Abstract
Description

公知の中間脊椎移植体の場合、頭部は、雄ねじを担持する円筒状部分と、端部領域で隣接する円錐状のねじのない部分とを有する。この中間脊椎移植体により、危険なしに、椎骨体の特に腹側周囲の完全な延伸を行うことは不可能であるが、この理由は、隣接した大静脈及び大動脈の損傷の危険が、完全には排除されないからである。

c)前記中央部分(5、6、7、8)に成形された下部構造(3)とを有し、
d)前記中央部分(5、6、7、8)が、前記頭部(2)から前記下部構造(3)に延びる複数のストラットによって形成され、この結果、前記頭部(2)と前記ストラット(5、6、7、8)と前記下部構造(3)との間に、大きな面積の窓部(9、10、11、12)が開放したままである中間脊椎移植体において、
e)前記頭部(2)の前記雄ねじ(14)が、前記円錐状部分(13)に形成され、

- 前記保護隆起部(15)が、閉じたキャップの形状を有することを特徴とする、請求項1〜4のいずれか1項に記載の中間脊椎移植体。
- 前記保護隆起部がリング状であり、かつ中央導通開口部を備えることを特徴とする、請求項1〜4のいずれか1項に記載の中間脊椎移植体。
- 前記下部構造(3)が、少なくとも5mmの軸方向延長部(D5)を備えることを特徴とする、請求項1〜6のいずれか1項に記載の中間脊椎移植体。
- 前記下部構造(3)がプレートであることを特徴とする、請求項1〜7のいずれか1項に記載の中間脊椎移植体。
- 前記下部構造(3)に、ねじ込み器具用の把持装置(18、19)が設けられることを特徴とする、請求項1〜8のいずれか1項に記載の中間脊椎移植体。
- 前記頭部(2)の前記円錐状部分(13)の円錐状外面と、想定される同軸の円筒外面とによって画定される角度(α)が、10°〜20°、好ましくは15°であることを特徴とする、請求項1〜9のいずれか1項に記載の中間脊椎移植体。
- 前記中央部分(5、6、7、8)及び前記下部構造(3)が、前記頭部(2)の前記円錐状部分(15)に対し逆方向に円錐状であることを特徴とする、請求項1〜10のいずれか1項に記載の中間脊椎移植体。
- 前記中央部分(5、6、7、8)及び前記下部構造(3)の前記円錐状外面と、想定される同軸の円筒外面とによって画定される角度(β)が約3°であることを特徴とする、請求項11に記載の中間脊椎移植体。
- 前記中央部分(5、6、7、8)及び前記下部構造(3)の前記円錐状外面と、想定される同軸の円筒外面とによって画定される角度(β)が約6°であることを特徴とする、請求項11に記載の中間脊椎移植体。
- ステンレス鋼から構成されることを特徴とする、請求項1〜13のいずれか1項に記載の中間脊椎移植体。
- 炭素セラミック材料から構成されることを特徴とする、請求項1〜13のいずれか1項に記載の中間脊椎移植体。
- アルミニウム合金から構成されることを特徴とする、請求項1〜13のいずれか1項に記載の中間脊椎移植体。
- チタンから構成されることを特徴とする、請求項1〜13のいずれか1項に記載の中間脊椎移植体。
- プラスチックから構成されることを特徴とする、請求項1〜13のいずれか1項に記載の中間脊椎移植体。
- ポリイソシアネートから構成されることを特徴とする、請求項18に記載の中間脊椎移植体。
- 生体吸収可能な材料から構成されることを特徴とする、請求項1〜13のいずれか1項に記載の中間脊椎移植体。
- ポリアクタイドから構成されることを特徴とする、請求項20に記載の中間脊椎移植体。
- 少なくとも1つの軸方向領域で、非回転対称の断面形状を備えることを特徴とする、請求項1〜21のいずれか1項に記載の中間脊椎移植体。
- 前記下部構造(3)が、非回転対称の断面形状を備えることを特徴とする、請求項22に記載の中間脊椎移植体。
- 前記保護隆起部(159が研磨されることを特徴とする、請求項1〜23のいずれか1項に記載の中間脊椎移植体。
- 2つの隣接した椎骨体(30、31)の間の椎骨の間隙に、少なくとも1つの中間脊椎移植体(1)が挿入される脊柱の脊椎固定術のための外科手術法において、
次の方法の段階、すなわち、
a)前記椎骨の間隙への入口が、顕微手術により小さな脊椎関節を部分除去して背側側面から開かれる段階と、
b)前記椎骨の間隙の上板及び底板の軟骨が取り除かれる段階と、
c)前記椎骨の間隙の背側領域に、ねじが切り込まれる段階と、
e)延伸器具を用いて前記椎骨の間隙が事前延伸される段階と、
f)前記事前延伸がスペーサ要素を用いて直立に保持されて、前記延伸器具が取り除かれる段階と、
g)前記椎骨の間隙のねじに、第1の中間脊椎移植体(1)がねじ込まれる段階と、
h)前記第1の中間脊椎移植体(1)から間隔を置いて挿入される第2の中間脊椎移植体(1’)のために、a)及びb)の前記段階が繰り返される段階と、を含む方法。 - 前記段階a)の後に、靱帯フラバムが外側から内側に薄くされることを特徴とする、請求項25に記載の方法。
- 前記段階b)の後に、露出された骨部分が新鮮にされることを特徴とする、請求項25又は26に記載の方法。
- 前記中間脊椎移植体(1、1’)が、前記椎骨の間隙に導入する際に海綿質で充填されることを特徴とする請求項25〜27のいずれか1項に記載の方法。
- 前記椎骨の間隙が、遅くとも第2の中間脊椎移植体(11)を導入する前に海綿質で充填されることを特徴とする、請求項25〜28のいずれか1項に記載の方法。
- 請求項1〜23のいずれか1項に記載の中間脊椎移植体(1)が使用され、前記頭部(2)の保護隆起部(15)と前記円錐状領域(13)との間の移行線(20)が、前記椎骨体(30、31)の腹側周囲の側方収容部の頂点(32、33)の高さに位置するまで、特に腹側領域を延伸しつつ、前記中間脊椎移植体がねじ込まれることを特徴とする請求項25〜29のいずれか1項に記載の方法。
- 前記中間脊椎移植体(1、1’)のねじ込みの際に、トルクが測定され、所定のトルクを下回った場合、より大きな最大直径(D2)を有する中間脊椎移植体(1、1’)が選択されることを特徴とする、請求項30に記載の方法。
