本発明は、インプラントその製造方法及びその使用方法に関する。

【従来の技術】
米国特許第５，２１７，４９６号（ブルース）には、組織適合型微粉材料及び
組織適合性生物学的分解材料の混合物からなる層を含むインプラント（人工補装
具）が開示されている。混合物の成分は、栄養溶液を加えることによって互いに
成長し且つリンクさせることができる。この特許には、更に、成形型を使用した
このようなインプラントの製造方法が開示されている。
【０００３】 米国特許第５，０１５，２５６号（ブルース）には、人工関節をセメントなし
で固定するための手段が開示されており、この手段は、５ｍｍ以下の大きさの本

【０００４】 上文中に説明した固定技術を使用して実施した実験により、顆粒状材料が、イ
ンプラントが挿入され又は配置されるべき組織と同じ種類の組織で形成された内
部成長材料を含む場合、例えば固定されるべき人工補装具が人工大腿骨である場
合に大腿骨窩からの骨組織を含む場合、人工補装具の固定が特に迅速に且つ安定
的に、永久的に且つ痛みなく正確に行われることがわかった。骨組織は、粗粒を
包囲し且つキャビティの壁から人工補装具まで延びる骨（細胞）を形成する。更
に、これらの実験によれば、リンクした粗粒材料（及び内方成長材料）により人
工補装具が迅速に固定されることが示されている。粗粒間の距離が短ければ短い
程、身体細胞の成長の傾向が高まるように見える。

【０００６】 米国特許第５，０１５，２５６号では、身体のキャビティは、米国特許第５，
２１７，４９６号による方法で成形型を構成し、細胞を成長するための自然の栄
養溶液、即ち内方成長体液、例えば血液を含むと言われている。

【０００９】 バッチによる嚢／ラップの好ましい充填は、インプラントの目的に必要な充填
程度まで行われる。インプラントの目的が、椎骨等の身体の部分を支持すること
又はこのような身体の部分間の距離を保持することである場合には、充填の程度
は、例えば形成手術の目的及び他の目的で、リウマチ用骨成長促進剤の場合に身
体のキャビティを充填する場合よりも大きくなければならず、即ち支持−離間機
能を提供できなければならない。
【００１０】 嚢／ラップ内のバッチは、有利には、振動によって圧縮できる。振動は、バッ
チの成分が適当に混合され、栄養物が粗粒／顆粒の孔に進入するといった別の利
点を提供する。振動は、混合を行うための特定の比較的高い周波数、及び圧縮を

によって椎骨間等に固定されることが期待される場合には、特別の固定手段を使
用する必要はない。
【００１３】 インプラントは、キャビティを良好に充填する形状に圧縮する際及び挿入され
るべき距離に合わせて形成される。このことは、そう（身体組織と嚢との間の距
離）でない場合に、インプラントが成長しない危険又は嚢と身体組織との間に連
結組織が形成される危険があるため、重要である。
【００１４】
【発明が解決しようとする課題】
本発明によるインプラントの形状は、平らなプレート、ストリップ片、円筒体
、ロッド、等の随意の形状であるのがよいということが明らかになった。
【００１５】 前記混合物が入った嚢は、バッチを捏ねることができ且つ捏ねることによって
形成された嚢／ラップの形状を保持できるようにする別の／何等かの別の組織適
合性成分をバッチで使用することによって賦形できる。適当な成分は、例えば、
イミューノ（ＩＭＭＵＮＯ）（スイス）ＡＧが市販している硬化性二成分フィブ
リン接着剤である。別の適当な成分は、米国のフォーカル社が販売している手術
用シーリング剤であるフォーカルシール（Ｆｏｃａｌ Ｓｅａｌ）である。しか
しながら、血液（フィブリン及び凝固剤を含有する）自体が、混合物を嚢／ラッ
プの内側又は外側で、インプラントが装着されるべきキャビティの形態に合わせ
て成形できる適当な別の成分であるということを強調しなければならない。成形
又は賦形を行うためのパターンとして、例えば、問題の身体キャビティのＸ線記
録を使用できる。
【００１６】 便利であることを考えると、嚢／ラップは、再吸収性材料でできているのがよ
い。一例は、エチコン社から入手できるサージセル（サージセル（ＳＵＲＧＩＣ
ＥＬ）は登録商標である）である。
【００１７】 組織適合性粗粒／顆粒の材料として、本発明によれば、先ず最初にチタニウム
を選択できるが、この目的について、バイオセラミックス、バイオガラス、ヒド
ロキシアパタイト、ポリマー、デキスラン等の当業者に周知の他の材料も適して
いる。それ自体が多孔質でないチタニウム等から、材料の溶融体にガス又は液体
を吹き込むことによって、従来技術の方法で多孔質粗粒／顆粒を得ることができ
る。
【００１８】 粗粒／顆粒は、粒径分布が本質的に均等であり、好ましくは塑性であり且つ不
規則である。この目的は、振動によって相互係止し圧縮する場合に、異なる粒径
が身体のキャビティ内に層をなして配置され、これにより不規則性の危険及びか
くして組織の成長を阻害が起こってはならないためである。本質的に均等な粒径
分布というのは、粗粒／顆粒直径の変化が±５０％、好ましくは±２５％又はそ
れ以下であるということを意味する。粗粒／顆粒の絶対粒径は、比較的広い範囲
で変化し、５ｍｍ以下の大きさの粗粒／顆粒が最も便利であると考えられる。下
限は設定が困難であり、非常に小さな粗粒子を小さな粒子（ダスト）を形成する
生体親和性液体と組み合わせて使用できる。しかしながら、通常は、０．１ｍｍ
以上の粗粒／顆粒が使用される。好ましくは、上限は約２ｍｍであり、下限は０
．５ｍｍである。一般的には、粗粒／顆粒の粒径は、手術の完了後に粗粒／顆粒
で充填しなければならない空間を考慮して選択される。即ち、大きな身体のキャ
ビティに対しては小さなキャビティよりも大きな粗粒／顆粒を選択できる。「粗
粒／顆粒」、「不規則」、及び「直径」という用語は、（ほぼ）球形以外の形態
を含もうとするものである。
【００１９】 骨組織の交換又は修復のために本発明によるインプラントを使用しようとする
場合には、粗粒／顆粒は、最も好ましくは、塑性即ち本質的には弾性でない連続
した多数の孔が設けられた生体親和性材料、好ましくは以下の多孔特徴を持つチ
タニウム等の金属又は金属合金でできている。多孔特徴は、 孔が連続しており、 ピット／窪み／ポケットの開口部及びこれらを相互連結するチャンネル／通路
が、骨組織について約５０μｍ以上の幅を有する。このような多孔特徴により、
粗粒内に空所ができ、これらの空所がチャンネルや通路によって相互連結され、
その結果、粗粒の外面の一部へ骨組織が成長することにより、成長が個々の粗粒
を通して連続し、粗粒の外面の他の部分を通して出る。
【００２０】
【発明が解決しようとする課題】
本発明によれば、バッチ成分を混合して上述のバッチを形成する工程は、バッ
チを嚢に導入する前に、又はバッチを包む前に実施できる。これに関し、栄養物
のバッチを混合物に加える。別の態様では、及び本発明によれば、嚢に導入した
後に又はバッチを包んだ後に混合を行うことができる。
【００２１】 例えば、椎骨間の摩耗した椎間板を交換するための脊柱手術では、椎骨間にね
じ込まれるインプラントが使用される場合が多い。このようなインプラントは剛
性であり、骨片を含む。米国特許第４，５０１，２６９号及び米国特許第５，４
８９，３０８号を参照されたい。しかしながら、このような骨片は十分な量を入
手できず、このようなインプラントは支持機能を有し、痛みを生じる。更に、こ
のようなインプラントは複雑であり、製造に費用がかかる。

【課題を解決するための手段】
本発明は、これを緩和し、脊柱を安定化させるための上述の種類のインプラン
トを教示する。

【発明の実施の形態】
以下、本発明の実施の形態を図面により説明する。 図１は、二つの椎骨Ｋ間に挿入した、上文中に説明した内容物が入った２つの
環状−円筒形の嚢１の概略図である。嚢１にはバッチ（粗粒／顆粒材料はチタニ
ウム製である）が十分に充填してあり、振動を加えて適切に混合し圧縮してあり
、その結果、椎骨間の距離を正しく保持できる。骨が迅速に形成し、支持機能を
取り戻す。嚢は、上述の例示の再吸収性材料でできている。図１のａは、ａ−ａ
線での断面図である。
【００２４】 図２は、寛骨インプラント３を寛骨窩に固定するため、寛骨窩Ｓに挿入した本
発明によるインプラント２を示す。前記寛骨インプラント３は、チタニウム５で
コーティングした従来のプラスチックカップ４を含み、このカップは、寛骨窩に
従って形成された薄い嚢１にプレス嵌めで載止している。嚢１には、粗粒／顆粒
がチタニウムでできた上文中に説明したバッチが入っている。嚢１もまた、織布
の形態の上文中に説明した例示の再吸収性材料でできている。
【００２５】 本発明の一つの別の実施例では、脊柱インプラントの内部チャンバは、例えば
米国特許第５，０１５，２４７号及び米国特許第４，５０１，２６９号に記載さ
れた種類であり、本発明によるインプラントによって充填される。

Claims (18)

    【特許請求の範囲】
  1. 【請求項１】 組織適合型の多孔質粗粒／顆粒材料及び組織適合性生物学的分解材料（好まし
    くは骨粉等の内方成長材料）の混合物のバッチを含むインプラント（人工補装具
    ）において、バッチは、バッチの賦形又は成形を可能にする一つの追加の組織生
    体親和性成分を含み、前記バッチは、組織適合性可撓性材料でできた、生物学的
    材料の組織の内方成長及び外方成長を可能にする大きさの小孔／孔／ミシン目線
    等を持つ嚢又はラップ内に封入されていることを特徴とするインプラント。
  2. 【請求項２】 前記可撓性材料は、再吸収性織布の一つであり、例えば再生セルロース又はポ
    リマーでできていることを特徴とする請求項１に記載のインプラント。
  3. 【請求項３】 前記粗粒／顆粒材料は、チタニウム又はポリマー又はデキストランでできてい
    ることを特徴とする請求項１又は２に記載のインプラント。
  4. 【請求項４】 前記バッチは、組織適合性生物学的材料の前記バッチ内での成長を促す種類の
    栄養物／栄養溶液を含むことを特徴とする請求項１、２、又は３に記載のインプ
    ラント。
  5. 【請求項５】 前記別の成分は、硬化性成分及びこの成分用の硬化剤であることを特徴とする
    請求項１乃至４のうちのいずれか一項に記載のインプラント。
  6. 【請求項６】 前記別の成分は、血液であることを特徴とする請求項１乃至５のうちのいずれ
    か一項に記載のインプラント。
  7. 【請求項７】 粗粒／顆粒の大きさは、０．１ｍｍ乃至５ｍｍであり、好ましくは０．５ｍｍ
    乃至２ｍｍであることを特徴とする請求項１乃至６のうちのいずれか一項に記載
    のインプラント。
  8. 【請求項８】 粗粒／顆粒の前記バッチは、嚢／ラップ内で圧縮されることを特徴とする請求
    項１乃至７のうちのいずれか一項に記載のインプラント。
  9. 【請求項９】 前記粗粒／顆粒は塑性であり即ち本質的に弾性でなく、並びに多孔質であり、
    以下の多孔特徴を有する、即ち孔が連続しており、ピット／窪み／ポケットの開
    口部及びこれらを相互連結するチャンネル／通路が、骨組織について約５０μｍ
    以上の幅を有することを特徴とする請求項１乃至８のうちのいずれか一項に記載
    のインプラント。
  10. 【請求項１０】 椎骨間人工補装具としての請求項１乃至９のうちのいずれか一項に記載のイン
    プラントの使用。
  11. 【請求項１１】 人工股関節用固定剤としての請求項１乃至９のうちのいずれか一項に記載のイ
    ンプラントの使用。
  12. 【請求項１２】 例えば形成手術での充填剤としての請求項１乃至９のうちのいずれか一項に記
    載のインプラントの使用。
  13. 【請求項１３】 リウマチを治療する場合の骨成長促進剤としての請求項１乃至９のうちのいず
    れか一項に記載のインプラントの使用。
  14. 【請求項１４】 身体のキャビティ、ピット、窪み、等の充填剤としての、又は身体の部分間、
    例えば椎骨間のキャリヤとしての請求項１乃至９のうちのいずれか一項に記載の
    インプラントの使用。
  15. 【請求項１５】 身体内でのスペーサとしての請求項１乃至９のうちのいずれか一項に記載のイ
    ンプラントの使用。
  16. 【請求項１６】 欠陥／除去組織の強化体としての請求項１乃至９のうちのいずれか一項に記載
    のインプラントの使用。
  17. 【請求項１７】 組織（骨）交換体としての請求項１乃至９のうちのいずれか一項に記載のイン
    プラントの使用。
  18. 【請求項１８】 従来のインプラントと相補的である請求項１乃至９のうちのいずれか一項に記
    載のインプラントの使用。
