Abstract
【解決手段】
ヒト又は動物の患者の脈管部位(例えば、動脈瘤)を閉塞するアッセンブリであり、脈管閉塞部材と、遠位端と、この遠位端の近位側に位置する分離可能な接合部を有する押し出し部材と、前記押し出し部材を前記閉塞部材に連結する回動可能なカップリングとを具える。分離可能な接合部を切断して、前記押し出し部材から前記脈管閉塞部材を切り離すので、脈管閉塞部材によって近位方向に軸方向の力がかかり、前記回動可能なカップリングを曲げて、軸方向の力が横方向に変位する。前記回動可能なカップリングの屈曲によって生じる前記軸方向の力の横方向への変位が、前記送出カテーテルが、前記軸方向の力によって、前記動脈瘤ネックからずれることを防ぐ。

Description
本発明は、ヒト又は動物の患者の血管中に塞栓又は脈管閉塞を確立するためのインビボ脈管閉塞デバイスをインプラントするアッセンブリに関する。
脈管閉塞デバイス又はインプラントは、様々な理由で用いられている。これらは、脈管内の動脈瘤の治療にしばしば用いられている。つまり、この治療は、動脈瘤内への脈管閉塞デバイスの配設を含んでおり、凝血塊を形成し、最終的にはこの脈管閉塞デバイスを含むコラーゲン塊の生成を形成させる。これらの閉塞は、動脈瘤を塞いで満たし、これによって、動脈瘤の脆弱した壁が、開放脈管内腔の脈動する血圧に曝露されるのを防ぐ。このような動脈瘤の治療は、一般的に行われている外科的方法の重要な改良である。
本発明の一の実施例によれば、ヒト又は動物の患者の脈管中に塞栓又は脈管閉塞を確立するアッセンブリが提供されている。このアッセンブリは、一般的に、脈管閉塞部材(例えば、コイル)と、遠位端とこの遠位端の近位に配置された分離可能な接合部(例えば、機械的又は電気分解によって分離可能な接合部)を有する押し出し部材と、前記閉塞デバイスに押し出し部材を連結する回動可能なカップリングとを具える。このアッセンブリは選択的にカテーテルを具えていてもよく、その中に前記押し出し部材が摺動的に配置されている。脈管閉塞部材がコイルを具える場合、前記アッセンブリは、選択的に抗伸長部材を具えていてもよく、この場合、前記回動可能なカップリングは、コイルの内腔内で前記抗伸長部材を固定することができる。抗伸長部材の固定を選択的に提供することに加えて、回動可能なカップリングは、押し出し部材の遠位端を脈管閉塞部材から電気的に絶縁する構成要素を含んでいてもよい。回動可能なカップリングは、種々の方法のいずれか1つで作られることができる。
図1を参照すると、一の実施例によって構成された脈管閉塞アッセンブリ10が示されている。方向付けの目的で、図に示すように、「近位」の用語は、ある要素を限定する場合に一般的にこの要素の左を意味し、「右」の用語は、ある要素を限定する場合に、一般的にこの要素の右端を意味する。脈管閉塞アッセンブリ10は、一般的に、細長の筒状カテーテル16と、押し出し部材18を有する送出デバイス12と、送出デバイス12の遠位端と、特に押し出し部材18の遠位端に取り外し可能に連結した脈管閉塞デバイス14とを具える。
Claims (18)
- ヒト又は動物の患者の脈管部位を閉塞するアッセンブリにおいて、前記アッセンブリが:
脈管閉塞部材と;
遠位端と前記遠位端の近位に位置する分離可能な接合部を有する押し出し部材と;
前記閉塞部材に前記押し出し部材を連結する回動可能なカップリングと;
を具えることを特徴とするアッセンブリ。 - 請求項1に記載のアッセンブリにおいて、前記分離可能な接合部が、機械的に分離可能な接合部を具えることを特徴とするアッセンブリ。
- 請求項1に記載のアッセンブリにおいて、前記分離可能な接合部が、電気分解で分離可能な接合部を具えることを特徴とするアッセンブリ。
- 請求項1に記載のアッセンブリにおいて、前記脈管閉塞部材がコイルを具えることを特徴とするアッセンブリ。
- 請求項4に記載のアッセンブリが、抗伸縮部材を更に具え、前記回動可能なカップリングが、前記コイルの内腔内で、前記抗伸縮部材を固定していることを特徴とするアッセンブリ。
- 請求項1に記載のアッセンブリにおいて、前記回動可能なカップリングが、脈管閉塞部材から前記押し出し部材の遠位端を電気的に絶縁する要素を具えることを特徴とするアッセンブリ。
- 請求項1に記載のアッセンブリが、前記押し出し部材を摺動自在に配置する送出内腔を有するカテーテルを更に具えることを特徴とするアッセンブリ。
- 請求項1に記載のアッセンブリにおいて、前記回動可能なカップリングが、前記押し出し部材の遠位端と脈管閉塞部材との間に連結された可撓性スリーブを具えることを特徴とするアッセンブリ。
- 請求項8に記載のアッセンブリにおいて、前記スリーブがゴム状弾性であることを特徴とするアッセンブリ。
- 請求項8に記載のアッセンブリにおいて、前記回動可能なカップリングが更に:
前記押し出し部材の遠位端に連結した近位側コイルと;
前記脈管閉塞部材に連結した遠位側コイルと;を具え、前記スリーブが前記近位側コイルと遠位側コイルとの上に配置されていることを特徴とするアッセンブリ。 - 請求項1に記載のアッセンブリにおいて、前記回動可能なカップリングが:
前記押し出し部材の遠位端に連結した近位側リンク部材と;
前記脈管閉塞部材に連結した遠位側リンク部材と;
を具え、前記近位側リンク部材及び遠位側リンク部材が、互いに回動可能に係合している
ことを特徴とするアッセンブリ。 - 請求項11に記載のアッセンブリにおいて、前記近位側リンク部材及び遠位側リンク部材が、フック又はループであることを特徴とするアッセンブリ。
- 請求項11に記載のアッセンブリが:
前記押し出し部材の遠位端に連結した近位側コイルであって、前記近位側リンク部材が、前記近位側コイルの遠位端上に配置されている近位側コイルと;
前記脈管閉塞部材に連結した遠位側コイルであって、前記遠位側リンク部材が、前記遠位側コイルの近位端上に配置されている遠位側コイルと;
を更に具えることを特徴とするアッセンブリ。 - 請求項1に記載のアッセンブリにおいて、前記回動可能なカップリングが:
前記押し出し部材の遠位端に連結した第1ボール部材と;
前記脈管閉塞部材に連結した第2ボール部材と;
前記第1及び第2ボール部材を保持するスリーブと;
を具えることを特徴とするアッセンブリ。 - 請求項14に記載のアッセンブリにおいて、前記スリーブが、メッシュ又はブレイドを具えることを特徴とするアッセンブリ。
- 請求項14に記載のアッセンブリにおいて、前記回動可能なカップリングが更に、前記脈管閉塞部材に取り付けたコイルを具え、前記第1ボール部材が前記押し出し部材の遠位端上に形成されており、前記第2ボール部材が前記コイルの近位端上に形成されていることを特徴とするアッセンブリ。
- 請求項1に記載のアッセンブリにおいて、前記回動可能なカップリングが、前記押し出し部材の遠位端と前記脈管閉塞部材との間に連結した第1コイルを具え、前記第1コイルが、間に空間をおいた、開ピッチの巻線を具え、前記脈管閉塞部材が、前記開ピッチの巻線のいくつかの間の前記空間内に配置した近位側巻線を具える脈管閉塞コイルを具え、前記開ピッチの巻線間の空間の少なくともいくつかが空のままであることを特徴とするアッセンブリ。
- 請求項17に記載のアッセンブリにおいて、前記回動可能なカップリングが、前記押し出し部材の遠位端と前記第1コイルの間に連結した第2コイルを具え、前記第2コイルが、前記開ピッチの巻線のいくつかの間の空間内に配置した遠位側巻線を具えることを特徴とするアッセンブリ。
