発明の分野
本発明は、中空の可撓性体構造の治療のための、より詳細には、肺の中のブレ
ブ(bleb)の治療のための装置および方法に関する。
発明の背景
ブレブは、大きさが約3mmから数センチメートルの範囲であり得る肺の中の異
常な小胞である。肺胞壁が悪化し、それによって個々の肺胞の一団が1つまたは
それ以上のブレブに変化したとき、ブレブはしばしば発達する。肺胞は小さく、
フィブリル化した結合組織から構成された多面体の凹部であり、そして少量の不

発明の要旨

囲の熱に変換される。温度が上がるにつれて、コラーゲンは構造変形を受け、そ

RFエネルギーは十分に変形してしまった後(例えば、治療部位からの加熱装
置の除去および視診の実行により判断し得るコラーゲンの収縮)、もはや適用さ
れない。十分な収縮はまた、透視、外部超音波スキャニング、パルス−エコー超
音波スキャニング、虚脱の検知あるいは適切なフィードバック可変物での加熱要
素の整備、インピーダンスモニタリングあるいは任意の他の適切な肺の機能試験
方法によって検出され得る。
実質的な変換は、特定の治療条件に依存して非常に速く達成され得る。変換は
むしろ迅速な速度で進行し得るので、RFエネルギーは低電力レベルで適用され
るべきである。好ましくは、RFエネルギーは、約1秒〜約120秒の範囲の時
間の長さの間適用される。適切なRF電源は市販されており、当業者に周知であ
る。1つの実施態様において使用されるRF発生装置は、単一チャンネルを有し
、約1〜10ワットのRFエネルギーを送達し、そして連続生産能力を有する。
変換速度は加熱要素に送達されるエネルギーを変化させることによって制御され
得る。
加熱要素を活性化させるためのRFエネルギーを使用する代わりに、エネルギ
ーの他の形態(例えば、交流、マイクロ波、超音波、および干渉性(例えば、レ
ーザー)または非干渉性(例えば、発光ダイオードまたはタングステンフィラメ
ント)のいずれかの光)が使用され得、そして抵抗コイル、熱流体要素(例えば

バルーンを形成する治療装置の部分は、膨張可能でありかつ空気または他の適
切なガスに実質的に不通気性の材料で作られる。この様では、細長い部材のこの
部分は、圧縮ガスまたは任意の他の適切な力あるいはこの細長い部材の内部領域
に適用された物質に応じて、放射状に膨張可能でありかつ変形可能である。さら
に、この細長い部材は、内力が非活性化されるかまたはこの材料が取り除かれた
場合、その元の膨張していない形状に実質的に戻る。
図3および4は、気管支鏡30を示し、気管支鏡30は治療装置10を有し、
治療装置10は管腔内に滑動可能に配置されている。この装置はまた、画像伝送
ファイバ50および照明ファイバ52を含む。適切なサイズと方向を持った作業
管腔を備えた任意の従来の気管支鏡が、用いられ得る。画像伝送ファイバは、治
療装置の遠位端から光を集め、そしてこの光を塞がれた気管の像を表示するため
に、観察装置(図示せず)に方向付ける。気管支鏡は、パンシステムを有し得、
これはその先端部を異なる方向に動かすことを可能にする。
特定の部位を治療する場合、過剰の体液がまず、従来の手段により(例えば、
吸引カテーテルを用いて)、ブレブから取り除かれる。その後、気管支鏡は、そ
のヒトの鼻腔または口腔から、気管、主気管支管を通って、ブレブに前進され得
る。加熱治療装置は、RF発生器に接続され、このRF発生器は、装置のハンドルに
設置され得るかまたは患者から遠く離れて設置され得る。
治療装置は、取付手段(例えば、フックまたは突起装置)が作動される前に、
気管支鏡から前進される。その後、RF発生器にエネルギーが与えられる。電極の
数および/または表面積に依存して、この治療装置は、さらなる加熱治療のため
に、別の部位へ動かされ得る。治療の完了後、電極へのRFエネルギーは中断され
、取付手段が取り外され、次いで、気管支鏡が患者から取り除かれる。
加熱装置は、コラーゲンの変性を起こし得る結合組織の過熱に対する保護を提
供するように作製され得る。温度モニタリングおよびインピーダンスモニタリン
グが、システムで利用され得、これは、使用者に、音、光、他の表示の形態のフ
ィードバックを提供するか、または、十分な変形が検出された場合、および治療
部位の焼成を避けるために、加熱要素から治療部位へのエネルギー適用を停止す
る。適用されるエネルギーの量は、特定の条件下で、手動または自動で、減少ま
たは除去され得る。例えば、気管壁または加熱要素の温度がモニターされ得、適
用されるエネルギーが従って調整され得る。外科医は、所望ならば、フィードバ
ック制御システムを無効にし得る。マイクロプロセッサが含まれ得、電源のスイ
ッチをオンおよびオフにするためならびに電源を調節するために、フィードバッ
ク制御システムに組み込まれ得る。マイクロプロセッサは、温度をモニターし、
電源を調節するための制御装置として働き得る。同様に、治療装置は、キャビテ
ィの過剰吸引および/またはバルーンの過剰膨張に対するフィードバック保護を
提供し得る。
図5は、本発明の別の治療装置の1実施態様を示し、この装置は、細長い円柱
状部材50を含み、この部材は加熱要素を有し、この加熱要素は、部材の外表面
に設置される複数の62および64と命名された電極を有する。電極は、RFエネ
ルギー源に、コネクタ68を介して、電気的に接続される。好ましくは、各電極
は、約0.5mm〜約3mmの幅を有するように示される帯のように構成され、好まし
くは、各電極帯は約1mm〜5mmの距離でその次の電極帯から離れている。加熱要
素は1つ以上の電極帯を含むことが理解される。この装置は、遠位端70を有し
、この遠位端70は、装置が気管内に前進されたときに出くわす抵抗の量を減少
するために、放物状に成形される。遠位端は、管腔72を含み、この管腔72は

この装置は、治療されるべきブレブに導く細気管支の所望の最終内径にほぼ等
しい(または等しいまで膨張可能であり得る)外径を有する。典型的には、この
外径は、約2フレンチ〜約6フレンチの範囲である。
治療要素の機能は、壁のコラーゲンに管腔を封止するための構造変形を起こさ
せるために十分な量のエネルギーを細気管支壁に適用することである。明らかな
ように、各電極帯の数および幅は、重要でない。電極帯が1つだけ存在する場合
、十分な量のコラーゲンを変形させるために、装置を動かし、管腔壁の1つより
多い領域を加熱する必要があり得る。部材50はまた、好ましくは、本明細書中
で記載されるカテーテルまたは気管支鏡を通して巧みに動くような可撓性材料か
ら作製される。
治療装置は、ブレブキャビティに導かれる細気管支内に挿入され得る。その後
、バルーン装置は、空気または適切な流体で膨張され、その結果、バルーン外表
面が細気管支内表面と物理的に接触する。例えば、過剰膨張を避けるために、フ
ィードバック機構として採用され得る従来の圧力ゲージ85が、組み込まれ得る
。次に、空気が管腔72を通してブレブから引かれ、この管腔72は、吸引装置
(図示せず)に接続されている。吸引器はブレブ壁の圧潰を起こし、最終的にブ
レブキャビティのサイズを減少させるのに十分な吸引を作り出す。
吸引が適用された場合、ブレブ壁を形成する組織が、管腔72に向かって引き
寄せられ、最終的に、ブレブは、組織が管腔内部を通るに従って、陥入または裏
返しにされる。この組織の加熱を容易にするために、1実施態様(図5で示され
る装置)では、電極88は、管腔72の内表面に配置される。この方法では、ブ
レブ組織が吸引によって管腔内に引かれるに従って、陥入したブレブは電極88
によって熱シールされ得る。あるいは、電極は、ブレブに導かれる細気管支を加
熱するために、または細気管支を陥入したブレブにシールするために、装置の先
端部に配置され得る。いずれの方法でも、バルーンは、加熱治療プロセスの間、
膨張されたままであり、真空が保たれる。この手順を容易にするために、電極帯
は、治療装置の遠位部分70付近に配置され得る。さらに、この遠位部分は、治
療装置が、その直径が減少するに従って、細気管支管腔からしだいに取り除かれ
るように先細にされ得る。
治療装置が治療部位に配置された場合、RF発生器が活性化されて、適切なRFエ
ネルギーが、好ましくは、10MHz〜1000MHzの範囲で選択された周波数で、提供さ
れる。放射エネルギーは、組織内で、約40℃〜約95℃の範囲の熱に変換される。
RFエネルギーは、コラーゲン繊維が十分に変形(例えば、収縮)した後(これは
、加熱装置を治療部位から取り除き、そして管腔が圧潰されないままであるかど
うかを視覚的に決定することによって判断され得る)は、もはや適用されない。
十分な収縮はまた、蛍光透視、外部超音波走査、パルス-エコー超音波走査、適
切なフィードバック変数を備えた加熱要素の圧潰または矯正のセンシング、イン
ピーダンスモニタリング、または任意の他の方法によって、検出され得る。
加熱要素にエネルギーを与えるためにRFエネルギーを用いる使用するのに加え
て、図1の装置について記載されたような他の形態のエネルギー(交流、マイク
ロ波、超音波、およびコヒーレント(例えば、レーザ)または非コヒーレント(例
えば、発光ダイオードまたはタングステンフィラメント)のいずれかの光を含む
)が使用され得ること、ならびに抵抗コイル、熱流体要素(例えば、循環液体、
気体、および液体と気体との組み合わせ)から産生される熱エネルギーもまた使
用され得ることが理解されるべきである。
図5に示される加熱要素は、患者の体内で、単極内部電極として作動する。電
極帯よりもずっと大きな表面積を有する外部電極(図示せず)は、患者の身体の
外表面に配置される。双極電極を含む加熱要素もまた使用され得る。
加熱要素が電極帯として示されているが、例えば、らせん状、環状および格子
パターンのような他の配置が使用され得る。これらの要素は、管腔壁に対応する
パターンを作製する。1つの制約は、熱要素は、加熱治療プロセスが合理的な時
間内で完了し得るような、管腔壁と接触した十分な表面積を有することである。
