Abstract
Description
組織の封止剤および接着剤として使用するために損傷治癒力を向上させた血小板血漿濃縮物を調整する装置および方法に関する。その産物は、血中よりも数倍高い、十分に活性な(変性されていない)フィブリノゲン濃度、および血中よりも遥かに高い血小板濃度を有している。
血液は、分画することができ、かつ血液の異なる分画は、異なる医療的な必要物として使用される。重力および遠心力の影響下において、血液は、自然に3つの層になって沈降する。平衡状態において、上部の低密度の層は、血漿と呼ばれる淡い黄色の透明な液体である。血漿は、塩、代謝生成物、ペプチド、および小さなもの(インスリン)から大きなもの(補体の構成要素)まで、多くのタンパク質の水溶液である。
本発明の目的は、十分に活性を有する濃縮された血漿に懸濁されている血小板の濃縮物を生成する、小型および自己充足型のシステムを提供することである。上記血小板は、水の除去によって濃縮され、かつ十分に活性を有する型のフィブリノゲンが残っている血漿に懸濁された実質的に不活性な血小板である。
添付の図面とともに以下の詳細な説明を参照することによって、本発明のより全面的な適用例および付随の利点について、容易に理解することができるであろう。
本発明の装置および方法は、フィブリノゲンを変性させることなく、または赤血球を活性化させることなく、血液からPRPを分離し、かつ上記PRPから水を除去する。本発明の1つの局面は、治癒を促進および早める損傷治癒組織封止剤および損傷治癒組織吸着剤として有用な、患者の血液を用いて自家産物を提供する方法を実現するために、必要なもの内蔵した装置である。本発明の1つの局面に係る上記装置は、さらに携帯型の装置である。本発明の他の局面は、上記方法および産物を提供する常設の動力ユニットが用いられている、携帯型の使い捨てシステムである。さらに他の局面は、患者の血液からPRPを生産する携帯型の使い捨てシステムである。
Claims (19)
- 外傷患者に対する診察室用または急患用に適している、携帯用の、密閉された、簡単に処分できる、自己充足型の、および使い捨てのPRP分離装置であって、
上記PRP分離装置は、
遠心分離ユニットとモータとを備え、
500mAh未満の総消費電力を用いて3分未満の間に、血液から赤血球を分離して、PRPを生成することができ;
(a)上記遠心分離ユニットは、遠心分離ドラムを備えており、
上記遠心分離ドラムは、上端部および下端部を含む内壁表面、ドラムの底、ならびに中心軸を有し、
上記遠心分離ドラムの床は上記下端部から上記ドラムの底まで傾斜しており;
(b)上記モータは、上記遠心分離ユニットと接続された駆動軸を備え、
少なくとも120秒間に2000rpmの速度によって上記遠心分離ドラムを回転させることができる、
PRP分離装置。 - 上記モータには、少なくとも500mAhの電力を上記モータに供給するバッテリが、スイッチを介して接続されている請求項1に記載のPRP分離装置。
- 上記分離装置は、外部コンテナに密閉されており、
上記外部コンテナは、排気口と、中心のくぼみと一直線に配置された滅菌シリンジ流入口とを有しており、
接続チューブが、上記シリンジ流入口に接続され、かつ上記シリンジ流入口から下方に延長されている
請求項1に記載のPRP分離装置。 - 上記遠心分離ユニットの内壁表面が、赤血球を捕捉し、かつ保持する通路を有するデプスフィルタであり、
上記通路は、血液から赤血球を遠心分離する間において、上記通路の孔の中に移動する赤血球を捕捉し、かつ遠心分離が終了したとき、上記デプスフィルタに上記赤血球を保持する大きさを有し、
上記デプスフィルタは、上記デプスフィルタの表面と接触する血小板を有為に活性化させない材料から選択され、
赤血球は、遠心分離の間において血漿を介して外側に移動して、上記デプスフィルタを通り、かつ上記デプスフィルタによって捕捉され、
上記中心のくぼみに流れて行く上記PRPは、遠心分離が終了すると、実質的に赤血球が取り除かれている
請求項1に記載の携帯用の分離装置。 - 上記遠心分離の上記内壁表面は、上記中心軸から1度〜15度の角度において、上記底から外側に傾斜しており、
上記遠心分離ドラムの上端は、外部の環状を有する赤血球捕捉チャンバによって取り囲まれ、かつ上記赤血球捕捉チャンバと一体化されており、
上記赤血球捕捉チャンバは、外壁、内壁および下部容量を備え、
上記外壁は、上記内壁よりも高い頂部を有し、
上記赤血球捕捉チャンバの上記下部容量は、上記内壁の下にある容量であり、かつ有為な量の上記PRPを保持することなく、血液から分離された赤血球の総量を保持する大きさを有しており、
遠心分離の流れの間において上記血漿を介して外側に移動する赤血球は、遠心分離において上記赤血球捕捉チャンバの上記外壁にぶつかって保持され、かつ遠心分離が終了すると、上記赤血球捕捉チャンバの上記下部容量に集まる
請求項1に記載の携帯用の分離装置。 - 上記赤血球捕捉チャンバの上記内壁は、上記中央軸に対して相対的に少なくとも25度の角度“a”を形成する傾斜を有する上部を含み、
上記傾斜は、遠心分離の間において、上記PRPにおける血小板の流れが、上記赤血球捕捉チャンバから逆流し、かつあふれ出し易くする
請求項5に記載の携帯用の分離装置。 - 上記遠心分離ドラムの上記底に向かう上記接続チューブと接続するためのルアー取り付け部を有する血漿濃縮シリンジと組み合わせた
請求項1に記載の携帯用の分離装置。 - 上記血漿濃縮シリンジは、
内面および出入り口を有する円筒形の容器、
上記容器の上記内面と合わさる外面を有する円筒形の作動ピストン、
上記ピストンと上記出入り口との間配置された濃縮用乾燥ヒドロゲルビーズ、ならびに
上記出入り口を隣接して配置され、かつ上記出入り口からの上記濃縮用乾燥ヒドロゲルビーズの漏れを防止するフィルタ
を備えており、
上記出入り口から離れる方向へ上記ピストンが移動することによって、上記濃縮チャンバにPRPが引き込まれ、かつ上記濃縮用乾燥ヒドロゲルによって上記PRPから水が除去され、これによって、血漿における血小板の活性化またはフィブリノゲンの変性を起こさせることなく、上記PRPを濃縮し、かつ
上記出入り口に向かって上記ピストンを移動させることによって、上記出入り口を介して濃縮されたPRPを放出する
上記血漿濃縮シリンジと組み合わせた
請求項8に記載の分離装置。 - 外傷患者に対する診察室用または急患用に適している、携帯用の、密閉された、簡単に処分できる、自己充足の、および使い捨てのPRP分離装置および濃縮装置であって、
上記PRP分離装置は、遠心分離ユニットとモータとを備え、
上記PRP分離装置および濃縮装置は、
500mAh未満の総消費電力用いて、PRPを分離および濃縮する能力を有し;
(a)上記遠心分離ユニットが、遠心分離ドラム、上記遠心分離ドラムの中に延長されている流入チューブ、および上記遠心分離ドラムの中に延長されている流出チューブを含み、
上記遠心分離ドラムが、上端部および下端部を含む内壁表面、くぼみおよび上記上端部からくぼみまで下方に傾斜する床を有するドラムの底、ならびに中心軸を有する
上記ドラムの底が、くぼみおよび上記上端部からくぼみまで下方に傾斜する床を有し、
上記流出チューブが、上記くぼみと隣接して配置された注入口を有する上記遠心分離ユニットと;
(b)駆動軸を有する駆動モータが、上記遠心分離ユニットと接続され;
(c)中空糸カートリッジが、カートリッジの入り口、カートリッジの出口、および薄膜フィルタを有し、
上記薄膜フィルタが、水および塩を移動させ、かつ血液中に存在する凝固因子および治癒因子の移動を阻害する複数の孔を有し、
上記カートリッジの入り口が、流路を介して上記流出チューブに接続されており;
(d)引き込み口および引き出し口を有するポンプであって、上記引き込み口が流路を介して上記カートリッジの出口と接続され、かつ引き出し口が流路を介して上記流入チューブと接続されている
PRP分離装置および濃縮装置。 - 上記中空糸カートリッジは、中空糸、上記中空糸を取り囲む抽出液体チャンバ、および上記抽出液体チャンバと連絡している抽出液体排出チャンバを含んでいる請求項9に記載のPRP分離装置および濃縮装置。
- 上記抽出液体排出チャンバは、上壁および上記上壁にある通気口を有し、上記通気口は、下部の親水性膜および上部の疎水性膜によって塞がれており、
上記排出チャンバが満ちていくと、上記通気口を介して空気が排出され、
上記排出チャンバが満たされると、上記親水性膜および上記疎水性膜を浸す液体が液体の流れを阻害し、
上記システムにおける内圧が、上記排出チャンバから液体を引き込んでいるとき、浸された上記親水性膜が、上記排出チャンバに空気が流れ込むことを防止する
請求項10に記載のPRP分離装置および濃縮装置。 - 中心軸を有するPRP分離装置および濃縮装置であって、
固定ハウジング、および上記固定ハウジングにおいて上記中心軸の周りを回転するために取り付けられた回転アセンブリを備え、
上記固定ハウジングについて、
回転可能な上記アセンブリが、回転可能な遠心分離装置および濃縮装置、駆動モータ、および上記モータと回転可能な上記アセンブリと接続する連結部とを含み、上記モータおよび駆動連結は、回転可能な上記アセンブリを、上記回転軸の周りを回転させるために配置されており、
上記遠心分離装置は、内部の分離チャンバおよび外部の赤血球補足系を有し、
上記濃縮装置は、乾燥ビーズを含む濃縮チャンバを備え、上記濃縮チャンバは、床および複数の直立遮蔽物支持体を有し、上記直立遮蔽物支持体は、内面および外面を有し、上記直立遮蔽物支持体の上記外面に円筒形の遮蔽物が支持されており、
軸方向に同軸の固定チューブが、上記ハウジングに固定され、かつ上記固定チューブに固定された固定ビーズレーキを有する上記濃縮チャンバを介して伸びており、半径方向の外側に広がっている上記レーキは、上記直立遮蔽物支持体の内面に隣接している遠心端を有し、
上記固定ハウジングに対して上記回転アセンブリが低速に回転することによって、上記固定レーキを通過した上記ビーズが引っ張られ、ゲル分極が低下し、かつ上記ビーズが凝集する
PRP分離装置および濃縮装置。 - 隣接する直立遮蔽物支持体の組のそれぞれが、上記レーキの上記遠心端から半径方向の外側に乾燥ビーズを保持する乾燥ビーズ受容部を決定しており、高速回転段階における上記レーキによるビーズの破壊が実質的に回避される請求項12に記載のPRP分離装置および濃縮装置。
- モータ制御部をさらに含み、
上記駆動モータは、遠心による赤血球の分離およびPRP回収段階に必要な高い回転速度を有し、かつ乾燥ビーズによる水の除去に必要な低い回転速度を有し、
上記モータ制御部は、上記回転アセンブリの高いおよび低い回転速度を開始させるスイッチを含んでいる
請求項13に記載のPRP分離装置および濃縮装置。 - 上記スイッチは、遠心分離段階およびPRP濃縮回収段階において上記回転アセンブリの高い回転速度を起こし、かつPRP濃縮回収段階において上記回転アセンブリの低い回転速度を起こす請求項14に記載のPRP分離装置および濃縮装置。
- 中心軸を有する固定ハウジングを備える回転可能なPRP濃縮装置であって、
PRP濃縮装置は、駆動モータおよび回転可能な遠心分離PRP濃縮分離装置、上記駆動モータと接続している連結部、ならびに中心軸を有し、かつその中心軸の周りを回転する遠心分離PRP分離装置を備え、
上記PRP分離装置は、乾燥ビーズを含む濃縮チャンバを備え、上記濃縮チャンバは、床および直立遮蔽物支持体を有し、上記直立遮蔽物支持体は、内面および外面を有し、円筒形の遮蔽物が上記直立遮蔽物支持体の上記外面上に支持されており、
軸方向に同軸の固定チューブが、上記ハウジングに固定され、かつ上記濃縮チャンバを介して伸びており、固定ビーズレーキが、上記チューブに固定され、かつ半径方向の外側に広がって上記直立遮蔽物支持体の内面と隣接しており、
上記固定ハウジングに対して上記回転アセンブリが低速に回転することによって、上記固定レーキを通過した上記ビーズが引っ張られ、ゲル分極が低下し、かつ上記ビーズが凝集する
PRP濃縮装置。 - 隣接する直立遮蔽物支持体の組、およびそれらの間に伸びている遮蔽部分のそれぞれが、上記レーキの上記遠心端から半径方向の外側に乾燥ビーズを保持する乾燥ビーズ受容部を決定しており、高速回転段階における上記レーキによるビーズの破壊が実質的に回避される請求項16に記載のPRP濃縮装置。
- モータ制御部をさらに含み、
上記駆動モータは、PRP回収段階に必要な高い回転速度を有し、かつ乾燥ビーズによる水の除去に必要な低い回転速度を有し、
上記モータ制御部は、上記回転アセンブリの高いおよび低い回転速度を開始させるスイッチを含んでいる
