Abstract
L'URETRO-RENOSCOPE EST CONSTITUE D'UNE PIECE RAPPORTEE D'INSTRUMENT 1 PASSANT DANS UNE TIGE EXTERIEURE ET ACCOUPLABLE DU COTE PROXIMAL, DANS LAQUELLE EST GUIDE UN DISPOSITIF OPTIQUE 2 DEVANT LEQUEL, DU COTE PROXIMAL DE SA SORTIE DISTALE, EST PREVU UN DISPOSITIF DE DEVIATION COMMANDABLE 3 SERVANT A DEVIER UN INSTRUMENT AUXILIAIRE FLEXIBLE 4 QUI PASSE DANS LA PIECE RAPPORTEE 1. L'URETRO-RENOSCOPE PERMET D'ENLEVER DES CALCULS QUELLE QUE SOIT LEUR POSITION DANS LE REIN.
Description
La présente invention concerne un urétro-rénoscope dans la tige duquel
passe une pièce rapportée d'instrument accouplable Pour les manipulations dans la région des groupes de calices moyen et inférieur du rein ont été utilisés jusqu'à maintenant des instruments auxiliaires flexibles précourbés qui cependant ne sont pas commandables de manière dirigée et ne peuvent pas conserver leur position pendant de longues durées. Ces instruments auxiliaires sont donc d'utilisation restreinte et de manipulation
très compliquée.
L'invention a pour objet de permettre d'enlever des calculs ou du tissu du rein, quelle que soit leur position dans le rein, au moyen d'instruments auxiliaires flexibles et sous contrôle visuel, de coaguler des tumeurs ou autres au laser ou par haute fréquence ou de passer
une canule de manière dirigée depuis le rein vers l'exté-
rieur pour poser un canal de néphrostomie (sous contrôle visuel). L'invention atteint ce but grace au fait que sur l'urétro-rénoscope mentionné au début, dans la pièce rapportée d'instrument est guidé un dispositif optique du côté de la sortie duquel, avant cette sortie dans la direction proximale est prévu un dispositif de déviation commandable du côté proximal servant & dévier un instrument
auxiliaire flexible qui passe dans la pièce rapportée.
Ainsi, le dispositif de déviation se trouve du coté de la sortie du dispositif optique, de sorte que la déviation de l'instrument auxiliaire peut Otre observée
dès sa sortie du canal d'instrument et contrôlée visuelle-
ment en permanence. L'urétro-rénoscope permet donc une
manipulation dirigée dans le rein.
L'invention est expliquée ci-après à l'aide du dessin annexé sur lequel: Fig. 1 est une vue de côté d'une pièce rapportée
d'instrument pour un urétro-rénoscope.

Fig. 2 montre l'extrémité distale de la pièce rapportée d'instrument avec un levier d'Albarran modifié, qui est représenté en coupe sur la fig. 2a, Fig. 3 montre l'extrémité distale de la pièce rapportée d'instrument avec un dispositif de déviation d'instruments auxiliaires modifié, et Fig. 3a et 3b montrent le dispositif de déviation
de la fig.3 en position de repos et en position déviée.
La pièce rapportée d'instrument 1 peut être
accouplée par un cône la à un urétro-rénoscope non repré-
senté. Dans cette pièce rapportée 1 est introduit un dispo-
sitif optique 2 et s'étend un canal pour le passage d'un
instrument auxiliaire 4 qui peut être dévié par un disposi-
tif de déviation 3 au moyen d'une commande proximale.
Ce dispositif de déviation se trouve avant la sortie du dispositif optique sur le côté de celle-ci et, sur les fig. 1 et la, est un levier d'Albarran 3 qui est pourvu, sur sa face dirigée vers l'extérieur, d'un creux en forme de rigole 3a destiné à recevoir l'instrument auxiliaire 4. Sur les fig. 2 et 2a, le levier d'Albarran 3 est pourvu d'un trou percé longitudinal 3b destiné au
guidage de l'instrument auxiliaire 4.
Sur les fig. 3, 3a et 3b, le dispositif de dévia-
tion 3 est constitué de segments annulaires articulés les uns aux autres dont l'ensemble peut, comme le montre
la fig. 3b, être courbé au moyen d'un fil de tirage manoeu-
vrable du côté proximal pour la déviation de l'instrument
auxiliaire 4 qui passe dans le dispositif.
Claims (3)
1. Urétro-rénoscope dans la tige duquel passe une pièce rapportée d'instrument, accouplable caractérisé en ce que dans la pièce rapportée d'instrument (1) est guidé un dispositif optique (2) du côté de la sortie duquel, avant cette sortie dans la direction proximale, est prévu un dispositif de déviation (3) commandable du côté proximal servant à dévier un instrument auxiliaire flexible (4)
qui passe dans la pièce rapportée (1).
2. Urétro-rénoscope selon la revendication 1, caractérisée en ce que le dispositif de déviation est un levier d'Albarran (3) pourvu d'un creux en forme de rigole (3a) ou d'un trou percé longitudinal (3b) pour
le guidage de l'instrument auxiliaire (4).
3. Urétro-rénoscope selon la revendication 1,
caractérisé en ce que le dispositif de déviation est cons-
titué de segments annulaires (3) articulés les uns aux autres et traversés par l'instrument auxiliaire (4) dont l'ensemble peut être courbé au moyen de fils de tirage
manipulés du côté proximal.
