Abstract
Description
1.針の平面が鉗子の軸に対して垂直になるように針を把持し、
2.針の平面を縫合対象の縁部に対して垂直に配置し、
3.針の平面に対して垂直な軸に従って針を回動させて縫合対象の組織内に針を挿入させる。
従って、好ましい条件で縫合を行なうためには、器具の軸を縫合対象の縁部に対して略平行に配置し、鉗子を軸のまわりで回動させなければならない。一部の処置においては、手術野に対する切開点の配置が、剛直な器具を用いると鉗子の軸を縫合対象の縁部と整合させることができないような配置となり、縫合を行なうことが実質的に難しくなる。こうした理由から、遠位側の可動性により鉗子の軸を器具の主挿入軸に対して配向することができる器具が開発された。
− 長手軸に従った細長アームであって、当該細長アームにピボットジョイントを用いて前記長手軸に対して実質的に直交する軸に従って取り付けられる遠位端を有する細長アームと、
− 細長アームと実質的に同軸をなす駆動シャフトであって、ピボットジョイントに面する(向かい合う)ユニバーサルジョイント(自在継手)を形成する手段を含む駆動シャフトと、
− 駆動シャフト上にしっかりと固定されるとともに細長アームの遠位端上及び該遠位端の延長線上において回転可能に取り付けられた遠位側道具であって、互いに異なる独立した2つの回転自由度、すなわち、一方が細長アームの長手軸に対して略垂直な軸のまわりの回転自由度、他方が遠位側道具の自らの軸に対して実質的に共線をなす軸のまわりの回転自由度、を有するようになっている遠位側道具と
を含む手術器具であって、
ユニバーサルジョイントを形成する手段はフレキシブル駆動スリーブを含むことを特徴とする手術器具を提供する。
− フレキシブル駆動スリーブはベローズを含む。
− フレキシブル駆動スリーブは長手軸に従った捩り応力に対して剛直であると共に、長手軸に対して垂直なあらゆる軸に従った捩り応力に対しては可撓性で弾性変形が可能である。
− ピボットジョイントは、直列に取り付けられた複数のリンク要素を有する。
− 2つの隣接するリンク要素は有限ピボットジョイントに従って互いに取り付けられる。
− 各リンク要素は、隣接するリンク要素の方へと延在する2つのラグ部を含む中空筒形要素であり、ラグ部は互いに対向して有限ピボットジョイントを形成する。
− 細長アームは中空な筒形の要素であり、少なくとも部分的に駆動シャフトを包囲する。
− 細長アームは中空であり、少なくとも部分的に駆動シャフトを包囲する。

Claims (12)
- − 長手軸に従った細長アーム(111;207)であって、当該細長アームにピボットジョイントを用いて前記長手軸に対して実質的に直交する軸に従って取り付けられる遠位端(105;204)を有する細長アーム(111;207)と、
− 前記細長アームと実質的に同軸をなす駆動シャフトであって、前記ピボットジョイントに向かい合うユニバーサルジョイントを形成する手段を含む駆動シャフト(103;3)と、
− 前記駆動シャフトにしっかりと固定されるとともに前記細長アームの前記遠位端上及び前記遠位端の延長線上において回転可能に取り付けられた遠位側道具(9)であって、互いに異なる独立した2つの回転自由度、すなわち一方が前記細長アームの前記長手軸に対して実質的に垂直な軸のまわりの回転自由度、他方が前記遠位側道具の自らの軸に対して実質的に共線をなす軸のまわりの回転自由度、を有するようになっている遠位側道具(9)と
を含み、
前記ユニバーサルジョイントを形成する手段はフレキシブル駆動スリーブ(104;206)を含むことを特徴とする手術器具(1)。 - 前記フレキシブル駆動スリーブはベローズを含むことを特徴とする請求項1に記載の手術器具。
- 前記フレキシブル駆動スリーブは長手軸に従った捩り応力に対して剛直であると共に、長手軸に対して垂直なあらゆる軸に従った捩り応力に対しては可撓性で弾性変形が可能であることを特徴とする請求項1または2に記載の手術器具。
- 前記ピボットジョイントは、直列に取り付けられた複数のリンク要素(101,102,105;202,203,204)を有することを特徴とする請求項1〜3のいずれか一項に記載の手術器具。
- 2つの隣接するリンク要素は有限ピボットジョイント(110;210)に従って互いに取り付けられることを特徴とする請求項1〜4のいずれか一項に記載の手術器具。
- 前記各リンク要素は、隣接するリンク要素の方へと延在する2つのラグ部を含む中空筒形要素であり、前記ラグ部は互いに対向して前記有限ピボットジョイント(110;210)を形成することを特徴とする請求項5に記載の手術器具。
- 前記細長アーム(103)は中空な筒形の要素であり、少なくとも部分的に前記駆動シャフト(111)を包囲することを特徴とする請求項1〜6のいずれか一項に記載の手術器具。
- 前記駆動シャフト(207)は中空であり、少なくとも部分的に前記細長アーム(3)を包囲することを特徴とする請求項1〜6のいずれか一項に記載の手術器具。
- 前記細長アーム(3)は長手軸に沿った貫通穴を含み、この貫通穴を介して1本以上のケーブルが前記細長アーム内において延在することを特徴とする請求項8に記載の手術器具。
- 長手軸に沿って延在する遠位側道具の作動ケーブルを含み、この作動ケーブルがシースにより取り囲まれることを特徴とする請求項1〜9のいずれか一項に記載の手術器具。
