本発明は、人間の骨格または骨組織に挿入して恒久固定するインプラントに関
する。特に、本発明は、第１直径の第１円筒形部分および第２直径の第２円筒形
部分を包含し、前記第２直径が前記第１直径より小さいねじ留め式固定構造を有
するインプラントに関する。 本発明は、限定するものではないが、特に、股関節人工補てつ具用の大腿骨固
定具という形をとるこのタイプのインプラントに関する。

【発明の背景】
股関節人工補てつ具用大腿骨固定具の形をしたこのタイプのインプラントは、
出願人の国際特許出願公報ＷＯ９３／０１７６９、ＷＯ９３／１６６６３および
ＷＯ９７／２５９３９で公知であり、そのインプラントにおいては、第１、第２
の円筒形部分が、大腿骨の骨組織と係合する外ねじ山を備えている。ＷＯ９３／
０１７６９に開示されている大腿骨固定具のねじ留め式第１、第２の円筒形部分
は、まず大転子の下方から第２円筒形部分を大腿骨の頸部の中間に挿入し、次い
で、大腿骨の頭部の切除後の残った切除部分を通して横方向に頸部内へ第１円筒
形部分を挿入することによって、一緒に大腿骨に組み込む。一方、ＷＯ９３／１
６６６３およびＷＯ９７／２５９３９に開示されている大腿骨固定具のねじ留め
式第１、第２の円筒形部分は、大腿骨頭部の切除後に残った切除部分を通して横

ラントの移植中にくさび留め効果を提供するということである。このくさび留め
効果は、インプラントの移植後の短期および長期の安定性を向上させる。これは
、主として、インプラントと、移植時に連結部分を取り囲む海綿骨組織との間の
通常の接触力の半径方向分力による。
【００１０】 インプラントの移植後に本発明インプラントおよび上記の従来技術のインプラ
ントに周囲組織によって加えられる長手方向の力がほぼ同じ場合、本発明のイン
プラントにかかる横方向力は、従来の段付きインプラントに比べて大きくなる。
これは、長手方向力が先細連結部分の斜面によって支えられ、それによって、そ
の結果生じる横方向力が半径方向分力を持つことになるからである。この半径方
向分力は、近位円筒形部分（たとえば、ＷＯ９７／２５９３９における図１の部
分２）の半径方向端部分（たとえば、ＷＯ９７／２５９３９における図１の部分
１０）のみが対応する長手方向力を支えるので、従来技術のインプラントには存
在しない。
【００１１】 全横方向力が増大する間、単位面積あたりの実際の力は必ずしも増大しない。
これは、長手方向接触力を支える接触面が、従来技術インプラントの前記半径方
向端部の場合におけるように本発明の先細連結部分においてかなり大きくなると
いう事実による。
【００１２】 また、本発明の先細連結部分を設けることで、インプラントと海綿骨組織との
一体結合（この技術分野においては、「骨一体形成（osseointegration）」と呼
ばれる）を向上させる。これは、主として、連結部分の接触表面の形状によるが
、海綿骨組織によってインプラントに加えられる増大する全接触力にもよる。 普通、第１円筒形部分は、後述する実施例におけるように、第２円筒形部分の
近位方向に配置され、連結部分のテーパが遠位方向において内方へ傾いている。 好ましくは、先細連結部分は、他の先細り形状が本発明の範囲から逸脱するこ
となく考えられる場合であっても、円錐台形の外形を有する。
【００１３】 本発明の好ましい実施例によれば、第１円筒形部分の直径は、インプラントを
意図した特定の患者の大腿骨の実際の寸法、形状に適応する。したがって、第１
円筒形部分の直径はかなり変化する可能性がある。しかしながら、第２円筒形部
分の直径は、短い限られた範囲内になるように寸法決めすると好ましい。したが
って、連結部分のフランク角は、第１、第２の円筒形部分の実際の寸法に応じて
変えるとよい。好ましくは、フランク角は、１０度〜５０度の範囲、より好まし
くは、２０度〜４０度の範囲にあるとよい。 さらに、連結部分の長手方向寸法は、５〜１５mmの範囲、好ましくは、７〜１
１mmの範囲にある。
【００１４】 有利には、第１円筒形部分とインターフェイス接続する先細連結部分の端は、
第１円筒形部分とほぼ同じ直径を有する。同様に、第２円筒形部分とインターフ
ェイス接続する先細連結部分の端は、有利には、第２円筒形部分とほぼ同じ直径
を有する。
【００１５】 本発明の好ましい実施例によれば、連結部分は、少なくとも部分的に、粗面を
備えている。これは、円筒形部分間の移行領域のところでの骨一体形成プロセス
をさらに促進する。粗面は、ブラスチング、好ましくは、グリット・ブラスチン
グ、エッチング等で達成することができる。あるいはまたはさらに、円周方向に
向いた粗面（好ましくは、機械加工した粗面）を連結部分に設けてもよい。この
ような円周方向に向いた粗面は、たとえば、溝、ビーズ、トラックまたはねじ山
の形で設けることができる。
【００１６】 本発明の好ましい実施例によれば、先細連結部分は、少なくとも部分的に、ね
じ山の外形を備える。先細連結部分のねじ山の外形は、第１、第２の円筒形部分
上にあるねじ山の外形と異なっていると好ましい。好ましくは、連結部分のねじ
山の外形の高さは、円筒形部分のねじ山の外形の高さよりも低くなっている。有
利には、連結部分のねじ山の外形は、０.３mmより低く、好ましくは、０.１〜０
.２５mmの範囲にあり、さらに好ましくは、約０.２mmである高さを有する微小ね
じ山の形をしている。
【００１７】 ねじ山の外形における他の違いも考えられる。たとえば、連結部分のねじ山の
外形が、円筒形部分のねじ山の外形のピッチより短いピッチを有する。 あるいは、連結部分のねじ山の外形は、円筒形部分のそれとほぼ同じものであ
る。
【００１８】 円周方向の粗面をビーズまたはトラックの形で設けた場合、前記円周方向粗面
の高さは、好ましくは０.３mm未満、より好ましくは０.１〜０.２５mmの範囲に
あり、さらに好ましくはほぼ０.２mmである。 あるいは、連結部分の表面の少なくとも一部あるいは全表面が、滑らかなまま
であってもよいし、さらには研磨すらしてもよい。
【００１９】 本発明の後述する実施例においては、先細連結部分上に少なくとも部分的に、
１つまたはそれ以上のセルフタッピング用切削くぼみが設けてある。前記切削く
ぼみは、好ましくは、等間隔に隔たっていて、円周方向に配置してある。
【００２０】 本発明の好ましい実施例によれば、インプラントは、また、その頭部と第１円
筒形部分を相互連結する先細近位部分を包含する。この先細近位部分は、インプ
ラントとそれを取り囲む皮質骨組織との接触を増大させ、骨組織内に固定したと
きにインプラントの安定性を向上させる。
【００２１】 先細近位部分が、好ましくは８度〜１５度の範囲、より好ましくは、１０度〜
１３度の範囲にあり、さらに好ましくは約１２度であるフランク角を持つ円錐台
形となっていると有利である。好ましくは、第１円筒形部分に当接している先細
近位部分の端は、第１円筒形部分の直径にほぼ等しい直径を有する。
【００２２】 好ましくは、先細近位部分の表面は、たとえば円周方向ビーズまたはねじ山の
形をした円周方向に向いた粗面を備える。ビーズまたはねじ山の高さは、好まし
くは、好ましくは０.３mm以下であり、より好ましくは０.１〜０.２５mmの範囲
にあり、さらに好ましくはほぼ０.２mmである。
【００２３】 好ましくは、前記ビーズまたはねじ山は、切除表面のところで大腿骨頸部の皮
層３４と係合する。その結果、インプラントのより強い短期間固定を行える。ま
た、先細近位部分とそれを取り囲む骨組織間との骨形成が改善されるため、より
強い長期固定も行うことができる。
【００２４】 本発明の実施例によれば、上述の頭部は、先細近位部分に当接するカラーを備
えている。このカラーは骨組織内へのインプラントの挿入限界を定める。好まし
くは、近位部分に面するカラーの表面は、内方へ傾いており、対応した傾斜形状
を与える切除骨組織面と一致する。好ましくは、傾斜角度は、１０度〜２０度の
範囲内、より好ましくは、ほぼ１５度である。あるいは、近位部分に面するカラ
ーの表面が凹形状となっており、骨組織凸面と一致するようになっている。この
ことにより、インプラントと骨表面との間の接触状態を向上させることができる
。
【００２５】 好ましくは、前記カラー表面は、半径方向に隔たった円形のビーズまたは溝を
備え、これは、挿入したインプラントの安定性を増大させると共に、インプラン
トと骨組織との間の骨一体形成を促進する。これらのビーズは、好ましくは０.
１〜０.５mmの範囲、より好ましくは０.２〜０.４mmの範囲にあり、さらに好ま
しくはほぼ０.３mmである高さを有する。 以下、添付図面を参照しながら本発明の実施例を説明する。

【実施例】
図１〜８を参照して、ここには、好ましくは市販の純チタンから作ってあり、
（ｉ）円形横断面の骨内固定部分３と、（ii）頭部分５とからなる股関節人工補
てつ具用の一体形成式大腿骨固定具１が示してある。固定部分３は、対応する外
形（図７）の空洞３０内に横方向に挿入することを意図している。前記空洞３０
は、大腿骨の頭部を切除することによって作った切除部３３を貫いて大腿骨頸部
に穿たれている。固定具の頭部分５（骨内固定部分３を空洞３０（図８）内に設
置したときに切除部３３から突き出ることになる）は、股関節人工補てつ具の球
体２５を支持するように配置する。この股関節人工補てつ具は、解剖学的寛骨臼
の空洞または股関節人工補てつ具全体を必要とする股関節人工補てつ具寛骨臼部
分と相互作用する。
【００２７】 図１〜３でわかるように、骨内固定部分３は、異なった外径の近位、遠位円筒
形部分１１、１３を有する。近位円筒形部分１１の直径は、遠位円筒形部分１３
のそれより大きい。骨内固定部分３は、さらに、遠位円筒形部分１３に隣接する
先細遠位末端部分１２と、近位円筒形部分１１を遠位円筒形部分１３に連結する
円錐台形連結部分１５と、近位円筒形部分１１を頭部分５に連結する円錐台形近
位部分１８とを有する。
【００２８】 近位円筒形部分１１は、前記空洞の外方骨空洞部３２に螺合するためのねじ付
き外面を有する。遠位円筒形部分１３もねじ付き外面を有し、これは、前記外方
空洞部３２と同軸である細い穿孔３１に螺合する。近位円筒形部分１１のねじ山
は、遠位円筒形部分（１３）のそれと同じピッチ、高さを有する。
【００２９】 近位、遠位円筒形部分１１、１３上のねじ山の外径（major diameter）は、外
方空洞部３２の対応する円筒形部分の内径および大腿骨頸部の空洞３０（図７参
照）に設けた穿孔３１より大きいように設定する。したがって、骨内固定部分３
は、空洞３０内に大腿骨固定具１を螺合させることによって空洞３０内に固定す
ることができる。そのとき、近位、遠位円筒形部分１１、１３上のねじ山は、空
洞３０の境界壁にある骨組織に螺合する。
【００３０】 図８でわかるように、近位円筒形部分１１の直径は、実際、ＷＯ９３／１６６
６３およびＷＯ９７／２５９３９に概説されるように、ねじ山が大腿骨頸部の皮
質骨３４の周辺層に整合するような寸法となっている。したがって、近位円筒形
部分１１上のねじ山は、よりスポンジ状の海綿骨３５ではなくより強い皮質骨３
４に固定され、それによって、大腿骨固定具１を大腿骨頸部により強く固定する
。大腿骨寸法が患者毎に異なるという事実により、近位円筒形部分の直径は、約
１６〜２６mm（図３、８参照）の範囲で変わり得る。
【００３１】 図８に示すように、骨内固定部分３の軸長は、骨内固定部分３の固定位置にお
いて、その遠位端１２が大腿骨の横方向の皮質骨層（cortex）３４を貫いて突出
するように決める。
【００３２】 図３〜５を参照して、円錐台形近位部分１８にもねじ山が設けてある。これら
のねじ山の高さは、近位、遠位円筒形部分１１、１３上のねじ山のそれより低い
０.２mm（いわゆる微小ねじ山）である。さらに、円錐台形の近位部分１８は、
微小ねじ山が切除表面のところで大腿骨頸部の皮質骨層３４と係合するような寸
法となっている。ここに説明する実施例において、円錐台形近位基部１８は、ほ
ぼ１２度のフランク角を有し、ほぼ８mmの軸線方向寸法を有する。
【００３３】 近位部分１８の遠位直径は、隣接した近位円筒形部分１１の直径になっており
、その結果、円錐台形近位部分１８と近位円筒形部分１１との間の移行領域には
鋭い縁がまったくない。したがって、円錐台形近位部分１８の近位直径は、ほぼ
２０〜３０mmの範囲にある。
【００３４】 遠位円筒形部分１３の直径は、患者の大腿骨寸法に依存して変化する必要はな
い。遠位円筒形部分１３の直径は、ほぼ１１mmであり、あるいは、１０〜１２mm
の範囲内にある。
【００３５】 円錐台形連結部分１５は、近位、遠位円筒形部分１１、１３を相互連結してい
る。本実施例において、連結部分１５のそれぞれの端の直径は、近位、遠位円筒
形部分１１、１３の直径のそれぞれと一致している。換言すれば、連結部分１５
の遠位端は、遠位円筒形部分１３とほぼ同じ直径を有し、連結部分１５の近位端
は、近位円筒形部分１１とほぼ同じ直径を有する。
【００３６】 近位円筒形部分１１の直径が異なった大腿骨固定具間で変わる可能性があり、
一方、遠位円筒形部分１３の直径が変化しないという事実の結果、連結部分の寸
法は、近位円筒形部分１１と遠位円筒形部分１３との間の直径の様々な違いに従
って変化することになる。連結部分の軸線方向寸法が比較的短く保たれる、すな
わち、おおよそ７.５〜１０.５mmの範囲内に保たれるので、連結部分のフランク
角は、最も細い固定具の場合には約２０度から変わり、最も太い固定具の場合に
はほぼ３７度まで変わる可能性がある。
【００３７】 本発明のここに説明した実施例においては、円錐台形連結部分１５の表面は、
この表面と周囲の海綿骨組織との間の骨形成を促進するためのグリット・ブラス
ト仕上げした表面を備えている。さらに、あるいは、代わりに、この表面は、前
記骨形成を促進し、大腿骨固定具１の固定を向上させるためのねじ山の外形を備
え得る。さらなる代替案として、円錐台形連結部分１５は、滑らかなままに残し
てもよいし、さらには研磨さえしてよい。
【００３８】 図２、３でわかるように、近位円筒形部分１１と円錐台形連結部分１５との境
界をつないで、一連の円周方向に等間隔に隔たっている、鋭利な縁のある切削く
ぼみまたは切り欠き１４があり、これらは予め切削した外方骨空洞部３２にセル
フタッピングすることができる。切削くぼみ１４の各々は、近位円筒形部分１１
に設けた縦溝１６と連通している。これらの縦溝は、ＷＯ９７／２５９３９に詳
述されるように、大腿骨固定具１を大腿骨頸部の内腔に螺合したときに切削くぼ
み１４で切削される骨の自家移植のためのものである。
【００３９】 さらに、遠位円筒形部分１３と先細遠位末端部分１２との境界をつないで、一
連の短い、鋭利な縁のある、円周方向配置の切削くぼみ１７があり、これらの切
削くぼみは、遠位円筒形部分１３を前記の比較的細い穿孔３１内にセルフタッピ
ングすることができる。
【００４０】 図１、７、８を参照して、大腿骨固定具１の頭部分５は、カラー部分２０と、
股関節人工骨頭の球体要素２５を装着するための先細取り付け部分２３とを有す
る。取り付け部分２３は、球体要素２５を受け入れるくぼみ２４を備えている。
カラー部分２０は、空洞３０に通じる開口に隣接して切除大腿骨部分３３と当接
することによって、骨内固定部分３の大腿骨頸部の内腔への挿入限界を定める。
図５でわかるように、遠位表面２１（図５）は、切除大腿骨部分３３（図７）の
対応した傾斜骨表面と一致するように内方に傾いている。ここに説明した実施例
における傾斜角度は、ほぼ１５度である。さらに、固定および骨形成を向上させ
るために、図６でわかるように、カラー部分２０の遠位表面２１は、半径方向に
隔たった円周方向ビーズ２２を備えている。これらのビーズは、ほぼ０.３mmの
高さを有する。
【００４１】 ＷＯ９３／１６６６３およびＷＯ９７／２５９３９に記載されている大腿骨固
定具を植設するための外科措置は、また、大腿骨固定具１の移植にも適用できる
ので、参照によって本明細書に加入する。
【００４２】 大腿骨固定具１の固定は、主として、大腿骨におけるねじ山の整合、特に、大
腿骨頸部の皮質骨層３４における近位円筒形部分１１上のねじ山の整合、そして
、大腿骨の横方向皮層３４における遠位円筒形部分１３上のねじ山の整合に依存
する。これは、たとえばＧＢ−Ａ−２０３３７５５に記載されているような固定
用のスラスト・プレート機構に依存する大腿骨固定具とは異なる。
【００４３】 添付図を参照しながらここに説明した大腿骨固定具１は、本発明の範囲内で多
数の方法によって変更可能である。たとえば、大腿骨固定具１は、構成部分を、
（ｉ）ＷＯ９３／１６６６３に開示されているように一体構造として内腔へ横方
向に挿入するように、あるいは、（ii）ＷＯ９３／０１７６９に開示されている
ように、内腔内で相互に部品を連結することによって組み立てる組立体の形をし
ていてもよい。大腿骨固定具１は、また、装着時に加えられる荷重に耐えるに充
分な強度を有する任意の生物適合材料で作ってもよい。
【００４４】 ここで、発明を実施例に関して説明してきており、添付の請求範囲内で多くの
異なった方法で発明を変更できることは了解されたい。たとえば、インプラント
は、股関節人工補てつ具用の大腿骨固定具としての用途に限定されない。一例と
して、インプラントは骨固定ねじの形を採り得る。さらに、図面の参照符号を請
求項に付けているが、これが説明の目的であり、請求項に制限を与えるものと解
釈されるべきではないことは了解されたい。

  1. 【請求項１】 骨組織に挿入して恒久固定するインプラント(１)であって、
    ほぼ円形の横断面の骨内固定構造(３)を包含し、この固定構造(３)が、第１直径
    の第１円筒形部分(１１)と第２直径の第２円筒形部分(１３)とを包含し、前記第
    ２直径が前記第１直径より小さくなっており、第１、第２の円筒形部分（１１、
    １３）の各々がねじ山の外形を備えているインプラントにおいて、固定構造(３)
    が、第１、第２の円筒形部分（１１、１３）間に設けてあり、それらを相互連結
    する先細連結部分(１５)を包含することを特徴とするインプラント。
