Abstract
Description
本発明は、米国国立癌研究所/NIH(米国国立衛生研究所)のグラントU01 CA88127号及びNIHの神経疾患卒中研究部門(National Institute of Neurological Disorders and Stroke)のグラントK08NS43147−01号に基づいてなされた。政府は本発明について特定の権利を有するものである。
本出願は、米国特許法第119条(e)に基づき、2002年11月5日に出願された米国仮特許出願第60/423,777号の利益を主張するものであり、その内容は参照として本明細書に取り込まれる。
PI3K/Akt経路の制御されていない活性化は多形性膠芽腫(GBM)を含む癌で一般に見られる。従って、この経路を評価することは標的とされたキナーゼ阻害剤治療について患者を分類するのに重要である。本明細書の開示は、PI3K/Akt経路の活性化異常(deregulated activation)に関連する一連のバイオマーカーを同定すると同時に、これらのマーカーを試験するために最適化された方法を提供する。従って、本明細書が提供する開示は、多形性膠芽腫等の癌においてこの経路の試験を可能にする。さらに重要なことには、これらのマーカーを試験するための開示された方法は、ホルマリン固定されパラフィン包埋された生検試料を含む幅広い種類の組織サンプルにおいて有用である。この開示の多様な側面はChoeら、Cancer Res.2003 Jun 1;63(11):2742−6に記載されている。

本明細書で提供される開示は、神経膠腫等の癌に共通して見られるPI3K/Akt経路の異常な活性化に関連した一連のバイオマーカーを同定する。本明細書で提供される開示はさらに、これらのバイオマーカーを試験するための最適化された方法も提供する。従って、開示は、多形性膠芽腫等の癌で、これらのバイオマーカーの活性化状態を試験することを可能にする。さらに重要なことに、これらのバイオマーカーを試験するための開示された方法は、ホルマリン固定されパラフィン包埋された生検試料を含む幅広い種類の組織試料に有用である。本明細書で開示されるように、これらのマーカーはリン酸特異的抗体等のパネル抗体を用いて試験され得る。これらの方法で、ホルマリン固定されパラフィン包埋された多形性膠芽腫の生検試料に由来する細胞等の哺乳動物の細胞を、リン酸化ポリペプチドを含む本明細書で開示される多様な標的分子の存在について、この細胞を含む組織試料を試験することによって、Akt経路の活性化の証拠について調べることができる。本発明の特定の態様では、ラパマイシン或いはその類似物又はZD−1839等のEGFR阻害剤或いはその類似物等の膠芽腫を処理するのに用いることができる治療剤を同定及び/又は評価する。
これらの腫瘍は周囲の正常組織に侵入し、グレードII又はそれ以上の腫瘍として再発するかもしれない。
グレードIIIの腫瘍は悪性である。これらの腫瘍は活発に再生する異常な細胞を含み、

本明細書に開示される本発明は多くの態様を有している。本発明の例示的な態様はホルマリン固定されパラフィン包埋された多形性膠芽腫生検試料等の腫瘍試料をPI3K/Akt経路の制御されていない活性化の証拠について試験する方法を含んでいる。これらの方法は、膠芽腫の処理に有用であるかもしれない治療剤を同定及び/又は評価するために、この経路と関連のある開示されたバイオマーカーの存在及び/又はリン酸化状態を試験することを包含する。本明細書の開示のように、開示されたバイオマーカーの存在及び/又はリン酸化状態は経路活性化のマーカー又は代用品として働く。
本発明の態様は、本発明の方法を容易に実施できるよう設計された製品及び/又はキットも包含する。一般的に、このようなキットはその中に本発明の方法に従った要素の使用についての説明書を含む。このようなキットはバイアル、チューブ等の1又はそれ以上の容器手段を狭い場所に受け入れるために区画分けされている運搬手段を含むことができ、それぞれの容器手段は方法で使用される分離された要素の1つを含んでいる。例えば、容器手段の1つは、検出可能なようにマーカーで標識されているか又はされ得る本明細書で開示された抗体(例えば抗S6抗体)を1つ又はそれ以上包含し得る。キットが、これらの方法のための緩衝液を含む容器及び/又は発色団又は放射性分子等のレポーター手段で標識された抗体を含む容器も有するのは、キットが標的タンパク質を検出するために免疫学的方法(例えば免疫組織化学及びウエスタンブロッティング)を利用するからである。加えて、アポトーシスについてカスパーゼ−3アッセイ又はtunelアッセイ等のさらなる方法を利用するキットについては、これらの手法に関連したさらなる試薬がさらにキットに含まれ得る。
本発明の方法は、一般的にPI3K/Akt経路のポリペプチドに対する抗体を利用する。本発明で有用な実例となる抗体組成物は、PI3K/Akt経路に関連するポリペプチドのリン酸化型に特異的に免疫反応する抗体分子を含んでいることを特徴とする抗リン酸化タンパク質抗体である。ポリペプチドは、例えば、S6、mTOR、FKHR、AKT又はPTENであっても良い。「特異的に免疫反応する」に関して言えば、それは、抗体がポリペプチドのリン酸化型に結合し(すなわちリン酸特異的)、同じポリペプチドの非リン酸化型には結合しないことを意味している。従って、経路の活性化に関連するリン酸化はこれらの抗体で試験し得る。それゆえに、本発明の抗体は、PI3K/Akt経路に関連するポリペプチドのリン酸化型及び非リン酸化型を区別し得る。従って、経路活性化に関連するリン酸化はこれらの抗体で試験し得る。一般的に、本発明のアッセイは本明細書で開示される抗体を使用する免疫組織化学的な手法を包含する。例えば、試料は配列番号8の202位でリン酸化されたスレオニン残基及び204位のチロシン残基を含むエピトープに結合する抗体を使用することによってリン酸化ERK等の生化学的な経路に関連するリン酸化ポリペプチドの存在について試験され得る。
本発明で有用な抗体はポリクローナル抗体、例えばアフィニティー精製されたポリクローナル抗体を包含しても良い。ポリクローナル抗体を調製する方法は熟練した技術者に公知である。ポリクローナル抗体は、例えば免疫原及び望まれるならばアジュバントを1回又はそれ以上注入することによって、哺乳動物で生じさせることができる。一般的に、免疫原及び/又はアジュバントは複数の皮下又は腹腔内注入によって哺乳動物に注入されるであろう。免疫原は適当なポリペプチドエピトープ(例えば、リン酸化されたセリン、スレオニン又はチロシン残基を有するS6ポリペプチド(配列番号1)、リン酸化されたセリン、スレオニン又はチロシン残基を有するmTORポリペプチド(配列番号2)、リン酸化されたセリン、スレオニン又はチロシン残基を有するFKHRポリペプチド(配列番号3)、リン酸化されたセリン、スレオニン又はチロシン残基を有するERKポリペプチド(配列番号8)、リン酸化されたセリン、スレオニン又はチロシン残基を有するAKTポリペプチド(配列番号4)又はPTENポリペプチド)又はそれらの融合タンパク質を含んでいても良い。
本発明は制御されていない細胞増殖に関連する細胞経路を試験するためのアッセイを提供する。本発明の特定の態様は、組織内のリン酸化されたS6、mTOR、FKHR、AKT又はERKポリペプチド、又は、PTEN及びEGFRポリペプチドの存在を検出する工程を包含する。これらのポリペプチドを検出する方法はよく知られ、例えば免疫沈降、免疫組織化学解析、ウエスタンブロット解析、分子結合アッセイ、ELISA、ELIFA等を含む。
(実施例)
この研究に参加する全ての患者は、UCLA Institutional Review Board Policiesに従って、手術前にインフォームドコンセントを与えた。ホルマリン固定されパラフィン包埋された組織片を、初期の外科的な切除で膠芽腫と診断され、UCLAの神経腫瘍学者によって治療された45患者から得た。診断は少なくとも2人の神経病理学者によって独立に行われた。腫瘍を切除する前に治療された患者はいなかった。3つの代表的な0.6mmの円筒形標本を、それぞれ初期GBM患者から採取した組織片の診断領域から得た。2つを腫瘍の部位的に異なる領域から得、1つを可能であれば(約2/3の場合が可能)は正常な脳組織の領域から入手した。円筒形標本は組織アレイヤーを用いてパラフィンブロック中に格子状に挿入された。5ミクロンの切片を組織アレイから切り出し免疫組織化学的に分析した。組織アレイからの連続切片を免疫組織化学解析に使用した。4つの腫瘍がPTEN、EGFR及びEGFRvIII解析に対して組織アレイ上に十分な材料を有していたが、p−Akt、p−mTOR、p−S6、p−FKHR及びp−Erkの解析に対しては十分な材料を有してはいなかった。
組織マイクロアレイからの切片をPTEN(クローン6H2.1,Cascade Bioscience,Winchester MA)、EGFR(クローン31G7,Zymed,San Francisco,CA)、EGFRvIII(クローンL8A4,Dr.Darrell Bignerより分与)それぞれに対するモノクローナル抗体、及び、p−Akt(セリン473)、p−FKHR(スレオニン24)/p−FKHRL1(スレオニン32)、p−mTOR(セリン2481)、p−S6リボゾーマルタンパク質(セリン235/236)及びp−44/42MAPキナーゼ(p−Erk)(スレオニン202/チロシン204)(Cell Signaling Technologies,Beverly,MA)それぞれに対するリン酸化特異的抗体で染色した。切片を60℃で焼き、キシレンで脱パラフィン処理し、エタノールまで徐々に変化させた。熱で誘導される抗原の修正を以下のように用いた。p−Erk、p−Akt、p−mTOR、p−FKHR/FKHRL1及びp−s6に対しては0.01Mクエン酸緩衝液、pH6、圧力釜で25分間、PTENに対しては0.01Mクエン酸緩衝液、pH6、電子レンジでで16分間、EGFR、プロナーゼ(0.03g/ml 0f 0.05M トリス緩衝液、pH7.4)37℃で8分間、EGFRvIIIに対しては0.01Mクエン酸緩衝液、pH6、野菜蒸し器で25分間。内在性のペルオキシダーゼ活性をメタノール中3%過酸化水素で失活させた。一次抗体(PTEN 1:400、EGFR 1:150、EGFRvIII 1:400、p−Akt 1:50、p−mTOR 1:50、p−FKHR/FKHRL1 1:50、pS6 1:50、p−Erk 1:50)を0.1%Tweenトリス緩衝生理食塩水で希釈し、4℃で16時間接触させ、1:100希釈した抗マウス又は抗ウサギビオチン化免疫グロブリン(Vector)でさらに1時間接触させ、最後にアビジン−ビオチン複合体(Elite ABC, Vector)で1時間接触させた。陰性対照スライドは正常マウス血清(DACO)を一次抗体として用いた。PTEN、EGFR及びEGFRvIIIに対する特異的抗体の局在を可視化するために、ジアミノベンジジン四塩酸塩を酵素の基質として使用し、Vector NovaRed(Vector)をリン酸特異的抗体に対して使用した。スライドをハリスヘマトキシリンで対比染色した。
PTEN−PTEN染色をこれまでに確立されている0−2の単位に従って評価したが、その単位では血管内皮細胞(スコアー2)が内部標準として働く(例えばPerrenら、Am J Pathol.157:1097−103.,2000、Perrenら、Am J Pathol.155:1253−60.,1999、Zhouら、Am J Pathol.161:439−47.,2002;Gimmら、Am J Pathol.156:1693−700.,2000参照)。腫瘍細胞の染色強度が血管内皮細胞のそれと同じであれば腫瘍細胞は2として格付けされ、血管内皮細胞と比べて減少していれば1、腫瘍細胞で検出されず血管内皮細胞に存在していれば0となる(例えばZhouら、Am J Pathol.161:439−47.,2002参照)。この格付けシステムは、乳(例えばPerrenら、Am J Pathol.155:1253−60.,1999参照)、卵巣(例えばMutterら、Cancer Res.61:4311−4314.,2001参照)、膵臓(例えばPerrenら、Am J Pathol.157:1097−103.,2000参照)及び結腸(例えばZhouら、Am J Pathol.161:439−47.,2002参照)を含む異なる癌の細胞種間で高い相関を示してきている。2人の神経病理学者がそれぞれ独立して腫瘍を評価した。さらに、神経病理学者の1人が独立した2回の機会に腫瘍を評価した。評価者内及び評価者間の意見は90%以上一致した。
マーカー間の関連をフィッシャーの正確検定を用いて解析した。ソフトウェアは「home.clara.net」という用語を用いてインターネット検索で同定できるthe Simple Interactive Statistical Websiteで入手できる(http://home.clara.net/sisa/index.htm)。予後要素の解析のために、我々は手術以外の治療を受けていない13患者を除外した。これらの患者は診断の時点で状態が悪く、さらなる治療を受けないので選択された。他の全ての患者は、領域分割放射線治療を含む少なくとも標準的な治療を受けていた。最初の診断から画像処理や臨床像によって進行の証拠が得られるまでの時間(腫瘍進行までの時間)及び最初の診断から死までの時間(全体の生存期間)と変量との関係を評価するために、カプラン−マイヤー曲線を作製した。腫瘍進行までの時間及び全体の生存期間において統計上有意な相違点を同定するために、ウィルコクソン二標本検定を用いた。
我々は未処理の初期GBM患者45人から得た試料からなる組織マイクロアレイを構築した(表1)。全ての腫瘍はde novoでグレードIV腫瘍(初期GBMs)として存在した(例えばKleihuesら、Neuro−oncol.l:44−51.,1999参照)。いずれの患者も外科的切除の前にいかなる放射線又は化学療法も受けていなかった。我々が初期GBMsに焦点を当てたのは、それらが高い確率でPTEN変異及びEGFR過剰発現を有しており(例えばKleihuesら、Neuro−oncol.l:44−51.,1999参照)、このことが以前に治療されていない状態でのPI3’K/Akt経路の活性化を解析することを可能にしたからである。患者は平均年齢58歳で28歳から88歳まで及んでおり(表1)、全ての患者は少なくとも2人の独立した神経病理学者によって生検でGBMであると診断された。
PTENの消失はAkt活性化の唯一の経路ではないように思われた。p−Akt及び下流の作用因子p−mTOR、p−FKHR及びp−S6の発現は免疫組織化学的なPTENの消失が見られないGBMsの28%でも検出された(表2)。PI3’K/Akt経路はEGFR情報伝達によって活性化され得るので、我々は正常なPTEN免疫組織化学染色の状況下で、EGFR及びEGFRvIIIの発現を解析し、それらとPI3’K/Akt経路活性化の関連を評価した。以前の報告と調和して(例えばSmithら、J Natl Cancer Inst.93:1246−56.,2001、Watanabeら、Brain Pathol.6.217−23;discussion 23−4.,1996、Ekstrandら、Proc Natl Acad Sci USA.89:4309−13.,1992、Frederickら、Cancer Res.60:1383−7.,2000、Hayashiら、Brain Pathol.7:871−5.,1997、Naganeら、Cancer Lett.162 Suppl: S17−S21.,2001、Nishikawaら、Proc Natl Acad Sci USA.91:7727−31.,1994、Wikstrandら、Cancer Res.57:4130−40.,1997参照)EGFR免疫染色陽性をGBMsの60%で検出した(図2)。構造的に活性な変異体EGFRvIIIの免疫組織化学的な発現をEGFR陽性腫瘍の56%(腫瘍全体の44%)で検出した(図2)(Smithら、J Natl Cancer Inst.93:1246−56.,2001、Naganeら、Cancer Lett.162 Suppl:S17−S21.,2001、Nishikawaら、Proc Natl Acad Sci USA.91:7727−31.,1994、Wikstrandら、Cancer Res.57:4130−40.,1997)。EGFR及びEGFRvIIIの発現を欠いているが正常なPTENの免疫組織化学的な発現を伴っているGBMsでは、強く活性化されたAkt(2+染色)を検出しなかった(表2)。対照的に、EGFR及びEGFRvIIIを共発現し、かつ正常なPTENの免疫組織化学的な発現を伴っているGBMsの36%は活性化されたAktを強く染色した(p=0.06)(表2)。腫瘍のサブセットは小さいが、EGFRを伴うEGFRvIIIの共発現は強いAkt活性化(2+染色)で必要とされているように思われた(表2)。これらの結果は、EGFR及びEGFRvIIIの共発現が正常なPTENタンパク質の発現を伴うGBMsでAkt活性化を促進することができる証拠を提供する。これに一致して、mTOR、FKHR及びS6の下流の活性化も有意に、EGFR及びEGFRvIIIを共発現している時に正常なPTENタンパク質の発現を伴うGBMsで強く活性化(2+染色)しているように思われた(p<0.002)。
以前の研究は、GBMsにおけるPTENの消失、EGFR過剰発現又はEGFRvIIIについて明白な予後との関係を示してきてはいなかった。このことに調和して、我々はPTENの消失、EGFR又はEGFRvIIIの発現と、進行までの時間又は全体の生存期間のいずれかとの間に統計的に有意な関係がないことを見出した。対照的に、協調した経路の活性化が予後と密接な関係を有しているように思われた。p−Aktの発現は生存や進行と有意な関連はなかった。対照的に、mTOR、FKHR及びS6の同時リン酸化によって検出されるような、下流の経路の活性化は進行へのより短い時間(p=0.002)及び減少した全体の生存期間(p=0.02)の両者に有意に関連した。この発見は、Erkが仲介するS6キナーゼの活性化及び栄養分が仲介するmTORの活性化等のAkt下流へのさらなる入力からの寄与を反映しているのかもしれない。あるいは、3つのリン酸特異的抗体のこのパネルは、単独のリン酸−Akt抗体のみに比べてAkt経路活性化を検出するより感受性のある方法であるかもしれない。Erk活性化も、初期のGBM患者のこのサブセットで、より急速な進行及び減少した全体の生存期間と有意に関連した(<0.04)(表5)。これらの発見は経路活性化がGBM患者の腫瘍進行に影響力を有していることを最初に示すものである。

(a)PTENポリペプチド(配列番号5)の発現
