Abstract
【課題】膀胱潰瘍を視覚的に簡易かつ明確に判断が可能で、麻酔下膀胱水圧拡張術を用いる必要なく、感度の高い下部尿路障害の疾患、特に間質性膀胱炎の疾患を診断する方法を提供すること。
【解決手段】特殊光観測装置を有する膀胱内視鏡システムを用いて、膀胱粘膜表層の血管及び/又は新生血管を観察することにより、下部尿路障害を診断することからなる。また、前記膀胱粘膜表層の血管及び/又は新生血管の観察は、狭帯域光観察(Narrow band imaging: NBI)装置を有する膀胱内視鏡システムを用いて取得した膀胱粘膜表層の画像と膀胱粘膜深部の画像とを視覚的に比較することで、血管及び/又は新生血管の異常のうち膀胱粘膜表層の血管及び/又は新生血管の異常を観察することからなる。

Description
本発明は下部尿路疾患、特に間質性膀胱炎の診断方法に関する。
下部尿路障害は下部尿路機能に関する異常の総称であり、下部尿路障害に起因する症状である下部尿路症状は、主に蓄尿症状(頻尿や尿意切迫感など)、排尿症状(尿勢低下や尿線分割など)、及び排尿後症状(残尿感や排尿後尿滴下など)の3種類に大別される。また、下部尿路症状には排尿痛、膀胱痛、及び尿道痛などの下部尿路痛、排尿筋過活動、及び排尿困難などが含まれ、下部尿路障害に際して血尿が認められることがある。下部尿路障害の原因となる疾患としては、間質性膀胱炎、前立腺肥大症、前立腺炎、前立腺症、膀胱頸部硬化症、過活動膀胱、膀胱痛症候群などが挙げられる。
これらのうち、間質性膀胱炎は、頻尿、尿意亢進、尿意切迫感、膀胱不快感及び膀胱痛などの症状を呈するが、尿路感染症や他の明らかな病的状態が認められない難治性疾患である。また、間質性膀胱炎は膀胱の非特異的な慢性炎症を伴った疾患とされているが、その病因は未だ明確にされてないために、明確な疾患の定義もない。また通常の膀胱炎と異なり、従来の抗生物質による療法は効果がない。
現在、間質性膀胱炎の診断基準として最もよく知られているのは、NIDDK(National Institute of Diabetics and Digestive and Kidney Diseases、米国立糖尿病・消化器・腎疾患研究所)の基準である。また、2006年に日本間質性膀胱炎研究会によって、「間質性膀胱炎診療ガイドライン」が公表され、臨床的な診断基準が提示されている。
上記の間質性膀胱炎の診断では、膀胱表層(上皮)の異常が診断の重要な根拠の1つとなる。そこで、1)膀胱潰瘍か2)麻酔下膀胱水圧拡張術によって生じる点状出血の観察が必要とされる。そして、膀胱潰瘍の観察には膀胱鏡検査が用いられるが、従来の可視光を用いた膀胱内視鏡所見で潰瘍が見つけられる頻度は10〜50%程度と低く(例えば、非特許文献1参照。)、感度良く検査するには大きな問題がある。
すなわち、従来の可視光を用いた所見では、ハンナー潰瘍のような明らかな潰瘍を形成しない限り診断することが不可能で、泌尿器科医はただ膀胱粘膜が赤いだけと思うだけで異常を見落とす場合が多い。すなわち、潰瘍は膀胱表層に新生血管が集族したものであるが、新生血管が膀胱表層にあろうが深部であろうが従来の膀胱鏡では赤くうつるだけであり、判断ができないか、非常に難しいという問題がある。
一方、麻酔下膀胱水圧拡張術を用いて行う診断では、患者は入院が必要で、しかも腰椎麻酔も含め全身麻酔下で水圧拡張術を用いて行う必要があり、さらに施行後疼痛を伴うなど、患者の身体的・精神的・経済的負担は極めて大きく、また診断する側の負担も同様に大きい。
そのため、特に中年の女性を中心に多くの人が間質性膀胱炎の症状に苦しんでいるにもかかわらず、その多数が診断を受けることができず、または正しく診断されずに苦労を強いられているという現状がある。

そこで、本発明の課題は、膀胱潰瘍を視覚的に簡易かつ明確に判断が可能で、麻酔下膀胱水圧拡張術を用いる必要なく、感度の高い下部尿路障害の疾患、特に間質性膀胱炎の疾患を診断する方法を提供することを目的とする。
本発明者は上記の課題を解決すべく鋭意研究を行った結果、間質性膀胱炎の潰瘍部分の粘膜層では、PD-ECGFなどの血管新生因子が高発現していて、血管新生と潰瘍とが密接に関係しており、もし膀胱鏡検査で血管新生の程度を感度よく検査できれば、間質性膀胱炎の新たな診断方法を提供することになることを見出した。さらに、現在癌の発見に限られて使用されている特殊光を用いた膀胱内視鏡システムが、間質性膀胱炎、膀胱痛症候群および慢性前立腺炎などの下部尿路障害疾患での膀胱粘膜表層の血管新生を感度よく検出する方法として有用であることを発見し、本発明を完成した。
すなわち、前記目的達成するため、本発明の診断方法は、特殊光観測装置を有する膀胱内視鏡システムを用いて、膀胱粘膜表層の血管及び/又は新生血管を観察することにより、下部尿路障害を診断することからなる。
また、前記特殊光観測装置が狭帯域光観察(Narrow band imaging: NBI)、蛍光観察(Auto fluorescent imaging: AFI)、又は赤外光観察(Infra red imaging)であることが好適である。
また、前記膀胱粘膜表層の血管及び/又は新生血管の観察は、狭帯域光観察(Narrow band imaging: NBI)装置を有する膀胱内視鏡システムを用いて取得した膀胱粘膜表層の画像と膀胱粘膜深部の画像とを視覚的に比較することで、血管及び/又は新生血管の異常のうち膀胱粘膜表層の血管及び/又は新生血管の異常を観察することであることが好適である。
また、下部尿路障害が間質性膀胱炎であることが好適である。
また、前記特殊光観測装置が狭帯域光観察(Narrow band imaging: NBI)、蛍光観察(Auto fluorescent imaging: AFI)、又は赤外光観察(Infra red imaging)であることが好適である。
また、前記膀胱粘膜表層の血管及び/又は新生血管の観察は、狭帯域光観察(Narrow band imaging: NBI)装置を有する膀胱内視鏡システムを用いて取得した膀胱粘膜表層の画像と膀胱粘膜深部の画像とを視覚的に比較することで、血管及び/又は新生血管の異常のうち膀胱粘膜表層の血管及び/又は新生血管の異常を観察することであることが好適である。
また、下部尿路障害が間質性膀胱炎であることが好適である。
膀胱内視鏡は、人体の膀胱内部を観察するための医療機器である。細長い形状に光学系を内蔵し、先端を体内に挿入することによって内部の映像を手元で見ることができる。更に、現在の膀胱内視鏡にはある程度の手術や標本採取ができるようになっているものがある。最近では、特殊光と画像処理の発展により、微細病変の早期発見、僅かな粘膜の肥厚や深部血管などが観察できるようになった。特殊光観測の方法としては、例えば狭帯域光観察(Narrow band imaging: NBI)、蛍光観察(Auto fluorescent imaging: AFI)、赤外光観察(Infra red imaging)などが実用化されつつある。NBIは、従来の膀胱内視鏡システムにフィルターとプロセッサーを組み込むことで使用可能になる。装置としては、例えば、癌早期発見のためのオリンパスメディカルシステムズの電子内視鏡システム「EVIS LUCERA SPECTRUM」(登録商標)が利用できる。
NBIは内視鏡の照明光にフィルターを通して取り出した波長400-430nm、好ましくは波長410-420nmから選択される波長の光,特に好ましくは415 nmの波長の光と、波長520-560nm,好ましくは波長530-550nmから選択される波長の光,特に好ましくは540 nmの波長の光とを用いる。両波長はヘモグロビンへの吸収率が高いため、毛細血管を黒く浮かび上がらせることができる。415 nmの光は粘膜の表層部分を、540 nmの光はやや深い部分を観測するために用いる。NBIでは、周囲組織とのコントラストが強調されて隆起状態が明瞭となるため、通常の内視鏡では見落としやすい非常に小さな腫瘍が発見可能となり、腫瘍の境界も他の方法と比較して非常に明確である。膀胱上皮内癌(CIS)では、粘膜の不整や発赤が強調され、さらに正常粘膜との境界が鮮明化するので、生検を行う部位の決定には非常に有用である。なお、従来の内視鏡システムにフィルターとプロセッサーを組み込むことで使用可能であり、内視鏡の手元スイッチで通常の可視光の内視鏡検査と1〜2秒で切り替え可能である。
従来の可視光を用いた間質性膀胱炎の内視鏡検査では、ハンナー潰瘍のような明らかな潰瘍を形成しない限り診断することが不可能であったが、NBIを用いた膀胱内視鏡では、潰瘍の前段階もしくは初期段階の粘膜層の変化を、血管新生像として観察することが可能となった。また、ハンナー潰瘍の場合でも、血管新生部分を観測することにより、潰瘍周辺部の病巣も診断することができ、潰瘍部レーザー焼灼術を施行する際のレーザー照射部位の決定にも有効である。
すなわち、NBIを用いた膀胱内検査では、波長415 nmの光で間質性膀胱炎に特徴的な粘膜表層の血管新生を観測することができる。一方、膀胱癌の場合には、540 nmの波長の光で粘膜の比較的深い部分に存在する血管新生を観測することにより診断する。それぞれの波長の光で血管新生部分の粘膜層の存在部分を観測することにより、癌と間質性膀胱炎を明確に区別して診断することが可能となる。
上記の方法を用いることで、下部尿路障害が前立腺炎、前立腺肥大症、前立腺症、前立腺膀胱炎、前立腺膿瘍、前立腺のうっ血及び出血、又は前立腺の萎縮;下部尿路障害が膀胱頸部硬化症、膀胱頸部閉塞症、膀胱腸瘻又は膀胱憩室;下部尿路障害が急性膀胱炎、慢性膀胱炎、間質性膀胱炎、膀胱三角部炎、放射線膀胱炎、癌化学療法剤による膀胱炎又は結核性膀胱炎;及び下部尿路障害が膀胱痛症候群頻尿、尿意切迫感症候群、過活動膀胱症候群、慢性骨盤痛症候群、無菌性膀胱炎、神経性(心因性)頻尿と診断される疾患のうち、間質性膀胱炎を診断することが可能となる。なお、前述のように間質性膀胱炎は、病因が未だ明確にされておらず明確な疾患の定義がないため、米国の国立衛生研究所(National Institute of Health)等では、最近は膀胱痛症候群/間質性膀胱炎(Painful Bladder Syndrome/Interstitial cystitis)と表記することもある。本明細書における間質性膀胱炎とはそのような概念の疾患を含む意味である。
また、蛍光観察(Auto fluorescent imaging: AFI)及び赤外光観察(Infra red imaging)も、NBIと同様に病変と正常組織を視覚的に区別することが可能であり、間質性膀胱炎、膀胱痛症候群および慢性前立腺炎などの下部尿路障害疾患での膀胱粘膜表層の血管新生を感度よく検出する方法として有用である。
膀胱内視鏡の照明光にフィルターを通して取り出した波長415 nm と540 nmの波長の光を用いて診断を行う。両波長はヘモグロビンへの吸収率が高いため、毛細血管を黒く浮かび上がらせることができる。415 nmの光は粘膜の表層部分を、540 nmの光はやや深い部分を観測するために用いる。間質性膀胱炎の診断には、415 nmの波長の光で観測される血管像の有無によって診断する。
間質性膀胱炎に特徴的な粘膜表層部分の血管新生は、NBIの415 nmの波長の光で観測でき、やや深い粘膜層の血管を観測する540 nmの光とは異なった色調で観測することができる。また、NBIでは、415 nmで観測される血管新生が盛んな部分が周囲組織とのコントラストが強調されて明瞭となるため、通常の内視鏡では見落としやすい血管新生部分が発見可能となり、正常部分との境界も他の方法と比較して非常に明確である。
以下、本発明を実施例によりさらに具体的に説明するが、本発明の範囲は下記の実施例に限定されることはない。
潰瘍型の間質性膀胱炎と診断した平均年令64歳の6名の患者の膀胱内視鏡検査を行った。6名の患者は、O'Leary and Santの症状質問で症状を呈している患者である。患者を脊椎麻酔下、通常の膀胱内視鏡で膀胱粘膜の潰瘍部分を観察し、次いで同じ場所をNBIで観察した。

下図に、前記患者の通常の可視光を用いた膀胱内視鏡による潰瘍性の間質性膀胱炎患者の潰瘍部分の写真と、その部分のNBI写真(415nm)を示す。左の写真は通常、右の写真は同じ場所のNBIの画像で、NBIでは血管新生部分及び潰瘍部分が明瞭に画像化されている。

すなわち、従来の可視光を用いた左の写真では、新生血管が膀胱表層にあろうが深部であろうが従来の膀胱鏡では赤く写ってしまい、判断が難しい。
これに対し、NBIを用いた右の画像では、症状を呈している6名の患者全員で、潰瘍部分に相当する粘膜表層部の血管が茶色(褐色)で表示されている。これは、粘膜深部の血管が青で表示されるのに対して、明確に区別された画像であった。 そのため、麻酔下膀胱水圧拡張術を用いた点状出血の観察をすることなく膀胱表層の新生血管の異常を視覚的に明確に判断することが可能である。
Claims (4)
- 特殊光観測装置を有する膀胱内視鏡システムを用いて、膀胱粘膜表層の血管及び/又は新生血管を観察することにより、下部尿路障害を診断する方法
- 前記特殊光観測装置が狭帯域光観察(Narrow band imaging: NBI)、蛍光観察(Auto fluorescent imaging: AFI)、又は赤外光観察(Infra red imaging)である、請求項1に記載の下部尿路障害を診断する方法
- 前記膀胱粘膜表層の血管及び/又は新生血管の観察は、狭帯域光観察(Narrow band imaging: NBI)装置を有する膀胱内視鏡システムを用いて取得した膀胱粘膜表層の画像と膀胱粘膜深部の画像とを視覚的に比較することで、血管及び/又は新生血管の異常のうち膀胱粘膜表層の血管及び/又は新生血管の異常を観察することである、下部尿路障害を診断する方法
- 下部尿路障害が間質性膀胱炎である請求項1から3のいずれか1項に記載の方法。
