Description
本発明のいくつかの実施形態の一側面は、少なくともノードを備える、女性の尿失禁の治療のためのデバイスに関する。任意選択的に、ノードの長さが、デバイス全体の長さの30%に過ぎないか、それより少ない。本発明のいくつかの実施形態においては、デバイスは、アンカー及びサポートをさらに備える。本発明のいくつかの実施形態においては、ノード構造は、機能改善のためのデバイスのより大きな柔軟性制御を可能とする。任意選択的に、柔軟なネック部分が、デバイスのノードに配置される。柔軟性、特に、中央の中核ボディでの柔軟性は、装着者の快適性を増すことができる。本発明のいくつかの実施形態においては、使い捨ての、女性の尿失禁の治療のためのデバイスが提供される。任意選択的に、デバイスは、単一のボディ構造からなっている。任意選択的に、デバイスは、一つの材料から構築されている。任意選択的に、デバイスは、種々のサイズに製造される。例えば、本発明のいくつかの実施形態において、全長が30mmから50mmの間のデバイスが提供される。任意選択的に、デバイスは、個々のユーザの必要に依存して、より大きい、あるいは、より小さい。本発明のいくつかの実施形態においては、デバイスがアプリケータを用いて挿入される。
本発明は、尿失禁女性の治療のためのデバイス及びデバイスの変形を提供する。本発明のデバイスは使い捨て可能で、短期間で使い切って捨てられ、(必要であれば)新しいデバイスに取り替えられるのに適している。代替的に、このデバイスは使用の合間に消毒することにより、リサイクルして使用される。本発明のデバイスは簡単で使い易く、任意選択的に生理中に膣にタンポンを挿入するのと変わらない、使い勝手が良く、慣れた方法で、楽々と挿入される。先行技術の大きく煩わしいデバイスとは対照的に、本発明のデバイスは快適で、一度挿入すると、再び取り出すまで女性はそのことを考える要がない。不随意の排尿が起こる場合、それはしばしば膀胱内の圧力の上昇のために発生するが、それを補償する膀胱頚部又は尿道からの反対圧力は全く無い。このことは、一般に膀胱頚部と尿道の腹腔内圧系から離れた低い位置への異常な下垂の結果である。これは、「運動機能亢進」として知られ、本来なら尿道と膀胱頚部とを恥骨の裏側に沿って高い位置に保持する支持機構が相当の損傷を受けた結果である。膀胱頚部と尿道の下降は、例えば、女性が咳やくしゃみをしたり、あるいは笑ったりするときに、不随意な尿の漏出を起こす。
図1Aを参照すると、失禁デバイス100の例示的な実施形態の側面図が示されている。説明の便宜のため、デバイス100は中心軸150が中心に配置され、3つの部分に分けられている。膣内でデバイスを安定させるために、「アンカー」要素として機能する上部106が与えられる。アンカーには2つのタイプがある。膣の中心軸方向に機能する軸アンカーと、左右すなわち膣の中心軸と実質的に垂直な方向に機能する放射アンカーである。尿道支持をもたらすために、「支持」要素の役割を果たす下部110が与えられる。本発明のいくつかの実施形態において、尿道中央部の位置に支持がもたらされる。本発明のいくつかの実施形態において、底部支持部110は、アンカーデバイス100を適所に配置させるための少なくとも一つのアンカーの形状を提供する。本発明のいくつかの実施形態において、デバイス100の全長は30mmと50mmの間である。任意選択的に、患者の個々の必要に応じて、デバイス100はそれよりも大きいか小さい。
次に図2Aを参照すると、女性尿失禁と闘うためのデバイス200の追加の実施形態が図示されている。この実施形態は、アンカー要素206、支持要素210、及びノード208から構成される。本発明のこの実施形態において、アンカー要素は、ノード208に実質的に垂直に放射状に広がるアーム212を備える。このアンカー要素アーム構成は、どちらの方向への移動にも抵抗するが、失禁デバイスの膣口に向かう不慮の移動を防ぐために特に有用である。図1Aに見られるように、支持要素アーム214はデバイス200の中心軸に向けて曲がるような形になっていて、アームの先端208はデバイスの中心軸から外側へ離れて実質的に展開することはない。このことは、アーム214の先端222がデバイス200の中心軸から大きく離れて展開した図2Dに示される実施形態220と対照的である。加えて、内側に向くアーム先端は、これらの形状は膣壁内に突出しない先端を提供するため、装着者の快適さを高め、本発明のいくつかの実施形態においてデバイス200の除去を容易にする。この実施形態は図1Aのデバイス100と同様に機能する。
失禁であれば、全ての女性、そして全ての活動を、全く同じデバイスで対処できるというわけではない。例えば、図3A〜Dは、激しい身体活動中に尿道を直接圧迫するのに適したデバイス300を図解する。任意選択的に、この実施形態は、ここに記載される他の実施形態では効果が無い、比較的に重症の失禁をもつ女性の治療のために使用される。本発明の例示的な実施形態において、このデバイス300は、アンカー要素、支持要素、及びノードを備える。任意選択的に、デバイス300は、アンカー要素アーム312及び支持要素アーム314を備える。
すでに述べた通り、全ての女性が同じデバイスで治療できるわけではない。同じ基本的な解剖学的特徴は一般に全ての女性に与えられているが、これら特徴のサイズや、その相互の関係は若干異なることがある。このため、「1サイズで全ての女性に適合する」デバイスの種類を提供することは困難であろう。図1に示されるデバイスは、多様な形状に適合できる柔軟なデバイスを提供することによって、この問題に対処する。一部の女性は図1のデバイスを使用することができない可能性があるが、その特徴は供給すべきサイズの数を確実に削減する。この問題を解決するための一つの任意選択の方法が、図4A〜Cに図解されている。図解されているデバイス400は、任意選択的に他の実施形態のいくつかと同じ基本要素(すなわち、アンカー要素406、支持要素410、及びノード408)から構成されている。しかしながら、デバイス400の可変形状を可能にするいくつかの変更がある。図4Aから分かるように、患者の個々の必要に応じて、支持要素アーム414の形状が調整可能なデバイス400が提供されている。
図6A〜Cも、形状が拡張挿入体800の位置に基づいて可変である、本発明600の例示的な実施形態を図解する。可変運動の操作は図4A〜Cに示される実施形態と本質的には同じである。しかしながら、アンカー要素アーム612は若干構成が異なることに留意されたい。このタイプのアーム構成(つまり、デバイス600の中心軸に実質的に垂直に展開する)は、図2A〜Dに示されるアンカー要素アーム212に類似した機能を果たす。図面に示される実施形態はほんの一例であり、ここに記載され又は示唆される可変形状失禁デバイスは何れも、デバイスのサイズ、アームの数、アームの構成、及び/又はアームの形状に関わらず、拡張挿入体と一緒に使用することができると理解されるべきである。
図9Aは、カバー900とともに使用される失禁デバイスを示す。本発明の例示的な実施形態において、カバー900は、柔軟で滑らかなメッシュ素材で作られている。任意選択的に、カバー900はデバイス904(図9に示される内部支持構造体として機能する)をカプセル化する小さな袋としてデザインされている。カバー900の使用は、デバイスを使用する上での一つ以上の利点を潜在的に提供する。例えば、カバーは挿入時のアプリケータとデバイスとの摩擦を低減する。加えて、カバーは、挿入中の膣とデバイスとの摩擦を低減する。本発明のいくつかの実施形態において、カバー900のメッシュ(デバイスのアームの間で伸ばされている)は、尿道用のスリング状支持としての機能を果たす。ストレスの多い出来事(咳やくしゃみ等の最中に腹圧が上昇するとき)の最中に尿を漏らす女性では、尿道は弛み下がっているが、その中央部でカバー900に接触している。このことは尿道内圧の上昇を引き起こし、その結果として排尿の自制を実現する。本発明の例示的な実施形態において、上述のように、デバイスは尿道又は膀胱頚部に圧力をかけずに、腹圧上昇があるときに支持を提供するだけである。任意選択的に、デバイスは尿道及び/又は膀胱頚部に直接圧力を加える。
次に図10Aを参照すると、膣内へのデバイスの挿入に役立つアプリケータが図示されている。挿入は、通常の生理用タンポンを挿入するのと同様に、このアプリケータを使用して達成される。失禁デバイスは、膣内部に挿入される遠位端1002の内部に保持される。近位端1004を押し込むときに、デバイスは出口1006を通って押し込まれ、一旦アプリケータが膣から取り除かれると直ぐに機能することを可能にする。本発明の例示的な実施形態において、出口1006は近位端1004が押し込まれて、失禁デバイスがアプリケータの外へ押し出されるまで閉じたままであることに注意されたい。任意選択的に、出口1006は花状である。糸902は目に見え、近位端1004の開口1008の外に突出している。アプリケータ1000の断面図を図11に示す。デバイスが未だアプリケータ1000の内部にあるときには、その柔軟なアーム1112及び1114は中心軸の方に集まり、展開した構成よりも非常に小さな輪郭を与えて、小さな径のアプリケータを介した挿入を可能にする。
失禁デバイスの除去は、任意選択的に、本発明の例示的な実施形態により、デバイス排除器1302によって支援される。図13Aは、膣から取り出されているデバイス1300を示す。デバイス除去の基本的なプロセスを上で説明した。しかしながら、糸1302に加わる下向きの力が、ここで説明するようにカバーに膣内でデバイスの輪郭を縮小させ、より容易な除去を可能にする点に留意することが重要である。カバーを使わない、しかし同じ効果を利用する、任意選択のデバイスの除去を、図13C及びDと併せて後述する。
本発明の例示的な実施形態において、膣から容易に除去されるのに適した失禁治療用デバイス1380が提供される。図13Cに示されるものは、一般に、アンカー部1386、支持部1390及びノード1388から構成される失禁デバイスである。糸1389などのデバイス排除器は、デバイスの支持要素アーム1394に直接取り付けられる。図13Dから、下向きの力(すなわち、膣口に向く力)が加えられ、支持要素アーム1394はデバイス1380の中心軸の方に折り畳まれることが分かる。任意選択的に、下向きの除去力からのアームへのダメージを防ぐために、アーム1394と糸との連結は強化されている。付加的な力が加えられてアームが折り畳まれることによりデバイスの直径が小さくなると、デバイス1380を膣の外へ引き抜くことが容易になる。加えて、デバイス排除器の使用は、非侵襲的かつ衛生的な方法で、気分を害することなく、女性がデバイス1380を除去することを可能にする。この技術は、剛直又は柔軟な支持要素アーム1394と共に利用できることに留意されたい。例えば、剛直なアームの場合、ノード1388が柔軟な材料で構成されている限り、アーム1394に加わる力は、結果としてノード1388が剛直なアームに加わるストレスを吸収し、アームが中心軸に向けて折り畳まれることを可能にする。本発明のいくつかの実施形態において、アームの先端はデバイスの中心軸から外側に向いておらず、デバイスと膣壁との摩擦が小さいために、より容易な除去を可能にする。
