5 El compuesto I se sometió a ensayó adicionalmente frente a aislados clínicos anaerobios y frente a bacterias aerobias. Se mostró que un panel de 207 aislados clínicos de C. difficile, era muy sensibles al compuesto I. El compuesto fue también activo frente a cepas clínicas de Staphylococcus sp. y Enterococcus sp. Estos resultados demuestran el estrecho espectro antimicrobiano de este compuesto frente a determinados organismos patógenos

15 En otro estudio, se sometieron a ensayo varios aislados clínicos de especies de Clostridium especies y se ensayaron aproximadamente 300 aislados clínicos de GI frente al compuesto I. El compuesto I era más activo frente a C. difficile, C. perfringens y C. sordellii con la CIM90 entre 0,062 y 0,25 µg/ml (Tabla 5 y 6). El compuesto I fue también activo frente a Staphylococcus y Enterococcus con una CIM90 a 1 y 8 µg/ml, respectivamente.

5 Ejemplo 2
Eficacia comparativa del compuesto 1, metronidazol, y vancomicina en el modelo 5 de hámster de diarrea asociada a Clostridium difficile.
10 Para evaluar la eficacia in vivo del compuesto 1 en el tratamiento de la colitis asociada a Clostridium difficile, se sometió a ensayo el compuesto 1 en un modelo de hámster de colitis inducida por clindamicina en comparación con vancomicina y metronidazol. Se trataron los animales con dos dosis orales de clindamicina a 100 mg/kg. Tres días después de la segunda dosis de clindamicina, se inocularon con esporas de C. difficile toxigénico. Ocho horas después de la infección, los animales recibieron el compuesto 1, vancomicina o metronidazol por vía oral, durante 7
15 días. Se observó a los animales diariamente para determinar la presencia o ausencia de diarrea. Se realizaron las necropsias en algunos animales que murieron durante el experimento, y se evaluó el contenido cecal para determinar la toxina A de C. difficile. Se vigiló a los hámsteres durante 20 días, y se registró la mortalidad acumulada durante este periodo (Fig. 1). Los tres antibióticos sometidos a ensayo protegieron a los animales de la infección, que -en ausencia de tratamiento- fue por otra parte uniformemente fatal entre los dos y los seis días después de la
20 infección. La DE50 del compuesto 1 estuvo por debajo de 0,3 mg/kg. El tratamiento con el compuesto 1 a concentraciones de 0,8 y 2,5 mg/kg fue tan eficaz como el tratamiento con vancomicina (5 mg/kg) o metronidazol (100 mg/kg).

Administración oral del compuesto 1 a seres humanos.
Se investigó la tolerabilidad y la farmacocinética del compuesto 1 tras la administración de una única dosis en 16 sujetos voluntarios sanos. El ensayo clínico fue un estudio de aumento de dosis con una única dosis, doblemente enmascarado aleatorizado, controlado por placebo.
5 Se administró el compuesto 1 por vía oral tras un desayuno por la mañana a 10 sujetos sanos. Se determinaron las concentraciones del compuesto 1 en plasma, orina, y las fecales para la evaluación farmacocinética.
Tras la administración oral, se detectó poco compuesto 1 en sangre; las concentraciones estuvieron próximas al límite inferior de cuantificación (LIC = 5 ng/ml). Solo un sujeto en el grupo de dosis de 450 mg tuvo concentraciones 10 en plasma que fueron detectables tan tarde como a las 8 horas. El nivel más alto en plasma observado fue de 37,8 ng/ml (en la dosis más alta, grupo de 450 mg).
La recuperación fecal del compuesto 1 sin cambiar como una dosis por ciento de compuesto administrado fue aproximadamente de 20 % en los grupos de dosificación con 200 y 300 mg y los valores promedio para la 15 concentración de los máximos fecales correspondientes fueron 157 y 248 µg/g, respectivamente.

Actividad in vitro del compuesto 1
20 Se evaluaron la eficacia in vitro del compuesto 1, metronidazol, y vancomicina frente a 110 aislados clínicos genéticamente distintos de C. difficile mediante dilución en agar. En las Tablas 7 y 8 se presentan los datos de la CIM.
