R−CO−O1CAP (I)
本発明の目的に対し、次の用語は以下の意味を持つこととする。
代替の疼痛管理方式の調査は、N‐メチル‐D‐アスパラギン酸(NMDA)受容体およびそれらの拮抗薬に重点を置いたものであり、それらが動物およびヒトモデルにおいて体性感覚およびニューロパシックペイン感覚を軽減することが近年証明された(Plesan et al, 1998; Klepstad et al, 1990; Eisenberg et al, 1998; Kinnman et al, 1997; and Kawamatugs to a et al, 1998)。これら薬剤の臨床的有用性は、NMDA受容体に対する薬物の高親和性結合に由来し、末梢の侵害刺激によって疼痛が生じる接合部に位置し、次いでA*感覚線維およびC感覚線維を介して中枢受容体へ運搬される、NMDA受容体を結果として遮断する(Woolf et al, 1993)。臨床的見地からすると、効果的な疼痛管理に必要とされる従来の鎮痛薬の用量は、非常に少ないものだろう。これらの化合物の1つは、鎮咳薬として臨床的安全性の長い歴史を持つ、低親和性、非競合的NMDA受容体拮抗薬のデキストロメトルファン(DM)である(Bem et al, 1992)。
デキストロメトルファン(DM)およびレボルファノールは、当初、モルヒネに対する薬理学的代替物として40年以上前に合成された。DMはコデイン類似体(レボルファノール)のD異性体であるが、そのL異性体とは対照的に、オピエート受容体に対して無効である(Benson et al, 1953)。当初から、その臨床用途は、成人用量が1日3回乃至6回、10mg乃至30mgの、主としてシロップ剤の鎮咳薬であった。DMが鎮咳効果を発揮する特異的中枢部位は未だ不確定だが、その効果がナロキソンによって抑制されない限り、オピエートの部位とは異なる(Karlsson et al, 1988)。さらに、オピエートと違って、DMには確立された安全実績があり、すなわち、治療用の鎮咳用量(1mg・kg−1・日−1)では、オピエートのような呼吸器または血行動態への主な副作用はなく、またヒスタミン放出の合併症も誘発しない。NMDA受容体への拮抗薬の結合によって、受容体依存開閉Ca2+電流が調整される。Ca2+電流の変化は、通常、NMDAに誘導されたニューロン発射を招き、それが持続する場合、一次侵害刺激の増強(すなわち「ワインドアップ」現象)および二次感覚疼痛の誘発が後続する(Mendell 1966; Church et al, 1985)。他のNMDA受容体拮抗薬とは対照的に、DMは、オピエートおよび他の神経伝達物質と違って中枢神経系に広範囲の結合部位を有しており、したがってブタおよびラットにおいて見られたように、DMの活性はNMDA受容体のみに限定されていない(Musacchio 1988, Church 1991)。NMDA受容体依存性チャネルを介する細胞内のCa2+流入を低減するDMの能力に加えて、DMはまた、通常、高濃度の細胞外K+によって活性化される電位依存Ca2+チャネルを調節する。これら多チャネル調節能力の生理学的影響の一つには、ヒトに加えて、ラットおよび神経細胞培養においても実験的に証明されたように、通常は発作に転換される脳内でのNMDA仲介性ニューロン発射のDMによる弱化がある(Ferkany 1988, Choi 1987)。
本発明において利用され得るNMDA拮抗薬の非限定リストには、デキストロメトルファン、デキストロルファン、ケタミン、アマンタジン、メマンチン、エリプロディル、イフェンプロジル、フェンシクリジン、MK‐801、ジゾシルピン、CCPene、フルピルチン、またはそれらの誘導体、塩、代謝物質もしくは複合体が含まれる。
(+/−)‐トラマドールは、コデインの合成4‐フェニル‐ピペリジン類似体である。トラマドールはオピエート受容体に対する親和性が低い中枢性鎮痛薬である。μ受容体に対するトラマドールの選択性が近年実証され、肝臓でのO‐脱メチル化によって生じるトラマドールのM1代謝物質はオピエート受容体に対して、親薬物よりも高い親和性を示す。このM1誘導体(O‐デメチルトラマドール)の生成速度は、デブリソキン型の多形性イソ酵素(シトクロムP450 2D6(CYP2D6))の影響を受ける。一つのメカニズムが、μ‐オピエート受容体に対するトラマドールの弱親和性に関連している(モルヒネの6,000倍弱く、d‐プロポキシフェンの100倍弱く、コデインの10倍弱く、デキストロメトルファンと同等である)。さらに、他のオピエートとは対照的に、トラマドールの鎮痛作用は、オピエート拮抗薬ナロキソンによって部分的にのみ阻害され、このことは他の作用メカニズムの存在を示唆している。これは、ノルアドレナリン(ノルエピネフリン)およびセロトニン(5‐ヒドロキシトリプタミン;5‐HT)の再取込みを阻害する、モノアミン作動性活性の発見によって実証され、脊髄レベルで侵害受容性インパルスを遮断することによって鎮痛作用に有意に貢献した(Dayer et al, 1994&1997)。
トラマドールは、本発明によるトラマドール類似体に置き換えても、または本発明によるトラマドール類似体との組合せで使用してもよい。本発明において利用してもよいトラマドール類似体の非限定リストには、トラマドールN‐オキサイド誘導体(「トラマドールN‐オキサイド」)、トラマドールO‐デスメチル誘導体(「O‐デスメチルトラマドール」)、ベンラファキシン、(R/S)‐1‐[2‐(ジメチルアミノ)‐1‐(4‐メトキシフェニル)エチル]シクロヘキサノールおよびO‐デスメチルベンラファキシン、またはそれらの混合物、立体異性体、ラセミ化合物、代謝物質、塩、もしくは複合体が含まれる。
ガバペンチン(GBP;Neurontin(米国登録商標))は、臨床的ニューロパシックペインの治療への有用性の向上が見出された抗痙攣薬である。当初は痙縮およびてんかんの治療用に開発されたが、最近では、臨床試験における有効性および最小の副作用特性に基づいて、ニューロパシックペインの治療におけるGBPの有用性に焦点が当てられている(Rice and Maton, 2001)。齧歯類のニューロパシックペインモデルにおいて、GBPは、末梢神経結紮に続く熱および機械的誘発性過敏症を効果的に弱化する(Xiao and Bennett, 1996; Hunter et al, 1997; Hwang and Yaksh, 1997)。GBPが、カラギーナン誘導性の炎症に後続する温熱性痛覚過敏および機械的痛覚過敏を阻害することも証明されたが(Field et al, 1997b; Lu and Westlund, 1999)、他の試験では、炎症性疼痛に対するGBPの有効性は限定的であることが報告された(Gould et al, 1997; Patel et al, 2001)。さらに、GBPは、それぞれ足底内ホルマリンまたは外科的切開によって引き起こされた自発的侵害受容行動および機械的痛覚過敏を阻害する(Field et al, 1997a,b)。急性侵害刺激への反応には影響がないので、ニューロパシックペイン、炎症性痛覚および術後疼痛のモデルにおけるGBPの抗侵害受容作用は、負傷誘導性過敏症に対して選択的であると考えられる(Field et al, 1997b; Hunter et al, 1997)。
本発明において利用できるガバペンチンの類似体の非限定的リストには、プレガバリン、3‐メチルガバペンチン、[(1R,5R,6S)‐6‐(アミノメチル)ビシクロ[‐3.2.0]ヘプタ‐6‐イル]酢酸、3‐(1‐アミノメチル‐シクロヘキシルメチル)‐4H‐[1,2,4]‐オキサジアゾール‐5‐オン、C‐[1‐(1H‐テトラゾール‐5‐イルメチル)‐シクロヘプチル)‐メチルアミン、(3S,4S)‐(1‐アミノメチル‐3,4‐ジメチル‐シクロペンチル)‐酢酸、(1α,3α,5α)(3‐アミノ‐メチル‐ビシクロ[3.2.0]ヘプタ‐3‐イル)‐酢酸、(3S,5R)‐3‐アミノメチル‐5‐メチル‐オクタン酸、(3S,5R)‐3‐アミノ‐5‐メチル‐ヘプタン酸、(3S,5R)‐3‐アミノ‐5‐メチル‐ノナン酸および(3S,5R)‐3‐アミノ‐5‐メチル‐オクタン酸、(1‐アミノメチル‐3‐メチルシクロヘキシル)酢酸、(1‐アミノメチル‐3‐メチルシクロペンチル)酢酸、(S)‐3‐(アミノメチル)‐5‐メチルヘキサン酸、3‐アミノメチル‐5‐メチルヘキサン酸、(1‐アミノメチル‐3,4‐ジメチルシクロペンチル)酢酸もしくはその薬学的に許容される塩、またはそれらのエステル誘導体もしくはアミド誘導体が含まれる。
カプサイシンは赤唐辛子に含まれる刺激性の天然材料である。カプサイシンは、使用濃度および適用方法によって、小径知覚求心性神経に対する神経毒性作用を選択的に活性化、脱感作、または発揮できる一方で、大径求心性神経には影響を及ぼさない(Holzer, 1991; Winter et al, 1995)。感覚ニューロンの活性化は、バニロイド受容体(VR‐1)と呼ばれるリガンド依存性の非選択性陽イオンチャネルとの相互作用によって生じ(Caterina et al, 1997)、受容体占有は、Na+およびCa2+のイオン流入、活動電位発射、ならびに香辛料の入った食物に関連した結果生じる灼熱感またはカプサイシンによって誘発される疼痛を誘発する。VR1受容体は、C線維上およびAδ線維上の両方に存在し、カプサイシンおよびその類似体、熱、酸性化、ならびに脂質代謝物質によって活性化される(Tominaga et al, 1998; Caterina and Julius, 2001)。脱感作は、カプサイシンの連続投与によって生じ、受容体媒介の過程であり、Ca2+およびカルモジュリン依存性の過程ならびに陽イオンチャネルのリン酸化を含む(Winter et al, 1995; Wood and Docherty, 1997)。
皮膚に対して低刺激、かつ胃の灼熱感が著しく低いカプサイシンを作製するために、カプサイシンをフェノール酸の位置でエステル化した。これらのエステルは一般式Iを有する:

カプサイシンのエステル化合物は、当業者に公知の任意の方法によって調製できる。例えば、本発明の化合物は、唐辛子の成分であるカプサイシンのエステルである。様々な方法が、多数のカルボン酸およびフェノールのエステルの合成に関する文献に記載されている(March’s Advanced Organic Chemistry: Reactions, Mechanisms, and Structure, 5th Edition, by Michael B. Smith and Jerry March, John Wiley and Sons, Inc, 2001)。
様々な実施形態において、三環系抗鬱薬が本発明の医薬組成物に含まれてもよい。本発明中で使用され得る三環系抗鬱薬の非限定リストには、アミトリプチリン、ブトリプチリン、アモキサピン、クロミプラミン、デシプラミン、ドチエピン、イミプラミン、ジベンゼピン、イプリンドール、ロフェプラミン、ノルトリプチリン、オピプラモール、プロトリプチリン、チアネプチン、ミルナシプランおよびトリミプラミンが含まれる。
本発明の医薬組成物(例えば、NMDA拮抗薬と、トラマドールと、ガバペンチンとの組合せ)は、癌性疼痛および手術後疼痛、リウマチ熱を含む様々な症状の熱および炎症、インフルエンザまたは他のウイルス感染に関連した症状、風邪、腰部および頚部の疼痛、月経困難症、頭痛、歯痛、筋違いおよび捻挫、筋炎、神経痛、関節滑膜炎、関節リウマチを含む関節炎、退行性骨関節症(骨関節炎、痛風および強直性脊椎炎等)、滑液嚢炎、熱傷、片頭痛、線維筋痛症候群、多発性硬化症候群、三叉神経痛、糖尿病性ニューロパシーに関連した症状ならびに損傷を含むが、これらに限定されない様々な病因から生じる中等度から重度の疼痛の治療に使用できる。
本発明の好ましい実施形態は、NMDA拮抗薬またはその薬学的に許容される塩と、抗痙攣薬(例えば、ガバペンチンまたはガバペンチン類似体)および/または三環系抗鬱薬またはその薬学的に許容される塩と、トラマドールまたはトラマドール類似体またはその薬学的に許容される塩との組合せを提供する、経口投与の疼痛緩和剤である。様々な実施形態において、これらの薬物の組合せは、相乗して疼痛軽減を提供でき、提供される薬剤の量を好適に最小限に抑えることによって、生じ得る副作用を軽減または解消するので、活性成分を低減させた投薬量は、本発明において好適に使用される。
本発明は、効果的な鎮痛量のデキストロメトルファンと、ガバペンチンまたはガバペンチンの類似体と、トラマドールまたはその類似体との組合せの即時放出投薬形態を包含する。即時放出投薬形態は、タブレット、カプセル、またはカプセル状の多重微粒子の経口製剤として製剤化してもよい。当該分野において公知の他の即時放出投薬形態を使用できる。特定の各実施形態において、本発明の医薬製剤は経口投与されてもよく、他の各実施形態において、医薬製剤の投与は、非経口投与、静脈内投与、吸入を介する投与等であってよい。
NMDA拮抗薬と、抗痙攣薬および/または三環系抗鬱薬と、トラマドールまたはその類似体との組合せ、または、NMDA拮抗薬と、抗痙攣薬および/または三環系抗鬱薬と、カプサイシンまたはカプサイシンのエステルと、トラマドールまたはその類似体との組合せは、当業者に公知の適切なタブレット、コーティングされたタブレット、または多重微粒子剤において、放出制御経口剤または徐放性経口剤として製剤化することができる。徐放性投薬形態は、トラマドールまたはその類似体と共にマトリックスに取り入れられるか、または徐放性コーティングとして適用される徐放性担体を任意で含んでもよい。様々な各実施形態において、医薬組成物はリポソームに含まれてもよい。
本発明の投薬形態は、放出の調節または製剤の保護に適した1つまたは複数の材料で、任意でコーティングされてもよい。ある実施形態において、コーティングは、例えば胃腸液に暴露された場合に、pH依存性放出またはpH非依存性放出を可能なように提供される。pH依存性コーティングの働きは、少なくとも約12時間、好ましくは最大24時間の鎮痛を患者にもたらすことが可能な吸収プロファイルを提供するように、胃腸(GI)管の所望領域(例えば、胃または小腸)に、トラマドールまたはその類似体を放出することである。pH非依存性コーティングが所望される場合、コーティングは、周囲の液体(例えばGI管)のpHの変化に関係なく最適な放出を達成するように設計される。GI管の所望領域(例えば胃)に一部の用量を放出し、GI管の他領域(例えば小腸)に残りの用量を放出する組成物を製剤化することもできる。
アルキルセルロースを含むセルロース系の材料およびポリマーは、本発明によるビーズのコーティングに適切な疎水性材料を提供する。単純に例として、好適なアルキルセルロース系ポリマーはエチルセルロースであるが、当業者にとっては言うまでもなく、本発明による疎水性コーティングのすべてまたは一部として、他のセルロースポリマーおよび/またはアルキルセルロースポリマーを単独または組合せて、直に使用してもよい。
放出制御コーティングを含む疎水性材料は、アクリル酸およびメタクリル酸コポリマー、メタクリル酸メチルコポリマー、メタクリル酸エトキシエチル、メタクリル酸シアノエチル、ポリ(アクリル酸)、ポリ(メタクリル酸)、メタクリル酸アルキルアミドコポリマー、ポリ(メタクリル酸メチル)、ポリメタクリル酸、ポリ(メタクリル酸メチル)コポリマー、ポリアクリルアミド、メタクリル酸アミノアルキルコポリマー、ポリ(メタクリル酸無水物)、ならびにメタクリル酸グリシジルコポリマーを含むがこれらに限定されない、薬学的に許容されるアクリルポリマーである。アクリルポリマーは、1つまたは複数のメタクリル酸アンモニオコポリマーで構成される。メタクリル酸アンモニオコポリマーは当該分野において周知であり、第四級アンモニウム基を低含有量である、アクリル酸エステルおよびメタクリル酸エステルの完全に重合したコポリマーとして、NF XVIIに記載されている。
コーティングが疎水性材料の水性分散液を含む本発明において、疎水性材料の水性分散液に有効量の可塑剤を含むことによって、徐放性コーティングの物理的特性がさらに改善されるだろう。例えば、エチルセルロースは比較的高いガラス転移温度を有し、通常のコーティング条件下では軟質フィルムを形成しないので、コーティング材として使用する前に、徐放性コーティングを含むエチルセルロースコーティングに可塑剤を加えることが好ましい。一般に、コーティング溶剤に含まれる可塑剤の量はフィルム形成剤の濃度に基づき、例えば、多くの場合、フィルム形成剤の重量の約1%乃至約50%である。しかし、特定のコーティング溶剤および適用方法について実験を十分に重ねた後でのみ、可塑剤の濃度を適切に決定できる。
疎水性材料の水性分散液を使用して、ヌ・パリエル(nu‐pariel)18/20ビーズ等の不活性の薬学的ビーズをコーティングする場合、結果として生じる複数の安定した固体の放出制御ビーズは、その後、摂取されて周囲の液体(例えば、胃液または溶解溶媒)に接触した際に効果的な放出制御用量を提供するのに十分な量で、ゼラチンカプセルに入れられる。
放出制御剤は、上記のように、放出制御コーティングを有するマトリックスを介して得られる。本発明は、トラマドールまたはその類似体の好ましい範囲内でのインビトロの溶解速度を提供し、pH依存的またはpH非依存的にトラマドールまたはその類似体を放出する、放出制御マトリックスを使用してもよい。放出制御マトリックスへの含有に適した材料は、マトリックスの形成に使用される方法によって決まる。
本発明による固体の経口用放出制御投薬形態の調製を容易にするために、当業者に公知であるマトリックス製剤を調製する任意の方法を使用してもよい。例えば、マトリックスへの組み入れは、例えば、(a)少なくとも1つの水溶性ヒドロキシアルキルセルロース、およびトラマドールもしくはその類似体、またはトラマドールもしくはその類似体の塩を含む顆粒を形成すること;(b)顆粒を含むヒドロキシアルキルセルロースを、少なくとも1つのC12〜C36の脂肪族アルコールと混合すること;および(c)任意で顆粒を圧縮および成形すること、によって達成できる。好ましくは、顆粒は、ヒドロキシアルキルセルロース/トラマドールまたはその類似体を水で湿式顆粒状にすることによって形成される。この過程の特に好ましい実施形態において、湿式顆粒工程中で追加される水の量は、トラマドールまたはその類似体の乾燥重量の、好ましくは1.5倍乃至5倍、特に好ましくは1.75倍乃至3.5倍である。
徐放性マトリックスは、溶融造粒技術または溶融押出技術でも調製できる。一般的に、溶融造粒技術には、ワックス等の通常は固体の疎水性材料を溶融すること、および散剤をその中に入れ込むことを含む。徐放性投薬形態を得るために、溶融ワックス疎水性材料に、エチルセルロースまたは非水溶性アクリルポリマー等の、追加の疎水性物質を入れ込む必要がある。溶融造粒技術によって調製される徐放投与剤の例は、本発明の出願人に譲渡され、その全体を参照することにより本明細書に組み入れられる米国特許第4,861,598号に見られる。
本発明による好適な溶融押出マトリックスの調製は、例えば、少なくとも1つの疎水性材料および好ましくは追加の疎水性材料と共に、トラマドールまたはその類似体を混合して、均質混合物を得る工程を含んでもよい。次いで、少なくとも混合物を押出加工するために軟化させるのに十分な温度まで均質混合物を加熱する。次いで、得られた均質混合物を押出加工して、ストランドを形成する。押出物は、好ましくは冷却され、当該分野において公知の手段によって多重微粒子に切断される。ストランドは冷却され、多重微粒子に切断される。次いで、多重微粒子は単位用量に分割される。押出物は、好ましくは約0.1mm乃至約5mmの直径を有し、約8時間乃至約24時間の期間にわたって治療活性薬剤を徐放する。
カプサイシンUSP27(HUBEI XIANGXI CHEMICAL INDUSTRY CO.,LTD, China)30.5gm(〜0.1M)、無水トリエチルアミン(Spectrum Chemicals)16.7ml(0.12M)、4‐(ジメチルアミノ)ピリジン220mg、および無水ジクロロメタン200ml、の混合物を、1000mlの二ツ口の丸底フラスコに入れた。内容物をアルミホイルで覆い、光の暴露から保護した。フラスコには、上部にモイスチャートラップを取り付けたコンデンサおよび滴下漏斗を取り付けた。フラスコを室温に維持し、塩化パルミトイル25.4ml(0.095M)を、漏斗から混合物中へ撹拌しながら徐々に追加した。追加後、混合物を3〜6時間還流させ、室温で10〜15時間撹拌した。混合物を分別漏斗に移し、水2×500ml、希塩酸2×500ml、10%炭酸水素ナトリウム溶剤2×500ml、およびI型水3×500mlで連続洗浄した。淡黄色固体(理論上95%)を生成するために有機層を分離し、無水硫酸マグネシウムで乾燥し、ジクロロメタンを真空下で除去した。上記で得られた淡黄色固体を、エタノールから再結晶化した。2リットルフラスコ内で、加熱した1リットルのエタノールに固体を溶解し、濾紙を用いて濾過した。次いで、濾過液を冷蔵庫で冷却し、白色結晶を得た。
患者1:症状:糖尿病性ニューロパシー
50歳の白人男性は糖尿病性ニューロパシーと診断され、実施例1の製剤1のカプセルが与えられた。この患者によるカプセル使用後の体験談は、以下の通りである。「糖尿病性ニューロパシーが原因で、脚と足に強い痛みを感じます。長期にわたって抗生物質の静脈注射を続けていましたが、その後ニューロパシーになりました。疼痛を和らげるために、アスピリンを過剰に服用したり、処方された抗鬱薬を服用したり、強い鎮痛剤を使ったこともあります。それでも疼痛は十分に緩和されず、夜は数時間しか眠れませんでした。でも、このカプセルを服用し始めてからは、痛みを全く感じません。効果が現れるまでに30分しかかからず、最長18時間も持続します。おかげで熟睡できるようになりました。」
64歳の女性は、関節痛および骨関節炎による硬直を患っている。この患者に実施例1の製剤1のカプセルを処方した。この患者によるカプセル使用後の体験談は、以下の通りである。「通常、1日のうちで一番具合が悪いのは朝です。動き出すにはストレッチとウォーミングアップが必要で、これには30分以上かかります。でも、カプセルを服用することで、この問題が解決しました。一度も夜中に目を覚まさずに眠ることができ、楽に朝をスタートできます。他の疼痛薬剤では、生活の質は、10中3のレベルでしたが、このカプセルのおかげで10中9のレベルで毎日を楽しむことができます。カプセルを服用する以前は、動き出すために朝の時間を費やし、関節炎の疼痛と筋肉の不快感が緩和するように残りの時間を費やしていました。」
44歳の白人女性は、両側腎摘出術を受け、慢性の背部痛および硬直を患っている。この患者による実施例1の製剤1のカプセル使用後の体験談は、以下の通りである。「私は腎臓を摘出した44歳の女性です。処方薬に関しては、服用するものすべてに注意しなければなりません。ついに、すべての痛みを取り除いてくれるものに出会いました。私は背部痛と硬直に悩まされています。数日間カプセルを使用し、カプセルがもたらす緩和レベルに非常に感動しました。20分以内に効果が現れ、私はまるでどこも悪くないかのように動き回ることができました。本当に効果のあるものが販売されていると知って、とても興奮しています。疼痛緩和がこの製品の唯一の効果であることにも気付きました。つまり悪心またはめまいが起こらないということです。」
44歳のヒスパニックの女性は線維筋痛症候群を発症し、症候群に起因する慢性疼痛を患っている。この患者に実施例1の製剤1のカプセルを処方した。この患者によるカプセル使用後の体験談は、以下の通りである。「私は線維筋痛に苦しむ非糖尿病患者です。3年程前に発症しました。強い鎮静剤を服用したくはありませんでしたので、トラマドールを処方してもらいました。でも、この薬物では中等度の緩和しか得られず、疼痛が持続します。胴部左側、腕および脚に痛みを感じます。痛みがあまりに強く、まるで彫像のように身をかがめることがあります。私の痛みは1〜10のスケールで11です。痛みを止めるために何度も自殺を考えました。今の生活には全く満足できません。
骨癌および疼痛を患う61歳の男性に、実施例1の製剤1のカプセルを処方した。この患者の体験談は、以下の通りである。「私は左側の肩に骨癌があります。疾患の広まりを抑える治療の末、強い痛みを患いました。2、3時間以上眠ることはできませんでしたし、残りの時間は痙攣がありました。TLI‐1026を服用した後、目が覚めたときには9時間も熟睡したことに驚きました。化学療法および放射線療法のニューロパシーからの疼痛を全て緩和したことに加えて、痙攣が全くありませんでした。」
46歳の男性は持続的な歯痛があり、この患者に実施例1の製剤1のカプセルを処方した。この患者の体験談は、以下の通りである。「全く良くならない歯痛のために、私はロラタブ(Loratab)5を服用していました。歯科医がこの問題を治す前に、口内の膿瘍の治癒を待たねばなりませんでした。治癒を待っている間、私は、TLI‐1026と呼ばれる新しい処方薬を試してみました。約30分で、疼痛の80%が消えました。そして数時間後には、痛みがすべて消えていました。疼痛は、膿瘍があった右下の下顎にありました。」
23歳の男性は、背中に急性疼痛を有しており、この患者に実施例1中の製剤1のカプセルを処方した。この患者の体験談は、以下の通りである。「私は、重い荷物を不適切に持ち上げた時に負った怪我の結果、今では2年以上にわたって腰部に急性疼痛を患っています。最初、痛みが消えるだろうと自分に言い聞かせていたときには耐えられました。けれど、しばらくして、疼痛の強いフレアよる縦方向の圧迫感を腰から取るために、1日に2、3回横にならなければならないことに気付きました。約2か月間、毎日耐え難い背中の痛みを体験した後、眠れなくなったので、医師に診察してもらいました。1日2回350mgのソーマ(カリソプロドール)を処方されました。単に疼痛を覆い隠しているみたいで、日中ひどく眠くなりました。このため、処方薬を全て使い果たせませんでした。次に、(500mgのアセトアミノフェンと共に)5mgのバイコディン(ヒドロコドン)を処方されました。この薬でも眠気がひどく、ソーマを服用していた時のように、2、3時間疼痛を覆い隠しているようでした。疼痛が悪化し、体を歪めても、まだ痛みを感じました。しかし、TLI‐1026製剤を一錠試した後に、痛みが和らぎました。半時間以内に痛みを忘れ、腰を捻っても痛みを感じませんでした。副作用はほとんど無く、眠気もありませんでした。痛みが18時間にわたって緩和され、何時間も身体を動かすことなく、子供のように眠ることができました。目を覚ますかどうかは、完全にまだ効いているこの製剤の残存効果次第でした。この製剤のおかげで、腰の痛みを心配する必要も、3時間ごとに薬を服用する必要も、楽になるためにベッドで時間を過ごす必要もなくなり、毎日の日課を全て続けることができました。アセトアミノフェンが含まれていないことは、肝臓にもよいと確信しています。」
23歳の女性はライム病のために線維筋痛であると診断された。この患者に実施例1の製剤1のカプセルを処方した。この患者の体験談は、以下の通りである。「1年前にリウマチ専門医が線維筋痛と診断しました。血液検査によれば、線維筋痛は、私が以前患ったライム病が原因でした。常に持続的なニューロパシックペイン、頭痛および疲労感があります。まだ若いので、普段は非常に活動的でありたいのです。カプセルを1つ服用すると、50分以内に頭痛と股関節の痛みがほとんど消えました。その12時間後に2個目のカプセルを服用してみると、疼痛、頭痛、および疲労感は完全に無くなりました。私はこの結果に驚きましたし、担当医はこれ程の改善を信じませんでした。」
女性は頚椎骨軟骨症と診断された。この患者に実施例1の製剤1のカプセルを処方した。この患者の体験談は、以下の通りである。「私が使っていた薬は、良く効いていました。効き目が完全に出るまで3、4日かかったようです。この薬を2007年8月25日に服用し始めました。1日目に、私は一種の立ちくらみ(浮遊感)を僅かな感じましたが、日々の活動を妨げるようには思われませんでした。私は今でも、朝晩1錠ずつ、(約)12時間毎にこの薬を服用しています。この薬剤を服用する前は、35歳の時に診断された頚椎骨軟骨症に伴う痛みのために、バイコプロフェンを服用していました。バイコプロフェンは、もはや痛みの軽減にはまったく役立ちませんでした。私はまた、自ら「気圧性頭痛」と呼ぶ痛みも患っており、これはホルモンの変化に関係すると考えています。どの薬も頭痛を和らげてはくれませんでした!!この薬を開始した時には、これら頭痛のうちの1つを患っていましたが、今や頭痛はありません!痛みを無くしてくれてありがとうございます!!」
線維筋痛と診断された女性患者に、実施例1の製剤1のカプセルを処方した。この患者の体験談は、以下の通りである。「私はディアン・トーマス、69歳です。マイク・タイラー先生に8〜10年間「線維筋痛」を診てもらっています。過去4、5年間、ノルコ10/325を1日6回服用しています。私とって効果はありましたが、完全に痛みがなくなるわけなく、痛みが和らぐだけでした。
1.糖尿病性ニューロパシー患者および線維筋痛患者
本発明の組成物の有効性を評価するために、2セットのカプセルを糖尿病性ニューロパシー患者および線維筋痛患者に処方した。カプセルの1つのセットはTLI‐1180と呼ばれ、35mgのトラマドールおよび45mgのデキストロメトルファンを含み、他方のカプセルのセットはTLI‐1026と呼ばれ、実施例1の製剤1の組成物を含んでいた。以下の経験談は、患者を観察した医師によるものである。

糖尿病性ニューロパシー患者
白人男性は糖尿病性ニューロパシーと診断され、この患者に実施例8の製剤1のカプセル(TLI‐1126)を処方した。この患者の体験談は、以下の通りである。「私は、糖尿病性ニューロパシーの疼痛用にガバペンチンを含むTLI‐1026を定期的に使用しています。ガバペンチンとアミトリプチリンの両方を含むと教えられたので、私はTLI‐1126の効果に興味がありました。カプセル1つを服用してから1時間以内に、痛みが完全に取り除かれたと感じ、足の刺痛感がなくなりました。この緩和があまりに完全であり、効果は非常に強かったので、私はほとんど12時間も眠ってしまいました。私にとって、TLI‐1126は強すぎるようです。私の疼痛には、TLI‐1126投薬量の半分で間に合うと思いました。」
実施例6の製剤1に記載された組成物を、糖尿病性ニューロパシーの疼痛と診断された患者2名に処方した。患者は12時間間隔で1日2個のカプセルを服用し、両者共にニューロパシー伴う疼痛の完全な緩和を感じた。
