Abstract
【解決手段】急性の虚血性または出血性脳卒中に罹患した患者における細菌感染の診断及び治療ガイダンスのためのインビトロ(in vitro)での方法であって、当該方法は、患者由来の体液のサンプル中のプロカルシトニン(PCT)または少なくとも12のアミノ酸残基を有するこのフラグメントのレベル、及び患者における細菌感染の診断との測定されたレベルの相関の測定を含んでなる方法。

Description
我々の社会では、脳卒中の発生率は年齢の増加とともに顕著に増加する(Modan 及び Wagener, 1992. Stroke 23:1230-36)。大抵の虚血性脳卒中は年齢71〜80歳で生じ、一方大抵の出血性脳卒中は60〜70歳で見られる(Colombo 等, 1989. Rivista de Neurologia 59: 1-7)。
脳卒中は、白血球(WBC)数の増加(Kazmierski 等. 2001. Wiad Lek 54:143-51)並びに中核体温の上昇(Boysen 及び Christensen 2001. Stroke 32:413-7)、脳温度(Schwab 等. 1997. Neurology 48:762-7)及びC反応性タンパク(CRP)の増加(Idicula 等. 2009. BMC Neurology 9:18)を有する炎症反応を末梢血で生じることがよく知られる。しかしながら、脳卒中後の全身性の炎症反応は感染よりもむしろ壊死組織それ自体に対する応答に起因するかもしれない。壊死組織は、炎症反応のあらゆる部分における細胞性、体液性、及び代謝性メカニズムによって除去される(Kogure 等. 1996. Acta Neurochir Suppl (Wien) 66:40-3)。体温の上昇は、発症の24時間以内に開始し、一方感染に起因する熱は遅れて生じるようである(Boysen 及び Christensen 2001. Stroke 32:413-7; Davalos 等. 1997. Cerebrovasc Dis 7:64-9; Castillo 等. 1999. Cerebrovasc Dis 9:22-7; Castillo 等. 1998. Stroke 29:2455-60)。感染の徴候のない虚血性脳卒中患者において、体温及びWBC並びにCRPの上昇及び増加は、病変体積及び初期脳卒中重症度と顕著に相関があった(Audebert 等. 2004. Stroke 35: 2128-2133)。さらに、全身性の炎症反応は、成功的な血栓溶解療法によって減弱されることが示された。したがって、大脳の壊死を回避することによって脳組織の救出が誘発されるのは、全身性の炎症が減弱される結果かもしれない。
PCTは抗生物質の治療ガイダンスに既に使用されてきたが、急性脳卒中後においては除かれる。救急科において下気道感染の症状を表わす患者では、PCTが測定され、そしてPCT濃度>0.25ng/mLまたは>0.5ng/mLを有する患者だけが抗生物質で治療を受けた(Christ-Crain 等. 2004. Lancet 363:600-7)。市中感染の肺炎(CAP)に罹患した患者では、抗生物質治療は、血清PCT濃度(PCT濃度<0.1ng/mLで強く非推奨;PCT濃度<0.25ng/mLで非推奨;PCT濃度>0.25ng/mLで推奨,及びPCT濃度>0.5ng/mLで強く推奨)に基づいて実施された(Christ-Crain 等. 2006. Am J Resp Crit Care Med 174:84-93)。PCTのガイダンスによって、患者の予後を悪化させることなく、実質的にCAPでの抗生物質の使用が減少される。同様に、上述の同一の決定閾値を使用したPCTガイドされた治療はまた、患者の予後を損なうことなく、急性の気道感染のための抗生物質のプライマリケアでの使用を顕著に減少した(Briel 等. 2008. Arch Intern Med 168:2000-7)。
急性脳卒中に罹患した患者での血清PCTレベルの調査からは、入院日〜7日目の間の有意差は明らかにならなかった(Miyakis 等. 2004. Clin Chim Acta 350: 437-9)。この実験では、PCTレベルの死亡率または神経学的予後との相関は見られなかった。モルナー(Molnar)及び同僚は、急性の虚血性脳卒中及びTIAを有する患者の血清でPCTを測定した(Molnar 等. 2008. J Clin Pathol 61: 1209-13)。PCTを連続的に測定したにも関わらず、72時間後のPCTのわずかな増加から脳卒中後の感染が続くことが示された以外は、著者は脳卒中後の異なる時点で測定したPCTレベルにおいていずれの相違も発見できなかった。両研究の結果は、使用したPCTテストシステム各々、すなわちマイヤキス(Miyakis)等の記載する機能分析感度(FAS)0.08 ng/mLでのPCT LIA、及びモルナー(Molnar)等の記載するFAS0.06ng/mLでのPCT Kryptorが原因かもしれない。
ヴォゲルゲサン(Vogelgesang)等は、患者の病院への入院後7日及び14日目に、>0.5ng/mLのPCTのカットオフを使用して、感染の3つの基準の中の1つとして脳卒中患者の血清中でPCT値を測定した(Vogelgesang 等. 2008. Stroke 39: 237-41)。
PCTのレベルを測定することによって脳卒中または一過性脳虚血発作に罹患した患者の予後またはリスクを評価するための方法が、EP 08167512.6及びEP08168671.9に記載される。
PCTレベルを測定すること、及び未だ顕在化していない及び/又は未だ症状のないさらなる疾患或いは内科疾患に罹患する患者のリスクとこのPCTレベルとを関連付けることによる、初期の非伝染性疾患に罹患した患者のための予後診断の方法がEP07015271.5に記載される。
従って、本発明の発明者は、急性の虚血性または出血性脳卒中或いは一過性脳虚血発作に罹患した患者由来の体液サンプル中のプロカルシトニンのレベルの測定を、脳卒中後/TIA後のこれらの患者における細菌感染の診断及び治療ガイダンスに使用できるかいなかを調査した。
(i)急性脳卒中または一過性脳虚血発作に罹患した患者からのサンプルを提供すること;
(ii)0.06ng/mL未満、より好適には0.04ng/mL以下、さらに好適には0.03ng/mL以下、最も好適には0.01ng/mL以下の機能分析感度でのPCT検出アッセイを使用して、上記サンプルで、プロカルシトニン(PCT)或いは少なくとも12アミノ酸長、より好適には50以上のアミノ酸長、さらに好適には110以上のアミノ酸長であるこのフラグメントのレベルを測定すること;並びに
(iii)上記の測定されたPCTレベルを所定の閾値レベルと比較することによって、上記患者が細菌に感染しているか否かを決定すること、を含んでなる。
PCTレベルの検出が高感度なため、本発明は急性脳卒中または一過性脳虚血発作に罹患した患者において細菌感染症の早期の検出、並びに続く当該細菌感染症の早期の治療を可能にし、このため患者の予後の改善を可能にする。従って、当該方法は、医療機関、例えば病院で集中治療中の患者からのサンプルに適用するのに特に適当である。少量の予後の改善でさえ主要な公衆衛生上の影響を有する可能性があるので、良好な予後において、細菌感染の早期の同定及び早期の抗生物質治療に起因する5%の絶対的(absolute)な改善でさえ価値がある、ということを留意すべきである(Lees 等. 2003. Lancet Neurology 2: 54-61)。従って、必要としている患者の早期治療は非常に重要である。
本発明は、急性脳卒中または一過性脳虚血発作(TIA)に罹患した患者における細菌感染を診断または同定するための方法、特にインビトロ(in vitro)での方法に関する。当該方法は、次のステップ:
(i)急性脳卒中または一過性脳虚血発作に罹患した患者からのサンプルを提供すること;
(ii)0.06ng/mL未満、好適には0.04ng/mL以下、より好適には0.03ng/mL以下、最も好適には0.01ng/mL以下の機能分析感度でのPCT検出アッセイを使用して、上記サンプルで、プロカルシトニン(PCT)或いは少なくとも12アミノ酸長、より好適には50以上のアミノ酸長、さらに好適には110以上のアミノ酸長であるこのフラグメントのレベルを測定すること;並びに
(iii)上記の測定されたPCTレベルを所定の閾値レベルと比較することによって、上記患者が細菌に感染しているか否かを決定すること、を含んでなる。
従って、7日、好適には12週、より好適には6月、さらに好適には1年までモニターすることが推奨される。
本発明の文脈で用語「レベル」は、対象から採取されたサンプル中のPCT(またはフラグメント / 前駆物質)の濃度(好適には、重量 / 容積 ; w/vとして表わされる)に関する。
ここで使用するように用語「患者」は、疾患が原因で医療を受けているまたは医療を受けるべき生存しているヒトまたはヒト以外の生物に言及する。これには、確定されていない疾患に罹患する人間であって症状の表れのため調査されている人間が含まれる。従って、ここに記載される当該方法及びアッセイは、ヒト及び動物の疾患いずれにおいても適用可能である。
ここで使用される用語「脳卒中後の感染」は、急性脳卒中またはTIAの症状の発生後生じている細菌感染に言及する。感染は、病院の入院後48時間以降或いは病院または健康管理サービスユニットで治療を受けて退院後30日以内に最初に生じた場合に、院内または医療関連のものとみなす。用語「医療関連の感染」(HAI)は、ホーラン(Horan)等によって確定されている(Horan 等. 2008. Am J Infect Control 36: 309-32)。
(i)急性脳卒中または一過性脳虚血発作に罹患した患者からのサンプルを提供するステップであって、当該サンプルが急性脳卒中または一過性脳虚血発作の症状の発生後72時間以内に当該患者から採取されたものであるステップ;
(ii)0.06ng/mL未満の機能分析感度でのPCT検出アッセイを使用して、上記サンプル中のプロカルシトニン(PCT)または少なくとも12アミノ酸長であるこのフラグメントのレベルを測定するステップ;並びに
(iii)上記の測定されたPCTレベルを所定の閾値レベルと比較することによって、上記患者が細菌に感染しているか否かを決定するステップ、を含んでなる方法である。
従って、本発明の好適な実施形態において、サンプルは、血液サンプル、血清サンプル、血漿サンプル、脳脊髄液サンプル、唾液サンプル及び尿サンプル或いは任意の前述のサンプルの抽出物から成る群から選択される。好適には、サンプルは、血液サンプル、最も好適には血清サンプルまたは血漿サンプルである。
ここで使用される用語急性脳卒中は、虚血性及び出血性の脳卒中いずれも言及する。
本発明の文脈においてプロカルシトニンは、好適にはアミノ酸残基1〜116、2〜116、または3〜116或いはこれらのフラグメンに及ぶペプチドに関する。従って、プロカルシトニンフラグメントは、少なくとも12アミノ酸長、好適には50以上のアミノ酸長、より好適には110以上のアミノ酸長である。PCTは、糖鎖付加、脂質過酸化(liposidation)または誘導体化等の翻訳後の修飾を含んでよい。PCTそのものは、カルシトニン及びカタカルシンの前駆物質である。PCT 1−116のアミノ酸配列は、配列番号1に示す。
PCT若しくはフラグメントまたは前駆物質或いはこのフラグメントのレベルの測定は、ここでは検出方法及び/又は診断アッセイを使用して実施される。
当該アッセイは、同種または異種のアッセイ、競合的及び非競合的アッセイとすることができる。特に好適な実施形態では、アッセイは、非競合的イムノアッセイであるサンドイッチアッセイの形態であり、検出及び/又は定量化するための分子は一次抗体及び二次抗体へ結合される。一次抗体は、固相、例えばビーズ、ウェルまたは他の容器の表面、チップまたはストリップへ結合されてよく、そして二次抗体は、例えば染料、放射性同位元素、または反応性のあるまたは触媒的に活性のあるモイエティで標識された抗体である。次に分析物へ結合される標識抗体の量は、適当な方法で測定される。「サンドイッチアッセイ」と関係のある一般的な組成物及び手順は、確立され当業者に知られている(参照により取り込まれた、The Immunoassay Handbook, Ed. David Wild, Elsevier LTD, Oxford; 3rd ed. (May 2005), ISBN-13: 978-0080445267; Hultschig C 等., Curr Opin Chem Biol. 2006 Feb;10(1):4-10. PMID: 16376134)。
さらに好適には、当該標識システムは、蛍光染料または化学発光染料、特にシアニン型染料と組合せて、希土類クリプテートまたは希土類キレートを含んでなる。
本発明の方法で診断される細菌感染症は、当業者に知られる適当な抗生物質で治療される。抗生物質の可能な種類は、ペニシリン(例えば、フルクロキサシリン、アモキシシリン、アンピシリン、メズロシリン)、セファロスポリン(例えばセファゾリン、セフロキシム、セフォタキシム、セファクロル、セファレキシン)、β−ラクタマーゼ阻害剤(例えば、スルバクタム、タゾバクタム)、テトラサイクリン(例えば、ドキシサイクリン、ミノサイクリン、テトラサイクリン、オキシテトラサイクリン)、アミノグリコシド(例えば、ゲンタマイシン、ネオマイシン、ストレプトマイシン)、マクロライド系抗生物質(例えば、アジスロマイシン、クラリスロマイシン、エリスロマイシン、ロキシスロマイシン、スピラマイシン、クリンダマイシン)、リンコサミド(例えば、リンコマイシン)、ギラーゼ阻害剤(例えば、シプロフロキサシン、オフロキサシン、ノルフロキサシン)、スルホンアミド、トリメトプリム、グリコペプチド系抗生物質(例えば、バンコマイシン)、ポリペプチド抗生物質(例えば、コリスチン、ポリミキシン)、並びにアンフェニコール(例えば、クロラムフェニコール)、から成る群から選択される。
本発明の方法はさらに、患者が細菌感染の臨床症状を示す前に、抗生物質を患者に投与するステップを含んでよい。
さらに、無症候性の細菌感染症を診断または同定するためにマーカーを測定してよい。これらのマーカーは、:C−反応性タンパク質(CRP)、ネオプテリン、白血球(WBC)の数、体温、好中球、赤血球沈降速度(ESR)から成る群から選択してよい。
さらなる態様では、本発明は、急性脳卒中または一過性脳虚血発作に罹患した患者の抗生物質での予防的治療のための記載された方法の使用に言及する。
さらに、本発明は、急性の虚血性または出血性脳卒中或いはTIAに罹患した患者における脳卒中後の細菌感染症の診断のために、PCTまたはこのフラグメント或いはPCT前駆物質またはこのフラグメントに対する1または複数の抗体を含んでなるキットの使用に関する。
研究はバーゼルの大学病院の救急並びに神経系及び脳神経外科クリニックで実施した。本研究には、直前3日間内に症状の発生のあり、世界保健機関の基準に従って虚血性または出血性の脳卒中或いは一過性脳虚血発作(TIA)で救急科へ入院した全継続患者が含まれた。インフォームドコンセントのない患者は排除した。
患者のベースラインのデータ収集には以下のものが含まれた:
a)年齢、
b)性別、
c)肥満度指数(BMI)、
d)病歴項目:入院に先行した実際の病歴;一過性脳虚血発作を有する患者でのABCDスコア(Rothwell 等. 2005. Lancet 366: 29-36);家族歴;チャールソンインデックスによってもまた評価した関連のある合併症(Goldstein 等. 2004. Stroke 35: 1941-5)(例えば高血圧、以前の脳卒中、以前のTIA、虚血性の心疾患、心房細動、糖尿病、腎臓及び肝臓機能不全、うっ血性心不全、脂質異常症;低ナトリウム血症のリスクを有する合併症(重度の甲状腺機能低下症、グルココルチコイド不全、腫瘍、HIV感染);喫煙歴(煙草の箱年数)及び状況(一日の箱数);現在の投薬;飲酒量(一日のグラス及びグラム);症状の発生から入院までの時間。
f)臨床項目:精神学的状態を含む健康診断、NIHSS(脳卒中の重症度を評価するため)及びグラスゴー昏睡尺度(GCS; Adams 等. 1999. Neurology 53: 126-31)、血圧、脈拍数、体重、ボリューム状態(皮膚弾力、頸部静脈の膨張(distension)、聴診、もし利用可能であれば水分摂取量及び体液喪失量のフローシート、を含む)、体温;日常の臨床マネージメント内で実施されるなら、脳神経外科の患者の脳内圧。
g)入院時に、及びナトリウム不均衡の場合には、神経疾患患者で低ナトリウム血症の臨床症状を評価した。頭蓋内手術を受けている患者では、臨床症状を毎日診断するつもりである。特に、頭痛、食欲不振、吐き気、嘔吐、筋けいれん及び痛み、発作、混乱、無欲または嗜眠の発生の存在をモニターした。
h)日常的/標準的な臨床検査:以下を含む日常的な血液サンプリング:ヘマトクリット、血中尿素窒素、炭酸水素、総タンパク質、アルブミン、尿酸血清及び尿中電解質、尿及び血清浸透圧、クレアチニン、脂質、TSH、fT4、T3、並びに基礎コルチゾール。可能であればいずれの食物摂取、または喫煙の前にも全血サンプリングを行った。また、影響因子をモニターした。
j)さらなる調査:脳卒中患者は超音波検査、心エコー検査、標準12誘導心電図検査及び24時間心電図検査を受け、そして患者を脳卒中の原因によって急性脳卒中治療におけるORG 10172の試行(Trial of Org 10172 in acute Treatment)(TOAST)に従って脳卒中亜型分類に分類したが、この分類は大動脈アテローム硬化、心塞栓、小動脈閉塞、他の病因、及び未確定の病因の間で異なる。
研究はバーゼルの倫理委員会によって承認された(Ethikkommission beider Basel)。これは、診査的及び観察的な研究であった;研究に関連する唯一の処置は、日常実施される血液サンプリングの間に得られた7.5mlの血漿であった。従って、患者は、科学的目的のための自分達のデータの使用に同意する、という書面によるインフォームドコンセントを準備した。急性のCNS病変の続発症によって「インフォームドコンセント」が実行可能でなかった患者では(後者は研究に含めるための必要条件)、患者の最近親者が患者の意志を推定的に述べるために同意フォームにサインした。万が一、最近親者とすぐに会うことができない場合には、治療を行っているが当該研究に関係していない医師が、自らの見地からこの研究に含めることに関して反対はなかったと認定した。これらのインフォームドコンセント手続きの後にのみ、患者を研究に含めた。
ステップ1.救急科または神経性病棟の適格な全患者を当該研究に加えた。
ステップ2.全基本データを収集した。
ステップ3.入院の間、退院までの体重、血圧、脈拍数、ボリューム状態及び体温を含む臨床項目をカルテ審査によって評価した。
・液体治療及び薬剤
・低ナトリウム血症の潜在的症状、すなわち頭痛、吐き気、嘔吐、筋けいれん及び痛み、食欲不振、意識障害、発作。
・血液サンプリングが病棟で通常行われる時点に、日常的に実施される臨床検査(ケモグラム(chemogramm)、血漿グルコース、血清浸透圧、尿浸透圧、尿中ナトリウム、ヘマトクリット)をサンプリングした。
ステップ4.全患者で、入院の5日目に、NIHSS、バーセルインデックス及びランキングスケールでの臨床検査を実施した(Collin 等. 1988. International Disability Study 10: 61-3; Bonita 及び Beaglehole. 1988. Stroke 19: 1497-1500)。
今後の居住地(すなわち依存した生活対自立した生活)を評価した。
ステップ5.虚血性脳卒中患者において3月後に、(バーセルインデックス及びランキングスケールによって評価されるように)病的状態及び死亡率に関して電話により経過観察を得た。好ましくない予後がバーセルインデックス<85または3〜6の修正ランキングスケールとして確定された。
モーゲンターラー等によって記載されるように機能分析感度0.007ng/mLを有する超高感度市販の試験システムを使用してPCTを測定した(Morgenthaler 等. 2002. Clin Chem 48:788-790)。簡潔に言えば、ヒツジ抗体をPCTのカルシトニンモイエティに対して産生させ、そしてマウスモノクローナル抗体をPCTのカタカルシンモイエティに対して産生させた。チューブを抗カタカルシン抗体でコーティングした。抗カルシトニン抗体をMACN アクリジニウムエステル(InVent GmbH, Hennigsdorf, Germany)で標識し、トレーサーとして供給した。標準的なウマ血清中での組み換え型PCTの希釈物を標準物質として使用した。100μLのサンプルまたは標準物質をコーティングしたチューブで30分間培養し、200μLのトレーサーを添加した。さらなる2時間の培養後、チューブを1mLのLIA洗浄液で4回洗浄し(BRAHMS AG, Hennigsdorf, Germany)、そしてLB952T ルミノメーターを使用して結合された化学発光を測定した(Berthold, Wildbad, Germany)。
(虚血性または出血性の)脳卒中或いは一過性脳虚血発作に罹患していると診断された患者の血液サンプル中で、プロカルシトニンのレベルを測定した。最初のアプローチでは、急性脳卒中またはTIAの症状の発生後最初の12時間以内に病院に入院した時に患者を分析に加えた(n=376)。入院後1日目(24〜36時)、3日目(72〜84時)及び5日目(120〜132時)に、(病院への入院時の)ベースラインでプロカルシトニンの濃度を測定した。この376人の患者のうち24人は脳卒中/TIA発生後に細菌感染症(例えば、肺炎、尿路感染)に感染した。細菌に感染している及びしていない(急性脳卒中及びTIA症状の発生後12時間以内に病院へ入院した)患者の経時的なPCT濃度を図1に示す。ロジスティック回帰分析により、PCT濃度の増加及び細菌感染の有無の間の相関が明らかとなった(入院時 (0-12h) p=0.12;1日目(24-36h) p=0.067 ; 3日目(72-84h) p=0.0042 及び5日目(120-132h) p=0.011 )。対応する感度及び特異度を測定するために、異なるカットオフ値(cut-off value)を使用した(表1)。PCTのカットオフ値の0.06ng/mLにおける、急性脳卒中/TIA症状の発生後12時間以内に入院した患者の入院時の感度及び特異度は、それぞれ9.1及び94.1%であった。このカットオフ値を2分の1の0.03ng/mLとすると、感度は2倍以上(22.7%)となり、特異度では中程度の減少(85.7%)となった。症状発生後24〜36時間内にPCTを測定した時、感度及び特異度はPCTのカットオフ値が0.06ng/mLの場合には各々28.2及び94.6%であり、一方PCTのカットオフ値が0.03ng/mLの場合には感度は33.3%まで上昇し、特異度は82.9%であった。

Claims (14)
- 急性脳卒中または一過性脳虚血発作に罹患した患者における細菌感染を診断するための方法であって、
(i)急性脳卒中または一過性脳虚血発作に罹患した患者からのサンプルを提供するステップであって、当該サンプルが急性脳卒中または一過性脳虚血発作の症状の発生後72時間以内に当該患者から採取されたものであるステップ;
(ii)0.06ng/mL未満の機能分析感度でのPCT検出アッセイを使用して、上記サンプル中のプロカルシトニン(PCT)または少なくとも12アミノ酸長であるこのフラグメントのレベルを測定するステップ;並びに
(iii)上記の測定されたPCTレベルを所定の閾値レベルと比較することによって、上記患者が細菌に感染しているか否かを決定するステップ、
を含んでなる、方法。 - 前記サンプルが、急性脳卒中または一過性脳虚血発作の症状の発生後48時間以内に、より好適には24時間以内に、最も好適には12時間以内に、前記患者から採取された、請求項1記載の方法。
- 前記サンプルが、体液、特に血液、血清、血漿、脳脊髄液、尿、唾液または胸水である、請求項1または2記載の方法。
- 前記急性脳卒中が、虚血性または出血性脳卒中である、請求項1〜3のいずれか1項に記載の方法。
- PCTの前駆物質のフラグメントのPCTのフラグメントが、少なくとも12アミノ酸残基長を有する、請求項1〜4のいずれか1項に記載の方法。
- 前記所定の閾値レベルが、0.02ng/mL〜0.5ng/mL、より好適には0.02ng/mL〜0.25ng/mL、さらに好適には0.02ng/mL〜0.1ng/mL、さらに好適には0.02ng/mL〜0.06ng/mL、最も好適には0.02ng/mL〜0.05ng/mL(未満)である、請求項1〜5のいずれか1項に記載の方法。
- 前記測定されたPCTレベルが前記所定の閾値レベル以上である場合に、前記患者は細菌に感染している、請求項1〜6のいずれか1項に記載の方法。
- 前記細菌感染が抗生物質で治療可能である、請求項1〜7のいずれか1項に記載の方法。
- 前記細菌感染が肺炎である、請求項1〜8のいずれか1項に記載の方法。
- 前記細菌感染が(細菌感染の臨床症状がない)無症候性である、請求項1〜9のいずれか1項に記載の方法。
- (iv)抗生物質を患者へ投与するステップをさらに含んでなる、請求項1〜10のいずれか1項に記載の方法。
- 少なくとも1または複数のマーカー、例えばC−反応性タンパク質(CRP)、ネオプテリン、白血球(WBC)の数、体温、好中球、赤血球沈降速度(ESR)等のレベルを測定するステップをさらに含んでなる、請求項1〜10のいずれか1項に記載の方法。
- 急性脳卒中または一過性脳虚血発作に罹患した患者への抗生物質の投与のための治療ガイダンスを提供するための、請求項1〜12のいずれか1項に記載の方法の使用。
- 急性脳卒中または一過性脳虚血発作に罹患した患者を抗生物質で治療するための、請求項1〜12のいずれか1項に記載の方法の使用。
