Abstract
Un compuesto que disminuye la concentración intracelular de cloro dentro de las neuronas para uso en el tratamiento del autismo, en el que dicho compuesto inhibe el cotransportador de Na-K-Cl (NKCC).

En una realización, el cotransportador de NKCC o el cotransportador de KCC usado en el método de selección pueden5 ser sus ortólogos y derivados como se define en la presente invención.
En general, tales métodos de selección implican proporcionar células apropiadas que expresan NKCC o KCC, sus ortólogos y derivados de los mismos en su superficie. En particular, se puede emplear un ácido nucleico que codifica NKCC o KCC para transfectar células para expresar así el receptor de la invención. Dicha transfección se puede realizar por métodos bien conocidos en la técnica.
10 En una realización particular, las células se seleccionan del grupo que consiste en células gliales, células neuronales, neuronas, líneas celulares transfectadas para investigaciones o células renales de cualquier especie (ratón, humanos ...).
El método de selección de la invención se puede emplear para determinar un antagonista o agonista poniendo en contacto dichas células con compuestos que se van a seleccionar y determinar si dicho compuesto inhibe o activa el
15 cotransportador.
La determinación de la inhibición de NKCC se puede evaluar determinando la viabilidad celular. Se considera que un compuesto disminuye la viabilidad celular si es negativo en uno cualquiera de los métodos descritos a continuación como ejemplos de actividad de rescate celular.
De acuerdo con una realización de la invención, el compuesto candidato se puede seleccionar de una biblioteca de
20 compuestos previamente sintetizados, o una biblioteca de compuestos para los cuales la estructura se determina en una base de datos, o de una biblioteca de compuestos que han sido sintetizados de compuestos nuevos o naturales.
El compuesto candidato se puede seleccionar del grupo de (a) proteínas o péptidos, (b) ácidos nucleicos y (c) compuestos orgánicos o químicos (naturales o no). Ilustrativamente, se pueden obtener bibliotecas de ácidos nucleicos candidatos preseleccionados realizando el método SELEX tal como se describe en los documentos US 5,475,096 y US
25 5,270,163. Además, ilustrativamente, el compuesto candidato se puede seleccionar del grupo de anticuerpos dirigidos contra NKCC o KCC.
Tal método se puede usar para seleccionar antagonistas de NKCC o agonistas de KCC según la invención.
La invención se ilustrará adicionalmente mediante las siguientes tablas y ejemplos. Sin embargo, estos ejemplos y figuras no deben ser interpretados en modo alguno como limitativos del alcance de la presente invención.

Material y Métodos
Los inventores han investigado en 5 lactantes autistas los efectos de bum con la vigilancia clínica y biológica en curso.
5 Fueron seleccionados sin a priori de un gran grupo de niños con IAS colocados en instituciones o en el hogar para proporcionar una variedad de casos. El diurético se administró (1mg/24h, 0.5mg dos veces al día) y el tratamiento continuó durante 3 meses, una duración mínima considerada suficiente para una evaluación de los efectos sobre el IAS. Se reporta una mejora de las manifestaciones de IAS en los 5 niños. Estas observaciones requieren una amplia gama de selección del uso de Bum en IAS.
10 Los niños fueron diagnosticados por un psiquiatra clínico experimentado usando criterios estrictos de ICD-10 (OMS, 1993) para el trastorno autista. Estos niños no tenían antecedentes de enfermedad neurológica (EEG normal). Las pruebas genéticas realizadas sistemáticamente fueron negativas, indicando que no había mutación identificable (cariotipo y X frágil). Se recogió la ADI-R (Le Couteur et al., 1989; Lord et al., 1994) para todos los participantes para confirmar los diagnósticos. Un examen clínico y biológico demostró que ninguno de los lactantes tenía una
15 contraindicación para bum (incluyendo ionograma sanguíneo, transaminasas, fosfatasas alcalinas, uremia, creatinemia, aclaramiento de creatinina, yGT, glicemia notablemente). Debido a que la hipopotasemia puede inducir una arritmia por ráfaga de ondas, se realizó un ECG para asegurar que ninguno de los pacientes tenía un alargamiento del intervalo QT debido a que tienen una mayor propensión a generar arritmia. Se realizó una vigilancia clínica semanal durante el primer, segundo y tercer mes después del inicio del tratamiento, incluyendo sodio y potasio en sangre una semana y 2
20 meses después del inicio del tratamiento. Ninguno de los lactantes tenía trastornos neurológicos asociados y ninguno estaba bajo otro tratamiento desde al menos tres meses.
Para determinar la posible eficacia del índice terapéutico, se basó en 5 determinaciones de comportamiento clásico de la gravedad de IAS incluyendo:
i) La escala de evaluación del autismo infantil (CARS) es una escala de calificación de 15 ítems que se utiliza como 25 instrumento de selección y para evaluar los cambios en los síntomas del autismo con el tiempo. Estos ítems

Discusión
Los resultados actuales sugieren que la bumetanida mejora los aspectos de comportamiento del IAS sugiriendo que el diurético tiene una acción global. Hasta donde sabemos, este es el primer informe que plantea la posibilidad de alteraciones del cloro en el autismo.
Las conclusiones derivadas de estas observaciones se ven obstaculizadas por varias limitaciones, incluyendo la falta de investigaciones aleatorias doble ciego y placebo, debido al número limitado de casos. Los efectos del placebo son más frecuentes en los niños que en los adultos (Rheims et al., 2008), esto también se aplica al autismo (Sandler 2005). Claramente, las investigaciones a gran escala son necesarias para confirmar o informar las observaciones. Se es consciente de estas limitaciones para demostrar los efectos positivos de la bumetanida e involucrar sus acciones en IAS. Sin embargo, las observaciones indican que la bumetanida no tiene efectos secundarios con una tolerancia general al diurético. También, se nos animó a presentar estas observaciones por la dramática mejoría del comportamiento sugerida por los resultados y la insistencia de los padres de que sus hijos están más presentes y su deseo de seguir el tratamiento hablando en favor de una acción significativa de bum. A pesar de la dificultad de traducir esta noción subjetiva, es interesante resaltar que el mismo término de presencia fue utilizado por todos los padres.
En la actualidad no es posible determinar si la bumetanida ejerce una acción preferencial sobre un aspecto de la sintomatología. Se espera que la falta de efectos de la bumetanida en ABC5 -discurso inapropiado y excesivo fuera de contexto-sea probable porque no es probable que mejore en 3 meses de habla. En contraste, casi todas las puntuaciones fueron mejoradas a grados variables, haciendo hincapié en la acción general de la bumetanida. La alteración del comportamiento cognitivo y emocional es una característica básica del IAS (Kanner, 1943; Hill and Frith, 2003; Hill, 2004; Bieberich and Morgan, 2004) y el hecho de que la bumetanida permite una mejor regulación cognitiva tal vez esté correlacionada con la mejor presencia reportada por los padres. Los resultados de las subescalas de ABC sugieren un progreso de los estados de vigilancia y las interacciones sociales, los movimientos estereotípicos y la hiperactividad de nuevo en consonancia con la noción de regulación cognitiva. La farmacodinámica de la bumetanida ha sido investigada en neonatos humanos (Sullivan et al., 1996; Lopez-Samblas et al., 1997) y un reciente informe sugiere que la bumetanida reduce la severidad de la convulsión en un niño epiléptico (Kahle et al., 2009). Una amplia gama de investigaciones experimentales sugiere que la bumetanida reduce la severidad de las convulsiones (Kahle and Staley, 2008; Kahle et al., 2008; Nardou et al., 2009). Actualmente se está investigando la bumetanida como un nuevo tratamiento para las convulsiones neonatales (EU FP7 Nemo project).
Las observaciones son compatibles con la hipótesis de trabajo de que la bumetanida mejora la eficacia de los procesos integradores neuronales reduciendo el cloro intracelular y reforzando las acciones inhibidoras de GABA. El marco conceptual básico de estas investigaciones es que las neuronas que no respetan el programa de desarrollo mantener las características inmaduras, incluyendo posiblemente alta (Cl-)I y otras propiedades eléctricas y de arquitectura (Ben-Ari, 2008). Otras observaciones sugieren un vínculo entre las señales GABAérgicas y el autismo (Minshew, 1997; Hussman, 2001; Schmitz et al., 2005). También, varias observaciones de imágenes cerebrales indican una pérdida significativa de receptores de GABA/benzodiazepinas en el autismo, especialmente en el hipocampo, el cerebelo y diversas estructuras límbicas (Garreau et al., 1993; Oblak et al., 2009; Guptill et al., 2007; Blatt, 2005).
En conclusión, una serie emergente de estudios sugiere que el cloro se acumula durante la maduración del cerebro en relación con diversas malformaciones de desarrollo. Las observaciones actuales sugieren que un diurético convencional que reduce esta acumulación y actúa para restablecer las acciones inhibitorias de GABA puede ejercer acciones beneficiosas en el autismo que requieren estudios clínicos y experimentales más detallados sobre los enlaces entre GABA/(Cl-)I e IAS.
