試料分析工程(工程b)は、質量分析計(MS、mass spectrometer)を用いて試料を分析することを含むことができる。例えば、それらに限定するものではないが、そのような質量分析計は、FTMS型、オービトラップ(orbitrap)型、飛行時間(TOF、time of flight)型又は四重極型でよい。代わりに、質量分析計は、さらなるプレ検出器マスフィルターを備えることができよう。例えば、それらに限定するものではないが、そのような器具は、四重極−FTMS(Q−FTMS、quadrupole-FMS)、四重極−TOF(Q−TOF、quadrupole-TOF)又は三重四重極(TQ、triple quadrupole、若しくはQQQ)と一般に呼ばれる。さらに、質量分析計は親イオン検出モード(MS)又は、n>=2であるMSnモードで作動させることができた。MSnは、親イオンが衝突誘発性解離(CID、collision induced dissociation)又は他の断片化手法によって断片化して断片イオンを形成し、その後、前記断片の1又は複数が質量分析計によって検出される状況を指す。次に、そのような断片をさらに断片化して、さらなる断片を形成することができる。代わりに、液体若しくはガスクロマトグラフィー系を用いて、又は、直接注入によって、試料を質量分析計に導入することができた。

A.血清バイオマーカー
代謝産物の構造解明のために用いることができる特性には、正確な質量及び分子式の決定、極性、酸/塩基性質、NMRスペクトル並びにMS/MS又はMSnスペクトルが含まれる。これらのデータ、特にMS/MSスペクトルは、特定の代謝産物のフィンガープリントとして用いることができ、完全な構造が決定されているかどうかに関係なく、特定の代謝産物の特異識別子である。
AD病状に特異的な代謝産物マーカーはエタノールアミンプラスマロゲン骨格を有するという事実に基づいて、他の血清プラスマロゲンがADの指標となることができるかどうかについて、さらに調査した。血清中のエタノールアミンリン脂質のこの特性評価は、質量分析検出器と組み合わせたクロマトグラフィー法を用いて実施した。
AD認知症及び非AD認知症状態の鑑別診断のための、ハイスループット方法を確立した。
様々なレベルの認知症を有する40〜95歳の752人の対象におけるエタノールアミンリン脂質の血清レベルに及ぼす加齢及び認知症程度の影響を調査した。対象コホートに関する臨床データを、表19に示す。
白質及び灰白質特異EtnPlスコアリングシステムを開発し、それによって、各対象の各EtnPlをそれらのそれぞれの性特異認知正常平均に標準化し、log2変換し、平均を中心にした。各対象の白質スコアを、そのようなプラスメニル16:0/18:1(M15)、16:0/18:2(M16)、18:0/18:1(M20)及び18:0/18:2(M21)EtnPlの最低値とし、彼らの灰白質スコアを、プラスメニル16:0/20:4(M17)、16:0/22:6(M19)、18:0/20:4(M22)及び18:0/22:6(M24)EtnPlの最低値とした。
認知症程度の影響は、68人の認知的に確認された非認知症の対象(MMSE≧28)及び、現時点でAD(ADAS−cogが6〜70、MMSEが0〜26)と診断された256人の対象が含まれた、56〜95歳の324人(女性176人、男性148人)の対象で判定した。AD病状の影響は、20人の死後確認されたAD及び19人の対照対象から収集した血清試料を用いて判定した(表19)。対象を、彼らのMMSEスコア[≧28=認知的に正常]又は彼らのADAS−cogスコア[5〜19=低い認知障害、20〜39=中程度、40〜70=重度]に基づいて、4つの認知症重症度コホートの1つに分類した。
EtnPl 16:0/22:6(M19)対PtdEt 16:0/18:0の比率(M01)(DHA−EtnPl)は、最も強い全体的な性別非依存性の認知症影響を示したので(表38、41)、以降の全ての人口分布及び比較のために用いた。この比率を用いた重要な比較の概要を、表52に記載する。次に、この比率をlog(2)変換し、認知障害が進行する各コホートの集団ヒストグラムを作成するために用いた(図35)。カットオフ値は、Bennett et al(非特許文献35)の知見に基づいて選択した(すなわち、CN群の約30%がADとして検出されること)(図35、点線)。このカットオフを用いて、軽度、中程度及び重度の認知症の対象のそれぞれ63%、79%及び83%が、その後ADとして分類された。
臨床的に診断されたAD集団においてEtnPlの減少及び認知症の増加の間に相関が存在するかどうかを調査するために、各認知症コホートの22:6含有EtnPlの平均レベル(CNに標準化したもの)及びこれら3つのコホートのそれぞれの平均ADAS−cogスコアを用いて、線形回帰分析を実施した(図37)。3つの認知症コホートの22:6含有EtnPlの平均レベル及び平均ADAS−cogスコアの間に、非常に高い相関が観察された(r2=0.99)。しかし、この線形の減少は、CN群へ外挿されなかった(X対CN)。1年に7.5ADAS−cog単位のADの臨床的進行を仮定すると、この外挿により、22:6含有EtnPlレベルは、臨床認知障害(ADAS−cog=15)が明白になる約7年前に低下し始めると予測される。
上記予測を実験的に検証することができるかどうかを調査するために、認知状態が未知であるが現時点で認知症と診断されていない209人の対象(男性110人、女性99人、表19)の血清中22:6含有EtnPlレベルを測定し、臨床AD及びCNコホートと比較した(図38)。この分析の結果から、50〜59歳のコホートに対して60〜69歳のコホートでの、血清中22:6含有EtnPlの有意な減少が明らかになった(図38A)。CN群は平均して13歳年長であったが、CN群に対しても、このコホートは有意に低いレベルを有していた。面白いことに、70〜95歳のコホートは、50〜59歳のコホート及びCNコホートから有意に異なることはなかったが、SDATコホートよりも有意に高いレベルを有した。
図38Bで示す各年齢群での血清DHA−EtnPlの分布も、検討した。年齢及び認知症状態によって区別した5群(3年齢群、CN及びAD)の人口分布は、3つの異なる集団(P1〜P3、図38B)の存在を明らかにする。これらの集団は、下記の通り指定した。P1−AD病状を有し、残りの余力のない対象;P3−AD病状がほとんど又は全くない対象;P2−P3からP1に移行する対象。これらのP2対象は、AD病状を有し、多少のレベルの残存余力を有すると仮定される。
