Abstract
Description
2および先端装置304を備える。先端装置304は吸引するための吸引管腔の詰まりを除去したり、塞栓を細分したりすることに使用する。カテーテル302は基端に取着されたマニホールド306を備え、マニホールド306は第1のポート308と第2のポート310とを有する。先端装置304は基端312を備え、基端312はカテーテル302の管腔中に配置された長尺状部材314に取着されている。先端装置304は弓形をなす形状を有したり、塞栓を細分するためのループ状、コイル状、パドル状、ほうき状、ジグザグ状、らせん状、または他の適切な形状をなしていたりする。長尺状部材314の基端側端部は固定されていなかったり、長尺状部材314の軸線に沿った動作付与することが可能な動作制御装置に取着されていたりする。動作制御装置は長尺状部材314の先端を、長手方向もしくは径方向の動作、または振動を行わせることができる。カテーテル302はまた真空源と連通している。
図1cは塞栓摘出装置500を図示する。装置500は装置300と類似であり、第1の管腔520と第2の管腔522とを備える。長尺状部材314は第1の管腔520内に配置され、真空源は第2の管腔522と連通している。使用時には、塞栓摘出装置500を塞栓に近接して配置し、真空源を稼動させる。先端装置304は手動によったり、動作制御装置を介したりして操作され、吸引管腔または他の管腔を開通させたり、塞栓を細分したりする。それにより先端装置304は塞栓を細分し、塞栓または塞栓の断片は第2の管腔522中に吸引される。先端装置304は、長尺状部材314の先端における振動または脈動により塞栓を変形させ、内腔の詰まりを除去したり、塞栓を細分したりする。これに代えて、第1の管腔520または付加された管腔を介して流体を灌注することができる。代替案または付加案として、先端装置304を用いて塞栓という負荷を除去することにより管腔から塞栓を除去し、それにより一層効果的な吸引のための開放された流路を形成することができる。
204はガイドカテーテル206内に配置されており、先端はほぼ平坦であり塞栓に近接して位置する。塞栓塊牽引装置212はその後シース204に挿入され、塞栓を捕捉すなわち保持する。さらにカテーテル206は基端方向に移動し、先端部210はガイドカテーテル206よりも先端側に配置され、図2dに図示するように拡径する。これに代えて、回収シース204をガイドカテーテル206よりも先端方向に移動させ、先端部210を拡径させることができる。その後塞栓塊牽引装置212は、塞栓が拡径可能な先端部210中に位置するように移動される。回収シース204および塞栓塊牽引装置212は、その後ガイドカテーテル206から基端方向に抜去される。所望であれば、塞栓物質は回収シース204および塞栓塊牽引装置212から除去され、これらの装置はガイドカテーテル206に再度挿入される。他の方法が想起されることは言うまでもない。例えば、回収シース204は先端方向に押圧され、先端部210は拡径され、それにより塞栓塊牽引装置212は回収シース204を介して先端側に挿入される。
Claims (29)
- 塞栓を変形させるためのカテーテルであって、
基端、先端、および同基端と先端との間に延びる管腔を有する第1の長尺状シャフトと、
第1の長尺状シャフトの管腔中に少なくとも部分的に配置された第2の長尺状シャフトと、第2のシャフトは基端、先端、および同基端と先端との間に延びる管腔を有することと、
第2のシャフトの先端に配置されるとともに、第2のシャフトの管腔と連通する空洞と先端側の開口とを有する先端部と、該先端部は第1の状態と第2の状態との間で変動可能であって、同先端側の開口は第2の状態において第1の状態よりも断面積が大きいことと、
第2の長尺状シャフトの管腔内に少なくとも部分的に配置された、塞栓を変形させるための振動可能なワイヤとを備えるカテーテル。 - ワイヤは第2の長尺状シャフトの管腔の詰まりを除去するように構成された請求項1に記載のカテーテル。
- ワイヤは塞栓を細分するように構成された請求項1に記載のカテーテル。
- 前記空洞は第2の状態において第1の状態よりも大きな体積を有する請求項1に記載のカテーテル。
- 第1の状態において前記先端側の開口は第1の長尺状シャフトの先端よりも基端側に位置し、第2の状態において前記先端側の開口は第1の長尺状シャフトの先端よりも先端側に位置する請求項1に記載のカテーテル。
- 第2の長尺状シャフトは形状記憶ポリウレタンよりなる請求項1に記載のカテーテル。
- 第2の長尺状シャフトはニチノール巻回シートカテーテルである請求項1に記載のカテーテル。
- 第2の長尺状部材は拡径可能な網組を備える請求項1に記載のカテーテル。
- 第1のシャフトの先端と連通した真空源をさらに備える請求項1に記載のカテーテル。
- 第2のシャフトの先端と連通した真空源をさらに備える請求項1に記載のカテーテル。
- 第2の長尺状シャフトの管腔内に配置された塞栓塊牽引装置をさらに備える請求項1に記載のカテーテル。
- 第2の長尺状シャフトの管腔中に少なくとも部分的に配置されるとともに、管腔を有する第3の長尺状シャフトをさらに備え、塞栓塊牽引装置はカテーテルの管腔中に少なくとも部分的に配置された請求項11に記載のカテーテル。
- 第2の状態において、前記先端側の開口は第1の長尺状シャフトの管腔の断面積より、先端において大きな断面積を有する請求項1に記載のカテーテル。
- 基端、先端、および同基端と先端との間に延びる第1の管腔を有する第1の長尺状シャフトと、
第1の長尺状シャフト中に少なくとも部分的に配置された基端および先端を有するワイヤと、同先端は第1の長尺状シャフトから先端方向に延伸することと、
ワイヤの基端に連結された動作制御装置とを備えるカテーテル。 - 動作制御装置はワイヤに対して振動を付与することが可能な請求項14に記載のカテーテル。
- 前記振動は約20kHz未満の周波数を備える請求項15に記載のカテーテル。
- 前記振動は約1Hzと約150Hzとの間の周波数を備える請求項16に記載のカテーテル。
- 前記振動は軸線方向の動作である請求項15に記載のカテーテル。
- ワイヤの先端に取着され、かつ塞栓を変形させるための装置をさらに備える請求項14に記載のカテーテル。
- 該装置は塞栓を変形させ、カテーテル管腔の先端側の詰まりを除去するように構成された請求項19に記載のカテーテル。
- 該装置は塞栓を細分するように構成された請求項19に記載のカテーテル。
- 該装置は弓形のワイヤである請求項19に記載のカテーテル。
- 該装置はジグザグ形状を有するワイヤである請求項19に記載のカテーテル。
- 該装置はループである請求項19に記載のカテーテル。
- 該装置は基端方向に約20mm、先端方向に約100mmの動作範囲を有する請求項19に記載のカテーテル。
- 該装置は基端方向に約2mm、先端方向に約15mmの動作範囲を有する請求項25に記載のカテーテル。
- 第1の長尺状シャフトの先端と連通する真空源をさらに備える請求項20に記載のカテーテル。
- 第1の長尺状シャフトに配置された第2の長尺状管腔をさらに備え、ワイヤは第2の管腔中に少なくとも部分的に配置された請求項27に記載のカテーテル。
