Abstract
Description
本発明は、経口投与が問題になる患者のためのチピファルニブの代替投与経路(すなわち、非経口的)を提供する。本発明の目的は、現状のチピファルニブの1日2回の経口投与に類似する臨床成果を可能にする、チピファルニブの十分な曝露を提供することができる、連続注入としても、または短期間注入(1時間以下〜数時間;1日当たり1回もしくはそれ以上の回数)としても、チピファルニブの投与のための静脈内経路を提供することであった。特に、本発明の目的は、現状のチピファルニブの1日2回の経口投与に類似する曝露を与えることができる、連続注入としても、または1日2回の2時間注入としても、チピファルニブの投与のための静脈内経路を提供することであった。
より正確な記述を提供するために、本明細書に与えられる若干の量的表現は用語「約」により修飾されない。用語「約」が明確に使用されるか否かは、本明細書で与えられる各数量が実際に与えられた値を指すことが意味され、そしてまた、それが、そのような与えられた値についての実験および/または測定条件による近似値を含む、当業者によって合理的に推定できるような与えられた値への近似値を指すことが意味されると理解される。

調合物1
調合物1の10mlが20mlバイアル中に充満され、そして凍結乾燥される(1バイアル当たり100mgのチピファルニブ)。使用前に、凍結乾燥粉末が調合物2の8.6mlに溶解されて、チピファルニブ10mg/ml注射溶液(pH〜4−5)の10mlを得る。このチピファルニブ10mg/ml溶液は、次に、0.9%NaCl溶液により所望の濃度(0.5〜2mg/ml)までさらに希釈される。

(1)ヒドロキシプロピル−β−シクロデキストリンの2kgを適当な容器に添加し、

(6)無菌の0.22μmフィルターを通して濾過し、
(7)(6)の10mlを20mlバイアル中に充満する。
(8)バイアルに栓をして、冷温トレーに移し、
(9)(8)を凍結乾燥機に移し、

− 一次乾燥:0.1mbarで10℃

(11)バイアルの密封(crimping)。
方法および材料
この無作為化されたオープン・ラベルの比較試験は、臨床試験実施規準(good clinical practice)にしたがって実施された。倫理委員会の承認は試験前に得られ、そして患者は参加することへの彼らの書面で示されたインフォームドコンセントを与えた。
治癒療法を受け入れる余地のない(not amenable)、病理学的に確認されたがんを有する年齢18歳以上の男性および女性がこの研究のために選ばれる資格があった。患者はECOG(Eastern Cooperative Oncology Group)Performance Status<2を有することが要求された。他の要件、研究開始前少なくとも4週(ニトロソ尿素またはマイトマイシンCでは6週)の照射療法および/または化学療法の不在および栄養の合理的状態を維持するための十分な経口摂取を含んだ。
本研究の第1(用量−決定)相の目的は、十分に許容される静脈内処置プログラム(regimen)を同定することであった。この処置プログラムは、健康な男性ボランティアにおける絶対生物利用度の予備試験的決定に基づいて、臨床的に適切な経口用量(200mg)の50%〜60%と同等であろうことが期待された。3人の被験者の第1コホートは、次のような中断することのないチピファルニブの4日連続の処置:第1〜4日における2時間静脈内注入b.i.d.として30mg、続いて第5〜8日における経口投与b.i.d.として200mg、続いて第9〜12日における連続静脈内注入として1日当たり60mgを受けた。同様に、3人の被験者の第2コホートは、チピファルニブの類似の4日処置プログラム:2時間静脈内注入b.i.d.として60mg、続いて経口投与b.i.d.として200mg、続いて連続静脈内注入として1日当たり120mgを受けた。前記の各々4日の経口またはi.v.処置の間にはウォッシュアウト(washout)はなかった。
用量−決定およびブリッジングの両相において、薬動学上のサンプリングは、各処置の4日目の朝(morning)の用量の開始から10時間の間実施された。全血液サンプルが、経口投与直前、および投与後0.5、1、2、3、4、6、8および10時間に採取された。2時間静脈内注入では、血液サンプルは、投薬直前、および投薬後1、2(すなわち、注入終了直前)、2.25、3、4、6、8および10時間に採取された。血液サンプルは、連続静脈内注入の開始直前、および開始後2、4、6、8および10時間に採取された。
チピファルニブは、不斉炭素原子において[14C]により特異的に標識された。7mlの血液サンプルが、4日目のチピファルニブの連続静脈内注入の開始後4時間に24人の患者から採取された。血漿サンプルは、500ng/mlにおける[14C]−チピファルニブにより強化された。強化された血漿サンプルが、同一のmacro−1 TeflonセルとDiachema10.17透析膜(MWカットオフ10,000)を備えたDiachemaシステムにおいて、37℃で4時間、0.067Mリン酸バッファー、pH7.17に対する平衡透析にかけられた。放射能レベルは、平衡透析の前後に強化された血漿の2並列の一定分量100μlについて、および透析後にメタノールと混合された透析セルのバッファー・コンパートメントの内容物の1000μlの2並列一定分量について、Packard Tri−Carb 1900 TR液体シンチレーション分光計で測定された。総タンパク質濃度は、比色ビューレット法、キットRoche Diagnostics 1929917により決定された。アルブミン濃度は、比色ブロモクレゾールグリーン法、キットRoche Diagnostics 1970909により決定された。α1−酸性糖タンパク質濃度は、免役比濁法、キットRoche Diagnostics 1557602により決定された。すべてこれらの試験はRoche Hitachi Molecularアナライザーにおいて実施された。

各投与期間の4日目の朝用量の開始から10時間の間のチピファルニブの個々の血漿濃度−時間データが、プログラムWinNonlinバージョン3.1(Pharsight Corporation,Cary,NC)を用いて非区分方法によって分析された。時間0〜10時間からの濃度−対−時間曲線下面積(AUC0−10h)は、直線台形法を用いて計算された。最大血漿濃度(Cmax)、定常状態における血漿濃度(Css、連続静脈内注入についてのみ)および最高血漿濃度に達する時間(tmax)は観察された値であった。経口投与後のチピファルニブの生物利用度は、
[AUCpoxDOSEiv/(AUCivxDosepo)]x100%
にしたがって、AUC0−10h値に基づいて計算された。
人口統計学的情報は、中間値、最小値および最大値として総括された。チピファルニブおよびその代謝物の薬動学的データもまた記述統計学を用いて総括された。全3処置に関する薬物動態サンプルが得られた被験者からの、経口および静脈内投与後のチピファルニブに対する全身性曝露(血漿CmaxおよびAUC0−10h)がANOVAを使用して評価された。チピファルニブCmaxおよびAUC0−10hについての静脈内注入(試験)対経口投与(参考)の最小平方平均比の周囲の点(point)推定値および対応する90%信頼区間が計算された。解析は、SASバージョン6.12統計学的ソフトウエアプログラム(SAS Institute,Cary,NC)を用いて対数変換された薬動学的パラメーターに関して実施された。
完全な患者の病歴が採られ、そして身体検査(生命徴候を含む)、血液学的試験および臨床化学試験が、基準線においておよび処置中に実施された。心臓血管安全性を評価するために、12リード(twelve−lead)E.C.G.が、スクリーニング、Cycle 1および治療の終わりにおいて記録された。目標の腫瘍応答はこの研究の主要目標ではなかったけれども、適当な測定値を得て応答を調査するために、1つの試みがなされた。すべての有害事象および臨床的に関連する実験室の異常は、Common Toxicity Criteria(CTCバージョン2.0)にしたがって格付けされた。
被験者の素因
32人の被験者(10人の男性および22人の女性)の全部が登録された;6被験者は用量決定相に、そして26被験者はブリッジング相に関与した。この研究における31人の被験者は、進展した固形腫瘍をもつと診断された。1人の被験者は高リスクの脊髄異形成症候群を有した。ブリッジング相では、12被験者が、無作為に、処置順序 1(2時間の静脈内注入b.i.d./経口投与b.i.d./連続静脈内注入)を受けるように割り当てられ、そして14被験者が、処置順序 2(連続静脈内注入/経口投与b.i.d./2時間の静脈内注入b.i.d.)を受けた。処置順序 1に無作為化された1被験者は、プロトコール違反のために4日目に研究から撤収され、そして処置順序 2の1被験者は、疾病の進行に関連する肝不全により8日目に撤収された。
第1コホートは、2時間静脈内注入として1日2回30mg、続いて経口投与として1日2回200mgおよび連続静脈内注入として1日当たり60mgの4日のチピファルニブ処置プログラムを受けた3被験者からなった。平均して、2時間静脈内注入として30mgb.i.d.および連続静脈内注入として1日当たり60mgについてのAUC0−10hに基づくチピファルニブの全身性曝露は、これらの個人によって受けられた200mgの経口用量後の曝露に較べて、それぞれ約64%および59%低かった(表II〜IV)(図2〜4)。静脈内投与後の用量制限毒性は観察されなかった。したがって、静脈内投与用量は次の用量レベルまで増大された。
予期されたように、チピファルニブ血漿濃度−時間プロフィルの形は、投与経路および投薬処置プログラムにより異なった(図1)。チピファルニブの最高血漿濃度は、経口投与および2時間静脈内注入後に明らかであった。反対に、連続静脈内注入は、明らかなピークのない薬物投与中の比較的一定した血漿濃度を生じた。
チピファルニブは、分析された全サンプルにおいて血漿タンパク質に広く結合されていた(図2)。平均遊離(未結合)フラクションは、連続静脈内注入の開始後4時間に採取された血漿サンプルにおいて0.62%(範囲0.45〜0.88%)であった。未結合チピファルニブのフラクションは、211ng/ml〜812ng/mlの範囲で総薬物血漿濃度とは独立していた。経口投与後または2時間静脈内注入後に採取された被験者サンプルからのタンパク質結合に関するデータは得られていない。しかしながら、4日間の200mgb.i.d.経口投与後の中央血漿濃度は、10時間の用量期間中、159ng/ml〜788ng/mlの範囲にある。
チピファルニブ代謝物の血漿薬物動態が、経口錠剤(200mgb.i.d.)、2時間静脈内注入(60および100mgb.i.d.)または連続静脈内注入(120および200mg/日)として4日投与後、4、8および12日目に分析された。分析された代謝物は、R130525、R101763、R104209、チピファルニブ−グルクロニド、R130525−グルクロニド、およびR104209−グルクロニドを含んだ。これらの代謝物の化学構造は、Garner RCらによって例証された。全身性循環において観察された主要代謝物はR130525およびチピファルニブ−グルクロニドであった(図3)。静脈内経路において決定されたチピファルニブの主要および少代謝物は、経口投与におけるそれらと同一であった。
全被験者が毒性について評価可能であった。1被験者は、研究のために不適格であることが分かった;有害な事象が報告されなかったので、3日後に撤収された。1被験者は、脊髄異形成症候群を有した。この被験者は、用量の減少および遅延によって管理される、グレード3〜4の好中球減少期間を有した。血小板カウントは正常に留まった。有意な非血液学的毒性は観察されなかった。
「用量−決定」相の目的は、1日2回経口的に投与される200mgに較べて、類似の曝露を生じるであろうチピファルニブの静脈内用量処置プログラムを同定することであった。より一般的に言えば、本目的は、現存の経口投与と類似の効果を与えるのに十分な曝露を提供できる静脈内用量およびスケジュールを同定することであった。低用量の静脈内処置プログラムによる第1および第2コホートでは、チピファルニブに対する全身性曝露は、200mg経口投与のそれよりもかなり低かった。これらの低用量処置プログラム後の用量制限毒性の不在を考慮して、最終静脈内投与用量は、本研究の「ブリッジング」相において、2時間注入としては100mgb.i.d.、そして連続注入としては200mg/日に増大された。
Claims (10)
- 静脈内投与に適したチピファルニブの製薬学的調合物。
- シクロデキストリンをさらに含む、請求項1に記載のチピファルニブの製薬学的調合物。
- シクロデキストリンがヒドロキシプロピル−β−シクロデキストリンである、請求項1または2に記載のチピファルニブの製薬学的調合物。
- チピファルニブの濃度が約10mg/mlである、請求項1〜3のいずれかに記載のチピファルニブの製薬学的調合物。
- シクロデキストリンを含んでなるチピファルニブの凍結乾燥組成物。
- シクロデキストリンがヒドロキシプロピル−β−シクロデキストリンである、請求項5に記載の凍結乾燥組成物。
- チピファルニブの凍結乾燥組成物を水および酸を含んでなる溶媒と混合する段階を含む、請求項1に記載の製薬学的調合物の製造方法。
- 酸が、酒石酸、マレイン酸、コハク酸およびクエン酸から選ばれる、請求項7に記載の製薬学的調合物の製造方法。
- 処置が請求項1の製薬学的調合物の投与を含むことを特徴とする、急性骨髄性白血病の処置のためのチピファルニブの使用。
- 処置が請求項1の製薬学的組成物の投与を含むことを特徴とする、乳がんの処置のためにチピファルニブの使用。
