1.発明の属する技術分野
本発明は、一般に、骨格関節に関係する組織の造影に関する。より詳細には、ラベルしたマーカーおよび関連する造影手段を使用した、筋骨格痛に関係する限局性因子の同定および/または特性解析に関する。
慢性背部痛(すなわち、一般的に12週間より長く持続する)は、米国において最も蔓延し、金のかかる非致命的な病態の1つであり、45歳未満の人々における障害の最大の原因であると考えられている。慢性背部痛で苦しむ人々の数は、全人口の25%を超えると見積もられている。毎年、米国の人口の約3−4%が一時的に障害を持つと見積もられ、労働年齢人口の約1%が、難治性の背部痛により、完全にかつ恒久的に障害者となると見積もられている。医学的に慢性背部痛を有する患者は、推計で1170万人存在する。この広く蔓延している病態のための国の障害費用は、1年につき300−700億ドルに及ぶ。この広く蔓延している病態を効果的に治療することは、依然として、医学における満たされていない最大の臨床上のニーズの1つである。疼痛の源を正確に診断し、場所を特定することもまた、成功すると予想されるターゲッティングされた手法でそのような治療を提供するためのクリティカル・パスにおいて、著しく不十分なままである。
したがって、本発明のある側面は、筋骨格関節の内部および周辺、特に有益な態様においては、背部痛に関連する脊椎椎間板の内部および周辺における、疼痛の源をより良く描写し、診断し、その場所を特定する、システム、組成物および方法を提供する。

椎間板起因の疼痛は、一般に多くの場合に多因子性の現象であると考えられている。特に、3つの実例となる因子を、椎間板起因の疼痛の発生と伝達に様々な様式で寄与する(またはその兆候である)と考えられる例として、ここに様々な詳細度で要約する。これら実例となる因子は、しばしば共存して組み合わせで作用し、度々同時に作用すると考えられている。これら因子のタイプは、以下のように要約される。

IL−1bおよびTNF−αは重複した、炎症促進効果をもたらす、共通のシグナルカスケードを活性化する、および同じようなターゲット遺伝子を誘発することが観察された(Faurの引例参照)。IL−1とTNF−αに対する炎症反応を媒介するエフェクターカスケードは、マイトジェンによって活性化されたタンパク質キナーゼ(MAPK)、NF−κβおよびプロスタグランジンシグナル伝達経路を含む(shalom-barak)。一酸化窒素シグナル分子は、また炎症性カスケードの重要な成分を形成し得る。
MAPKs形態は、細胞内のシグナル経路を、自己伝搬のリン酸化システム上に形成する(図6)。MAPKsの活性化は、サイトカイン受容体により誘発される極めて重要な細胞内の経路の1つである(Shalomberak)。3つのMAPKサブグループは特定された:細胞外のシグナル調節キナーゼ(ERK);JunNH2(アミノ)末端キナーゼ(JNK);およびp38(geng、その他)。軟骨細胞において、JNKおよびp38はIL−1およびTNF−αに対応してのみ見られるが、ERK活性は多様な刺激に対応して生じる(Firestein,Iiancini):このシグナル経路は、軟骨退化に関与すると考えられている(geng)。JNKおよびp38は、まとめてストレス活性化タンパク質キナーゼ(SAPKs)と呼ばれる。シグナルは、p38のリン酸化および活性化の代わりに、Rho系の膜近位小GTPアーゼ、MLK活性、並びにMKK3/6のリン酸化反応および活性によって開始される(Faur)。
さらに、MAPK誘導に関し、IL−1およびTNF−αは、NF−κβを活性化する。NF−κβは転写因子であり、未刺激の細胞の細胞質において潜在型で存在し、タンパク質抑制剤(Iκβ)に結合する転写活性のあるダイマー(p65およびp50)から構成される(Bowie,Magnani)。NF−κβは、多数の異なるシグナルにより活性化され、SAPKsを活性化する類似の細胞ストレスシグナルを含む。IL−1およびTNF−αは、Iκβのリン酸化反応および分解を誘発し、細胞核に入るためにNF−κβの放出を引き起こす(refs in Shalom;Baeuerle)。NF−κβ活性は、キナーゼ含有NF−κβ(NIK)から開始したカスケードを介して生じ、それからNF−κβ(Iκβ)キナーゼの阻害剤を活性化する。Iκβのリン酸化反応は、NF−κβに、コラゲナーゼ(MMP−1;Barchowsky)(Mengshol,magnani)及びCOX−2(Mifflin)を含有し、転写因子としての役割を果たすと共にそのターゲット遺伝子を制御する、細胞核へ転座することを可能としながら、Iκβ阻害サブユニットのユビキチン化および分解を引き起こす。図7は、このカスケードのいくつかの更なる詳細および成分間の関係を示す。
エイコサノイドは、自己分泌方法において役割を果たすシグナル分子である。炎症促進性刺激は、3つの酵素反応のカスケードを含むリン脂質由来のエイコサノイド合成の増加に導くことができる(図8)。第一にアラキドン酸(AA)は、ホスホリパーゼA2(PLA2)により、リン脂質貯蔵場所から遊離されている。次の律速段階は、シクロオキシゲナーゼ(COX)によるプロスタグランジン(H2)へのAAの転換である。

- 患者の組織の領域と関係する位置での、限局性の活発な疼痛の源に関する医学的処置の実施方法であって、
造影手段を用いて、疼痛因子を造影する能力を実質的に増大させるように、当該位置で該疼痛因子を人工的にラベリングすること;および
当該位置における第1の濃度のラベルされた疼痛因子と、当該位置に隣接する組織における第2の濃度のラベルされた疼痛因子とを、選択的に区別するのに十分な様式で、ラベルされた疼痛因子を造影することを含む方法。 - ターゲッティングされるラベルを含み、当該位置で疼痛の源と関係する疼痛因子に示差的に結合して、これをラベリングするように適合させた、実質的にターゲッティングされる薬剤を患者体内に送達させること;および

一定量のターゲッティングされる薬剤;

ここで該エネルギー送達システムは、患者にエネルギーを送達するように構成され;
