Instrumento quirúrgico con indicador
Antecedentes
Campo técnico
La presente descripción está relacionada generalmente con un instrumento quirúrgico de grapado para aplicar grapas quirúrgicas al tejido corporal. Más particularmente, la presente descripción está relacionada con un instrumento quirúrgico de grapado adecuado con un indicador de disparo.
Antecedentes de la técnica relacionada
La anastomosis es el empalme quirúrgico de secciones separadas de un órgano hueco. Típicamente, un procedimiento de anastomosis sigue a una cirugía en la que se extrae una sección defectuosa o enferma del tejido hueco y las secciones extremas que quedan se van a empalmar. Según el procedimiento deseado de anastomosis, las secciones extremas pueden empalmarse mediante métodos de reconstrucción de órganos, ya sean circulares, de extremo con extremo o de lado con lado.
En un procedimiento de anastomosis circular, los dos extremos de las secciones del órgano se empalman por medio de un instrumento de grapado que impulsa una distribución circular de grapas a través de la sección extrema de cada sección de órgano y simultáneamente extrae el núcleo del interior del tejido de la distribución circular impulsada de grapas para liberar el paso tubular. Unos ejemplos de instrumentos para realizar una anastomosis circular de órganos huecos se describen, por ejemplo, en los números de patente de EE.UU. 6.053.390, 5.588.579, 5.119.983, 5.005.749, 4.646.745, 4.576.167 y 4.473.077. El documento nº de patente de EE.UU. 5.005.749 describe un instrumento quirúrgico de grapado para anastomosis según el preámbulo de la reivindicación adjunta 1. Típicamente, estos instrumentos incluyen un tronco alargado que tiene una parte de asidero en un extremo proximal para accionar el instrumento y un componente que sostiene grapas dispuesto en un extremo distal. Un conjunto de yunque que incluye una varilla de yunque con un cabezal de yunque conectado se monta en el extremo distal del instrumento adyacente al componente que sostiene grapas. Unas partes extremas opuestas de tejido del órgano(s) hueco(s) a grapar se sujetan entre el cabezal de yunque y el componente que sostiene grapas a medida que estos componentes se aproximan. El tejido sujeto se grapa al impulsar una o más grapas desde el componente que sostiene grapas de modo que los extremos de las grapas pasan a través del tejido y son deformados por el cabezal de yunque. Concurrentemente se hace avanzar una cuchilla anular para extraer tejido dentro del órgano hueco para liberar un paso tubular dentro del órgano.
Además de la anastomosis de órganos huecos, para tratar hemorroides internas en el recto se han utilizado instrumentos quirúrgicos de grapado para realizar anastomosis circular. Típicamente, durante el uso de un instrumento de grapado circular para tratamiento hemorroidal, el cabezal de yunque y el componente que sostiene grapas del instrumento quirúrgico de grapado se insertan a través del ano y dentro del recto con el cabezal de yunque y el componente que sostiene grapas en una posición abierta y sin aproximar. A continuación, se utiliza una sutura en bolsa de tabaco para tirar del tejido hemorroidal interno hacia la varilla de yunque. A continuación, el cabezal de yunque y el componente que sostiene grapas se aproximan para sujetar el tejido hemorroidal entre el cabezal de yunque y el componente que sostiene grapas. El instrumento de grapado se dispara para retirar el tejido hemorroidal y grapar el tejido cortado. En una realización preferida el indicador de disparo es rotatorio con respecto a un asidero estacionario del conjunto de asidero e incluye una garra a la que se acopla el gatillo de disparo cuando se mueve desde su primera posición sin accionar hacia su segunda posición de accionado. En una realización preferida, la estructura de acoplamiento del gatillo de disparo incluye una pestaña que se extiende hacia el asidero estacionario. Ahora se describirán con detalle unas realizaciones del instrumento quirúrgico de grapado descrito actualmente, haciendo referencia a los dibujos, en los que números de referencia semejantes designan elementos idénticos o correspondientes en cada una de las varias vistas.
Por toda esta descripción, el término “proximal” se refiere a la parte del instrumento más cercana al operador y el término “distal” se refiere a la parte del instrumento más alejada del operador.

Las FIGS. 1-3 ilustran una realización del instrumento quirúrgico de grapado descrito actualmente mostrado generalmente con el número de referencia 10. Brevemente, el instrumento quirúrgico de grapado 10 incluye un conjunto de asidero 12, una parte alargada de cuerpo 14 que incluye un tubo exterior alargado curvado 14a y una parte de cabezal 16. Como alternativa, en algunos procedimientos quirúrgicos, p. ej. el tratamiento de hemorroides, es deseable que una parte de cuerpo sea substancialmente recta. La longitud, forma y/o el diámetro de la parte de cuerpo 14 y la parte de cabezal 16 también pueden variarse para adecuarse a un procedimiento quirúrgico en particular.
Continuando con la referencia a las FIGS. 1-3, el conjunto de asidero 12 incluye un asidero estacionario 18, un gatillo de disparo 20, un mando rotatorio de aproximación 22, un mecanismo indicador de aproximación 100 y un indicador de disparo 200. La parte de cabezal 16 incluye un conjunto de yunque 30 y un conjunto de carcasa 31. El conjunto de yunque 30 es movible con respecto al conjunto de carcasa 31 entre unas posiciones de espaciamiento y de aproximación.
Todavía haciendo referencia a las FIG. 1-3, en funcionamiento, la rotación del mando de aproximación 22 provoca el movimiento del conjunto de yunque 30 con respecto al conjunto de carcasa 31 entre la posición de espaciamiento (FIG. 1) y la de aproximación (FIG. 2-3), ya que el mando de aproximación 22 está acoplado funcionalmente con el retenedor 38 de yunque (FIG. 1), que se conecta con el tronco o varilla central 33 de yunque del conjunto de yunque

Como se muestra en la FIG. 4A, la parte proximal del tornillo 40 tiene un canal helicoidal 35 y está colocado de manera deslizante dentro de un agujero central 33a del manguito rotatorio 34. La parte proximal del manguito 34 se extiende a través de una abertura en el extremo proximal del asidero estacionario 18 y unas nervaduras alargadas 39 en su superficie exterior son recibidas en unas ranuras internas 49a del mando de rotación 22 para fijar el manguito 34 al mando 22. Un pasador 37 se extiende a través de un collarín cilíndrico 36 del manguito 34 adentro de un canal helicoidal 35. Dado que el manguito 34 está fijado axialmente con respecto al asidero estacionario 18, la rotación del manguito 34 alrededor del tornillo 40 hace que el pasador 37 se mueva a lo largo del canal 35 del tornillo 40 para efectuar el movimiento axial del tonillo 40. En la patente de EE.UU. nº 7.303.106 se describen más detalles del mecanismo de aproximación. La rotación del mando de aproximación 22 en el primer sentido hace que la parada 40 de tornillo se mueva proximalmente, que a su vez provoca el movimiento proximal del retenedor 38 de yunque para aproximar el yunque conectado hacia los sujetadores.
Adicionalmente, las FIGS. 1-4 muestran un mecanismo de trabado que incluye una traba 50 de gatillo. La traba 50 de gatillo se dispone en cooperación mecánica con el conjunto de asidero 12 y mantiene el gatillo de disparo 20 en una posición de apertura sin activar hasta que el conjunto de yunque 30 y el conjunto de carcasa 31 se han aproximado. Con la traba 50 de gatillo en la posición de bloqueo mostrada en las FIGS. 1 y 4B, en donde el yunque está en una posición sin aproximar, el gatillo de disparo 20 no se puede apretar. Como se aprecia, el mecanismo de trabado ayuda por lo tanto a impedir que las grapas sean disparadas prematuramente, es decir antes de la aproximación del conjunto de yunque, por el movimiento de bloqueo del gatillo de disparo 20.
Cuando se hace rotar el mando de rotación 22 para aproximar el conjunto de yunque 30, el tornillo 40 y la parada 49 de tornillo se mueven proximalmente, y una parte de pestaña 43, que se extiende hacia abajo, de la parada 49 de tornillo insta a una placa de seguridad 44 del mecanismo de trabado proximalmente contra la predisposición de un resorte 42. Esto mueve una pestaña 47 de la placa de seguridad 44 (FIG. 6A) fuera el acoplamiento con la superficie superior de la traba 50 de gatillo (visto en la orientación de la FIG. 6A) para permitir que la traba 50 de gatillo pivote en la dirección de la flecha “B” en la FIG. 2, desbloqueando de ese modo el gatillo de disparo 20 y permitiendo el disparo del instrumento quirúrgico de grapado 10. Esto se describe con detalle en la susodicha patente de EE.UU. nº. 7.303.106.
Como se muestra en las FIGS. 1-4B, el mecanismo indicador de aproximación 100 de la presente descripción incluye un indicador de aproximación bulboso 102, una tapa 104 de lente (FIG. 4B) y un miembro de deslizamiento
110. El indicador 102 se soporta de manera pivotante mediante un pasador 109 en la parte proyectada 112 del miembro de pivote 106 que puede formarse monolíticamente con el conjunto de asidero 12. La cubierta 104 de lente (o ventana del conjunto de asidero 12) se coloca encima del indicador 102 y puede formarse de material amplificador para facilitar la visualización del indicador 102. En una realización alternativa, en lugar del indicador conectado al miembro de pivote 106 mediante un pasador, la parte proyectada 112 se acopla a una superficie interior del indicador 102 para mantener el indicador en la primera posición.
Durante la aproximación del conjunto de yunque 30 y el conjunto de carcasa 31, una pestaña 41 de la parada 49 de tornillo pasa a través de una ranura del miembro de deslizamiento 110 y se acopla a un extremo proximal de la ranura. Al continuar la aproximación del instrumento 10, el tornillo 40 (una parte del cual está en contacto con el extremo proximal de la ranura) mueve el miembro de deslizamiento 110 proximalmente dentro del asidero estacionario 18 contra la predisposición del resorte 120 de tal manera que se hace rotar el indicador 102 debido a la conexión por pasador. El indicador de aproximación 100 pivota (o rota) a una segunda posición de la misma manera que la descrita en la patente de EE.UU. nº 7.303.106. En una realización alternativa en la que el indicador no está sujeto por pasador, la parte proyectada 112 del miembro de deslizamiento 110 se acopla a una parte del indicador 102 haciendo que el indicador 102 pivote (o rote) a una segunda posición. En la segunda posición, el indicador 102 proporciona una indicación al cirujano de que el instrumento se ha aproximado y ahora está en la posición preparado para el disparo de la FIG. 2.
El accionamiento del gatillo de disparo 20 (es decir, pivote en la dirección de la flecha “A” en la FIG. 1) hace que las grapas sean eyectadas desde el conjunto de carcasa 31 hacia el conjunto de yunque 30. Esto es, el gatillo de disparo 20 se dispone en cooperación mecánica con un empujador (en las realizaciones ilustradas no se muestra explícitamente), de tal manera que el accionamiento del gatillo de disparo 20 provoca el avance del empujador hacia el contacto con las grapas para eyectar las grapas adentro de unos huecos de deformación de grapa del conjunto de yunque 130. Más específicamente, cuando se acciona el gatillo de disparo 20, es decir se pivota alrededor del miembro de pivote 29, la unión de disparo 27 (FIG. 4B) se mueve proximalmente. El gatillo de disparo 20 se mueve distalmente para hacer avanzar la unión 26 de empujador distalmente contra su predisposición de resorte. El extremo distal de la unión 26 de empujador, conectada a una parte trasera de empujador (no se muestra), hace avanzar el empujador hacia atrás para eyectar las filas anulares de grapas desde el conjunto de carcasa 31. En la patente de EE.UU. nº 7.303.106 se describen detalles del mecanismo de disparo.
Haciendo referencia ahora a las FIGS. 4B-6, un accionamiento completo del gatillo de disparo 20 hace que el indicador de disparo 200 pivote (o rote) (p. ej. contra una predisposición inicial) desde su primera posición (FIGS. 4B y 4C) a su segunda posición (FIGS. 3 y 5). Específicamente, una vez que se acciona el gatillo de disparo 20, una estructura de acoplamiento del gatillo de disparo 20 (p. ej. una pestaña 24 en el mismo) contacta con una parte del indicador de disparo 200 (p. ej. una garra 210 que se extiende desde el indicador de disparo 200 y que está formada monolíticamente con el mismo o conectada funcionalmente al mismo). El indicador de disparo 200 se dispone en cooperación mecánica (p. ej. pivotable o rotatorio) con el asidero estacionario 18. El acoplamiento entre el gatillo de disparo 20 y el indicador de disparo 200 (p. ej. el acoplamiento entre la pestaña 24 y la garra 210 mostradas en la FIG. 5) hace que la garra 210 del indicador de disparo 200 (p. ej. la garra 210) rote en la dirección general de la flecha “C” (FIG. 6). El indicador 200 rota alrededor de un punto de pivote (p. ej. un poste dentro del asidero estacionario 18 que se extiende a través de una abertura 219 (FIG. 6) del indicador de disparo 200. En la FIG. 6, por claridad, se ha omitido el poste. En la realización ilustrada, la pestaña 24 incluye una superficie de acoplamiento angulada 24a (FIG. 4C) que está angulada con respecto a un eje longitudinal “A-A” definido por el asidero movible (gatillo de disparo) 20. La superficie de acoplamiento angulada 24a facilita el movimiento de la garra 210 del indicador de disparo 200 en sentido distal. Cabe señalar que en los dispositivos de grapado que aplican filas anulares de grapas que tienen dos asideros movibles, la pestaña 24 puede colocarse en uno de los asideros.
La FIG. 6 ilustra el indicador de disparo 200 en su primera posición con unos fijadores 240, colocados en lados opuestos del indicador 200, colocado por debajo de unas nervaduras 252 (solo de muestra una de ellas) en la respectiva mitad de alojamiento. Después de que el indicador de disparo 200 se mueva a su segunda posición de la FIG. 3, el indicador de disparo 200 es mantenido en su segunda posición por unos fijadores 240 que se acoplan a un respectivo rebaje 250 (en la FIG. 6, por claridad, solo se muestra un único rebaje 250, ya que la otra mitad de alojamiento se ha eliminado) entre las nervaduras 252, 254 en cada una de las mitades de cuerpo. Después de disparar, el indicador de disparo 200 está en la segunda posición (es decir, con los fijadores 240 entre las nervaduras 252, 254), hasta que el usuario restablece manualmente el indicador de disparo 200 a su primera posición por el movimiento (rotación) de una parte del indicador de disparo 200 (p. ej., garra 210) proximalmente en sentido opuesto a la flecha C para sobrepasar el acoplamiento de fijador 240/rebaje 250.

La presente descripción también está relacionada con un ejemplo de método para realizar un procedimiento quirúrgico. El ejemplo de método incluye proporcionar un instrumento quirúrgico 10 que incluye un conjunto de asidero 12, un parte alargada de cuerpo 14 y una parte de cabezal 16. A instrumento quirúrgico de grapado (10), que comprende:un conjunto de asidero (12) que incluye un gatillo de disparo (20) y un indicador de disparo (200), el gatillo de disparo (20) es movible entre una primera posición sin accionar y una segunda posición de accionado, el gatillo de disparo (20) incluye una estructura de acoplamiento;una parte alargada de cuerpo (14) que se extiende distalmente desde el conjunto de asidero (12); yuna parte de cabezal (16) dispuesta adyacente a una parte distal de la parte alargada de cuerpo (14) y que incluye un conjunto de yunque (30) y un conjunto de carcasa (31), el conjunto de yunque (30) es movible con respecto al conjunto de carcasa entre una posición de espaciamiento y una de aproximación;caracterizado por que la estructura de acoplamiento del gatillo de disparo (20) contacta con una parte (210) del indicador de disparo (200) y mueve el indicador de disparo (200) desde una primera posición a una segunda posición cuando el gatillo de disparo (20) se mueve desde su primera posición sin accionar a su segunda posición de accionado;

- El instrumento quirúrgico de grapado (10) de las reivindicaciones 1 o 2, en donde el indicador de disparo
(200) incluye una garra (210) y el gatillo de disparo (20) se configura para acoplarse a la garra (210) cuando el gatillo de disparo (20) se mueve desde su primera posición sin accionar hacia su segunda posición de accionado. - 4. El instrumento quirúrgico de grapado de la reivindicación 3, en donde la garra (210) y el indicador de disparo(200) se forman monolíticamente.

- El instrumento quirúrgico de grapado (10) de las reivindicaciones 3 o 4, en donde la garra (210) se extiende desde la superficie del indicador de disparo (200) hacia el gatillo de disparo (20).

- El instrumento quirúrgico de grapado (10) de cualquier reivindicación precedente, en donde el conjunto de asidero (12) incluye un asidero estacionario (18) y la estructura de acoplamiento incluye una pestaña (24), la pestaña (24) se extiende hacia el asidero estacionario (18) y se configura para hacer un contacto inicial con el indicador de disparo (200) cuando el gatillo de disparo (20) se mueve a su segunda posición de accionado.
