Abstract
腸管粘膜の腺管構造を簡便に蛍光観察することを可能にする検査試薬及びその用途を提供することを課題とする。緩衝液のpHよりも低いpH域においてpH低下に伴い蛍光強度が上昇する蛍光色素を主成分として含有し、pH 7~8に調整された緩衝液からなる腸管蛍光観察用検査試薬が提供される。
Description
本発明は検査試薬に関する。詳しくは、消化器内科の分野等で有用な腸管粘膜蛍光観察用検査試薬に関する。本出願は、2009年2月10日に出願された日本国特許出願第2009-28071号に基づく優先権を主張するものであり、当該特許出願の全内容は参照により援用される。
内視鏡検査は炎症疾患や新生物(良性腫瘍及び悪性腫瘍)を調べるために必須な検査である。内視鏡機器の進歩により従来の内視鏡では得ることが出来なかった炎症疾患や新生物に関する新しい情報を得ることが可能になり、その結果として適切な診断をすることが出来るようになった。最近の新しい内視鏡機器には蛍光を利用した共焦点レーザー内視鏡や自家蛍光内視鏡があるが(非特許文献1~3)、さまざまな蛍光色素を用いることでこれら蛍光内視鏡の有用性はいっそう高まる。これまで蛍光色素として汎用されているものがフルオレセイン(図1左欄)であるが、フルオレセインは生理的pH 7.4~7.5で既に蛍光強度が高いため、これを利用した場合には、静脈注射を行い腸管粘膜に血管から漏出してくるフルオレセインの蛍光を観察するか、もしくは酢酸液を腸管粘膜に散布してからフルオレセインを散布する必要があった(非特許文献1、2)。静脈注射を行うことは蛍光色素を全身投与することになり煩雑であり、また酢酸を散布することは粘膜刺激となるため好ましくはない。更にフルオレセインは非特異的な蛍光色素であるため粘膜の間質を一様に染めるだけであり、腸管粘膜の腺管そのものを特異的に染めることは出来ない。このように、従来、腸管粘膜の腺管を特異的に観察するための検査試薬はなかった。

以上の背景の下、本発明の課題は、腸管粘膜の腺管構造を簡便に蛍光観察することを可能にする検査試薬及びその用途を提供することである。
本発明者らは、上記の課題に鑑みて鋭意研究を行った結果、pH低下に伴い蛍光強度が上昇する蛍光色素を含有する生理的pHに調整された緩衝液が有用であることを見出した。また、比較実験の結果、短時間での蛍光観察を可能にするためには低分子量の蛍光色素が好ましいことを見出した。本発明は、これらの成果に基づくものであり、次の通りである。
[1]蛍光色素を主成分として含有し、pH 7~8に調整された緩衝液からなる腸管蛍光観察用検査試薬であって、前記蛍光色素が、前記緩衝液のpHよりも低いpH域においてpH低下に伴い蛍光強度が上昇する蛍光色素であることを特徴とする、腸管粘膜蛍光観察用検査試薬。
[2]前記緩衝液のpHが7.4~7.5である、[1]に記載の腸管粘膜蛍光観察用検査試薬。
[3]前記蛍光色素が、分子量250以下の蛍光色素である、[1]又は[2]に記載の腸管粘膜蛍光観察用検査試薬。

本発明で使用する蛍光色素は、試薬を構成する緩衝液のpHよりも低いpH域においてpH低下に伴いその蛍光強度が上昇する。適用前の蛍光色素の蛍光強度が低いことから、腸管粘膜に適用した際、背景となる蛍光は低く蛍光観察の妨げにはならない。従って、本発明の試薬は直接腸管に適用することができる。しかも蛍光観察を行う前に洗浄する必要がない。
ここで、腸管粘膜の腺管を構成する杯細胞の粘液顆粒は酸性ムコ多糖体を含有するため、杯細胞の粘液顆粒ではpHが低下している。その結果、本発明の試薬を適用すると、蛍光色素の蛍光強度は杯細胞の粘液顆粒で上昇し、腺管構造を反映する円環状蛍光像を観察することが出来るようになる。
このように、本発明の試薬によれば簡便に腸管粘膜の腺管を蛍光観察することが可能となる。従って、本発明の試薬は腸炎や大腸新生物の診断に極めて有用である。
本発明の腸管粘膜蛍光観察用検査試薬(以下、「本発明の試薬」と呼ぶ)は、腸管粘膜の腺管を構成する杯細胞の粘液顆粒のpHが低いことを利用して腺管の構造を観察するための検査試薬である。蛍光観察の手法は特に限定されないが、蛍光内視鏡(例えば共焦点蛍光内視鏡)による蛍光観察が好適である。本発明の試薬は蛍光色素を主成分として含有し、pH 7~8に調整された緩衝液からなる。蛍光色素としては、緩衝液のpHよりも低いpH域においてpH低下に伴い蛍光強度が上昇する蛍光色素が用いられる。典型的には、適用前の状態(即ち、試薬のpH域のとき)では蛍光を実質的に発することがない蛍光色素が用いられる。
好ましくは、本発明の試薬を構成する緩衝液のpHは生理的pHのpH 7.4~7.5である。このときには、pH 7.4~7.5以下のpH域において蛍光強度が上昇する蛍光色素が用いられることになる。該当する蛍光色素の例(3種類)を以下に示す。
4-nitro-7-piperazino-2,1,3-benzoxadiazole (NBD-PZ)
当該化合物は、LysoSensorTM Green DND-153(商品名、Invitrogen社)の名称で市販されている。

尚、上記NBD-PZのように、発色団(NBD-PZではnitrobenzoxadiazole(NBD))に電子供与体として塩基(NBD-PZではpiperazine(PZ))が結合した形で表される蛍光色素を好適に用いることができる。
本発明者らの検討の結果、良好な蛍光像が観察できるまでに要した時間は、LysoSensorTM Green DND-153及びLysoSensorTM Green DND-189よりもNBD-PZの方が格段に短かった。この事実と、各化合物の分子量(LysoSensorTM Green DND-153の分子量356、LysoSensorTM Green DND-189の分子量398、NBD-PZの分子量249)を考え合わせると、杯細胞の粘液顆粒に蛍光色素が短時間で到達するためには分子量が250以下と小さいことが重要であると考えられる。換言すれば、短時間での蛍光観察を可能とするためには、分子量250以下の蛍光色素(例えば上記NBD-PZ)を採用することが好ましいといえる。
本発明の試薬は、例えば、pH7~8において緩衝能を示す緩衝液に所望の蛍光色素を溶解することによって調製することができる。緩衝液には例えばリン酸緩衝液、乳酸緩衝液、クエン酸-リン酸緩衝液、トリス(tris(hydroxymethyl)aminomethane)-塩酸緩衝液などを用いることができる。蛍光色素の含有量は特に限定されないが、例えば0.1μg/ml~10μg/ml、好ましくは0.25μg/ml~2.5μg/mlとする。
本発明の試薬によれば、腸管粘膜の腺管の構造を簡便に蛍光観察できる。本発明の試薬を生体の蛍光観察に適用する際には次の二つのステップを行う。まず、本発明の試薬を被検者の腸管(大腸表面など)に適用する(第1ステップ)。適用方法の一例は散布であるが、これに限定されるものではない。次に、所定時間経過後に適用部位の蛍光を観察する(第2ステップ)。ここでの所定時間は、線管構造を観察するために必要な蛍光が生ずる限りにおいて特に限定されず、使用する蛍光色素の性質(主としてpHと蛍光強度の関係)を考慮して設定すればよい。例えば、ここでの所定時間を3分~30分の間で設定することができる。蛍光色素にNBD-PZを採用した場合には短時間で蛍光観察が可能となることから、本発明の試薬を適用後、例えば3分~5分経過したときに蛍光観察(第2ステップ)を行えばよい。尚、本発明の試薬の適用対象は生体に限定されるものではなく、生体から採取した組織片などの蛍光観察にも本発明の試薬を利用することができる。
1.マウス大腸粘膜の観察
蛍光色素を含む緩衝液(蛍光観察試薬(1)~(3))を調製した。NBZ-PZの構造を図1の右欄に示した。比較のため、フルオレセインの構造を同左欄に示す。
(1)NBD-PZ(東京化学工業株式会社) 1μg/ml含有リン酸緩衝液
(2)LysoSensorTM Green DND-189(Invitrogen社) 1μg/ml含有リン酸緩衝液
(3)LysoSensorTM Green DND-153(Invitrogen社) 1μg/ml含有リン酸緩衝液
蛍光色素を含む緩衝液(蛍光観察試薬(1)~(3))を調製した。NBZ-PZの構造を図1の右欄に示した。比較のため、フルオレセインの構造を同左欄に示す。
(1)NBD-PZ(東京化学工業株式会社) 1μg/ml含有リン酸緩衝液
(2)LysoSensorTM Green DND-189(Invitrogen社) 1μg/ml含有リン酸緩衝液
(3)LysoSensorTM Green DND-153(Invitrogen社) 1μg/ml含有リン酸緩衝液
実験用マウスの大腸粘膜組織を採取し、ヘマトキシリン・エオジン染色、アルシアンブルー染色、及び蛍光観察試薬による染色に供した。前2者については常法に従い、後者については蛍光観察試薬に組織を浸漬後、37℃で保温した後に蛍光実体顕微鏡で観察した。
染色像及び蛍光観察像を図2に示す。線管構造が明瞭に観察される(A)。アルシアンブルー染色(B)では胚細胞の杯細胞の粘液顆粒が青色に強染し、酸性ムコ多糖体が含まれていることがわかる。粘膜をリン酸緩衝液(蛍光色素非含有)に浸し蛍光実体顕微鏡で観察しても蛍光像は認めない(D)。これに対して、NBD-PZ含有の蛍光観察試薬(1)に浸し37℃で3分間保温した後に蛍光実体顕微鏡で観察すると腺管を示す円環状蛍光像を認めた(E)。同様に、粘膜をLysoSensorTM Green DND-189含有の蛍光観察試薬(2)に浸し37℃で20分間保温した後に蛍光実体顕微鏡で観察すると円環状蛍光像を認めた(H)。尚、LysoSensorTM Green DND-153含有の蛍光観察試薬(3)の場合も同様の蛍光像を認めた(図示せず)。
粘液顆粒は酸性ムコ多糖体を含有するため、杯細胞の粘液顆粒ではpHが生理的pH7.4-7.5より低くなっている。その結果、生理的pH7.4-7.5以下で蛍光強度が上昇するNBD-PZ(非特許文献4)の蛍光強度は杯細胞の粘液顆粒で上昇し、腺管構造を反映する円環状蛍光像を観察することが出来るようになった(図2E,F)。LysoSensor Green DND-153(pH7.5以下で蛍光強度が上昇する)、DND-189(pH5.2以下で蛍光強度が上昇する)でも同様な所見が得られた(図2G, H)。但し、円環状蛍光像を観察するまでNBD-PZでは3分間であるのに対してLysoSensorTM Green DND-153およびDND-189では20分間であった。NBD-PZの分子量が249であるのに対してLysoSensorTM Green DND-153及びDND-189の分子量はそれぞれ356及び398であるため、杯細胞の粘液顆粒に蛍光色素が短時間で到達するためには分子量が250以下と小さいことが重要であると考えられた。
2.マウスの腸炎の蛍光観察
TNBS(Trinitrobenzenesulfonic acid)誘発性マウス大腸炎モデル、DSS(デキストラン硫酸)誘発性マウス大腸炎モデル、及びIL-10(interleukin-10)欠損マウス大腸炎モデルを作製した。各大腸炎モデルから大腸を摘出し、実体顕微鏡像(図3A, D, G)、NBD-PZ含有の蛍光観察試薬による蛍光実体顕微鏡像(同図B, E, H)、ヘマトキシリン・エオジン染色組織像(同図C, F, I)を得た。いずれの腸炎でも腺管の破壊・杯細胞の消失に伴い正常腸管粘膜で見られたNBD-PZによる円環状蛍光像が消失していることがわかる。このように、BD-PZ含有の蛍光観察試薬を適用し、円環状蛍光像の消失の程度を観察することによって、腸炎による腺管の破壊の程度を評価することが可能であった。

3.マウスの腸管ポリープの蛍光観察
IL-10欠損マウス大腸炎モデルの大腸に生じたポリープの実体顕微鏡像(図4A, D)、NBD-PZ含有の蛍光観察試薬による蛍光実体顕微鏡像(同図B, E)、ヘマトキシリン・エオジン染色組織像(同図C, F)を得た。腺管の増大に伴い、NBD-PZによる円環状蛍光像も増大していることがわかる。また、構造異型による杯細胞分布の乱れによってNBD-PZによる円環状蛍光像も乱れ、歪んでいたり途中で途切れたり断片的になっている箇所がある。このように、NBD-PZ含有の蛍光観察試薬を適用し、円環状蛍光像を観察することでポリープなどの新生物に認められる腺管増大や分化異常を評価することが可能であった。

4.NBD-PZの毒性評価
マウスの大腸にNBD-PZ含有(2.5μg/ml)の蛍光観察試薬100μlを肛門から注入してから24時間後に大腸粘膜の組織像をヘマトキシリン・エオジン染色により調べた。その結果、無処置のマウスの大腸(正常)と比べ異常を認めなかった(図5A)。また、血液中のALT、BUNを測定することにより肝機能、腎機能を評価したところ異常を認めなかった(同図B)。このように、NBD-PZを2.5μg/mlの濃度でマウスの腸管内に投与しても粘膜障害、肝障害、腎障害を認めることはなかった。尚、NBD-PZは2.5μg/ml(10μM)の濃度まで、消化管粘膜由来培養細胞とともに24時間培養しても細胞毒性がないことが報告されている(非特許文献4)。

通常の内視鏡では潰瘍、腫瘍および粘膜色調変化を観察することが出来るが、蛍光観察を行うことにより通常内視鏡では得ることが出来ない病変の画像所見を得ることが可能となる。この蛍光観察が可能な内視鏡では生検を行わなくてもリアルタイムで組織像を反映した情報を得ることができ、その結果、腫瘍を早期発見したり炎症の状態を正確に評価したりすることが出来るようになる。これまで腸管粘膜の腺管を特異的に蛍光染色する検査試薬はなかった。本発明の検査試薬は、これまで得ることが出来なかった腺管の構造に関する情報を得ることを可能とし腸炎や腫瘍の診断に極めて有用であり、蛍光内視鏡の有用性を拡大するものである。
腸炎、特に炎症性腸疾患の一つである潰瘍性大腸炎において通常内視鏡では炎症の程度を評価することが時として困難である(非特許文献6)。メチレンブルーを用いた拡大内視鏡で得られる所見は腺管構造を反映し、潰瘍性大腸炎の再発を予見するのに有用であることが報告されている(非特許文献7)。したがって、腺管構造を反映した蛍光像が得られる、本発明の検査試薬も潰瘍性大腸炎などの腸炎の予後を判定するに有用であると考えられる。
大腸癌は西欧諸国における癌死の主要な原因であり、日本でも癌死の中で占める割合は大きくなってきている(非特許文献8)。大腸癌の発生過程には腺管の増大、腺管の分化異常が含まれる(非特許文献8)。杯細胞は腺管を占める主要な細胞であり、また正常分化を示す細胞でもある(非特許文献5)。したがって、その杯細胞の粘液顆粒を蛍光染色する我々が開発した検査試薬を用いた蛍光内視鏡を行うことで、大腸癌の発生過程を反映した蛍光像を観察することが出来ることになり、大腸癌の早期発見だけでなくその病理所見を反映した診断を行うことが可能となる。
本発明の検査試薬では従来使用されてきたフルオレセインとは全く異なる腸管粘膜の蛍光像を得ることが出来る。更に腸管粘膜に必要に応じて散布するだけで、洗浄する必要もなく、蛍光観察が出来る。したがって、本発明の検査試薬を用いることで、これまで得ることが出来なかった腸管粘膜の画像所見を蛍光内視鏡で極めて容易に観察することが可能となり、蛍光内視鏡検査の利点と適応範囲を更にいっそう拡大することに貢献する。
この発明は、上記発明の実施の形態及び実施例の説明に何ら限定されるものではない。特許請求の範囲の記載を逸脱せず、当業者が容易に想到できる範囲で種々の変形態様もこの発明に含まれる。

- 蛍光色素を主成分として含有し、pH 7~8に調整された緩衝液からなる腸管蛍光観察用検査試薬であって、
