Abstract
Un instrumento (10) de grapado quirúrgico, que comprende: un conjunto (22) de cartucho de grapas que tiene una pluralidad de filas de ranuras (36) de recepción de grapas, un conjunto (30) de yunque que tiene un miembro de yunque (26), que define una pluralidad de filas de rebajes (27) de conformación de grapas, teniendo el conjunto de cartucho de grapas, el conjunto de yunque, o ambos, uno o más miembros de fijación (132, 232); y un material (50, 50a) de refuerzo de la línea de grapado soldado ultrasónicamente a los miembros de fijación, definiendo el material de refuerzo de la línea de grapado unas perforaciones (68) adyacentes a los miembros de fijación, siendo las perforaciones para facilitar el desprendimiento del material de refuerzo de la línea de grapado con respecto al instrumento de grapado quirúrgico, en donde el uno o más miembros de fijación están dispuestos en el conjunto de cartucho de grapas, y situados hacia fuera de las filas de ranuras de recepción de grapas y/o el uno o más miembros de fijación están dispuestos en el conjunto de yunque y situados hacia fuera de las filas de rebajes de conformación de grapas.

Grapadora circular y material de refuerzo de lmea de grapado Antecedentes
1. Campo de la invencion
La presente solicitud se refiere al grapado quirurgico y a materiales de refuerzo de la lmea de grapado. En particular, se fija el material de refuerzo de la lmea de grapado a una o ambas superficies de contacto con tejido de una grapadora quirurgica, utilizando un reten.
2. Antecedentes
Es conocido el uso de materiales de refuerzo de la lmea de grapado, o contrafuertes, junto con las grapadoras. La Patente de Estados Unidos n.° 5.542.594, de McKean y otros, describe un aparato de grapado quirurgico con un tejido quirurgico biocompatible, unido al aparato. El tejido quirurgico y las grapas se fijan al tejido corporal mediante el aparato de grapado. El tejido se asegura a las superficies del aparato de grapado mediante pasadores o presillas.
La Patente de Estados Unidos n.° 7.128.748 describe una grapadora circular y un contrafuerte. El material de contrafuerte esta situado en el cartucho de grapas de la grapadora, y sobre el componente de yunque de la grapadora. El material de contrafuerte del yunque presenta una porcion central cilmdrica elevada, adaptada para encajar en la abertura central rebajada del yunque, y el contrafuerte del cartucho de grapas presenta una porcion central cilmdrica elevada, adaptada para encajar en una abertura central rebajada del componente de cartucho de la grapadora.
La Patente de Estados Unidos n.° 6.503.257 describe un adhesivo que se utiliza para unir de manera desprendible un material de contrafuerte a los miembros de sujecion de una grapadora. El material de contrafuerte se fija de forma desprendible con el material adhesivo.
El documento US 5.263.629 describe un dispositivo de grapado quirurgico que corta y une entre sf las paredes adyacentes de dos secciones de un organo, logrando la hemostasia a lo largo de la lmea de grapado. El dispositivo de grapado incluye un medio para sujetar las paredes adyacentes de las secciones del organo, junto con un material de compresa absorbible que se situa adyacente a la superficie de al menos una de las paredes de seccion del organo. Un medio de grapado forma al menos un par de lmeas paralelas, separadas, de grapas quirurgicas que se extienden a traves de las paredes de las secciones del organo, y del material de compresa absorbible, para distribuir de forma sustancialmente uniforme la presion a lo largo de las lmeas de grapas. El material de compresa se asegura a los bordes longitudinales del cartucho de grapas mediante multiples soldaduras ultrasonicas, separadas, o bien mediante uniones adhesivas separadas.
Existe la necesidad de un tipo de fijacion del material de refuerzo de la lmea de grapado, o del material de contrafuerte, que no complique el montaje o la fabricacion, no interfiera con el funcionamiento del instrumento quirurgico y fije el material de forma segura, al tiempo que permita desprender el material de forma fiable cuando se desee.
Compendio
En un aspecto de la presente descripcion, un instrumento de grapado quirurgico comprende un conjunto de cartucho de grapas, que tiene multiples filas de ranuras de recepcion de grapas, un conjunto de yunque que tiene un miembro de yunque, que define una pluralidad de filas de rebajes de conformacion de grapas. El conjunto de cartucho de grapas, el conjunto de yunque, o ambos, presentan uno o mas miembros de fijacion. Un material de refuerzo de la lmea de grapado esta unido a los miembros de fijacion, mediante soldadura ultrasonica.
El material de refuerzo de la lmea de grapado define unas perforaciones adyacentes a los miembros de fijacion. Dichas perforaciones son utiles para facilitar la liberacion del contrafuerte desde el aparato de grapado quirurgico. En ciertas realizaciones, el instrumento de grapado quirurgico es una grapadora circular. La pluralidad des filas de ranuras de recepcion de grapas pueden ser filas circulares. Tales grapadoras son utiles en procedimientos de anastomosis intestinal, y en otros procedimientos quirurgicos. La pluralidad des filas de rebajes de conformacion de grapas pueden ser filas circulares.
En ciertas realizaciones, el uno o mas miembros de fijacion se forman sobre el miembro de yunque moldeando un plastico. El uno o mas miembros de fijacion pueden disponerse sobre el miembro de yunque; el miembro de yunque se fabrica con metal, mientras que los miembros de fijacion pueden fabricarse con plastico.
El uno o mas miembros de fijacion estan dispuestos hacia fuera sobre el conjunto de cartucho de grapas, con respecto a las filas de ranuras de recepcion de grapas. El material de refuerzo de la lmea de grapado puede estar unido a uno o mas miembros de fijacion, y definir perforaciones adyacentes al uno o mas miembros de fijacion. De esta manera, el material de refuerzo de la lmea de grapado se eleva y aleja del conjunto de cartucho de grapas, separandose en las perforaciones. Cierto material marginal permanece en el conjunto de cartucho de grapas.

En otra disposicion, el uno o mas miembros de fijacion estan dispuestos hacia fuera en el conjunto de yunque, con respecto a las filas de rebajes de conformacion de grapas. El material de refuerzo de la lmea de grapado esta unido al uno o mas miembros de fijacion, y define unas perforaciones adyacentes al uno o mas miembros de fijacion.
El conjunto de yunque puede incluir un cono unido al miembro de yunque. El conjunto de yunque puede incluir un vastago y comprender adicionalmente una porcion de cuerpo tubular, pudiendo montarse el conjunto de cartucho de grapas en la porcion de cuerpo tubular; la porcion de cuerpo tubular tiene una varilla, pudiendo sujetarse el vastago del conjunto de yunque al vastago. En ciertas realizaciones, el instrumento de grapado quirurgico comprende un conjunto de empunadura.
En otro aspecto de la presente descripcion, un instrumento de grapado quirurgico comprende un conjunto de cartucho de grapas que presenta una pluralidad de filas de ranuras de recepcion de grapas, un conjunto de yunque que tiene un vastago y un miembro de yunque; el miembro de yunque define una pluralidad de filas de rebajes de conformacion de grapas. Un reten esta acoplado al vastago y el reten sujeta al conjunto de yunque un material de refuerzo de la lmea de grapado.
El instrumento de grapado quirurgico puede ser una grapadora circular. La pluralidad des filas de ranuras de recepcion de grapas pueden ser filas circulares, mientras que la pluralidad des filas de rebajes de conformacion de grapas seran filas circulares.
En ciertas realizaciones, el reten tiene forma circular y presenta una abertura central para recibir el vastago. El reten puede acoplarse por friccion con el vastago. Por ejemplo, el reten estara formado por un material que presente un coeficiente de friccion con el vastago, seleccionandose el coeficiente de friccion de modo que retenga sobre el vastago el reten y el material de refuerzo de la lmea de grapado.
En ciertas realizaciones, el reten se asegura al vastago utilizando un dispositivo de sujecion. Alternativamente, el reten se asegura al vastago utilizando una relacion de encaje a presion, entre el reten y el conjunto de yunque. El reten, el vastago, o ambos, pueden presentar una textura para mejorar el acoplamiento por friccion de dichas partes. Por ejemplo, la superficie del vastago estara tratada mecanicamente, o el vastago o el reten, o ambos, contaran con un recubrimiento que aumente la friccion entre el vastago y el reten.
El conjunto de yunque puede incluir un cono, unido al miembro de yunque. En ciertas realizaciones, el instrumento de grapado quirurgico presenta una porcion de cuerpo tubular, pudiendo montarse el conjunto de cartucho de grapas en la porcion de cuerpo tubular; la porcion de cuerpo tubular tiene una varilla, pudiendo sujetarse el vastago del conjunto de yunque al vastago. En ciertas realizaciones, el instrumento de grapado quirurgico presenta un conjunto de empunadura.

Descripcion detallada
Se describiran ahora con detalle una realizacion o realizaciones del instrumento de grapado, el reten y el material de refuerzo de la lmea de grapado descritos en la presente memoria, haciendo referencia a los dibujos. En los dibujos, los mismos numeros designan elementos identicos o correspondientes en cada una de las diversas vistas. Como es comun en la tecnica, el termino “proximal” se refiere a la parte o componente que esta mas cerca del usuario del instrumento, mientras que el termino “distal” se refiere a la parte o componente que esta mas alejada del usuario del instrumento.
La FIG. 1 ilustra un instrumento de grapado quirurgico circular, que se designa generalmente con el numero 10. El instrumento 10 de grapado quirurgico incluye un conjunto 12 de empunadura, que tiene al menos una empunadura 14 de accionamiento giratorio y un accionador giratorio 18. Una porcion 20 de cuerpo tubular se extiende desde el conjunto 12 de empunadura. La porcion 20 de cuerpo tubular, que generalmente tiene una forma de seccion transversal circular, puede tener una forma recta o curvada a lo largo de su longitud, y puede ser flexible o relativamente ngida. Se contemplan otras formas de seccion transversal que no sean circulares, de manera que la porcion 20 de cuerpo tubular puede tener una forma poligonal, elfptica, semicircular, ovoide u otras formas. La porcion 20 de cuerpo termina en un conjunto 22 de cartucho de grapas, que incluye una superficie de contacto con tejido orientada distalmente, que define una o mas filas 37 de ranuras 36 de recepcion de grapas. Cada ranura de recepcion de grapas tiene una grapa (no mostrada) dispuesta en la misma. Habitualmente, se proporciona un par de filas circulares 37 de ranuras 36 de recepcion de grapas, aunque se contemplan otras formas, tales como anulares. Un conjunto 30 de yunque esta situado distalmente con respecto al conjunto 22 de cartucho de grapas, e incluye un miembro de yunque 26 y un vastago 28 del yunque asociado operativamente con el mismo. El conjunto de yunque presenta una superficie de contacto con tejido, orientada proximalmente, que define unos rebajes 27 de conformacion de grapas que corresponden a las filas circulares de ranuras de recepcion de grapas. La porcion 20 de cuerpo tubular tiene una correspondiente varilla o vastago 40, situado centralmente con respecto al conjunto 22 de cartucho de grapas. El vastago 28 del conjunto de yunque puede conectarse de forma desprendible a la varilla o vastago 40 de la porcion 20 de cuerpo tubular. El vastago del yunque define un eje longitudinal “x”.
El conjunto 22 de cartucho de grapas puede conectarse al extremo distal de la porcion 20 de cuerpo tubular, o puede estar configurado para encajar concentricamente dentro del extremo distal de la porcion 20 de cuerpo tubular. Habitualmente, el conjunto 22 de cartucho de grapas incluye un dispositivo de empuje de grapas (no mostrado) con una porcion distal, que define dos anillos concentricos de unas perifericamente separadas (no mostradas), cada una de las cuales se recibe dentro de una respectiva ranura 36 de recepcion de grapas. Habitualmente, dentro del conjunto 22 de cartucho de grapas esta dispuesta una cuchilla (no mostrada), que tiene un borde cortante. El borde de la cuchilla es circular, y esta dispuesto radialmente hacia el interior de las filas de grapas. La cuchilla esta

montada de manera que, cuando se haga avanzar axialmente el dispositivo de empuje de grapas en la direccion del conjunto de yunque, la cuchilla tambien avance axialmente. Alternativamente, la cuchilla puede accionarse por separado. El dispositivo de empuje de grapas se hace avanzar en la direccion distal, para accionar las grapas situadas en las ranuras 36 de recepcion de grapas contra el miembro de yunque, de modo que los rebajes 27 de conformacion de grapas hagan adoptar a las grapas una forma cerrada. La cuchilla se hace avanzar y se impulsa hacia el conjunto 30 de yunque, para cortar el tejido. La Patente de Estados Unidos n.° 5.915.616, de Viola y otros, describe un instrumento de grapado circular. Aunque en la FIG. 1 se muestra un instrumento de grapado circular, el dispositivo de grapado puede estar dispuesto para desplegar las grapas de forma semicircular, lineal, o de otra forma deseada. Aunque se analizan con referencia al tejido intestinal, los dispositivos de acuerdo con la presente descripcion pueden disponerse para unir y/o tratar otros tejidos, durante otros procedimientos.
El miembro de yunque 26 puede ser o no ser pivotante alrededor del vastago 28 del yunque, desde una primera posicion inicial en la que un plano, definido por la superficie 52 de contacto con tejido del miembro de yunque 26, este sustancialmente perpendicular al eje longitudinal del vastago 28 del yunque, hasta una segunda posicion, inclinada con respecto al eje longitudinal “x”. Deseablemente, la segunda posicion es una posicion de perfil reducido, en la que el miembro de yunque 26 define un angulo con respecto al eje longitudinal “x”. Alternativamente, el miembro de yunque 26 esta fijado ngidamente al vastago 28 del yunque.
El vastago del yunque 28 esta dimensionado para acoplarse de forma desprendible con un reten de yunque (no mostrado), situado sobre la varilla o vastago 40 de un instrumento de grapado quirurgico circular, tal como el instrumento 10 de grapado quirurgico. En la solicitud de patente provisional de Estados Unidos con n.° de serie 60/281.259, presentada el 3 de abril de 2001, (“la solicitud '259”), se describe tal dispositivo de grapado quirurgico con reten de yunque, y con el cual puede utilizarse el conjunto 30 de yunque.
El reten de yunque funciona para alinear el conjunto 30 de yunque con la porcion 20 de cuerpo (FIG. 1) del instrumento 10 de grapado quirurgico, durante el movimiento del conjunto 30 de yunque desde una posicion separada del conjunto 22 de cartucho de grapas, del dispositivo de grapado quirurgico, hasta una posicion proxima en estrecha alineacion con el conjunto 22 de cartucho de grapas.
El instrumento de grapado circular puede utilizarse para formar una anastomosis entre secciones de tejido intestinal. Como se muestra en las FIGS. 2 y 3, se separa el conjunto 30 de yunque de la varilla o vastago 40 de la porcion 20 de cuerpo tubular, y se asegura al conjunto 30 de yunque una seccion de vaso corporal tubular, tal como una seccion de intestino, habitualmente atando una sutura “P” alrededor del vastago 28 del conjunto 30 de yunque. Se asegura otra seccion de tejido tubular a la porcion 20 de cuerpo tubular, atando una sutura alrededor del vastago 40. Vease la FIG. 2. Se conecta entonces el vastago 28 al vastago 40. Se hace girar el accionador 18 para retirar los vastagos 28, 40, aproximando de este modo el conjunto 30 de yunque al conjunto 22 de cartucho de grapas. Para disparar las grapas, se aprietan las empunaduras 14 de accionamiento, lo que hace avanzar el dispositivo de empuje de grapas y la cuchilla (no mostrada). Las grapas pasan a traves de cada seccion de tejido tubular, y adoptan su forma contra el yunque, de manera que las secciones de tejido tubular queden unidas entre sf. La cuchilla corta el tejido radialmente hacia el interior de las filas de grapas, y se retira el material suturado con el dispositivo de grapado circular.
De acuerdo con la presente descripcion, un instrumento de grapado quirurgico presenta un material de refuerzo de la lmea de grapado, retenido en el mismo. En ciertas realizaciones, la cabeza 42 del yunque tiene un rebaje interior 53 que es generalmente anular, y puede incluir un anillo hendido para recibir la cuchilla. Como se muestra en la FIG. 4, un material 50 de refuerzo de la lmea de grapado presenta una abertura central 51 para recibir el vastago 28 del yunque, y esta dimensionado de manera que el material de refuerzo de la lmea de grapado quede superpuesto con los rebajes 27 de conformacion de grapas, del miembro de yunque 26, cuando se coloca el material de refuerzo de la lmea de grapado sobre el vastago 28. Puede quedar una pequena cantidad de material sobrante sobre el borde exterior del miembro de yunque. Un reten 60, que puede estar configurado como una arandela con una abertura central 61, presenta un diametro exterior ligeramente menor que el diametro interior de la cuchilla, de manera que el reten no interfiera con el corte. El diametro interior esta dimensionado para recibir el vastago del yunque, y para quedar retenido por friccion sobre el vastago. La FIG. 5 muestra el conjunto 66 de retencion de material de refuerzo de la lmea de grapado. Cuando se coloca el material de refuerzo de la lmea de grapado sobre el vastago del yunque 28, y se coloca entonces el reten 60 sobre el vastago del yunque 28 por encima del material de refuerzo de la lmea de grapado, el enganche por friccion entre el reten 60 y el vastago mantendra en su sitio el material de refuerzo de la lmea de grapado, durante el uso. El reten 60 puede estar formado por un material que tenga un coeficiente de friccion deseado con el vastago, que normalmente sera de metal, tal como acero inoxidable.
En otra realizacion, se asegura el reten al conjunto de yunque utilizando un dispositivo de sujecion (tal como un tornillo o perno), una presilla, un fiador, o mediante una relacion de encaje a presion entre el reten y el conjunto de yunque. En una alternativa adicional, el reten, el vastago 28, o ambos, pueden presentar una textura para mejorar el acoplamiento de friccion de dichas partes. Esto puede incluir un tratamiento mecanico de las superficies del vastago y/o el reten, o puede incluir revestimientos.
Durante el uso, se manipula el accionador giratorio 18 para aproximar el conjunto 30 de yunque hacia el conjunto 22 de cartucho de grapas, y retener el tejido situado entre los mismos. Cuando se desplaza la empunadura 14 de

accionamiento giratorio, la cuchilla y/o el dispositivo de empuje se moveran en direccion hacia el conjunto de yunque, para disparar las grapas y cortar el tejido. El conjunto 66 de retencion de material de refuerzo de la lmea de grapado esta dispuesto de manera que el reten 60 este situado hacia dentro con respecto a la cuchilla. El reten retiene el material 50 de refuerzo de la lmea de grapado, contra la superficie 52 de contacto con tejido del miembro de yunque 26. La cuchilla corta el tejido y el material de refuerzo de la lmea de grapado, facilitando la retirada del reten con el instrumento 10 de grapado circular.
En otra realizacion de la presente descripcion, un instrumento de grapado circular como el analizado anteriormente en relacion con las FIGS. 1 a 3 tiene un conjunto 130 de yunque, con un miembro de yunque 126 unido a una cabeza 142 de yunque. El conjunto 130 de yunque incluye adicionalmente un vastago 128 del yunque que puede sujetarse a la varilla o vastago 40. En la FIG. 7 se muestra un miembro de yunque 125 comun de la tecnica anterior, que presenta dos filas de rebajes 127 de conformacion de grapas formados en la superficie 152 de contacto con tejido del miembro de yunque 125. En la FIG. 8 se muestra un miembro de yunque 126 de acuerdo con una realizacion de la presente descripcion. Unos miembros de fijacion 132, que pueden tener la forma de pestanas, estan unidos al miembro de yunque 126 de manera que los miembros de fijacion 132 esten generalmente al ras con la superficie 152 de contacto con tejido. Tambien se contemplan miembros de fijacion rebajados o sobresalientes. De esta manera, el miembro de yunque 126 puede estar formado con un material que sea util para definir rebajes de conformacion de grapas en el mismo, y para conformar las grapas. Asf, el miembro de yunque esta formado habitualmente con un metal, tal como acero inoxidable. Los miembros de fijacion 132 estan formados con un material que sea util para su fijacion a un material de refuerzo de la lmea de grapado. El material 50 de refuerzo de la lmea de grapado se fija a los miembros de fijacion utilizando adhesivos, soldadura ultrasonica o de otros tipos, u otros metodos. El miembro o miembros de fijacion 132 pueden estar formados por diversos materiales polimericos, tales como aquellos materiales polimericos con los que este fabricado el material de refuerzo de la lmea de grapado. En un ejemplo, se forman rebajes en el miembro de yunque, por ejemplo, mediante amolado, taladrado, mecanizado, etc. Se sobremoldea un material polimerico sobre el miembro de yunque, de manera que los miembros de fijacion del material polimerico queden dispuestos en los rebajes. En una realizacion adicional, se fija al miembro de yunque una estructura o anillo 141 de plastico, que tiene unas pestanas que se extienden hacia dentro (vease la FIG. 10), ya sea antes o despues de ensamblar el miembro de yunque 126 con la cabeza 142 de yunque. Las pestanas se encajan a presion en su sitio, en los rebajes preformados en el miembro de yunque. Alternativamente, el estructura o anillo 141 puede fijarse al miembro de yunque de manera que la pestana o pestanas queden dispuestas adyacentes a la superficie 152 de contacto con tejido, pero fuera de los rebajes 127 de conformacion de grapas. En otra realizacion, puede usarse un anillo de plastico sin pestanas, de manera que el material de refuerzo de la lmea de grapado se fije al anillo.
Pueden utilizarse uno o mas miembros de fijacion 132, que pueden presentar diversas formas. Las pestanas mostradas en la Fig. 9 tienen generalmente una forma trapezoidal, y se disponen entre los rebajes de conformacion de grapas del miembro de yunque, como se muestra en FIG. 8. Los miembros/pestanas de fijacion no interfieren con la formacion de las grapas en los rebajes de recepcion de grapas.
Puede resultar deseable fijar un material 50 de refuerzo de la lmea de grapado de manera adyacente al conjunto 22 de cartucho de grapas. Como se muestra en la FIG. 11, el conjunto 22 de cartucho de grapas presenta un rebaje central 24, a traves del cual pasa la varilla o vastago 40 (que se ha omitido en pos de la claridad). Un miembro o miembros de fijacion 232, que pueden tener la forma de pestanas, estan unidos a la superficie 25 de contacto con tejido del conjunto 22 de cartucho. Los miembros de fijacion 232 pueden estar formados de modo que esten generalmente al ras con la superficie 25 de contacto con tejido, pero se contemplan miembros de fijacion rebajados o sobresalientes. El miembro o miembros de fijacion estan formados con un material util para su fijacion al material de refuerzo de la lmea de grapado. El material 50 de refuerzo de la lmea de grapado se fija a los miembros de fijacion utilizando adhesivos, soldadura ultrasonica o de otros tipos, u otros metodos. Los miembros de fijacion 232 pueden estar formados con diversos materiales polimericos, tal como aquellos materiales polimericos con los que este fabricado el material de refuerzo de la lmea de grapado. En un ejemplo, se forman rebajes en la superficie 25 de contacto con tejido del cartucho de grapas, y se sobremoldea un material polimerico sobre el cartucho de grapas en los rebajes. De esta manera, el material del cartucho de grapas puede ser diferente del material de fijacion del material de refuerzo de la lmea de grapado. En otra realizacion, se fija al cartucho de grapas una estructura o anillo 241 de plastico (FIG. 11), que tiene unas pestanas que se extienden hacia dentro, de modo que se encajen a presion las pestanas en su sitio, en los rebajes preformados. Alternativamente, el estructura o anillo 241 puede fijarse al conjunto de cartucho o a la porcion 20 de cuerpo, de manera que la pestana o pestanas queden dispuestas sobre la superficie 25 de contacto con tejido, pero queden situadas hacia fuera de las ranuras 36 de recepcion de grapas. En otra realizacion, puede utilizarse un anillo de plastico sin pestanas, de manera que el material de refuerzo de la lmea de grapado se fije en el anillo.
Alternativamente, el material del cartucho 23 de grapas, que normalmente es plastico, puede seleccionarse de manera que sea util para fijar el material de refuerzo de la lmea de grapado, mediante adhesivos, soldadura ultrasonica o de otros tipos, u otros metodos. En ciertas realizaciones, el conjunto 130 de yunque incluye un anillo hendido dispuesto en el rebaje 153. El anillo hendido puede estar formado con unas pestanas que se extiendan proximalmente hacia la superficie 152 de contacto con tejido, de modo que sea accesible para la fijacion al material de refuerzo de la lmea de grapado.

En las realizaciones analizadas en relacion con las FIGS. 6 a 12, el material 50 de refuerzo de la lmea de grapado puede estar dimensionado para que tenga un diametro interior que sea inferior al diametro de la cuchilla del instrumento de grapado circular. Cuando se acciona la cuchilla, se extirpara parte del material de refuerzo de la lmea de grapado. Esto puede tender a un aumento de las fuerzas de disparo del instrumento de grapado circular. Alternativamente, el material 50 de refuerzo de la lmea de grapado puede estar dimensionado de modo que tenga un diametro interior que sea mayor que el diametro de la cuchilla del instrumento de grapado circular, de manera que la cuchilla no corte el material de refuerzo de la lmea de grapado. Cuando se accione el disparo de grapas, las grapas incidiran sobre el material de refuerzo de la lmea de grapado, pasando a traves del mismo. Las grapas que incidan cerca de la soldadura, u otros puntos de fijacion, aplicaran una fuerza suficiente como para separar sustancialmente la conexion entre el material de refuerzo de la lmea de grapado y el miembro o miembros de fijacion. De este modo, se libera el material de refuerzo de la lmea de grapado. Alternativamente, puede manipularse delicadamente el instrumento de grapado circular para separar suavemente de los puntos de fijacion el material de refuerzo de la lmea de grapado.
En otra realizacion de la presente descripcion, un instrumento de grapado quirurgico y un material de refuerzo de la lmea de grapado de acuerdo con las FIGS. 1 a 3 y 6 a 12 tienen un material 50a de refuerzo de la lmea de grapado, unido a un miembro o miembros de fijacion 132 dispuestos en la superficie 52 de contacto con tejido del conjunto de yunque, y/o dispuestos en la superficie 25 de contacto de tejido del conjunto 22 de cartucho de grapas. El miembro o miembros de fijacion 132 estan formados con un material que sea util para fijar el material 50a de refuerzo de la lmea de grapado. El material 50a de refuerzo de la lmea de grapado esta unido al miembro o miembros de fijacion 132 permanentemente, de manera que el material 50 no esta destinado a su desprendimiento cuando se disparen las grapas, o cuando se manipule el instrumento para retirarlo de la zona quirurgica. El material de refuerzo de la lmea de grapado incluye unas perforaciones 68, o areas de debilidad, adyacentes al miembro o miembros de fijacion. Vease la FIG. 19. El diametro interior del material 50a de refuerzo de la lmea de grapado es mayor que el diametro de la cuchilla. De manera deseable, el miembro o miembros de fijacion 132 se extienden hacia fuera de los rebajes 27 de conformacion de grapas del conjunto 30 de yunque, y/o hacia fuera de las ranuras 36 de recepcion de grapas del conjunto 22 de cartucho de grapas. Las perforaciones 68 estan dispuestas hacia dentro del miembro o miembros de fijacion 132, pero hacia fuera de la cuchilla. Durante el uso, se aproximara el conjunto de yunque al conjunto de cartucho de grapas para sujetar el tejido, y se dispararan las grapas y se desplegara la cuchilla. Al retirar el instrumento de grapado, el material 50a de refuerzo de lmea de grapado se separara del conjunto de yunque y/o del conjunto de cartucho, en las perforaciones. Cierto material marginal permanecera con el conjunto de yunque y/o el conjunto de cartucho.
En otra realizacion, el material 50a de refuerzo de la lmea de grapado tiene un diametro interior que es menor que el diametro de la cuchilla, de manera que se extirpara con la cuchilla una porcion del material 50a de refuerzo de la lmea de grapado.
En otra realizacion de la presente descripcion, se utiliza un instrumento de grapado lineal con un material de refuerzo de la lmea de grapado sobre el yunque, el conjunto de cartucho. o ambos. El instrumento 300 de grapado lineal tiene unas mordazas 310, 320 de grapado. Vease la FIG. 14. La mordaza 310 de grapado es un conjunto de cartucho de grapas que presenta una o mas filas 337 de ranuras 336 de recepcion de grapas. Cada ranura de recepcion de grapas tiene una grapa (no mostrada) dispuesta en su interior. Habitualmente, se proporcionan tres filas lineales 337 de ranuras 336 de recepcion de grapas, a cada lado de un canal 339. La otra mordaza es un conjunto 320 de yunque situado en oposicion al conjunto 310 de cartucho de grapas, y montado de modo pivotante de manera que puedan aproximarse entre sf el conjunto de yunque y el conjunto de cartucho de grapas, para retener tejido entre los mismos. El conjunto de yunque incluye un miembro de yunque 326, que define una pluralidad de rebajes 331 de conformacion de grapas que se corresponden con las filas lineales 337, de manera que el instrumento de grapado forme lmeas rectas de grapas. Las mordazas 310, 320 de grapado estan dispuestas en un extremo distal de un vastago endoscopico 340. Un conjunto 301 de empunadura incluye una empunadura pivotante 303 que acciona el movimiento de un miembro de accionamiento a traves del conjunto 310 de cartucho de grapas. El miembro de accionamiento (que no se muestra) pasa a traves del canal 339 y empuja una corredera o barra de leva, a traves del cartucho de grapas, para accionar unos elementos de empuje de grapas, y las grapas, a traves de las ranuras 336 hacia los rebajes de formacion de grapas del miembro 326 de yunque. En la patente de Estados Unidos n° 6.241.139, de Milliman y otros, se describen ciertas realizaciones de tal instrumento quirurgico.
Para fijar el material de refuerzo de la lmea de grapado a la mordaza o mordazas 310, 330, unos miembros de fijacion estan formados en o sobre el conjunto 310 de cartucho y/o el conjunto 330 de yunque. Habitualmente, el miembro de yunque 326 esta formado con un metal, tal como acero inoxidable. Los miembros de fijacion 332 estan formados con un material que sea util para su fijacion a un material de refuerzo de la lmea de grapado. El material 350 de refuerzo de la lmea de grapado se une a los miembros de fijacion 332 utilizando adhesivos, soldadura ultrasonica o de otros tipos, u otros metodos. El miembro o miembros de fijacion 332 pueden estar formados por diversos materiales polimericos, tales como los materiales polimericos con los que este fabricado el material de refuerzo de la lmea de grapado. En un ejemplo, se forman unos rebajes en el miembro de yunque, por ejemplo, mediante molienda, taladrado, mecanizado, etc. Se sobremoldea un material polimerico sobre el miembro de yunque, de manera que los miembros de fijacion del material polimerico queden dispuestos en los rebajes. En una realizacion, los miembros de fijacion 332 incluyen un primer miembro distal de fijacion 332a, un segundo miembro distal 332b de fijacion, un primer miembro proximal de fijacion 332c, y un segundo miembro proximal de fijacion

332d, de manera que haya uno o mas miembros de fijacion en cada uno de los extremos distal y proximal del miembro de yunque. Vease la FIG. 16.
En una realizacion adicional, se fija al miembro de yunque un estructura o anillo 341 de plastico, que presenta unas pestanas 342 que se extienden hacia dentro (vease la FIG. 18), ya sea antes o despues de montar el miembro de yunque 326 en el conjunto de yunque. Las pestanas se encajan a presion en su sitio, en los rebajes preformados en el miembro de yunque. Alternativamente, se fija el estructura o anillo 341 al miembro de yunque de manera que la pestana o pestanas queden dispuestas adyacentes a la superficie 352 de contacto con tejido, pero fuera de los rebajes 327 de conformacion de grapas. En otra realizacion, puede usarse un estructura o anillo de plastico sin pestanas, de modo que el material de refuerzo de la lmea de grapado este sujeto al anillo.
Puede resultar deseable sujetar un material de refuerzo de la lmea de grapado a la superficie 311 de contacto con tejido del conjunto 310 de cartucho. Se proporcionan unos miembros de fijacion 334 para el conjunto de cartucho de grapas, que estan formados con un material que sea util para su fijacion a un material de refuerzo de la lmea de grapado. El material 350 de refuerzo de la lmea de grapado se une a los miembros de fijacion 334 utilizando adhesivos, soldadura ultrasonica o de otros tipos, u otros metodos. El miembro o miembros de fijacion 334 pueden estar formados por diversos materiales polimericos, tales como aquellos materiales polimericos con los que este fabricado el material de refuerzo de la lmea de grapado. El miembro o miembros de fijacion 334 pueden fabricarse mediante la provision de rebajes en la superficie 311 de contacto con tejido del conjunto 310 de cartucho, y mediante sobremoldeo. Alternativamente, un estructura o anillo de plastico como el mostrado en la FIG. 18 puede incluir o no pestanas, y proporcionar una superficie en la que pueda fijarse el material de refuerzo de la lmea de grapado usando adhesivos, soldadura ultrasonica o de otros tipos, y otros metodos. Alternativamente, el material del cartucho de grapas puede seleccionarse de modo que sea util para sujetar el material de refuerzo de la lmea de grapado.
En otra realizacion de la presente descripcion, un instrumento de grapado quirurgico y un material de refuerzo de la lmea de grapado de acuerdo con las FIGS. 14 a 18 tiene un material 350A de refuerzo de lmea de grapado, unido a un miembro o miembros de fijacion 332 dispuestos en la superficie de contacto con tejido del conjunto 320 de yunque, y/o al miembro o miembros de fijacion 334 dispuestos en la superficie de contacto con tejido del conjunto 310 de cartucho de grapas. El miembro o miembros de fijacion 332, 334 estan formados con un material que sea util para sujetar el material 350A de refuerzo de la lmea de grapado. El material 350A de refuerzo de la lmea de grapado esta sujeto al miembro o miembros de fijacion 332, 334 permanentemente, de manera que el material 350A no esta destinado a su desprendimiento cuando se disparen las grapas, o cuando se manipule el instrumento para retirar el mismo de la zona quirurgica. El material de refuerzo de la lmea de grapado incluye unas perforaciones 368, o areas de debilidad, adyacentes al miembro o miembros de fijacion. Vease la FIG. 20. De manera deseable, el miembro o miembros de fijacion 332, 334 estan situados hacia fuera de las filas 337 de los rebajes 331 de conformacion de grapas del conjunto 30 de yunque, y/o hacia fuera de las filas 337 de las ranuras 336 de recepcion de grapas del conjunto 310 de cartucho de grapas. Las perforaciones estan dispuestas hacia el interior del miembro o miembros de fijacion 332, 334, pero hacia fuera de las filas 337 de rebajes 331 de conformacion de grapas del conjunto 30 de yunque, y/o hacia fuera de las filas 337 de ranuras 336 de recepcion de grapas. En uso, se aproximara el conjunto de yunque al conjunto de cartucho de grapas para retener el tejido, y se dispararan las grapas y se desplegara la cuchilla. Al retirar el instrumento de grapado, se separara el material 350A de refuerzo de lmea de grapado del conjunto de yunque y/o del conjunto de cartucho, en las perforaciones. Cierta cantidad de material marginal de refuerzo de la lmea de grapado permanecera en el instrumento de grapado quirurgico.
Se contempla que los materiales de refuerzo de la lmea de grapado analizados anteriormente incluyan o se fabriquen con un material de grado quirurgico, biocompatible y no absorbible, y pueden comprender una malla. Por ejemplo, el material de refuerzo de la lmea de grapado puede fabricarse con “TEFLON”, que es una marca registrada propiedad de DuPont de Nemours & Co. Se contempla adicionalmente que la porcion 102 de cuerpo pueda fabricarse con una espuma polimerica biocompatible, fieltro, politetrafluoroetileno (ePTFE), gelatina, una tela o similar, o cualquier otro material biocompatible.
Los materiales no absorbibles usados para el material de refuerzo de la lmea de grapado incluyen, pero no se limitan a, aquellos que esten fabricados a partir de polfmeros tales como polietileno, polipropileno, nailon, tereftalato de polietileno, politetrafluoroetileno, fluoruro de polivinilideno, y similares. Otros materiales no absorbibles incluyen, pero no se limitan a, acero inoxidable, titanio y similares.
En una realizacion, el material de refuerzo de la lmea de grapado puede fabricarse con un material bioabsorbible. En otras realizaciones, el material de refuerzo de la lmea de grapado presenta al menos una porcion que es absorbible, y al menos una porcion que no es absorbible. Los materiales bioabsorbibles utilizados para el material de refuerzo de la lmea de grapado incluyen, pero no se limitan a, los fabricados a partir de homopolfmeros, copolfmeros, o mezclas obtenidas a partir de uno o mas monomeros seleccionados del grupo que consiste en glicolida, acido glicolico, lactida, acido lactico, polidioxanona, a-caprolactona y carbonato de trimetileno. Otros materiales bioabsorbibles incluyen, pero no se limitan a, acido poliglicolico (PGA) y acido polilactico (PLA), por ejemplo. En una realizacion, el material de refuerzo de la lmea de grapado puede fabricarse con fieltro bioabsorbible.
El material de refuerzo de la lmea de grapado puede incorporar un material “W” de tratamiento de heridas, que incluye, pero no se limita a, uno de entre adhesivos, hemostaticos, agentes sellantes, coagulantes, astringentes y

medicamentos, o una combinacion de los mismos. Otros materiales “W’ de tratamiento de heridas quirurgicamente biocompatibles que pueden emplearse en instrumentos quirurgicos o aplicarse mediante instrumentos quirurgicos, incluyendo grapadoras quirurgicas, incluyen adhesivos de fijacion o retencion de organos, tejidos o estructuras; agentes sellantes para evitar fugas de fluido; hemostaticos para detener o prevenir el sangrado; coagulantes, astringentes (por ejemplo, sulfatos de aluminio) y medicamentos. Ejemplos de adhesivos que pueden emplearse incluyen materiales adhesivos a base de aldehudos derivados de protemas, por ejemplo, los materiales de albumina/glutaraldehudo comercializados por Cryolife, Inc. con la denominacion BioGlue©, y materiales a base de cianoacrilato comercializados por Tyco Healthcare Group, LP y Ethicon Endosurgery, Inc. con las denominaciones comerciales Indermil® y Derma Bond®, respectivamente. Ejemplos de agentes sellantes que pueden emplearse incluyen agentes sellantes de fibrina y agentes sellantes de tejido a base de colageno y de polfmero sintetico. Ejemplos de agentes sellantes que se comercializan son materiales de hidrogel a base de polietilenglicol sintetico, comercializados por Cohesion Technologies y Baxter International, Inc. con la designacion comercial CoSeal®. Ejemplos de materiales hemostaticos que pueden emplearse incluyen hemostaticos topicos a base de fibrina, a base de colageno, a base de celulosa regenerada oxidada, y a base de gelatina. Ejemplos de materiales hemostaticos que se comercializan son materiales con una combinacion de fibrinogeno y trombina, comercializados por Tyco Healthcare Group, LP, con la denominacion CoStasis®, y por Baxter International, Inc. Con la denominacion Tisseel®. El material W puede incluir medicamentos. Los medicamentos pueden incluir una o mas sustancias con utilidad medica y/o quirurgica, tales como farmacos, encimas, factores de crecimiento, peptidos, protemas, colorantes, agentes de diagnosis o agentes de hemostasia, anticuerpos monoclonales, o cualquier otro medicamento que se utilice en la prevencion de la estenosis.
El material de refuerzo de la lmea de grapado puede incluir una capa individual que incluya un conjunto homogeneo de materiales bioabsorbibles o no absorbibles, o un conjunto heterogeneo de materiales bioabsorbibles y/o no absorbibles. El material de refuerzo de la lmea de grapado puede incluir una porcion de cuerpo con capas, que incluya al menos dos capas compuestas por primera capa, pelmula u oblea, y segunda capa, pelmula u oblea. En esta realizacion, cada capa puede incluir un conjunto homogeneo o heterogeneo de materiales bioabsorbibles y/o no absorbibles.
En ciertas realizaciones preferidas, el material de refuerzo de la lmea de grapado es una tela no tejida. La tela no tejida puede formarse utilizando un proceso de soplado y fundido, que incluye las siguientes etapas. Se extruye por fusion la resina polimerica. Una bomba de fusion dosifica el polfmero fundido a una cabeza de matriz, que tiene un conjunto de orificios. A modo de ejemplo, los orificios tienen un diametro de aproximadamente 0,175 milfmetros a aproximadamente 0,25 milfmetros. Se fuerza el polfmero a traves del conjunto de orificios de la matriz. Las fibras de polfmero salen de la matriz, y se fuerzan las mismas sobre una cinta transportadora. Puede utilizarse una corriente de aire de soplado caliente, para forzar las fibras de polfmero sobre la cinta transportadora. Puede utilizarse succion a traves de la superficie de la cinta transportadora para compactar las fibras contra la cinta, y las unas contra las otras, a medida que se enfnan las mismas. Puede aplicarse compresion adicional sobre las fibras, por ejemplo, utilizando un rodillo de calandrado, que puede incluir una funcion de calentamiento o enfriamiento. A continuacion, puede recocerse la tela no tejida. Por ejemplo, puede utilizarse tension isometrica u otro tipo de compresion uniforme para promover la cristalizacion y eliminar el monomero. Deseablemente, el polfmero sera un polfmero bioabsorbible o no bioabsorbible, tal como un copolfmero de glicolido de lactida (el material utilizado en las suturas Polysorb®), un terpolfmero compuesto de glicoluro, carbonato de trimetileno y dioxanona (el material utilizado en las suturas Biosyn®), un polfmero de glicolida, caprolactona, carbonato de trimetileno y lactida (el material utilizado en las suturas Caprosyn®), y un copolfmero de glicolida trimetilen carbonato (el material utilizado en las suturas Maxon®).
En ciertas realizaciones, la tela no tejida es porosa. Por ejemplo, la tela no tejida puede tener una porosidad de aproximadamente un 50 % a aproximadamente un 90 %. El diametro de las fibras puede ser de aproximadamente 5 pm a aproximadamente 100 pm. El espesor del tejido puede ser de aproximadamente 150 pm a aproximadamente 400 pm.
Debe comprenderse que pueden efectuarse diversas modificaciones en las realizaciones descritas en la presente memoria. Por ejemplo, el instrumento de grapado quirurgico no tiene por que aplicar grapas, sino que puede aplicar dos elementos de sujecion tal como se conoce en la tecnica. Adicionalmente, puede modificarse la longitud de la fila lineal de grapas o de elementos de sujecion, o la longitud o diametro de una fila circular de grapas o elementos de sujecion, para satisfacer los requisitos de un procedimiento quirurgico particular. Por lo tanto, la descripcion anterior no debe interpretarse como limitativa, sino meramente como ejemplos de realizaciones preferidas. Los expertos en la tecnica concebiran otras modificaciones dentro del alcance de las reivindicaciones adjuntas a la presente memoria.
Claims (9)
- 51015202530REIVINDICACIONES1. Un instrumento (10) de grapado quirurgico, que comprende:un conjunto (22) de cartucho de grapas que tiene una pluralidad de filas de ranuras (36) de recepcion de grapas, un conjunto (30) de yunque que tiene un miembro de yunque (26), que define una pluralidad de filas de rebajes (27) de conformacion de grapas, teniendo el conjunto de cartucho de grapas, el conjunto de yunque, o ambos, uno o mas miembros de fijacion (132, 232); yun material (50, 50a) de refuerzo de la lmea de grapado soldado ultrasonicamente a los miembros de fijacion, definiendo el material de refuerzo de la lmea de grapado unas perforaciones (68) adyacentes a los miembros de fijacion, siendo las perforaciones para facilitar el desprendimiento del material de refuerzo de la lmea de grapado con respecto al instrumento de grapado quirurgico,en donde el uno o mas miembros de fijacion estan dispuestos en el conjunto de cartucho de grapas, y situados hacia fuera de las filas de ranuras de recepcion de grapas y/o el uno o mas miembros de fijacion estan dispuestos en el conjunto de yunque y situados hacia fuera de las filas de rebajes de conformacion de grapas.
- 2. El instrumento de grapado quirurgico segun la reivindicacion 1, en donde el material de refuerzo de la lmea de grapado esta unido de forma desprendible a los miembros de fijacion.
- 3. El instrumento de grapado quirurgico segun cualquier reivindicacion precedente, en donde la pluralidad des filas de ranuras de recepcion de grapas son filas circulares.
- 4. El instrumento de grapado quirurgico segun la reivindicacion 3, en donde la pluralidad de filas de rebajes de conformacion de grapas son filas circulares.
- 5. El instrumento de grapado quirurgico segun cualquier reivindicacion precedente, en donde el uno o mas miembros de fijacion se forman sobre el miembro de yunque moldeando un plastico.
- 6. El instrumento de grapado quirurgico segun cualquiera de las reivindicaciones 1 a 4, en donde el uno o mas miembros de fijacion estan dispuestos sobre el miembro de yunque, estando el miembro de yunque hecho de metal y los miembros de fijacion hechos de plastico.
- 7. El instrumento de grapado quirurgico segun cualquier reivindicacion precedente, en donde el conjunto de yunque incluye un cono fijado al miembro de yunque.
- 8. El instrumento de grapado quirurgico segun cualquiera de las reivindicaciones precedentes, en donde el conjunto de yunque incluye un vastago y comprende adicionalmente una porcion de cuerpo tubular, pudiendo montarse el conjunto de cartucho de grapas en la porcion de cuerpo tubular, y en donde la porcion de cuerpo tubular tiene una varilla, siendo acoplable el vastago del conjunto de yunque al vastago.
- 9. El instrumento de grapado quirurgico segun la reivindicacion 8, que comprende adicionalmente un conjunto de empunadura.
