Abstract
【解決手段】曲率を有する表面に沿って骨の端部を切除するための骨切除器具であって、ガイド、切削器具、およびトラックフォロアを含む。このガイドは、骨の端部に対して固定位置に取り外し可能に取り付けできるように構成されている。ガイドはまた、骨の切除する表面の曲率に概ね一致する曲率を有するトラックを含むように構成されている。切削器具は切削面を有する。トラックフォロアは、切削器具に結合し、トラックと協働して、ガイドに対する切削器具の往復運動を容易にし、切削面が曲率を有する表面に沿って骨を切除できるように構成されている。プロテーゼコンポーネントの下側の曲面の曲率に一致する曲面に沿って骨を切除する方法も提供する。

Description
本発明は、関節形成術に関連し、詳細には、関節置換システムのコンポーネントを受容するように骨を整形する器具および技術に関する。
損傷した関節や悪化した関節を置換するためにプロテーゼ関節コンポーネントが用いられることは良く知られている。通常の関節置換術では、関節が置換される肢の長さを変えることなくプロテーゼ装置を骨に確実に取り付けできるように、置換される関節を構成する1または複数の骨の遠位端部または近位端部を切除する必要がある。関節コンポーネント製造者は、切開部が小さく、運動学的により適切なインプラントを用いる、低侵襲性外科手術を必要とする関節置換術から恩典が得られることを認識している。加えて、若い患者が関節置換術を受けるようになってきたため、患者が術後に早期に回復し、優れた機能が得られ、より耐久性が得られる関節置換術が要望されている。
限定目的ではない添付の図面を用いて例示的な装置を説明する。全ての図面において、同じ参照記号は同じ構成要素を示し、類似の参照記号は類似の構成要素を示す。
(1) 曲率を有する表面に沿って骨の端部を切除するための骨切除器具であって、
前記骨の端部に対して取り外し可能に固定位置に取り付けでき、かつ前記骨の切除する表面の曲率に概ね一致する曲率を有するトラックを含むように構成された、ガイドと、
切削面を有する切削器具と、
前記切削器具に結合するように構成され、かつ前記トラックと協働して前記ガイドに対する前記切削器具の往復運動を容易にし、前記切削面が前記曲率を有する上面に沿って骨を切削するように構成されたトラックフォロアと、を含む、骨切除器具。
(2)実施態様(1)に記載の骨切除器具であって、
前記切削器具に結合されたシャフトをさらに含み、このシャフトが、前記切削器具と協働して前記切削面が骨を切削するように構成されている、骨切除器具。
(3)実施態様(2)に記載の骨切除器具であって、
前記シャフト、前記切削器具、前記トラックフォロア、および前記ガイドが、協働して前記曲率を有する表面に対して概ね垂直に前記シャフトを維持する、骨切除器具。
(4)実施態様(3)に記載の骨切除器具であって、
前記シャフトに結合して前記シャフトをその長軸を中心に回転させるドライバをさらに含む、骨切除器具。
(5)実施態様(4)に記載の骨切除器具であって、
前記シャフトは、前記切削面に固着されている、骨切除器具。
前記トラックフォロアおよび前記トラックは、前記切削器具が前記ガイドに対して前記トラックに沿って往復運動し易くするように構成されている、骨切除器具。
(7)実施態様(6)に記載の骨切除器具であって、
前記切削面は、その端部および側面で切削するように構成されている、骨切除器具。
(8)実施態様(5)に記載の骨切除器具であって、
前記ガイドは、前記曲率を有する表面に沿ってその上に延在する離間したレールを含み、
前記レールは、前記トラックを画定するよう協働する、骨切除器具。
(9)実施態様(8)に記載の骨切除器具であって、
湾曲スロットが前記各レールを通るように形成され、前記湾曲スロットが協働して前記トラックを画定している、骨切除器具。

前記シャフトの一部が前記離間したレール間に配置されている、骨切除器具。

前記シャフトにストッパーが形成されており、このストッパーが、前記トラックフォロアと協働して前記シャフトの骨に向かう往復運動を制限する、骨切除器具。

前記ストッパーが、前記シャフトに対して長手方向に調節できるように構成されている、骨切除器具。
(15)プロテーゼコンポーネントの下側の曲面の曲率に一致する曲面に沿って骨を切除する方法であって、
切削する骨の表面を覆う組織を、低侵襲性式に切開するステップと、
前記プロテーゼコンポーネントの下側の曲面の曲率に一致するように構成されたトラックを含むガイドを、切除する骨の表面に沿って配置するステップと、
前記ガイドを前記骨に固定するステップと、
切削面を有するカッターを前記ガイドに相互連結するステップと、
前記切削面を、前記曲面の接線に概ね平行に維持するステップと、
前記カッターを前記トラックに沿って案内し、前記切削面を前記骨に沿って横断させるステップと、を含む、方法。
前記カッターが、前記切削面に対して垂直に延びたシャフトを含み、
前記シャフトを前記曲面の接線に対して垂直に維持しながら、前記カッターを前記骨に沿って横断させるステップをさらに含む、方法。
(17)実施態様(16)に記載の方法であって、
前記曲面の接線に垂直な軸に沿ってシャフトを長手方向に移動させて、前記切削面を前記骨内に前進させるステップをさらに含む、方法。
(18)実施態様(17)に記載の方法であって、
前記シャフトをその軸を中心に回転させるステップをさらに含む、方法。
(19)実施態様(18)に記載の方法であって、
前記カッターを操作して前記トラックを連続的に移動させるステップをさらに含む、方法。
Claims (14)
- 曲率を有する表面に沿って骨の端部を切除するための骨切除器具であって、
前記骨の端部に対して取り外し可能に固定位置に取り付けでき、かつ前記骨の切除する表面の曲率に概ね一致する曲率を有するトラックを含むように構成された、ガイドと、
切削面を有する切削器具と、
前記切削器具に結合するように構成され、かつ前記トラックと協働して前記ガイドに対する前記切削器具の往復運動を容易にし、前記切削面が前記曲率を有する上面に沿って骨を切削するように構成されたトラックフォロアと、を含む、骨切除器具。 - 請求項1に記載の骨切除器具であって、
前記切削器具に結合されたシャフトをさらに含み、このシャフトが、前記切削器具と協働して前記切削面が骨を切削するように構成されている、骨切除器具。 - 請求項2に記載の骨切除器具であって、
前記シャフト、前記切削器具、前記トラックフォロア、および前記ガイドが、協働して前記曲率を有する表面に対して概ね垂直に前記シャフトを維持する、骨切除器具。 - 請求項3に記載の骨切除器具であって、
