Abstract
【解決手段】ヒトもしくは他の哺乳類に、月に連続して1日、2日もしくは3日の継続服薬スケジュールに従って、約65%〜約110%の累積的な有効量のリセドロネートまたは製薬上許容できるその酸、塩、エステル、溶媒和化合物もしくはそれらの多形体を含む医薬組成物を経口投与することを含む方法は、骨粗鬆症やその他の代謝性骨疾患を治療または予防するのに有用である。

Description
本発明は、更に、月に連続して1日、2日もしくは3日の継続服薬スケジュールに従って投与するのに好適な医薬組成物であって、前記医薬組成物が、
(a)約65%〜約110%の累積的な有効量のリセドロネートと、
(b)1つ以上の製薬上許容できる賦形剤と、
を含む、前記医薬組成物にも関する。
本発明のキットは、月に連続して1日、2日または3日の継続服薬スケジュールに従ってリセドロネートを投与するのに特に有用である。かかるキットは、1つ以上の単位用量のリセドロネートと、本発明の方法の服薬遵守を促進するための手段を含む。本発明のキットは、治療されるべき対象者が的確な方式で的確な用量の適切な活性物質を摂取するのを保証するのに便利で効果的な手段を提供する。このようなキットの服薬遵守手段には、本発明の方法による活性物質の投与を促進するあらゆる手段が包含される。このような服薬遵守手段には、使用説明書、包装、および分配手段、並びにこれらの組み合わせが挙げられる。キットはまた、これらに限定されないが、当該週のその日を記載すること、番号を付けること、図表、矢印、点字、カレンダー用のシール、備忘カード、または患者によって具体的に選択されるその他の手段を包含する、記憶を助ける手段を含むこともできる。包装および分配手段の例は、当該技術分野において周知であって、特許文献6および特許文献7に記載のものを包含する。単位用量の特別な配列の例としては、特許文献8に記載のものが挙げられる。
閉経後骨粗鬆症と診断された65kgの女性に、リセドロネート150mgを含む医薬組成物を月1回服用するよう処方する。前記患者は、各暦月の最初の日に前記経口剤形を服用する。2年後に腸骨稜の骨の生検を採取すると、その患者のベースライン生検に比べて再構築単位の平均壁圧の増加が示される。
骨粗鬆症と診断された70kgの男性に、リセドロネート125mgを含む医薬組成物を月1回服用するよう処方する。前記患者は、各暦月の末日に前記経口剤形を服用する。2年後に腸骨稜の骨の生検を採取すると、その患者のベースライン生検に比べて再構築単位の平均壁圧の増加が示される。
閉経後骨粗鬆症と診断された62kgの女性に、医薬組成物を月2回服用するよう処方する。前記医薬組成物の各単位用量には、リセドロネート75mgが含まれる。前記患者は、各暦月の最初の週末の土曜日と日曜日に1日1回、医薬組成物の単位用量を服用する。2年後に腸骨稜の骨の生検を採取すると、その患者のベースライン生検に比べて再構築単位の平均壁圧の増加が示される。
骨粗鬆症と診断された72kgの男性に、医薬組成物を月3回服用するよう処方する。前記医薬組成物の各単位用量には、リセドロネート50mgが含まれる。前記患者は、各暦月の最初の週末の金曜日、土曜日および日曜日に1日1回、医薬組成物の単位用量を服用する。2年後に腸骨稜の骨の生検を採取すると、その患者のベースライン生検に比べて再構築単位の平均壁圧の増加が示される。
非盲検で、多施設の無作為化パラレルグループ試験を行って、6ヶ月の間、月に連続して3日間毎日リセドロネート50mgを服用した参加者の腰椎の骨密度(LSBMD)を、6ヶ月間毎日リセドロネート5mgを服用した参加者のものと比較する。LSBMDは、二重エネルギーX線吸収測定法(DXA)により、ベースライン時および治療から6ヶ月後に測定する。治療期間終了時に、月に連続して3日間、毎日リセドロネート50mgを服用した参加者は、ベースラインLSBMD測定値と比較してLSBMDの増加を示す。更に、月に連続して3日間毎日リセドロネート50mgを服用した参加者において記録されたLSBMDの増加は、6ヶ月間毎日リセドロネート5mgを服用した参加者において記録されたものにも匹敵する。

Claims (11)
- 骨疾患の治療または予防を必要としているヒトもしくは他の哺乳類に、月に連続して1日、2日もしくは3日の継続服薬スケジュールに従って、約65%〜約110%の累積的な有効量のリセドロネートを含む医薬組成物を経口投与することを含むことを特徴とする、前記のヒトもしくは他の哺乳類における骨疾患の治療または予防方法。
- 前記医薬組成物が、約100〜約165mgのリセドロネート、好ましくは約150mgのリセドロネートを含む単位用量の形態で月に1日投与されることを特徴とする、請求項1に記載の方法。
- 前記医薬組成物が、月に連続して2日投与され、2回の単位用量がそれぞれ約50〜約82.5mgのリセドロネート、好ましくは約75mgのリセドロネートを含むことを特徴とする、請求項1に記載の方法。
- 前記医薬組成物が、月に連続して3日投与され、3回の単位用量がそれぞれ約33〜約55mgのリセドロネート、好ましくは約50mgのリセドロネートを含むことを特徴とする、請求項1に記載の方法。
- 骨疾患が骨粗鬆症であることを特徴とする、請求項1〜4のいずれか一項に記載の方法。
- 月に連続して1日、2日もしくは3日の継続服薬スケジュールに従って投与するのに好適な医薬組成物であって、前記医薬組成物が、
(a)約65%〜約110%の累積的な有効量のリセドロネートと、
(b)1つ以上の製薬上許容できる賦形剤と、
を含むことを特徴とする、前記医薬組成物。 - 前記の剤形が錠剤であることを特徴とする、請求項6に記載の医薬組成物。
- フィルムコーティングを更に含むことを特徴とする、請求項7に記載の医薬組成物。
- 月に連続して1日、2日もしくは3日の継続服薬スケジュールに従って投与されるべきリセドロネートの単位用量をそれぞれ1つ、2つもしくは3つ含むことを特徴とする、リセドロネート治療処方計画の服薬遵守を促進するためのキット。
- 前記キットが更に、栄養素の単位用量を少なくとも1つ含み、前記栄養素が、カルシウム、ビタミンD、またはカルシウムとビタミンDとの複合単位用量から成る群より選択されることを特徴とする、請求項9に記載のキット。
- 骨粗鬆症の治療または予防を必要としているヒトもしくは他の哺乳類に、月に1日の継続服薬スケジュールに従って約150mgのリセドロネートを含む医薬組成物を経口投与することを含むことを特徴とする、前記のヒトもしくは他の哺乳類における骨粗鬆症の治療または予防方法。
