Abstract
Description
本願は、米国仮特許出願第60/970,619号(2007年9月7日出願)の優先権の利益を主張し、この出願は、その全体が本明細書に参考として援用される。
本発明は、肥満および他の病状を治療するための胃腸用装置に関する。より具体的には、本発明は、胃内容物の流動を断続的に遮断または低減するために患者の胃腸管の中に経管的に配置される、装置に関する。
Claims (63)
- 胃の開口部を断続的に遮断する装置であって、該装置は、
第一の閉塞部材とエプロン部材とを含む、近位部材と、
テザーによって該近位部材に接続される第二の閉塞部材を含む、遠位部材と
を備え、
該第一の閉塞部材は、細長くより狭い構成から、収縮したより広い構成へと形成可能であり、
該エプロン部材は、該第一の閉塞部材を包囲する近位開放端と、該テザーに接続される遠位閉鎖端とを有する、装置。 - 前記エプロン部材の前記近位端は、間隙によって前記第一の閉塞部材から離間している、請求項1に記載の装置。
- 前記第一の閉塞部材は、該第一の閉塞部材内に物質を注入することによって、前記細長くより狭い構成から、前記収縮したより広い構成へと形成可能である、請求項1に記載の装置。
- 前記細長くより狭い構成は、複数の巻きを有する螺旋構成であり、前記収縮したより広い構成は、該複数の巻きを一方を他方に隣接して入れ子にすることによって形成される、請求項1に記載の装置。
- 前記第一の閉塞部材は、前記細長くより狭い構成から、前記収縮したより広い構成へと可逆的に形成可能である、請求項4に記載の装置。
- 前記第一の閉塞部材は、該第一の閉塞部材の第一端における接続部材を該第一の閉塞部材の第二端と連結させることによって、前記細長くより狭い構成から前記収縮したより広い構成へと形成可能である、請求項4に記載の装置。
- 前記接続部材は、前記第二端における噛合空洞の中に該接続部材を係合することによって、該第二端に連結される、請求項6に記載の装置。
- 前記接続部材は、該接続部材に接続される紐を遠位に引くことによって、前記第二端に連結される、請求項6に記載の装置。
- 前記紐は、前記テザーを通って前記第一の閉塞部材から前記第二の閉塞部材まで延在する管腔内に、前記装置の長さに沿って延在し、該紐は、前記接続部材を通って輪になり、該紐は、該接続部材が前記第二端に係合した後に該装置から除去可能である、請求項8に記載の装置。
- 前記紐は、該紐の残りの部分の第二の色とは異なる第一の色を有する部分を備える、請求項8に記載の装置。
- 前記第一端は、前記第一の閉塞部材の残りの部分よりも高い機械的性質を有する該第一端に補強材料を含むことによって補強される、請求項6に記載の装置。
- 前記第一の閉塞部材は、前記第一端から前記接続部材を切断することによって、前記収縮したより広い構成から前記細長くより狭い構成へと可逆的である、請求項6に記載の装置。
- 前記装置は、弾力性プラスチック材料から製造される、請求項1に記載の装置。
- 前記エプロン部材は、前記第一の閉塞部材を包囲することから前記テザーの近位部分を包囲することへと可逆的である、請求項1に記載の装置。
- 前記エプロンは、近位の本質的に円筒形の部分と、前記近位部分を前記テザーに接続する遠位の本質的に円錐形の部分とを含む、請求項1に記載の装置。
- 前記近位部分と前記遠位部分とは、異なる壁厚を有する、請求項14に記載の装置。
- 前記第二の閉塞部材は、該第二の閉塞部材の残りの部分よりも重い重量を有する挿入物を含む、請求項1に記載の装置。
- 前記第一または第二の閉塞部材のうちの少なくとも1つは、球根形状を有する、請求項1に記載の装置。
- 前記第一の閉塞部材、前記テザー、および前記第二の閉塞部材を通って軸方向に延在する管腔をさらに備える、請求項1に記載の装置。
- 前記装置は、該装置から前記胃腸管の中へ溶出可能な化合物を含む、請求項1に記載の装置。
- 前記化合物は、前記第二の閉塞部材の中に設置される貯留部の中に収容される、請求項20に記載の装置。
- 前記化合物は、前記装置の外側表面上に設置される、請求項20に記載の装置。
- 前記治療化合物は、インスリンである、請求項20に記載の装置。
- 閉塞部材に連結され、前記第一の閉塞部材をその収縮構成で維持するように構成される、係止機構をさらに備える、請求項1に記載の装置。
- 前記第一の閉塞部材の前記収縮構成を確認および/検出するように構成されるセンサをさらに備える、請求項24に記載の装置。
- 前記センサは、音響、超音波、電気、電磁、光学(例えば、色、波長、周波数等の変化を検出する)、化学、および張力から成る群より選択されるモダリティを介して、前記収縮構成を検出するように構成される、請求項25に記載の装置。
- 疾患を治療する方法であって、該方法は、
患者の胃の中に装置を配置することであって、該装置は、第一の閉塞部材およびエプロン部材を含む近位部材と、テザーによって該近位部材に接続される第二の閉塞部材を含む遠位部材とを備える、ことを含み、
該エプロン部材は、該第一の閉塞部材を包囲する近位開放端と、該テザーに接続される遠位閉鎖端とを有し、
該胃の運動は、該エプロン部材を該胃の開口部に対して断続的に設置した状態にさせることによって、該装置に該胃の開口部を断続的に遮断させる、方法。 - 前記疾患は、肥満、過食症、または脂質異常症のうちの1つ以上である、請求項27に記載の方法。
- 前記疾患は、プラダー・ウィリー症候群、フレーリッヒ症候群、コーエン症候群、サミット症候群、アルストレム症候群、ベルエソン症候群、バルデー・ビードル症候群、または高リポタンパク血症I型、II型、III型、およびIV型のうちの1つ以上である、請求項27に記載の方法。
- 前記装置は、前記患者の胃の中に細長くより狭い構成で該装置を導入することによって、および該細長くより狭い構成から収縮したより広い構成へと前記第一の閉塞部材を形成することによって、該患者の胃の中に設置される、請求項27に記載の方法。
- 前記装置から前記患者の胃腸管の中へ化合物を拡散させるステップをさらに含む、請求項27に記載の方法。
- 前記化合物を拡散させることは、前記装置の中に収容される貯留部から該化合物を拡散させることを含む、請求項31に記載の方法。
- 前記化合物を拡散させることは、前記遠位部材に収容される貯留部から該化合物を拡散させることを含む、請求項32に記載の方法。
- 前記化合物は、前記装置の外側表面上に設置される、請求項31に記載の方法。
- 前記化合物は、インスリンである、請求項31に記載の方法。
- 前記エプロン部材内において、前記第一の閉塞部材を収縮したより広い構成へと再構成することをさらに含む、請求項27に記載の方法。

患者の胃の中に装置を配置することであって、該装置は、第一の閉塞部材およびエプロン部材を含む近位部材と、テザーによって該近位部材に接続される第二の閉塞部材を含む遠位部材とを備え、該エプロン部材は、該第一の閉塞部材を包囲する近位開放端と、該テザーに接続される遠位閉鎖端とを有し、該胃の運動は、該エプロン部材を胃の開口部に対して断続的に設置した状態にさせることによって、該装置に該胃の開口部を断続的に遮断させる、ことと、
該装置から該患者の胃腸管の中へインスリンを拡散させることと
を含む、方法。 - 所定の割合まで胃内容排出を遅くするように前記近位部材を構成することによって、グルコースピークを管理するステップをさらに含む、請求項40に記載の方法。
- 前記インスリンを拡散させることは、前記装置の中に含まれる貯留部から該インスリンを拡散させることを含む、請求項40に記載の方法。
- 前記インスリンを拡散させることは、前記遠位部材に含まれる貯留部から該インスリンを拡散させることを含む、請求項40に記載の方法。
- 前記装置から前記インスリンを拡散させることは、該装置の表面から該インスリンを溶出させることを含む、請求項40に記載の方法。
- 前記インスリンを拡散させることは、該インスリンを受動的に拡散させることを含む、請求項40に記載の方法。
- 前記インスリンを拡散させることは、インスリン拡散システムを作動させることによって該インスリンを拡散させることを含む、請求項40に記載の方法。
- 前記インスリン拡散システムは、ポンプである、請求項46に記載の方法。
- 前記インスリン拡散システムは、前記装置による所定の状態の検出に応じて作動させられる、請求項46に記載の方法。
- 前記所定の状態は、炭水化物レベル、脂肪レベル、および糖レベルのうちの1つ以上を備える、請求項48に記載の方法。
- 前記患者に、または臨床医に、フィードバック信号を提供するステップをさらに含む、請求項48に記載の方法。

該装置から該患者の胃腸管の中へインスリンを拡散させることと
を含む、方法。 - 所定の割合まで胃内容排出を遅くするように前記近位部材を構成することによって、患者体重を管理するステップをさらに含む、請求項52に記載の方法。
