Description
本発明は、消化管内腔を蛍光色素で染色した像を明瞭で診断精度を上昇させるためのコントラスト増強剤に関する。
近年、欧米において、胃食道逆流関連疾患として逆流性食道炎及びBarrett食道の発生頻度が増加している。Barrett食道は食道腺癌と深く関わりがあり、様々な治療法が開発されている。食道壁の内面を覆う粘膜の上皮は重層扁平上皮と呼ばれ、細胞が幾層も並んで成り、胃粘膜は単層円柱上皮の構造を取っていて、明らかに異なった形状をしている。すなわち、上皮細胞の形状が明確に変化するところが食道と胃の境界である。
胃液が食道に逆流し食道粘膜に炎症を起こして発病する逆流性食道炎が長期の経過を有するとBarrett食道になる。これは食道胃接合部から全周性に3cm以上の長さで円柱上皮が胃側より連続して食道にまで存在するものである (非特許文献1)。
扁平上皮が円柱上皮化する機序に関する病理学的報告は少なく、明らかになっていない部分が多い。Barrett粘膜内にあるmultilayered epithelium(ME)という扁平上皮と円柱上皮の形態学的、組織学的特徴を有した上皮が4〜8層の細胞からなり、その深部は基底細胞に、表層は円柱上皮に類似している。さらにMEはサイトケラチンの発現が重層扁平上皮ならびに円柱上皮両者の性格を有している。このMEがBarrett食道の円柱上皮の起源細胞である説がある(非特許文献2)。
上述した胃食道関連疾患診断においては、早期での病変の発見が重要であり、そのための病気の進行を知る上で、組織への浸潤度を知ることが必要である。早期に発見することにより、外科的切除ではなく内視鏡的粘膜切除が可能になるからである。症状を訴えた患者の内視鏡所見を行い、それが肉眼で判別可能な程度の明らかなびらんや潰瘍を有するものであれば、容易に発見できる。これに対し、胸やけなどの症状があるにもかかわらず、明確な食道炎が認められなければ24時間の食道pHのモニターという方法がとられが、これは患者の精神的苦痛を伴う。
Barrett食道の症例で胃と食道の境界判断が白色光源による通常の内視鏡観察では容易ではなく、粘膜組織の変性を確認するためにランダムに生検が行われていたが、生検で癌と診断されてもその部位あるいはその範囲を同定するのに困難を感じることも少なくない。そこで色素内視鏡観察が行われるようになり、メチレンブルー染色が用いられた(非特許文献3)。しかし、メチレンブルー染色は粘液を完全に除去する必要があり、また、クリスタルバイオレットを用いた染色では Barrett粘膜を鮮明に染色してくれるが、癌の発見との直接の関連が不明確であった(非特許文献4)。
拡大内視鏡は血管網を診て深達度を診断するが、癌の分化度及び異型度、浸潤形式からも血管網の変化は異なるためどこまで判断が可能かは未知である。拡大内視鏡を用いれば、癌あるいは非癌の質的診断および深達度診断がきわめて有効に行うことができる(非特許文献5)とされ、通常内視鏡および色素内視鏡では正診率が45%であったの比し、拡大内視鏡では71%に向上したとの報告がなされている。
通常内視鏡に共焦点撮像システムを搭載した共焦点内視鏡は、通常内視鏡観察で病変の疑われるような生体組織を発見し、共焦点撮像システムによって観察すると、細胞や結合組織が複雑に多層化した生体表面の断面像を得ることができる。これは組織を採取することなく、細胞の形状や組織の形状による診断を非侵襲的に行える手段になる。断面像を得るために通常、観察部位に蛍光物質を投与する。
蛍光物質とは蛍光を発する物質であり、生体内での毒性がほとんどない物質であるほど良い。
そこで従来、フルオレセインは、生体に安全な蛍光造影剤として、水溶液を静脈に注射して眼底検査等に用いられてきた(非特許文献6)。

しかし本発明者の検討によれば、フルオレセインを静脈内投与し、消化管内腔側の蛍光観察を行った場合には、明瞭な蛍光染色像が得られない場合があることが判明した。そしてさらに検討したところ、このような明瞭な蛍光染色像が得られない場合が、特に胃、十二指腸や食道などの上部消化管において顕著であることが判明した。
従って、本発明は、蛍光色素を静脈内投与して消化管内腔蛍光染色像を得る際の染色像をより明瞭にするための手段を提供することを目的とする。

そこで本発明者は、消化管内腔蛍光染色像が明瞭にならない原因について種々検討したところ、消化管内腔のpHに依存していることを見出し、さらに検討したところ、消化管内腔側にpH6〜10の緩衝能を有する水溶液を撒布して蛍光観察したところ、蛍光染色像が顕著に明瞭になることを見出し、本発明を完成した。
すなわち、本発明は、pH6〜10の緩衝能を有する水溶液を含む消化管内腔用剤であって、静脈内投与された蛍光色素による消化管内腔蛍光染色像のコントラスト増強剤を提供するものである。
本発明のコントラスト増強剤は、静脈内投与された蛍光色素による消化管蛍光染色像を、消化管内腔に撒布又は塗布するという簡便な操作により、顕著に明瞭にできる。従って、本発明のコントラスト増強剤を用いれば、特に上部消化管の内視鏡による蛍光観察により、従来診断が困難であった早期癌等の診断が可能になる。
本発明のコントラスト増強剤は、pH6〜10の緩衝能を有する水溶液を含む消化管内腔用剤である。用いられる緩衝能を有する水溶液のpHは6〜10であるが、コントラスト増強効果及び安全性の点からpH6〜9が好ましく、pH6〜8が特に好ましい。
用いられる緩衝能を有する水溶液は、pHを6〜10の範囲にすることができるものであればよく、この範囲内の所望のpHになるように種々の酸性物質及び/又は塩基性物質を選択して緩衝能を示すように配合すればよい。
酸性物質としては、例えばリン酸、塩酸、炭酸等の無機酸;酢酸、クエン酸などの無毒性の有機酸が用いられる。塩基性物質としては、例えば水酸化ナトリウム、酢酸ナトリウム、リン酸水素ナトリウム、グリシンなどが用いられる。これらを組み合せた緩衝剤の例としては、リン酸ナトリウム、酢酸ナトリウム、クエン酸ナトリウム、炭酸ナトリウム、トリス(ヒドロキシメチル)アミノメタン−塩酸、リジン−塩酸、アルギニン−塩酸などが挙げられる。本発明においては、これらの緩衝剤を含有する水溶液が特に好ましい。
またこの水溶液には、中性多糖類等の増粘剤、粘稠剤、甘味剤、防腐剤、香料、ゲル化剤等を添加することもできる。
また、本発明のコントラスト増強剤は、消化管内腔に撒布、塗布、噴霧等して用いるものである。かかる撒布や塗布は、内視鏡に備えられた撒布チューブや、鉗子チャンネルを通じて行うのが好ましい。ここで消化管内腔としては、胃、十二指腸、食道等の上部消化管及び小腸、結腸、直腸、肛門の下部消化管が挙げられるが、本発明のコントラスト増強剤は上部消化管に用いた場合に特に効果が顕著であることから、上部消化管に用いるのが好ましく、さらに、食道に用いるのが特に好ましい。
本発明のコントラスト増強剤は、静脈内投与された蛍光色素による消化管内腔蛍光染色像のコントラスト増強させるものである。ここで蛍光色素としては、静脈内投与によって、消化管内腔を染色できるものであれば特に制限されないが、フルオレセイン又はその塩が好ましい。
蛍光観察は、励起光を照射して測定すればよい。蛍光染色像は、蛍光内視鏡又は共焦点撮像システムを有する内視鏡等により観察するのが好ましい。
より具体的には、蛍光色素を静脈内投与した後、消化管内腔に本発明のコントラスト増強剤を撒布等として、消化管内腔を蛍光観察すればよい。

次に実施例を挙げて本発明をさらに詳細に説明する。
実施例1
フルオレセインはpHの影響で染色性が変化しやすい蛍光染色剤である。マウス(ddY、10週齢、オス)から食道を摘出してすぐにフルオレセインナトリウム(0.1mg/mL)を染色剤が流失しないように摘出した食道の両端を留め、3分経過後に管状のまま0.1Mリン酸ナトリウム緩衝液(pH6)で洗浄し、内腔側を共焦点顕微鏡観察した。共焦点顕微鏡により撮影したものを図1に示す。使用した共焦点顕微鏡はライカ社製TCS SP2であり、観察条件はピンホール径1.00airy(36.24μm)、ゲイン値443.7V、レンズは20倍レンズを使用した。
食道壁の内面が重層扁平上皮という、細胞が幾層も並んでなっている構造であるため通常の観察が困難であるが、フルオレセインナトリウムで染色し酸性の本発明コントラスト増強剤を撒布することによって、共焦点撮像システムによる観察が可能である。

実施例2
マウスの食道が酸性状態にあるときコントラスト増強剤を用いて共焦点撮像システムによる観察を容易に行った。
マウス(ddY、13週齢、オス)を24時間絶食させた後、マウスの食道を酸性状態にするために0.1Mリン酸ナトリウム緩衝液(pH4)を若干量、口腔投与した。
フルオレサイト(アルコン社製)を生理食塩水で10mg/mLに調製し、それを心臓から300μlを灌流させた後、食道を摘出した。このときのpH値はpH5であり酸性であると確認された。
図2に共焦点顕微鏡(ライカ社製、TCS SP2)で撮影した画像を示す。撮影条件はピンホール径が1.00airy(36.24μm)で使用レンズは20倍、ゲイン値は495.6Vであった。
同様の処理を施した食道片に本発明のコントラスト増強剤(0.1Mリン酸ナトリウムをpH9に調製したもの)を塗布し共焦点顕微鏡観察を行った。このときのpH値は8でありアルカリ性になっていることを確認している。
図2に取得画像を示す。撮影条件はピンホール径が1.00airy(36.24μm)で使用レンズは20倍、ゲイン値は393.6Vであった。
撮影に際してゲイン値は自動で設定されるようになっており、ゲイン値(増幅率)が小さいと、よりよいと判断できる。
本実施例から、静注による染色観察を行う際、観察部位が酸性状態であると明瞭な染色像が得られない(図2)が、本発明のコントラスト増強剤を用いてpHを8にすると、共焦点撮像システムによる組織内部の観察が良好に行われることが明らかになった(図2)。

Claims (4)
- pH6〜10の緩衝能を有する水溶液を含む消化管内腔用剤であって、静脈内投与された蛍光色素による消化管内腔蛍光染色像のコントラスト増強剤。
- 消化管内腔が、上部消化管内腔である請求項1記載のコントラスト増強剤。
- 緩衝能を有する水溶液のpHが6〜8である請求項1又は2記載のコントラスト増強剤。
- 蛍光色素がフルオレセイン又はその塩である請求項1〜3のいずれか1項記載のコントラスト増強剤。
