Medicamento para el tratamiento del cáncer de hígado.
La presente invención se refiere a un medicamento y a una composición farmacéutica, respectivamente, que es adecuada para el tratamiento o prevención de cáncer de hígado, especialmente carcinoma hepatocelular (HCC) en 5 pacientes humanos.
Estado de la técnica
Hoy en día, las únicas opciones curativas para el tratamiento del cáncer de hígado son la resección quirúrgica o el trasplante de hígado. Sin embargo, en el momento del diagnóstico, la mayoría de los pacientes presentan un crecimiento tumoral avanzado y, por lo tanto, no son aptos para estas opciones de tratamiento. El cáncer de hígado
10 es un tumor principalmente quimiorresistente y solo recientemente Llovet y sus colaboradores, describieron el inhibidor de multicininasa Sorafenib como el primer tratamiento sistémico que puede prolongar la supervivencia de los pacientes con HCC. Parekh et al, Experimental and Toxicologic Pathology 167-173 (2011) describen que el tratamiento con resveratrol disminuyó la expresión de ciclina D1 y p38 MAP cinasa, Akt y Pak1 en células HepG2, y que las células tratadas con resveratrol mostraron un aumento de la actividad de ERK.

Descripción general de la invención
La invención logra estos objetos mediante las características definidas por las reivindicaciones, especialmente
35 proporcionando una composición farmacéutica, comprendiendo la composición Sorafenib y, además de Sorafenib, un inhibidor de Mapk14 (inhibidor de p38α cinasa) para su uso como medicamento en el tratamiento y/o en la prevención del cáncer, especialmente del cáncer de hígado. Se ha demostrado durante la preparación de la invención que un agente que inhibe la actividad de Mapk14 aumenta la eficacia de Sorafenib, como un medicamento en el tratamiento o prevención del cáncer de hígado. Así, la inhibición de la actividad de Mapk14 puede obtenerse por inhibidores de la actividad de Mapk14 que se utilizan en
5 una combinación con Sorafenib para su uso como un medicamento para el tratamiento o prevención de cáncer de hígado.
En una realización preferida, el inhibidor de la actividad de Mapk14 es de los 4 compuestos que tienen preferencia o especificidad para inactivar el producto génico Mapk14, cuyos compuestos se dan en la siguiente tabla 1. Generalmente, un inhibidor de Mapk14 de acuerdo con la invención tiene una CI50 de como máximo 30 nM, 10 preferiblemente como máximo 20 nM, más preferiblemente como máximo 10 nM o máx. 5 nM o como máximo 1 nM, mucho más preferiblemente como máximo 0,1 o como máximo 0,01 nM. Preferiblemente, la CI50 se determina en un ensayo in vitro en Mapk14, por ejemplo, determinando la CI50 (concentración inhibitoria para un 50 % de inhibición) dependiendo de la concentración del inhibidor. Adicionalmente, o como alternativa, la actividad inhibitoria del inhibidor de Mapk14 se determina en un ensayo in vitro basado en células usando, por ejemplo, células cancerosas, por ejemplo, Hep3B. En el ensayo, la actividad inhibitoria de los inhibidores de p38 se determina para la proteína de sustrato de Mapk14 en relación con un gen 25 constitutivo, por ejemplo, α-tubulina. Adicionalmente, o como alternativa, la combinación de Sorafenib con el inhibidor adicional de Mapk14 de la invención en un ensayo basado en células tiene una toxicidad significativamente mayor o inhibición de la tasa de proliferación frente a las células cancerosas cultivadas, especialmente frente a las células de cáncer de hígado, por ejemplo, frente a Hep3B. En el ensayo, las células vivas cultivadas se cuentan tras la incubación con una 35 combinación de Sorafenib y el inhibidor de Mapk14, o con la misma concentración de Sorafenib o inhibidor frente a Mapk14 en solitario o vehículo, respectivamente, por ejemplo, a concentraciones de como máximo 20 µM, preferiblemente, como máximo 12 µm, más preferiblemente como máximo 5 µm.
