Abstract
Description
の使用、並びに神経根障害の処置方法に関するものである。

経根障害を処置する可能性を得ることにある。

脳椎における盤ヘルニア化は腕における放射性疼痛および筋肉機能不全を誘発さ
せ、腰椎におけるヘルニア化は脚部における放射状疼痛および筋肉機能不全を誘
発しうる。脚部における放射性疼痛は一般に「坐骨神経痛」と称される。盤ヘル
ニア化は種々異なる程度にトラブルを生ぜしめ、疼痛は1−2ヶ月または重度の
場合は6ヶ月まで持続しうる。盤ヘルニア化の結果として生じうる腕もしくは脚
部の疼痛は極めて強烈であり、従って病状の期間に際し個人的患者の全生命状態
に影響を及ぼしうる。

も軽減させうることが示された。

、ペントキシフィリン、ナプトピラン、可溶性サイトキンリセプタ、TNF−α
に対するモノクローナル抗体、アムリノン、ピモベンダン、ベスナリノン、ホス
ホジエステラーゼIII抑制剤、ラクトフェリンおよびラクトフェリン誘導同族
体、メラトニン、ノルフロキサシン、オフロキサシン、サイプロフロキサシン、
ガチフロキサシン、ペフロキサシン、ロメフロキサシンおよびテマフロキサシン
である。これらは塩基または付加塩の形態で存在することができ、いずれの場合
も最も良好な医薬作用を有すると共に医薬上適する組成物にしうる最も良好な性
質を有する。

接種した。髄核投与の7日後、神経根伝達速度を投与帯域にわたりシリーズ−2
に従い局部的電気刺激により決定した。

(24、36)を誘発しうる共に、髄核細胞の膜結合構造もしくは物質がこれら
作用の原因になる(24、37)ことが確立された。さらに、これら作用はメチ
ルプレドニゾロンおよびサイクロスポリンA(2、38)により効率的に阻止さ
れることも判明した。これらデータを重視すれば、これら作用の全てに関連する
少なくとも一種のサイトキン、すなわち腫瘍壊死因子α(TNF−α)の存在が
認識される。TNF−αが髄核誘発神経根障害に関与しうるかどうかを評価する
ため、髄核細胞におけるTNF−αの存在を評価すると共に、髄核誘発作用がド
キシサイクリン、可溶性TNF−リセプタおよび選択性モノクローナルTNF−
抗体により阻止されうるかどうかを検討し、後者は髄核に局部的および全身的の

ットランド)と10%胎児ウシ血清(FCS)(ギブコ・BRL、ペースレー、
スコットランド)とが補充されたDMEM/F12 1:1培地(ギブコ・BR
L、ペースレー、スコットランド)に懸濁させた。これら細胞を37℃にて空気
中5%CO2で3〜4週間にわたり培養し、次いで組織培養処理ガラススライド
(ベクトン・ディキンソン・アンド・カンパニー・ラブウェアー、フランクリン
・レークス、NJ、USA)で直接培養した。ガラススライド上にて5日間の後
、細胞をその場にアセトンにより10分間にわたり固定させた。3%H2O2(
シグマ・ケミカル・カンパニー、セントルイス、MO、USA)の30分間にわ
たる添加およびウシ血清(イミュノピュアABC、ペルオキシダーゼ、ネズミI
gG染色キットNo.32028、ピアス、ロックフォード、IL)の20分間
にわたる添加により不適切な抗原を阻止した後、一次抗体(抗−ブタTNF−α
モノクローナル精製抗体、エンドゲン、ケンブリッジ、MA、USA)を+40
℃にて1晩にわたり添加し、1:10、1:20および1:40にて希釈した。
比較のためPBS(燐酸緩衝塩水、メルク、ダルムスタット、ドイツ国)に懸濁
されたBSA(ウシ血清アルブミン、インテルゲン・カンパニー、ニューヨーク
、USA)を同様に添加した。翌日、各細胞をPBSにおける1%BSAで洗浄
し、二次抗体(イミュノピュアABC、ペルオキシダーゼ、ネズミIgG染色キ
ットNo.32028、ピアス、ロックフォード、IL)を30分間施した。こ
の反応を増進させるため、細胞をアビジン−ビオチン複合体にさらに30分間に
わたり露呈させた(イミュノピュアABC、ペルオキシダーゼ、ネズミIgG染
色キットNo.32028、ピアス、ロックフォード、IL)。次いで各細胞を
20mgのDAB(3,3−ジアミノゲンジジン テトラヒドロクロライドNo
.D−5905、シグマ・ケミカル・カンパニー、セントルイス、MO、USA
)および塩水10mlにおける0.033mlの3%H2O2に10分間露呈さ
せた。各細胞をPBSで洗浄し、一連のエタノールにて脱水し、偏見のない観察
者により光学顕微鏡を装着すると共にTNF−αの存在を示す褐色着色の存在に
関し検査した。

ー国)を静脈内注射した。麻酔を2mg/体重1kgのハイプノジル(登録商標
)および0.05mg/体重1kgのストレスニル(登録商標)の追加静脈注射
により維持した。これらブタにはさらに0.1mg/体重1kgのステソリッド
・ノブム(登録商標)(ジアゼパム、ズメックス、ヘルシングボルグ、スウェー
デン)を術後に静脈注射した。

(処理なし)。4頭の他のブタには100mgのドキシサイクリン(ビブラマイ

0mlにおける静脈内輸液を1時間にわたり接種した。5頭のブタにおいて、髄
核をシリーズ1で用いた抗−TNF−α抗体の100gの1.11mg/ml懸
濁物と混合した後、施した。

より再麻酔した。これらブタをレスピレータで換気した。100mg/体重1k
gのクロラロース(α)−D(+)−グルコ−クロラロース(メルク、ダムスタ
ット、ドイツ国)の静脈内ボルス注射および30mg/kg/hrのクロラロー
スの連続供給により維持した。第四仙骨から第三尾骨椎までの椎弓切除を行った
。神経根をスポンゴスタン(登録商標)(フェロサン、デンマーク国)で覆った

ジオスII AIDコンバータ(GWインスツルメンツ、ソマービル、MA)が
グラスP18プレアンプリファイアー(グラス・インスツルメント・カンパニー
、クインシー、MA)と一緒に装着されたマッキントッシュIIciコンピュー
タにより可視化させた。2つの記録からのEMGの第1ピーク間における分離間
隔を決定すると共に、馬尾における2つの刺激部位間の分離間隔をキャリパーで
測定した。2つの刺激部位間における神経伝達速度をかくしてこれら2つの測定
値から計算することができた。

ンダ国)12.5mgを術前にs.c.投与接種すると共に、さらに術後の3日
間にわたり12.5mgをs.c.投与した。髄核投与の7日後に、神経根伝達
速度を投与帯域にわたりシリーズ−2に従って局部的電気刺激により決定した。
試験を盲倹するため神経生理学的評価を他の試験と並行して行い、分析を行った
人間は試験と各特定動物が受けた処理とを知らなかった。シリーズ−3には、髄
核または脂肪(比較)投与の7日後における神経伝達速度の予備知識に基づき、
処理なし動物を含ませなかった。群、すなわちインフリキシマブおよびエタネル
セプト、処理なし髄核(予備データからのプラス比較)と、レトロペリトニアル
脂肪(予備データからのマイナス比較)の投与との間の統計上の差をANOVA
およびフィッシャーPLSD(5%)を用いて評価した。
SにおけるBSAを用いたセクションには染色が観察されず、不適切な抗原の標
識および可視化が存在しないことを確認した。抗−TNF−α抗体を1:40希
釈で施した場合、弱い染色のみが存在した。しかしながら、染色は抗体の希釈が
減少するにつれ増大した。染色が細胞のソーマに見られ、これはTNF−αが細
胞質、細胞膜に結合した細胞表面、またはその両者に位置するかどうかを区別す
ることができなかつた。
誘発した(表1)のに対し、ドキシサイクリンでの処理はこの低下を完全に阻止
した(p<0.01スチューデント試験)。抗−TNF−α−抗体の局部的投与
もこの低下の部分的阻止を誘発したが、ドキシサイクリンほど完全でなく非処理
シリーズと統計上有意差がなかった。
なら、これら両処理群の平均神経根伝達速度は事前の試験(表2)に見られるよ
うに脂肪投与シリーズの平均伝達に近かったからである。髄核を投与するが、処
理なしの場合は両薬剤につき見られるように統計的有意差が存在した:
TNF−αを局部的投与の選択性モノクローナル抗体により阻止した場合、神経
根伝達速度の髄核誘発低下は部分的に阻止されたが、処理なし動物のシリーズと
比較して統計上の有意差はなかった。しかしながらドキシサイクリン、インフリ
キシマブおよびエタネルセプトでの全身的処理を用いてTNF−αを抑制した場
合、神経根伝達速度の低下が顕著に防止された。
って盤ヘルニア化にて見られる神経根障害は単に神経根の機械的変形のみに基づ
かず、ヘルニア化した髄核の硬膜存在に関連する未知の「生化学的作用」によっ
ても誘発されうることが明らかとなった。この新規な研究分野は多くの実験的研
究をもたらしたが、関与するメカニズムおよび物質は充分知られていない。自家
髄核の局部的投与は軸索障害(24、37、38、40〜42)、ミエリンシー
スの特徴的障害(24、38、40〜42)、血管浸透性の局部的増大(9、3
6、44)、血管内凝集、神経内血流の低下(43)、およびロイコタクシス(
36)を誘発させうることが見られた。さらに髄核関連作用はメチルプレドニゾ
ロン(38)およびサイクロスポリンA(2)により効率的に阻止されうるが、
インドメタシン(3)およびリドカイン(69)による効率は若干低いことが見
られた。さらに、これら作用は髄核細胞(37)により、特に細胞膜に結合した
物質もしくは構造体(25)により媒介されることが了解された。これらデータ
を重要に考慮すれば、少なくとも1種の特定サイトキンはこれら観察作用(すな
わち腫瘍壊死因子−α(TMF−α))に関連したことが明らかとなる。TNF
−αは、主として髄核誘発ミエリン障害(29、47、51、54、62、64
、66、70)に近似した特徴的ミエリン障害として見られる神経障害(29、
31、45、50、66)を誘発しうる。さらにTNF−αは血管浸透性におけ
る増大(47、66)をも誘発すると共に、凝集をも開始させる(22、34、
63)。さらに、TNF−αはステロイド(4、8、21、61、68)および
サイクロスポリンA(11、55、67、68)によっても阻止されうる。しか
しながら、TNF−αに対する阻止作用はNSAID(14、17、20)によ
りそれほど顕著でなく、リドカインにより極めて低く或いは反対である(5、3
2、46、60)。髄核の局部的投与はラットにおける疼痛関連挙動(特に熱ハ
イパーアルゲシア(23、40)をも誘発しうることが最近観察された。さらに
TNF−αはこの種の挙動特徴的変化(12、35、56、66)にも関連する
と共に、一般に神経病(30、54、56、57)にも関連することが判明した
。しかしながら、髄核の細胞におけるTNF−αの存在可能性を評価した試験は
存在しない。
かを評価するには、先ず最初に髄核細胞にTNF−αが存在したかどうかを分析
する必要があった。データは、TNF−αがこれら細胞に存在したことを明らか
に示した。TNF−αは先駆体(プロ−TNF)として生成され、これは膜に結
合すると共に亜鉛依存性メタロ−エンドペプチダーゼ(TNF−α変換酵素、T
ACE)により細胞膜から分離させて活性化される(6、15、16、48、4
9)。従って、これは自家髄核細胞の単なる細胞膜の投与が神経伝達速度低下を
誘発するという実験的知見に良く関連し、これは作用が膜結合物質により媒介さ
れることを示した。第2に、TNF−αの作用は制御的に阻止せねばならかった
。先ず最初に、シリーズ−1における免疫組織化学につき使用したと同じ選択的
抗体(これはTNF−αの作用を阻止することも知られる)を投与前の髄核に添
加するよう選択する。さらに、ブタをTNF−αを阻止することが知られたドキ
シサイクリンで処理するよう選択する(26、27、33、52、53)。しか
しながら、ドキシサイクリン製剤の低pHに基づき、髄核への局部的添加の代わ
りに静脈内注射によりブタを処理するよう選択した。何故なら、低pHにおける
髄核は髄核の作用を強化することが判明しているからである(38、39)。

モノクローナル抗体であってヒトTNF−αに特異的に結合する。3日間の観察
期間にわたりシリーズ−2で使用したモノクローナル抗体とは異なり、インフリ
キシマブを自家移植髄核に局部的に投与しなかったが、その代わりに臨床的に推
奨される投与量(4mg/kg)にて全身的に投与した。エタネルセプトは、ヒ
トIgGのFc部分よりなるダイマー融合蛋白である。この薬物を、小児科用途
につき推奨された投与量(0.5mg/kg、1週間2回)に匹敵する投与量に
て投与した。
共に、これらシリーズにおける神経伝達速度が事前の研究からの比較物質(レト
ロ腹部脂肪)投与の後の伝達速度に近かったことを示した(42)。髄核への抗
−TNF−α−抗体の投与も神経伝達速度の低下を部分防止したがドキソサイク
リンほど顕著でなく、このシリーズにおける速度は未処理動物でのシリーズにお
ける速度に対し統計上の有意差がデータの広い偏差に基づき存在しなかった。
み阻止したという事実およびデータの高い標準偏差は恐らく少なくとも3つの異
なる説明を有する。第1に、この群内の特定データを見れば、神経伝達速度は2
匹の動物にて低く(平均37.5m/s)かつ3匹の動物で高い(平均81.3
m/s)であることが判明した。従って、抗−TNF−α処理シリーズには明確
に異なる2群のデータが存在する。これは高い標準偏差の説明となり、阻止作用
が3匹の動物にて充分であると共に2匹の動物では不充分であることを示唆する
。これら動物における作用の欠如は充分でないTNF−α分子に関する抗体の量
に単に基づき、高投与量の抗体を使用した場合はTNF−α作用がこれら動物に
おいても阻止されたであろう。次いで、この種のシナリオは、TNF−α単独が
観察髄核誘発作用の原因となること、およびこれが低過ぎる抗体の量に基づき実
験的に証明しえなかったことを理論的に示唆する。
クリンも多数のサイトキンおよび他の物質を阻止しうることが知られている。た
とえば、これらはIL−1(1、28、58)、IFNγ(27)、NO−シン
テターゼ1およびメタロプロテナーゼ(1、53、58)を阻止することができ
る。特にIL−1およびIFNγはTNF−αと相乗作用することが知られてお
り、多かれ少なかれ神経毒性であることが知られている(7、10、13、18
、19、56、59)。これら物質はステロイドおよびサイクロスポリンAによ
って阻止され、これらは髄核誘発作用がこれら物質により阻止されうることを示
した(8、67)髄核誘発神経根障害に対する事前の観察によく対応する。従っ
て、TNF−αの選択的阻止は神経作用に対する髄核誘発作用を完全に阻止する
には充分でないこと、および他の相乗性物質の同時的阻止も必要であるという可
能性も考えられる。たとえば他方において、このシナリオは、TNF−αが単に
髄核誘発作用の原因となるに過ぎないこと、およびドキシサイクリンによっても
阻止される他の相乗性物質も必要であることを示唆する。

に必要であり、従って神経障害に到るイベントを阻止するのはこれら白血球であ
る。

り一層多量のTNF−αの放出を開始させうる(65)。従って、少量のTNF
−αでさえこれらプロセスを開始させるのに充分であり、かつサイトキン生成細
胞の局部的再生、並びに他のサイトキンおよびTNF−αの生成および放出にお
けるその後の増大も存在すると信ずる理由がある。TNF−αは従って病理学的
プロセスの「発火キー」として作用しうると共に、髄核誘発神経障害の背後にあ
る病理学的連鎖の開始につき重要な役割を演ずる。しかしながら、TNF−αの
主たる貢献は再生され、凝集し、かつ恐らく血管外滲出した白血球から再生する
こともでき、この薬理学的阻止の成功は全身的処置によってのみ達成されうる。
、特定物質(TNF−α)が髄核誘発神経根障害に関連していると初めて結論し
うる。この新規な情報は、髄核誘発神経障害の持続的理解、並びにTNF−αお
よび関連物質による坐骨神経痛の処置に関する薬理学的介入の有力な将来の臨床
用途の問題を提起するのに極めて重要である。
局部投与されたモノクローナル抗体によるTNF−αの阻止は神経根伝達速度の
髄核誘発低下を部分的に制限するのに対し、ドキシサイクリン、インフリキシマ
ブおよびエタネルセプトによる静脈内処置はこの低下を顕著に阻止した。これら
データは始めて1種の特定物質(すなわちTNF−α)を髄核誘発神経障害に関
連させた。
根障害にて酸化窒素(NO)の放出を抑制することを示し、従ってアミノグアニ
ジンはTNF−αの放出により停止される化合物を阻止する1種の化合物である
。
被覆錠、液体溶液の形態で;経腸的に座薬の形態で;非経口的にたとえば筋肉内
または静脈内注射により或いは輸液によるような各種の投与形態にて投与するこ
とができる。種々異なる臨床徴候に対する治療方式を病理学の種類に考慮適合さ
せねばならず、通常のように投与のルート、化合物の投与形態、並びに関与する
患者の年齢、体重および症状にも適合せねばならない。
。緊急の症例では好ましくは静脈注射が挙げられる。これら目的で、本発明の化
合物は約20〜約1500mg/1日の範囲の投与量にて経口投与することがで
きる。これら投与量は最適治療反応を与えるよう調整しうることは勿論である。
有する医薬組成物の性質は、所望の投与ルートに依存することは勿論である。こ
の組成物は通常の成分にて常法で処方することができる。たとえば本発明の化合
物は水溶液もしくは油性溶液もしくは懸濁液、錠剤、ピル、ゼラチンカプセル(
硬質もしくは軟質)、シロップもしくはドロップまたは座薬の形態で投与するこ
とができる。
錠剤、ピルもしくはゼラチンカプセルであって活性物質をたとえば乳糖、デキス
トロース、蔗糖、マニトール、ソルビトール、セルロースのような希釈剤;滑剤
、たとえばシリカ、タルク、ステアリン酸、ステアリン酸マグネシウムもしくは
カルシウムおよび/またはポリエチレングリコールのような希釈剤と一緒に含有
し、或いはこれらはさらにたとえば澱粉、ゼラチン、メチルセルロース、カルボ
キシメチルセルロース、アラビヤゴム、トラガカント、ポリビニルピロリドンの
ような結合剤;たとえば澱粉、アルギン酸、アルギネート、グリコール酸ナトリ
ウム澱粉、微結晶セルロースのような崩壊剤;たとえば炭酸塩および酸のような
起泡剤;染料;甘味料;たとえばレシチン、ポリソルベート、ラウリルサルフェ
ートのような湿潤剤、並びに一般に医薬組成物の処方に用いられる無毒性かつ医
薬上不活性な物質と一緒に含有する。この種の医薬組成物は公知方法にて、たと
えば混合、粒状化、錠剤化、糖衣もしくは薄膜被覆プロセスによって製造するこ
とができる。フィルム形成性化合物は、吸収および最大効果に関し腸管における
適正箇所にて放出するよう選択することができる。すなわちpH依存性フィルム
形成体を用いてそのまま腸にて吸収させることができ、これにより異なるフタレ
ートを使用し或いはアクリル酸/メタクリル酸誘導体およびポリマーを用いるこ
とができる。

- 【請求項1】 メチルプレニゾロンを除くメタロプロテナーゼ抑制剤、化学
改質テトラサイクリンを包含するテトラサイクリン、キノロン、コルチコステロ
イド、タリドマイド、ラザロイド、ペントキシフィリン、ヒドロキサミン酸誘導
体、炭素環式酸、ナプトピラン、可溶性サイトキンリセプタ、TNF−αに対す
るモノクローナル抗体、アムリノン、ピモベンダン、ベスナリノン、ホスホジエ
ステラーゼIII抑制剤、ラクトフェリンおよびラクトフェリン誘導同族体およ
びメラトニンよりなる群から選択される塩基またはその付加塩の形態としての、
脊髄盤TNF−αを抑制することによるTNF−αの放出または存在により停止
されるTNF−αおよび化合物の放出によって生ずる神経根障害としての脊髄障
害を処置する医薬組成物を作成する際の、TNF−α抑制剤の使用。 - 【請求項2】 活性成分がテトラサイクリン、ドキシサイクリン、リメサイ
クリン、オキシテトラサイクリン、ミノサイクリンおよび化学改質テトラサイク
リン、デジメチルアミノテトラサイクリンよりなる塩基または付加塩の形態から
選択される請求項1に記載の使用。 - 【請求項3】 活性成分がドキシサイクリンである請求項2に記載の使用。
- 【請求項4】 活性成分がヒドロキサミン酸化合物、炭素環式酸および誘導
体、タリドマイド、ラザロイド、ペントキシフィリン、ナプトピラン、可溶性サ
イトキンリセプタ、TNF−αに対するモノクローナル抗体、アムリノン、ピモ
ベンダン、ベスナリノン、ホスホジエステラーゼIII抑制剤、メラトニンから
選択される塩基または付加塩の形態の請求項1に記載の使用。 - 【請求項5】 活性成分がノルフロキサシン、オフロキサシン、サイプロフ
ロキサシン、ガチフロキサシン、ペフロキサシン、ロメフロキサシンおよびテマ
フロキサシンから選択される塩基または付加塩の形態の請求項1に記載の使用。 - 【請求項6】 活性成分が塩基または付加塩の形態におけるメタロプロテナ
ーゼ抑制剤である請求項1に記載の使用。 - 【請求項7】 活性成分が塩基または付加塩の形態におけるTNF−α(た
とえばインターフェロン−γ、インターロイキン−1および酸化窒素(NO))
の放出により停止される化合物を抑制する物質からなる請求項1に記載の使用。 - 【請求項8】 人間を含め哺乳動物におけるTNF−αの放出によって生じ
た神経根障害としての脊髄障害の処置方法において、メチルプレドニゾロンを除
くメタロプロテナーゼ抑制剤、化学改質テトラサイクリンを含めテトラサイクリ
ン、キノロン、コルチコステロイド、タリドマイド、ラザロイド、ペントキシフ
ィリン、ヒドロキサミン酸誘導体、炭素環式酸、ナプトピラン、可溶性サイトキ
ンリセプタ、TNF−αに対するモノクローナル抗体、アムリノン、ピモベンダ
ン、ベスナリノン、ホスホジエステラーゼIII抑制剤、ラクトフェリンおよび
ラクトフェリン誘導同族体およびメラトニンよりなる群から選択される塩基また
