疾患を真に反映することも裏付けるであろう。これを調べるため、抗マイコバクテリア治療を開始した2カ月後及び12カ月後において、活動性TB患者7例から再度試料採取し、それらの血液を、独立のテスト集合(n=12)に由来する、それらのベースラインにおける治療前の試料、及び健常対照試料と併せて、前出で説明した通りに、再度マイクロアレイ解析にかけた。活動性TB患者における393転写物の特徴は、ここでもまた、健常対照の393転写物の特徴とは異なることが観察された(図3a)。活動性TB患者の特徴が、健常対照の特徴により酷似し始めるように、治療の2カ月後、大半の活動性TB患者において、この転写サインは減殺され、治療の12カ月後には完全に消失した。治療の2カ月後における転写物プロファイルのこの変化は、転写物量の増大との関連でより顕著であり、TB患者の約50%では転写物量の増大が減殺された。これは、量の減少する転写物と対照的であり、量の減少する転写物は、治療の2カ月後にはまだ存在したが、治療の12カ月後にはベースラインの発現に復帰した。活動性TB患者の治療中に血中転写サインが消失したことは、X線写真による改善を反映すると考えられた(図3b)。次に、本発明者らは、治療中の各時点間における、健常までの分子的距離スコアの差違を解析した。治療の12カ月後における活動性TB患者の「健常までの分子的距離」スコアは、ベースラインの治療前より有意に低かった(p<0.001;フリードマンによる反復測定検定)(図3c及びd)。これらのデータは、活動性TB患者の血液における転写サインを用いて、治療の有効性をモニタリングしうることを示唆する。さらに、このことは、393転写物の特徴が、結核菌感染に対する宿主反応を真に反映することの証拠ももたらす。こうして、奏効する治療においては、活動性TBの転写サインが減殺され、これにより、新規の治療剤についての臨床試験を含めた、抗マイコバクテリア治療の効果を定量的にモニタリングする方法がもたらされる。
転写サインの解析を迅速化し、これに焦点を当て、活動性TB疾患における宿主反応を特徴づけるため、本発明者らは、モジュールデータマイニング戦略(Chaussabel, D. et al. A modular analysis framework for blood genomics studies: application to systemic lupus erythematosus. Immunity 29, 150-64 (2008))を用いた。この戦略は、遺伝子のクラスターが、ある範囲にわたる、異なる炎症性疾患及び感染性疾患において協調発現するという観察に基づく。このような遺伝子の離散的クラスターは、文献による不偏プロファイリングを介して、一貫した機能的関係を有することがしばしば示されうる特定のモジュールとして定義することができる(Chaussabel, D. et al. A modular analysis framework for blood genomics studies: application to systemic lupus erythematosus. Immunity 29, 150-64 (2008))。モジュール解析は、健常対照と比較した、活動性TB患者の血液における、機能的関与性を有する転写物量の変化の評価及び同定(少なくとも2例の個体において検出されなかった転写物だけをフィルタリングにより除外した(α=0.01)、全マイクロアレイのデータセットについて実施した)を容易とした(図4a)。健常対照と比較した、活動性TB患者の血液において観察されるモジュール特徴(モジュール)は、ロンドンにおける訓練集合及びテスト集合についても、南アフリカにおける独立の検証集合についても、画像的に極めて類似し(図4a)、古典的なクラスタリング解析を用いて観察される転写サインの再現性が、個別の不偏解析を介して裏付けられた(図1)。活動性TB患者のモジュール特徴は、B細胞関連転写物(モジュールM1.3)量及びT細胞関連転写物(モジュールM2.8)量の減少、並びに骨髄関連転写物(モジュールM1.5及びモジュールM2.6)量の増大を明らかにし、より程度は劣るが、好中球関連転写物(モジュールM2.2)量の増大を明らかにした。活動性TB患者の血液において、対照と比較して変化する転写物の最大部分は、インターフェロン(IFN)誘導性モジュール(モジュール3.1;転写物のうちの75〜82%)内の転写物であった(図4a;及び図10a〜10c)。
モジュール解析により示された活動性TB患者におけるIFN誘導性遺伝子の過剰表出(表4a)を裏付けるため、Ingenuity Pathway Analysisソフトウェアを用いて、393転写物の特徴を構成する転写物を解析した。文献により例示された他の生物学的経路と比較した、BH多重検定補正を伴うフィッシャーによる正確検定(p<0.0000001)を用い、393転写物において最も有意に過剰表出した機能経路として、IFNによるシグナル伝達経路が裏付けられた(図13)。活動性TB患者の血中では、IFN−γ受容体によるシグナル伝達及びI型IFNα/β受容体によるシグナル伝達の両方の下流にある遺伝子が、有意に過剰表出した(図4dでは赤色で印をつけた)ことは興味深い。活動性TB患者の血清中では、IFN−α2aタンパク質又はIFN−γタンパク質のいずれも検出されなかった(図13b及び13c)が、活動性TB患者の血中では、対照と対比して、IFN誘導性ケモカインであるCXCL10(IP10)レベルの上昇が検出されたことは、注目に値する(図4c)。
活動性TB患者の血中で量が増大し、IFN誘導性転写物と共にクラスタリングする1つの遺伝子は、多様な細胞において発現する免疫制御性リガンドである、プログラム細胞死リガンド1(また、CDE274及びB7−H1としても表記される、PDL−1)であった(図6)。PDL−1は、慢性ウイルス感染において、プログラム細胞死1受容体(PD−1)に結合することにより、T細胞の増殖及びエフェクター機能を抑制することが報告されている(Barber, D. L. et al. Restoring function in exhausted CD8 T cells during chronic viral infection. Nature 439, 682-7 (2006)、Day, C. L. et al. PD-1 expression on HIV-specific T cells is associated with T-cell exhaustion and disease progression. Nature 443, 350-4 (2006))。どの細胞がPDL−1を過剰発現させうるのかを決定するため、フローサイトメトリーにより活動性TB患者及び健常対照に由来する全血集団を解析したところ、活動性TB患者の全白血球では、対照/検証(南アフリカ)集合における潜在性TB患者と比較して、PDL−1が上方制御されることが示された(図6a及び図14)。PDL−1発現の増大は、活動性TB患者に由来する好中球において最も明らかであり、程度は劣るが、活動性TB患者に由来する単球でも明らかであったが、活動性TB患者に由来するリンパ球では明らかでなかった(図6b及び図14)。活動性TB患者から精製した好中球が発現するPDL−1転写物レベルが、健常対照に由来する好中球が発現するPDL−1転写物レベルより高レベルであったことは、フローサイトメトリーによるこれらの知見と符合する。これに対し、単球においてPDL−1が発現したのは、活動性TB患者7例中の2例に過ぎず、T細胞では検出可能な発現が見られなかった(図6c)。活動性TB患者の血液におけるPDL−1転写物量の増大は、大半の患者において、治療開始の2カ月後でもなお存在したが、治療が奏効した後では消失した(図6d)。
被験者の募集及び患者の特徴づけ:St.Mary's Hospital London、UKの地域研究倫理委員会(REC 06/Q0403/128)、及びUniversity of Cape Town、Cape Town、Republic of South Africaの地域研究倫理委員会(REC 012/2007)が、研究を承認した。すべての被験者は、18歳を超え、書面による説明同意文書を提出した。被験者は、St. Mary's Hospital and Hammersmith Hospital、Imperial College Healthcare NHS Trust、London、UK;Hillingdon Hospital、The Hillingdon Hospitals NHS Trust、Uxbridge、UK;及びthe Ubuntu TB/HIV clinic、Khayelitsha、Cape Town、South Africaから募集された。患者は、抗マイコバクテリア治療を開始する前に、将来を見越した形で募集及び試料採取したが、関与する研究群の臨床基準を完全に満たした場合に限り、最終的な解析に組み入れた。ロンドンで募集した第1のコホートに募集された活動性TB患者の部分集合からはまた、治療開始の2カ月後及び12カ月後にも試料採取した。妊娠しているか、免疫抑制されているか、又は糖尿病若しくは自己免疫疾患を有する患者は、不適格とされ、本研究からは除外された。南アフリカでは、すべての被験者に、Abbott Determine(登録商標)HIV1/2迅速抗体アッセイ検査キット(Abbott Laboratories社、Abbott Park、Illinois、USA)を用いて、日常的なHIV検査を施した。The Royal Brompton Hospital Mycobacterial Reference Laboratory、London、UK;又はThe Reference Lab of the National Health Laboratory Service、Groote Schuur Hospital、Cape Townが実施した感受性検査による呼吸器標本(痰又は気管支肺胞洗浄液)のマイコバクテリア培養物上で、検査室における結核菌の単離を行うことにより、活動性TB患者を確認した。英国では、IGRAを用いる陽性結果と併せて陽性TSTを伴う、TB診療室への照会患者から、潜在性TB患者を募集した。南アフリカにおける潜在性TB被験者は、Ubnutu TB/HIV診療所におけるボランティア検査診療室に自己照会する個体から募集し、TSTの結果によらず(TSTもなお実施したが)、IGRAが陽性であることだけを用いて、診断を裏付けた。健常対照被験者は、National Institute for Medical Research (NIMR)、Mill Hill、London, UKにおけるボランティア被験者から募集した。研究に組み入れるための最終的な基準を満たすには、健常ボランティア被験者が、TST及びIGRAのいずれによっても陰性でなければならなかった。
標準化:Illumina BeadStudio v2ソフトウェアを用いて、バックグラウンドを差し引き、各試料についての平均シグナル強度を、全試料についての全平均シグナル強度に照らして計量した。遺伝子発現解析ソフトウェアプログラムであるGeneSpring GX、version 7.1.3(Agilent Technologies社、Santa Clara、CA、USA;以後、GeneSpringと称する)を用いて、さらなる標準化を実施した。10未満の全シグナル強度値を、10に等しいとした。次に、各試料中の各プローブのシグナル強度を、全試料にわたる該プローブの中央値の強度で除することにより、遺伝子ごとの標準化を適用した。以下で詳述する、健常までの分子的距離の評価を除き、これらの標準化データを下流の解析すべてに用いた。

Claims (25)
- 潜在性/無症候性と考えられる活動性結核菌(Mycobacterium tuberculosis)感染を検出する方法であって、
潜在性/無症候性の結核菌感染を疑われる患者から、患者の遺伝子発現データセットを得るステップと、
前記患者の遺伝子発現データセットを、結核菌感染と関連する1又は2以上の遺伝子モジュールに類別するステップと、
前記1又は2以上の遺伝子モジュールの各々に対する前記患者の遺伝子発現データセットを、同様に同じ遺伝子モジュールに類別した非患者に由来する遺伝子発現データセットと比較するステップであって、前記1又は2以上の遺伝子モジュールに対する前記患者の遺伝子発現データセットにおける全遺伝子発現の増大又は減少が、潜在性/無症候性の結核菌感染ではなく、活動性結核菌感染を示唆するステップと
を含む方法。 - 遺伝子産物について決定した比較情報を用いて、診断、予後診断、又は治療計画のうちの少なくとも1つを下すか、又は策定するステップをさらに含む、請求項1に記載の方法。
- 潜在性結核(TB)患者を、活動性TB患者から識別するステップをさらに含む、請求項1に記載の方法。
- 患者の遺伝子発現データセットを、全血、末梢血単核細胞、又は痰のうちの少なくとも1つから得られる細胞から得る、請求項1に記載の方法。
- 患者の遺伝子発現データセットを、表2の遺伝子から選択される、少なくとも10、20、40、50、70、80、90、100、125、150、200、250、300、350、又は393遺伝子と比較する、請求項1に記載の方法。
- 患者の遺伝子発現データセットを、モジュールM1.3、M2.8、M1.5、M2.6、M2.2、及びM3.1のうちの少なくとも1つにおける遺伝子から得られる、少なくとも10、20、40、50、70、80、90、100、125、150、200の遺伝子と比較する、請求項1に記載の方法。
- 結核菌感染と関連する遺伝子モジュールが、モジュールM1.3、モジュールM2.8、モジュールM1.5、モジュールM2.6、モジュールM2.2、及びモジュール3.1からなる群から選択される、請求項1に記載の方法。
- 結核菌感染と関連する遺伝子モジュールが、B細胞関連遺伝子の減少、T細胞関連遺伝子の減少、骨髄関連遺伝子の増大、好中球関連転写物及びインターフェロン誘導性(IFN)遺伝子の増大の変化により選択される、請求項1に記載の方法。
- 患者の疾患状態を、患者の肺の放射線解析によりさらに決定する、請求項1に記載の方法。
- 患者を治療した後において、前記治療した患者の遺伝子発現データセットを決定し、前記治療した患者の遺伝子発現データセットが、正常な遺伝子発現データセットに復帰したかどうかを決定し、これにより、前記患者を治療したかどうかを決定するステップをさらに含む、請求項1に記載の方法。
- 潜在性/無症候性と考えられる結核菌感染が、活動性結核菌感染となるかどうかを予測する方法であって、
活動性結核菌感染を伴う第1の臨床群から得られる第1の遺伝子発現データセット、潜在性結核菌感染患者を伴う第2の臨床群から得られる第2の遺伝子発現データセット、及び非感染個体の臨床群から得られる第3の遺伝子発現データセットを得るステップと、
前記第1のデータセット、第2のデータセット、及び第3のデータセットのうちのいずれか2つの間で示差的な遺伝子発現を含む遺伝子クラスターのデータセットを作成するステップと、
潜在性感染、活動性感染、又は健常であることを示唆する、固有の発現/表出パターンを決定するステップであって、前記患者の遺伝子発現データセットが、モジュールM1.3、M2.8、M1.5、M2.6、M2.2、及びM3.1のうちの少なくとも1つにおける遺伝子から得られる、少なくとも6、10、20、40、50、70、80、90、100、125、150、又は200の遺伝子を含み、1又は2以上の遺伝子モジュールに対する前記患者の遺伝子発現データセットにおける全遺伝子発現の増大又は減少が、潜在性/無症候性の結核菌感染ではなく、活動性結核菌感染を示唆するステップと
を含む方法。 - 結核菌に感染していることが疑われる患者における感染を診断するためのキットであって、
前記患者から患者の遺伝子発現データセットを得るための遺伝子発現検出器であって、前記発現した遺伝子を前記患者の全血から得る検出器と、
前記遺伝子発現データセットを、結核菌感染と関連し、感染患者と非感染患者とを識別する、あらかじめ規定された遺伝子モジュールデータセットと比較することが可能なプロセッサーであって、全血が、対応する非感染患者と比較して、1又は2以上の転写遺伝子発現モジュールにおけるポリヌクレオチドレベルの総計の変化を示し、これにより、潜在性/無症候性の結核菌感染と、活動性となる結核菌感染とを識別するプロセッサーと
を含むキット。 - 患者の遺伝子発現データセットを、末梢血単核細胞から得る、請求項12に記載のキット。
- 患者の遺伝子発現データセットを、表2の遺伝子から選択される、少なくとも10、20、40、50、70、80、90、100、125、150、200、250、300、350、又は393遺伝子と比較する、請求項12に記載のキット。
- 患者の遺伝子発現データセットを、モジュールM1.3、M2.8、M1.5、M2.6、M2.2、及びM3.1のうちの少なくとも1つにおける遺伝子から得られる、少なくとも10、20、40、50、70、80、90、100、125、150、200の遺伝子と比較する、請求項12に記載のキット。
- 結核菌感染と関連する遺伝子モジュールが、モジュールM1.3、モジュールM2.8、モジュールM1.5、モジュールM2.6、モジュールM2.2、及びモジュール3.1からなる群から選択される、請求項12に記載のキット。
- 結核菌感染と関連する遺伝子モジュールが、B細胞関連遺伝子の減少、T細胞関連遺伝子の減少、骨髄関連遺伝子の増大、好中球関連転写物及びインターフェロン誘導性(IFN)遺伝子の増大の変化により選択される、請求項12に記載のキット。
- 遺伝子が、PDL−1、CASP5、CR1、CASP5、TLR5、MAPK14、STX11、BCL6、及びC5から選択される、請求項12に記載のキット。
- 潜在性/無症候性と考えられる活動性結核菌感染を検出するシステムであって、
患者から患者の遺伝子発現データセットを得るための遺伝子発現検出器であって、前記発現した遺伝子を前記患者の全血から得る検出器と、
前記遺伝子発現データセットを、結核菌感染と関連し、且つ、活動性疾患に増悪するリスクのある、潜在性結核菌感染を伴う患者を識別する、あらかじめ規定された遺伝子モジュールデータセットと比較することが可能なプロセッサーであって、全血が、対応する非感染患者と比較して、1又は2以上の転写遺伝子発現モジュールにおけるポリヌクレオチドレベルの総計の変化を示し、これにより、活動性疾患に増悪するリスクのある、潜在性結核菌感染を伴う患者を識別し、前記遺伝子モジュールデータセットが、モジュールM1.3、M2.8、M1.5、M2.6、M2.2、及びM3.1のうちの少なくとも1つを含むプロセッサーと
を含むシステム。 - 患者の遺伝子発現データセットを、表2の遺伝子から選択される、少なくとも10、20、40、50、70、80、90、100、125、150、200、250、300、350、又は393遺伝子と比較する、請求項19に記載のシステム。
- 患者の遺伝子発現データセットを、モジュールM1.3、M2.8、M1.5、M2.6、M2.2、及びM3.1のうちの少なくとも1つにおける遺伝子から得られる、少なくとも10、20、40、50、70、80、90、100、125、150、200の遺伝子と比較する、請求項19に記載のシステム。
- 結核菌感染と関連する遺伝子モジュールが、モジュールM1.3、モジュールM2.8、モジュールM1.5、モジュールM2.6、モジュールM2.2、及びモジュール3.1からなる群から選択される、請求項19に記載のシステム。
- 結核菌感染と関連する遺伝子モジュールが、B細胞関連遺伝子の減少、T細胞関連遺伝子の減少、骨髄関連遺伝子の増大、好中球関連転写物及びインターフェロン誘導性(IFN)遺伝子の増大の変化により選択される、請求項19に記載のシステム。
- 遺伝子が、PDL−1、CASP5、CR1、CASP5、TLR5、MAPK14、STX11、BCL6、及びC5から選択される、請求項19に記載のシステム。
- 治療剤の試験において有効性をモニタリングする方法であって、
結核菌に感染していることが疑われる患者から、患者の遺伝子発現データセットを得るステップと、
前記患者の遺伝子発現データセットを、結核菌感染と関連する1又は2以上の遺伝子モジュールに類別するステップと、
前記1又は2以上の遺伝子モジュールの各々に対する前記患者の遺伝子発現データセットを、非患者に由来する遺伝子発現データセットと比較するステップと、
前記患者を、前記治療剤により治療するステップと、
前記治療剤が、前記患者の遺伝子発現プロファイルを、非患者に由来する前記遺伝子発現データセットに変化させたかどうかを決定するステップであって、前記1又は2以上の遺伝子モジュールに対する前記患者の遺伝子発現データセットにおける全遺伝子発現の増大又は減少が、活動性結核菌感染を示唆するステップと
を含む方法。
