Description
Claims (25)
- 自立した心臓の感知及びエネルギー送出を提供するように作動可能な胸腔内心臓刺激デバイスであって、
患者の心臓に対して胸腔内に配置されるよう構成されたハウジングと、
前記ハウジングによって支持され、心臓組織又は心血管系内の植込み位置に前記ハウジングを固定するように構成された固定構成と、
前記ハウジングによって支持され、心臓活動を感知して前記心臓組織又は心血管系に刺激エネルギーを送出するように構成された電極構成と、
前記ハウジング内に設けられ、前記電極構成に結合されたエネルギー送出回路と、
前記ハウジング内に設けられ、前記電極構成に結合された検出回路と、
前記ハウジング内に設けられ、前記エネルギー送出回路及び前記検出回路に結合されたコントローラであって、前記植込み位置にふさわしいエネルギー送出プロトコルに従って前記心臓組織又は心血管系へのエネルギーの送出を調整する、コントローラと、
を含む、胸腔内心臓刺激デバイス。 - 前記電極構成が心外膜又は心内膜電極構成を含む、請求項1に記載のデバイス。
- 前記電極構成が静脈内電極構成を含む、請求項1に記載のデバイス。
- 前記電極構成が前記ハウジングの一部の周囲に支持された1つ以上の環状電極を含む、請求項1に記載のデバイス。
- 前記ハウジングが、植込み駆動部材の遠位端の受入と摘出部材取付機構の受入のうちの一方又は双方のために構成された1つ以上の凹部を含む、請求項1に記載のデバイス。
- 前記エネルギー送出プロトコルが前記心臓の特定の心腔にふさわしいペーシングプロトコルを含む、請求項1に記載のデバイス。
- 前記エネルギー送出プロトコルが多心腔ペーシングプロトコルを含む、請求項1に記載のデバイス。
- 前記エネルギー送出プロトコルが除細動又はカルディオバージョンプロトコルを含む、請求項1に記載のデバイス。
- 前記コントローラに結合された通信回路を更に含み、前記コントローラは患者の体外にあるシステムとの通信を前記通信回路を通じて調整する、請求項1に記載のデバイス。
- 前記コントローラに結合された通信回路を更に含み、前記コントローラは1つ以上の他の胸腔内心臓刺激デバイスとの通信を前記通信回路を通じて調整する、請求項1に記載のデバイス。
- 各々が前記患者の心臓に対して胸腔内に配置されるよう構成された請求項1に記載の心臓刺激デバイスを複数含むシステムであって、前記複数の心臓刺激デバイスの各々の前記コントローラは、前記複数の心臓刺激デバイスの各々が固定された前記植込み位置にふさわしいエネルギー送出プロトコルに従ってエネルギーの送出を調整する、システム。
- 自立した心臓の感知及びエネルギー送出を提供するように作動可能な胸腔内心臓刺激デバイスであって、
複数の胸腔内位置から心臓活動を感知する手段と、
感知された前記心臓活動に応じて前記複数の胸腔内位置から心臓組織又は心血管系にエネルギーを送出する手段と、
分散型エネルギー送出プロトコルに従って、前記複数の胸腔内位置で前記エネルギーの送出を調整する手段と、
を含む、胸腔内心臓刺激デバイス。 - 前記エネルギー送出プロトコルがペーシングプロトコルを含む、請求項12に記載のデバイス。
- 前記エネルギー送出プロトコルが再同期ペーシングプロトコルを含む、請求項12に記載のデバイス。
- 前記エネルギー送出プロトコルが、共通の心腔を用いて多部位ペーシングを行うためのペーシングプロトコルを含む、請求項12に記載のデバイス。
- 前記エネルギー送出プロトコルが刺激エネルギー送出プロトコルを含み、該刺激エネルギー送出プロトコルに関連する刺激エネルギーは捕捉を行うには不十分である、請求項12に記載のデバイス。
- 前記エネルギー送出プロトコルが除細動又はカルディオバージョンプロトコルを含む、請求項12に記載のデバイス。
- 前記エネルギーの送出を調整する前記手段が、前記エネルギーの送出の調整を容易にするように前記複数の胸腔内位置間の通信を行う手段を含む、請求項12に記載のデバイス。
- 前記複数の胸腔内位置のうちの少なくとも1つと患者の体外の位置との間の通信リンクを確立する手段を更に含む、請求項12に記載のデバイス。
- 自立した心臓の感知を提供するように作動可能な胸腔内の心臓モニタシステムであって、
心臓の複数の位置に配置されるよう構成された複数の単体感知ユニットを含み、前記単体感知ユニットの各々が、
患者の心臓に対して胸腔内に配置されるよう構成されたハウジングと、
前記ハウジングによって支持され、心臓組織又は心血管系内の植込み位置に前記ハウ
ジングを固定するように構成された固定構成と、
前記ハウジングによって支持され、心臓活動を感知するように構成された電極構成と、
前記ハウジング内に設けられ、前記電極構成に結合された検出回路と、
前記ハウジング内に設けられ、前記検出回路に結合されたコントローラであって、前
記植込み位置にふさわしい心臓感知プロトコルに従って心臓の電気的な活動の感知を調
整する、コントローラと、
を含む、胸腔内の心臓モニタシステム。 - 前記複数の単体感知ユニットの各々の前記コントローラが、前記複数の単体感知ユニットのうちの他の単体感知ユニットに対して心臓の電気的活動の感知を独立して調整する、請求項20に記載のシステム。
- 前記複数の単体感知ユニットの各々の前記コントローラが、前記複数の単体感知ユニットのうち少なくとも1つの他の単体感知ユニットと協働して心臓の電気的活動の感知を調整する、請求項20に記載のシステム。
- 前記コントローラに結合された通信回路を更に含み、前記コントローラは患者の体外にあるシステムとの通信を前記通信回路を通じて調整する、請求項20に記載のシステム。
- 前記患者の体外にあるシステムが、前記複数の単体感知ユニットのうち少なくとも1つの前記コントローラと協働して心臓の電気的活動の感知を調整する、請求項23に記載のシステム。
- 前記患者の体外にあるシステムが、前記複数の単体感知ユニットの各々の前記コントローラと協働して心臓の電気的活動の感知を調整する、請求項23に記載のシステム。
