Description
本出願は、2005年2月7日に出願された仮出願No.60/651,050、2005年2月17日に出願された仮出願No.60/654,718、および2005年10月4日に出願された仮出願No.60/723,312の出願日に関して、35USC120の下での利益を主張する。
本発明は、組織封止剤(tissue sealant)および組織接着剤(tissue adhesive)として使用できる、改良された創傷治癒特性を有する血小板−血漿濃縮物を作製するための装置および方法に関するものである。上記濃縮物には、十分に活性な(変性していない)フィブリノーゲンが濃縮されており、当該フィブリノーゲンの濃度は、血液中および血小板中のフィブリノーゲン濃度よりも数倍高いとともに、血液中の血小板の濃度よりも高い。
血液は分画(fractionated)され得、血液の異なる画分(fraction)は、異なる医学上の目的に利用され得る。引力または遠心力が作用している状況下では、血液は、3つの層に自然に分離する。平衡状態では、最上部の低密度層は淡黄色の透明な液体であって、当該液体は、血漿(plasma)と呼ばれる。血漿は、塩、代謝産物、ペプチド、および低分子(インスリン)から巨大分子(補体成分)に至るまで多岐にわたる多くのタンパク質の水溶液である。
それ故に、本発明の目的は、新規のPRP濃縮物を調製するための装置および方法を提供することにある。なお、当該新規のPRP濃縮物では、濃縮された血漿中において血小板濃度が増加されており、当該濃縮された血漿中では、フィブリノーゲンは大して変性していない。
本発明の装置および方法は新規のPRP濃縮物を調製し、当該新規のPRP濃縮物では、濃縮された血漿中において血小板濃度が増加されており、当該濃縮された血漿中では、フィブリノーゲンは大して変性していない。上記新規の生産物は、ある種の手術において高く評価されている血漿濃縮物が有する封止特性および止血特性と、血小板濃度が上昇することによって付与される増強された治療特性とを併せ持っている。
乾燥したヒドロゲルビーズ19は濃縮されて、バスケット18の下半分を満たす。それ故、一方の側のみが空であって、窓186および網目188(図17および図18)の遮蔽されない視界を可能にする。
〔標準的なシステムの操作〕
本明細書に示されるとともに説明されている装置を用いて、血液が処理された。
1)最初の回転は、250rpmにて10秒間、行われた。当該回転によって、十分に低い回転(rpm)条件下で、ビーズがケージ(cage)内に投げ出される。なお、当該回転条件下では、初期の不均衡によって、過剰な振動が発生することはない。熊手の外側末端(最も外側の枝)は、ビーズのバランスをとるために、バスケットの周囲の近辺のビーズを平らにする。
2)赤血球は、3200rpmにて90秒間の赤血球分離用の回転によって分離された。これによって、赤血球は、デプス型フィルター内に詰め込まれた。
3)PRPは、50rpmにて45秒間の低速回転によって濃縮された。そして、PRPは濃縮チェンバー内に引き抜かれるとともに、上記PRPとビーズとが混ぜ合わされた。
4)次いで、PRP濃縮物は、3200rpmにて45秒間の最後の高速回転によって、上記ビーズから除去された。
Claims (19)
- 筺体と、前記筐体内の中心軸の周囲に配置されたPRP分離アセンブリーおよびPRP濃縮アセンブリーとを備えるPRP分離濃縮装置であって、
前記PRP濃縮アセンブリーは、
濃縮アセンブリーの床プレートに支持された乾燥ビーズと、網目によって閉じられる少なくとも1つの壁に設けられた開口を有する、床プレートに固定された壁と、を有する濃縮チェンバーを備え、
前記網目は、前記濃縮チェンバー中に前記乾燥ビーズを保持し得るように大きさが規定された開口を有し、
前記濃縮チェンバーは、PRP濃縮槽を備え、
静止した排出チューブは、前記筺体に固定されているとともに、前記PRP濃縮槽にまで伸びており、
前記PRP分離アセンブリーは、分離チェンバーを備え、前記PRP濃縮アセンブリーに取り付けられるとともに前記PRP濃縮アセンブリーの上側に配置されて、前記静止した排出チューブの周りを回転することができる一体化した分離濃縮装置複合体を形成し、
前記濃縮チェンバーと分離チェンバーとを接続する中央通路をさらに備えることを特徴とするPRP(血小板に富んだ血漿(platelet rich plasma))分離濃縮装置。 - 前記PRP濃縮槽は、PRP濃縮槽を形成する濃縮チェンバーの中心軸に向かって傾斜したバケット底部によって規定されていることを特徴とする請求項1に記載のPRP分離濃縮装置。
- 前記分離チェンバーから濃縮チェンバーへのPRPの流れを制御するバルブアセンブリーをさらに備えることを特徴とする請求項1に記載のPRP分離濃縮装置。
- 筺体と、前記筐体内の中心軸の周囲に配置されたPRP分離アセンブリーおよびPRP濃縮アセンブリーとを備えるPRP分離濃縮装置であって、
前記PRP濃縮アセンブリーは、
床プレートに支持された乾燥ビーズと、網目によって閉じられる少なくとも1つの開口を有する壁と、を有する濃縮チェンバーを備え、
前記網目は、前記濃縮チェンバー中に前記乾燥ビーズを保持し得るように大きさが規定された開口を有し、
前記濃縮チェンバーは、PRP濃縮槽を備え、
静止した排出チューブは、前記筺体に固定されているとともに、前記PRP濃縮槽にまで伸びており、
前記PRP分離アセンブリーは、分離チェンバーを備え、前記PRP濃縮アセンブリーに取り付けられるとともに前記PRP濃縮アセンブリーの上側に配置されて、前記静止した排出チューブの周りを回転することができる一体化した分離濃縮装置複合体を形成し、
前記濃縮チェンバーと分離チェンバーとを接続する中央通路をさらに備え、
前記濃縮チェンバー内に、乾燥ビーズとPRPとを撹拌する静止した熊手をさらに備えることを特徴とするPRP(血小板に富んだ血漿(platelet rich plasma))分離濃縮装置。 - 前記熊手が排出チューブに固定されていることを特徴とする請求項4に記載のPRP分離濃縮装置。
- 前記PRP分離アセンブリー内にデプス型フィルターをさらに備え、PRP分離アセンブリー内のデプス型フィルター中に血液中の赤血球を捕捉することによって血液からPRPが調製され、前記PRP中の水を、PRP濃縮アセンブリー内に保持された乾燥ビーズで吸収することによってPRP濃縮物が調製されることを特徴とする請求項4に記載のPRP分離濃縮装置。
- 前記PRP濃縮槽は、PRP濃縮槽を形成する濃縮チェンバーの中心軸に向かって傾斜したバケット底部によって規定されていることを特徴とする請求項4に記載のPRP分離濃縮装置。
- 前記分離チェンバーから濃縮チェンバーへのPRPの流れを制御するバルブアセンブリーをさらに備えることを特徴とする請求項4に記載のPRP分離濃縮装置。
- PRP分離アセンブリー内のデプス型フィルター中に血液中の赤血球を捕捉することによって血液からPRPを調製し、前記PRP中の水を、PRP分離アセンブリーの下側に配置され、PRP濃縮アセンブリー内のPRP濃縮槽にまで伸びている静止した排出チューブを備えたPRP濃縮アセンブリー内に保持された乾燥ビーズに吸収させることによってPRP濃縮物を調製する、請求項1または4に記載のPRP分離濃縮装置の作動方法において、
PRP濃縮アセンブリー内の回転している濃縮チェンバー内において、乾燥ビーズを撹拌しながら、前記PRPと乾燥ビーズとを接触させることによってPRP濃縮物を調製することを特徴とするPRP分離濃縮装置の作動方法。 - 前記PRP濃縮物は、前記濃縮チェンバーを遠心分離の速度にて回転させて、前記ビーズからPRP濃縮物を分離することによって回収されることを特徴とする請求項9に記載のPRP分離濃縮装置の作動方法。
- 前記PRP分離アセンブリーは、デプス型フィルターによって裏打ちされる平衡が保たれた円筒状の分離チェンバーを備え、前記分離チェンバーは放射状に伸びるプレートによって分離領域に区分され、前記分離チェンバーが回転している間、前記分離領域は分離動作を通じて振動を減少させることを特徴とする請求項1または4に記載のPRP分離濃縮装置。
- 前記プレートは、前記分離チェンバーの外壁から、前記分離チェンバーの中心に向かって放射状に伸びていることを特徴とする請求項11に記載のPRP分離濃縮装置。
- 前記分離チェンバーから濃縮チェンバーへのPRPの流れを制御するバルブアセンブリーをさらに備えることを特徴とする請求項11に記載のPRP分離濃縮装置。
- 前記濃縮チェンバー内に、乾燥ビーズとPRPとを撹拌する静止した熊手をさらに備えることを特徴とする請求項1に記載のPRP分離濃縮装置。
- PRP分離アセンブリー内のデプス型フィルター中に血液中の赤血球を捕捉することによって血液からPRPが調製され、前記PRP中の水を、PRP濃縮アセンブリー内に保持された乾燥ビーズに吸収させることによってPRP濃縮物が調製され、
前記プレートは、前記分離チェンバーが回転している間、前記分離領域内の血液の分布が実質的に平衡となるように保ち、前記プレートによって、振動を減少させるとともに、デプス型フィルターからの赤血球の移動を減少させることを特徴とする請求項11に記載のPRP分離濃縮装置。 - PRP分離アセンブリー内のデプス型フィルター中に血液中の赤血球を捕捉することによって血液からPRPを調製する工程と、前記PRP中の水を、PRP分離アセンブリーに隣接されたPRP濃縮アセンブリー内に保持された乾燥ビーズに吸収することによってPRP濃縮物を調製する工程と、を含む請求項11に記載のPRP分離濃縮装置の作動方法において、
前記デプス型フィルターによって裏打ちされた分離チェンバー内において、遠心分離の速度にて血液を回転させることによって、前記赤血球が捕捉され、
前記分離チェンバーおよびデプス型フィルターは、放射状に伸びるプレートによって分離領域に区分され、
前記プレートは、前記分離チェンバーが回転している間、前記分離領域内の血液の分布が実質的に平衡となるように保ち、前記プレートによって、振動を減少させるとともに、デプス型フィルターからの赤血球の移動を減少させることを特徴とするPRP分離濃縮装置の作動方法。 - 前記分離チェンバーの回転速度は、前記分離領域内の血液の分布が平衡となる割合で加速され、
前記遠心分離が終了した後、前記分離チェンバーの回転速度は、前記分離領域内のPRPの分布が平衡となる割合で減速され、
前記割合にて加速および減速することによって、振動を減少させるとともに、前記デプス型フィルターからの赤血球の移動を減少させることを特徴とする請求項16に記載のPRP分離濃縮装置の作動方法。 - 前記ビーズが固定された熊手によって撹拌されている間に、PRP濃縮物を製造するために、PRP濃縮アセンブリーが備える回転している濃縮チェンバー内において、前記PRPと乾燥ビーズとを接触させることを特徴とする請求項16に記載のPRP分離濃縮装置の作動方法。
- 前記PRP濃縮物は、前記濃縮チェンバーを遠心分離の速度にて回転させて、前記ビーズからPRP濃縮物を分離することによって回収されることを特徴とする請求項18に記載のPRP分離濃縮装置の作動方法。
