Abstract
Description
本出願は、2017年7月11日に出願された米国仮特許出願第62/530,845号の優先権を主張し、その内容は全体として参照によりここに組み込まれる。
本発明は、PKA活性化を引き起こすことができる薬剤の、対象における癌細胞を選択的に死滅させるための医薬の調製における使用をさらに提供した。いくつかの実施形態において、前記細胞はPAR2発現の増殖細胞である。
本発明は、対象における過剰増殖性障害(特に、癌)、免疫応答関連障害の治療もしくは予防、および/または疼痛制御のために用いられ、あるいはこれらの障害または疾患を治療または予防するための方法に用いられる、プロテインキナーゼAの活性化を引き起こすことができる薬剤、または該薬剤を含む医薬組成物を提供する。
1. 細胞培養実験
1.1初代マウス肝細胞および角化細胞の調製および培養。
初代マウス肝細胞および角化細胞の起動および培養は、前述のように実施された。簡単に説明すると、肝細胞培養を開始するために、まずペントバルビタールナトリウム(400mg/kg、ip)で動物を麻酔し、次に腹腔を開き、37℃でカルシウムを含まないHEPES緩衝液を門脈のインサイチュー(in situ)から肝臓に4分間灌流し、および0.5mg/mLコラゲナーゼD(Life Technologies、米国)および3mM CaCl2を含有するHEPES緩衝液を8〜10分間灌流した。灌流速度は5mL/minに設定した。10%ウシ胎児血清(Life Technologies、米国)を添加したWilliams’ medium E(Life Technologies、米国)に、12ウェルプレートの各ウェルに400,000個細胞/ウェルの密度で細胞を播種し、そしてウェル壁に2時間貼付させる。壁に貼付していない細胞を捨て、貼付している細胞(肝細胞)を新鮮な培地に保存した。
37℃および5% CO2の標準組織培養条件下で、10%ウシ胎児血清を含む1640組織培地(Corning、米国)において、癌細胞系および不死化ヒト細胞系を培養した。
ミモシンの非存在下または200μMミモシンの存在下で28時間の場合、約60%コンフルエントのHepG2細胞は、無血清1640培地(Corning、米国)に48時間維持された。
初代肝細胞については、無処理(対照として)または複数のタイプの処理を48時間に至るまで行った場合、播種した細胞をモニタリングした。初代角化細胞については、無処理または複数のタイプの処理を96時間に至るまで行った場合、48ウェルプレートにおける細胞をモニタリングした。HepG2については、無処理または複数のタイプの処理を48時間に至るまで行った場合、96ウェルプレートにおける細胞をモニタリングした。IncuCyte Zoomは、3時間ごとに固定位置で1群の画像を撮影する(96ウェルプレートにウェルあたり4枚撮影し、および48ウェルプレートおよび12ウェルプレートにウェルあたり16枚撮影する)ように設定された。
処理後、細胞を収集してRIPA緩衝液(Solarbio)での溶解によりタンパク質抽出物を調製した。ウエスタンブロットは、特異的抗体およびSuperSignal(登録商標)West Pico化学発光基質(Pierce)を使用して実施された。抗体は、ポリクローナルウサギ抗PAR2抗体(ABGENT)、ポリクローナルウサギ抗β−アクチン抗体(Cell Signaling Technology)、抗ERK(Cell Signaling Technology)、抗リン酸化ERK(Cell Signaling Technology)、抗リン酸化ヒストンH3(Abchem)、抗CDK1(Abcam)のような商業的企業から購入された。
実験は、製造業者から提供された扱い説明書に基づいてわずかな変更を加えて「Click−iT Plus EdUイメージングキット」(Life Technologies, Carlsbad, California、米国)を用いて実行された。簡単に説明すると、24ウェルプレートの各ウェル内のカバースリップ(ウェルあたり1つのカバースリップ)上に、30,000個細胞/ウェルの密度でHepG2細胞を播種した。HepG2細胞は未処理であるか、または200μMで28時間処理し、または無血清培地で48時間インキュベートした後、HepG2細胞は、通常培地に異なる回復期間にある。その後、Edu(10μM)を30分間供給する前に、異なる時点で、薬物を含まない培地培地に細胞を戻して放出した。その後、室温で細胞を3.7%ホルムアルデヒド含有PBSで15分間固定し、次いで室温で0.5%Triton X−100で20分間透過処理した。その後、Click−iT Plus反応混合物を加えて30分間インキュベートした。3% BSA含有PBSでカバースリップを洗浄した後、Hoechst 33342(5μg/mL)で染色した。クリック反応は、Alexa Fluor蛍光色素をEduに接続させるようにデザインされており、Edu含有DNAと、Eduが追加された時にDNA合成を受けていた細胞とを可視化することができる。Hoechst 33342蛍光色素は、DNAに特異的に結合し、すべての核の可視化を可能にする。クリック反応とHoechst 33342染色をした後に、蛍光顕微鏡で検査して複数のタイプの処理を受けた細胞のEdu取り込み特性を評価した。
AntiCancer Biotech(Beijing) Co., Ltd.により臨床前実験を行った。
本研究では、雌の無胸腺ヌードマウス(6週齢)を使用した。動物はBeijing HFK Bioscience, Co., Ltdから購入され、ケージ、食物および放射線またはオートクレーブで滅菌済みの寝具(bedding)を備えた、高効率微粒子エアフィルター(HEPA)で濾過された環境で飼育し維持した。合計30匹のヌードマウスが研究に使用された。
トリプトニド含有懸濁液は、カルボキシメチルセルロース懸濁液における、すぐに投与できる(ready−to−administer)経口製剤として調製された。トリプトニドの濃度は、動物あたりの所望な医薬量が約0.2mLの体積で取得できるように選択される。動物研究の過程において、ビークル懸濁液およびトリプトニド含有懸濁液を試薬Aおよび試薬Bとして指定された。試薬Aおよび試薬Bの性質に関する情報は、動物実験を行ったAntiCancer Biotech (Beijing) Co., Ltdに保存された。
HepG2−GFPヒト肝細胞癌細胞(AntiCancer, Inc., San Diego, CA)は、10% FBS含有RPMI−1640(Gibco−BRL, Life Technologies, Inc.)とともにインキュベートされた。細胞は37℃および5% CO2/95%空気雰囲気に維持されたCO2 Water Jacketedインキュベーター(Forma Scientific)で増殖させた。トリパンブルー排除アッセイ法により細胞活性(細胞生存率)を確定した。
雌の無胸腺ヌードマウスマウスに、それぞれ単回用量での5×106個のHepG2−GFP細胞を皮下注射した。腫瘍サイズが約1cm3に達した時に腫瘍を収集した。
同所性ヒト肝細胞癌モデルを確立する方法は以前に記載されている(62)。HepG2−GFP細胞の由来とする皮下腫瘍を約1mm3の断片に切断し、6週齢の雌BALB/cnuヌードマウス(Beijing HFK BioScience Co., Ltd.)の肝臓右葉に同所移植し、動物あたり1つの移植物にした。簡単に説明すると、麻酔下で1cmの上腹部切開を行った。肝臓の右葉を曝露し、肝臓表面の一部をハサミで機械的に損傷した。次に、1つの腫瘍断片を肝臓組織内に固定して肝臓を腹膜腔に戻し、そして腹壁を縫合した。マウスは、特定の病原体のない条件下で層流キャビネットに保管された。
移植後3日目に、蛍光イメージングの結果に基づいて、移植した腫瘍が約2mm2である動物は選択された。該実験では、所望な腫瘍を有する動物を、群あたり10匹で無作為に群分けた。また、各マウスには、それぞれ識別用記号が耳に付けられた。
まず、1日おきに0、5、10、25、50、100、200mg/kg体重の経口投与量のトリプトニドを1回投与することにより、予備実験を行って最大無毒用量を確定した。該実験により、100mg/kg体重まで高い用量レベルでは、如何なる顕著な有害な影響が生じないことが明らかになった。したがって、担癌ヌードマウスに対して2回目の予備実験を行って無毒範囲内に抗腫瘍効果を達成可能であるか否かを確定した。第1群の実験において、1日おきに1つの投与量の方案で胃内投与によりトリプトニド0、1、5、10、25mg/kgで担癌マウスを処理した。結果は、投与量25mg/kgによる処理が1週間以内に、腫瘍塊を減少させることに非常に有効であることが示された。したがって、臨床前実験では、1日おきに1回25mg/kgのレジメンが選択された。単一の腫瘍の体積は、生物発光イメージングにより評価された。
研究期間中に、すべての実験マウスの死亡率または苦痛の症状を毎日チェックした。動物は腫瘍移植後28日間まで観察された。
マウスの体重は、研究期間中3日ごとに測定された。
腫瘍の増殖および進行の画像は、FluorVivoイメージングシステム、型番300/Mag(INDEC、CA、USA)を使用して研究期間中に3日ごとに取得された。
ペントバルビタールナトリウムの過剰注射により動物を安楽死させた。
各動物を安楽死させた直後、それぞれ皮膚を除去して肝臓を露出させ、すぐに最終的なGFP蛍光イメージングを行った。イメージング後、各肝臓の移植部位の腫瘍またはその残留物を検査した。腫瘍が明確に識別できる場合、腫瘍を切除してその重量を電子天秤(Sartorius BS 124 S、ドイツ)で測定した。腫瘍が見えない場合、移植部位周辺の腫瘍組織を収集し、さらなる分析のためにホルマリンで保存した。
標準的なヨウ化プロピジウム(PI)染色法を使用して、細胞周期プロファイルを分析した。簡単に説明すると、HepG2細胞を播種し、そして200μMミモシンを使用したかまたは使用しなく28時間処理した。未処理の細胞を非同期化対照として使用した。次に、別途の薬物処理なしで一部のミモシン処理細胞を培養し、0、11、24、および37時間で収集してフローサイトメトリー分析に使用した。
約70%コンフルエントになった培養HepG2細胞を200μMミモシンで28時間処理した。次に、ミモシン処理細胞を通常培地で2時間回復させてから、無血清1640基礎培地においてビークル溶液、1μMトリプトニドまたは50nMトリプシンで処理した。処理後の異なる時点で細胞を収集した。1群のサンプルに対して、細胞を通常培地に4時間回復させた後、1μMトリプトニドで処理した。その後、製造業者によって提供されたプロトコルにより、cAMP ELLISAキット(CELL BIOLABS、カタログ番号STA−501)を使用して、単一サンプルのcAMP濃度を測定した。
約70%コンフルエントになった培養HepG2細胞を200μMミモシンで28時間処理した。ビークル溶液または1μMトリプトニドで処理する前に、ミモシン処理細胞を通常培地で2時間回復した。PKA活性に対する治療の急性効果を測定するために、無血清1640基礎培地において処理を行った。処理後の異なる時点で細胞を収集した。処理の長期効果を測定するのために、通常培地において処理を1時間行った。処理後、細胞を通常の培養条件に戻し、処理後の異なる時点で収集した。単一サンプルのPKA活性レベルは、PepTag(登録商標)非放射性cAMP依存性プロテインキナーゼアッセイシステム(Promega、カタログ番号:V5340)を使用して製造業者によって提供されたプロトコルにより測定された。
PKI−14−22アミド(ミリストイル化)はTocrisから購入され、ビダラビンはSelleckから購入された。他のすべての化学薬品は、特に明記しない限り、Sigma−Aldrichから購入された。
発明者らは、初代マウス肝細胞(PMH、非癌細胞を表す)と、HepG2細胞(HCC癌細胞)との両方を個別の試験試薬に1時間だけ曝露し、PMHではなくHepG2細胞に対して有意な増殖阻害効果を示す試薬を探すスクリーニングスキームをデザインした。1時間の曝露は、一部の経口投与された医薬または局所送達された医薬の初回通過効果による肝臓の一時的な高濃度をシミュレートすることを目的としており、そのような短期間の曝露は、細胞内吸収による如何なる有意なオフターゲット効果を引き起こすことなく、一部の細胞表面受容体に影響を与えるのに依然として十分であると予想される。このような戦略は、可逆的な細胞周期停止による誤りヒットを減らすことにもなる。IncuCyte Zoomシステム(Essen BioScience、アナーバー、ミシガン州)を使用して、各試験化合物の短期(1時間)曝露のIC50値(IC50)を確定し、実験中に2分間ごとに培養細胞に対して撮影して単一細胞の動的変化を記録した。
該実施例において、HepG2細胞に対するトリプトニドとトリプトリドの効果を比較した。
トリプトニドの標的を同定するために、発明者らは、トリプトニドの増殖阻害効果がPAR2に依存しているかどうかを調べた。
興味深いことに、トリプトニドは50nMの低濃度で野生型角化細胞に対して有意な増殖阻害効果を示したが、その密接に関連するトリプトリドは、延いて800nMの高濃度で野生型またはPar2ノックアウトの角化細胞に対して如何なる有意な増殖阻害効果を持っていなかったであり(図9A、9B)、Par2依存性細胞死を引き起こすには、トリプトリドがトリプトニドよりも効果が低いか、Par2依存性細胞死を引き起こすのにまったく効果がないことを示している。しかしながら、これとは鮮明に対照的に、連続的に暴露した場合、1.25nMという低濃度(検出の最低濃度)のトリプトリドは、野生型とPar2ノックアウトの角化細胞と共に完全な致死ををもたらした(図9C、9D)。それに比べて、320nMという高濃度のトリプトニドはPMHに対して有意な毒性効果を示さなかった(図9D)。これらのデータは、トリプトリドとトリプトニドの異なる効果を実証する追加の証拠を提供している。すなわち、1時間スキームで投与した場合、トリプトリドではなくトリプトニドは、マイトジェン活性化細胞での独特なPar2/PAR2依存性細胞死を有効でもたらす可能性があり、そして1.25nM〜320nMの濃度で連続に適用した場合、トリプトリドは非常に強力な毒性があったが、トリプトニドは毒性がなかった。
細胞質におけるPKA活性の上昇は、哺乳類卵母細胞と体細胞の両方でG2/M移行を抑制するための効果的なメカニズムであり、G2/M移行の長期または永続的な阻害が分裂期細胞死を引き起こすをもたらす可能性があることがすでに実証されている。PAR2活性化がGαs−AC−cAMP−PKA経路の活性化、すなわちPKA活性の上昇と共役する可能であるため、発明者らは、トリプトニドがPAR2を介してGαs共役経路を活性化し、すなわちAC−cAMP−PKA経路を活性化して細胞質のPKA活性を上昇させることにより分裂期細胞死誘導効果をもたらす、という仮説を検討した。この仮説は、次のいくつかの予測を導いた。すなわち、(1)CDK1活性化は、G2/M移行時間の前に発生しない、(2)AC−cAMP−PKA経路は活性化される、(3)細胞質のPKAのレベルは、G2/M移行の時間の前後に上昇を維持する、(4)トリプトニドの増殖阻害と分裂期細胞死の誘導効果は、AC−cAMP−PKA経路に依存する。その後、これらの予測の妥当性を調べるために、次の実験を実行した。
これまで、PAR2は多くの癌細胞系および検出された多くの腫瘍タイプの大きい部分で発現していることが報告されていた。したがって、発明者らは、そのような新規な抗癌パラダイムは、HCCおよび/または他のタイプ癌の多くのケースにおける癌細胞を特異的に死滅させるために使用できるか否かを調べた。
上記実験には、2種の不死化細胞系(LO2、不死化肝細胞、およびGES−1、不死化胃上皮)も非癌対照細胞系として含まれていた。驚くべきことに、発明者らは、検出されたこれら2種の不死化細胞系は、両方ともトリプトニドに敏感であることに気付いた(表1)。この発見により、発明者らは、PAR2がこれらの非癌細胞で発現し、または言い換えれば、PAR2の活性化が腫瘍形成の一般的な初期イベント(early event)(または、いわゆる「ドライバーイベント」)(且つ、このため、抗癌医薬開発の望ましい標的である)を構成する可能性がある、という仮説を立てることができた。実際に、PAR2はHCC細胞系(図8)および胃癌細胞系(図16)だけでなく、調べた2種の不死化細胞系(図8、図16)においても明確に発現していることが分かった。また、予想どおり、Par2は、トリプトニド処理(図1)に耐性のある初代マウス肝細胞抽出物から検出できなかった(図8)。したがって、これらのデータは、PAR2が不死化細胞系において活性化であることが明らかに示している。不死化が細胞形質転換と腫瘍形成の初期イベントを構成するため、これは、一部の癌に対して、PAR2活性化が腫瘍形成の初期イベントであるという可能性も高めた。なお、このデータは、マウスおよびヒト細胞において、トリプトニドに対する感受性とPar2/PAR2発現との相関関係を実証した。
前述のように、インビボで非分裂細胞がPAR2の発現を制限する状況で、独特なトリプトニド媒介性のPar2依存的に増殖細胞を死滅させることにより、発明者らは、PAR2発現の癌に対する標的抗癌治療を推進するためにトリプトニドおよび/または同様のPAR2リガンドを使用する実現可能性をさらに評価するよう促された。しかしながら、50nM低濃度のトリプトニドへの長期曝露がPAR2発現の初代マウス角化細胞に対して強力な毒性を持つことを考えると(図7)、PAR2依存的に標的サイクルの癌細胞を死滅させると共に、PAR2発現の非癌細胞に対する有意な非特異的毒性を避けるために、短期曝露の実施、すなわち所望な血漿濃度の短期持続時間とすることが不可欠である。これに関して幸いなことは、トリプトニドがげっ歯類で非常に迅速な再分布動態を示すように見えることであり、これは、経口投与後の血漿濃度の数十倍の減少と相関している。したがって、トリプトニドの潜在的な抗腫瘍活性を評価することにした。具体的には、HepG2細胞は、トリプトニドに対する感受性レベルが、試験された細胞系の66.7%よりも低く、試験されたすべてのHCC細胞系で最も低いことを示したため(表1)、HCCに対する且つ望ましくは10タイプの癌の大部分の症例を対象とする評価を提供するために、HepG2に基づく同所性異種移植腫瘍モデルにおいてヒトHCCに対するトリプトニドの抗腫瘍効果の評価を最初に選択した。
ヒトHCCの標的抗癌治療の候補物を同定する試みたところ、発明者らは、トリプトニドが所望な癌細胞特異的な増殖阻害効果を有することを発見した(図1)。追跡の研究によりこの特異的な増殖阻害効果は、静止状態の非分裂細胞を損傷しないと共に、マイトジェン活性化細胞に対するトリプトニドの独特な分裂期細胞死の誘導効果によるものであることが明らかになった(図2−4)。次に、トリプトニドの該独特な分裂期細胞死の誘導効果とトリプトリドの細胞死滅効果と異なっており(図4と図5)、トリプトリドは、よく研究された抗白血病/抗癌剤であり、主にG1細胞の細胞周期停止とS期細胞のアポトーシス引き起こした(図6)。培養されたマウス角化細胞では、トリプトニドのようなこの独特な効果はGPCR受容体であるPar2に依存していた(図7−8)。ナノモル範囲で1時間曝露する場合、トリプトリドではなくトリプトニドは、この独特なPar2依存効果を示した(図9)。総合すると、これらのデータは、トリプトニドを、Par2/PAR2発現の細胞における分裂期細胞死を誘導し、Par2/PAR2発現の増殖細胞を選択的に死滅させることをもたらすために使用できる独特な薬剤として定義した。

Claims (15)
- 対象における過剰増殖性障害、好ましくは癌を治療または予防するための方法であって、プロテインキナーゼAの活性化を引き起こすことができる、治療または予防有効量の薬剤、好ましくは、トリプトニドまたはその機能的等価物もしくは薬学的に許容される塩、あるいはトリプトニドまたはその機能的等価物もしくは薬学的に許容される塩を含む医薬組成物が前記対象に投与されることを含む、方法。
- 請求項1に記載の方法であって、前記治療は、癌細胞、好ましくは、プロテアーゼ活性化受容体2(PAR2)の増殖細胞を選択的に死滅させることを含む、および/または、前記予防は、悪性転化前および/または悪性転化部位に発現するPAR2の細胞を選択的に死滅させることを含む、方法。
- トリプトニドまたはその機能的等価物もしくは薬学的に許容される塩、および薬学的に許容される担体を含む、必要に応じて、プロテインキナーゼAの活性化を引き起こす1種以上のその他薬剤をさらに含む、医薬組成物。
- 請求項3に記載の医薬組成物であって、対象における過剰増殖性障害、好ましくは癌を治療または予防するための、医薬組成物。
- 請求項3に記載の医薬組成物であって、対象における癌細胞、好ましくはPAR2の発現する増殖細胞を選択的に死滅させるために用いられる、医薬組成物。
- 請求項3〜5のいずれか1項に記載の医薬組成物であって、前記医薬組成物は、薬学的に許容される剤形、好ましくは、経口液剤、カプセル剤、散剤、錠剤、顆粒剤、丸剤、シロップ剤、注射剤等に製剤化される、医薬組成物。
- プロテインキナーゼAの活性化を引き起こすことができる薬剤、好ましくは、トリプトニドまたはその機能的等価物もしくは薬学的に許容される塩、あるいはトリプトニドまたはその機能的等価物もしくは薬学的に許容される塩を含む医薬組成物の、対象における過剰増殖性障害、好ましくは癌を治療または予防するための医薬の調製における使用。
- 請求項7に記載の使用であって、前記医薬は、経口、皮下、筋肉内または腹腔内を介して投与される、好ましくは、前記医薬は経口投与される、使用。
- 前記癌は、肝細胞癌、乳癌、結腸癌、非小細胞肺癌、胃癌、卵巣癌、腎癌、前立腺癌、中枢神経系癌および黒色腫からなる群より選ばれる、請求項1または2に記載の方法、請求項4〜6のいずれか1項に記載の医薬組成物、あるいは請求項7または8に記載の使用。
- 対象における免疫応答関連障害の治療もしくは予防、および/または疼痛制御のための方法であって、治療または予防有効量のトリプトニドまたはその機能的等価物もしくは薬学的に許容される塩、あるいはトリプトニドまたはその機能的等価物もしくは薬学的に許容される塩を含む医薬組成物が前記対象に投与されることを含む、方法。
- トリプトニドまたはその機能的等価物もしくは薬学的に許容される塩、あるいはトリプトニドまたはその機能的等価物もしくは薬学的に許容される塩を含む医薬組成物の、対象における免疫応答関連障害の治療もしくは予防および/または疼痛制御のための医薬の調製における使用。
- PKAの持続的活性化を引き起こすことができる薬剤を同定する方法であって、候補薬剤の分裂期細胞死の誘導効果を評価することを含む、方法。
- 請求項12に記載の方法であって、前記候補薬剤は分裂間期に投与される、方法。
- 請求項12または13に記載の方法であって、前記候補薬剤は、数分間〜数時間での短期治療として投与される、方法。
- PAR2発現の増殖細胞においてPKAの持続的活性化を誘導する方法であって、トリプトニドまたはその機能的等価物もしくは薬学的に許容される塩、あるいはトリプトニドまたはその機能的等価物もしくは薬学的に許容される塩を含む医薬組成物を、前記細胞に接触させることを含む、方法。
