Abstract

Caja intervertebral, presentando una forma alargada y delimitando cuatro caras laterales (2,3), es decir dos caras laterales principales (2), colocadas paralelamente al eje de la columna vertebral cuando se coloca la caja (1) y definiendo entre ellas el espesor de la caja, y dos caras laterales secundarias (3), relacionando entre sí las dos caras principales (2), y destinadas a contactar con las vértebras (50); dichas caras secundarias (3) presentando, vistas según una dirección perpendicular a dichas caras principales (2), una curvatura de forma convexa correspondiente a la forma cóncava que presentan los platos vertebrales (51) en el plano antero-posterior, siendo el espesor de la caja (1) reducido, es decir, entre 3,5 y 7 mm, la caja presentando, a nivel de al menos una de dichas caras secundarias (3), una ranura longitudinal (4), caja caracterizada en que la ranura longitudinal (4) es única y está dispuesta en la zona mediana de dicha cara secundaria (3), y en que la caja tiene una forma convexa a nivel de esta misma cara secundaria (3), vista en la dirección longitudinal de la caja (1).

Description

Caja intervertebral para ser colocada entre dos vértebras.
La presente invención desarrolla una caja intervertebral destinada a ser colocada entre dos vértebras.
Se utiliza una caja de este tipo para restablecer la distancia anatómica de dos vértebras cuando el disco que relaciona estas vértebras está aplastado, y para producir una inmovilización relativa de estas vértebras para el crecimiento óseo en el espacio intervertebral.
Para la colocación de una caja semejante, se dispone un alojamiento adecuado a través del disco y en los platos vertebrales, las vértebras son separadas y posteriormente se inserta la caja en el alojamiento por medio de un instrumento de colocación e impacción.
Las cajas existentes presentan generalmente formas cilíndricas, troncocónicas o paralelipédicas. Tienen dimensiones relativamente importantes en el sentido de su espesor con el objeto de delimitar dos superficies de contacto relativamente extendidas con las vértebras. Estas superficies tienen la finalidad de prevenir al máximo el riesgo de inserción de las cajas en las vértebras, resultando de las solicitaciones importantes y repetidas que las vértebras ejercen sobre ellas. Los alojamientos de recepción de estas cajas deben ser de dimensiones importantes, de forma que su manejo representa una ablación de hueso en consonancia, muy delicada de realizar teniendo en cuenta la proximidad de la médula espinal y las terminaciones nerviosas.
Por otra parte, ciertas cajas de espesor relativamente importante ocupan un espacio no desdeñable y conducen en consecuencia a un riesgo de que la fusión de las vértebras no sea perfectamente sólida.
Los documentos nº WO 95/08306 o US 6080158 describen una caja presentando una forma alargada que delimita cuatro caras laterales, es decir, dos caras laterales principales, situadas paralelamente al eje de la columna vertebral cuando la caja es colocada, definiendo entre ellas el espesor de la caja, y dos caras laterales secundarias, relacionando entre sí las dos caras principales, destinadas a contactar con las vértebras; dichas caras secundarias presentando, vistas según una dirección perpendicular a dichas caras principales, una curvatura de forma convexa correspondiente a la forma cóncava que presenta los platos vertebrales en el plano antero-posterior, siendo el espesor de la caja reducido, es decir, puede tener de 3,5
a 7 mm.
Las cajas según la técnica anterior tienen el inconveniente de presentar riesgos de basculación no desdeñables, particularmente cuando presentan espesores reducidos, así como riesgos de inserción dolorosa en las vértebras bajo el efecto de la carga que les es transmitida.
La presente invención viene a remediar dichos inconvenientes esenciales.
Su objetivo es proporcionar una caja intervertebral poco invasiva, es decir, limitando las operaciones a realizar para la confección del alojamiento de recepción de esta caja, presentando un riesgo limitado de inserción en los platos vertebrales y garantizando la obtención de una fusión perfectamente sólida de las vértebras.
La caja desarrollada es del tipo de las conocidas por los documentos anteriores mencionados
WO 95/08306 o US 6080158.
Según la invención, la caja presenta, a nivel de al menos una de dichas caras secundarias, una ranura longitudinal única dispuesta en la zona media de esta cara secundaria, en que la caja tiene una forma convexa a nivel de esta misma cara, vista en la dirección longitudinal de la caja.
Esta ranura y esta forma convexa permiten disponer dos zonas de apoyo desplazadas lateralmente, lo que favorece la estabilidad de la caja contra un riesgo de basculación. Además, la ranura permite disponer un espacio longitudinal que puede recibir virutas de hueso o pudiendo ser invadida por células óseas durante su crecimiento, lo que permite formar una nervadura ósea adecuada para fijar la caja en relación a las vértebras. Por otra parte, la curvatura convexa de dichas caras secundarias aumenta la superficie de estas caras y suprime la existencia de un ángulo saliente entre estas caras secundarias y las caras principales de la caja, lo que favorece un apoyo no agresivo de la caja contra los platos vertebrales, reduciendo de este modo el riesgo de inserción dolorosa de la caja en las vértebras.
Preferentemente, la caja presenta la ranura y la forma convexa mencionada a nivel de estas dos caras secundarias.
Ventajosamente, la caja presenta al menos una serie de entallas a nivel de al menos una de sus caras secundarias, estas entallas estando conformadas para delimitar unos dientes que favorecen una ligera inserción de la caja en los platos vertebrales.
Esta ligera inserción contribuye a favorecer el mantenimiento de la caja en posición en relación a las vértebras durante el tiempo en que se produce el crecimiento de las células óseas.
Con la misma finalidad, al menos una de las paredes principales y/o secundarias de la caja puede comprender al menos un orificio destinado a ser invadido por las células óseas durante su crecimiento.
Ventajosamente, la caja es perforada longitudinalmente por un orificio que desemboca en una de las caras extremas, y comprende canaladuras dispuestas en la pared que delimita este orificio.
Este orificio y estas acanaladuras están destinadas a recibir el extremo de forma correspondiente del instrumento de colocación de la caja. La profundidad de este agujero y la presencia de estas acanaladuras permiten una perfecta inmovilización de la caja en relación al instrumento de colocación con la finalidad de permitir un posicionamiento fiable de la caja en relación al alojamiento de recepción durante su inserción y de la impacción de esta caja en su alojamiento.
Según una forma de realización preferida de la invención la caja presenta un taladrado roscado en el fondo de dicho orificio, coaxial a dicho orificio.
Este taladrado está destinado a recibir el extremo fileteado de un vástago incluido en dicho instrumento de colocación, este vástago permitiendo una extracción de la caja si fuera necesario. Se ha hecho así posible verificar la implantación de la caja con objeto de verificar si el alojamiento de recepción está dispuesto correctamente.
En consecuencia a cuanto precede, el instrumento de colocación de la caja comprende dos piezas, a saber:
- una pieza tubular, cuyo extremo incluye una forma que se corresponde a la de dicho orificio y dichas canaladuras, y
- un vástago encajado que puede deslizarse en esta pieza tubular, cuyo extremo incorpora un fileteado que permite su fijación en dicho orificio roscado.
Para su mejor comprensión, la invención es descrita a continuación en relación a los dibujos esquemáticos anexados que representan, a título de ejemplo no limitativo, una forma que la desarrolla de una realización preferida de la caja intervertebral y de su instrumento de colocación.
La figura 1 es una vista en perspectiva de la caja;
La figura 2 es una vista lateral;
La figura 3 es una vista por un extremo;
Las figuras 4 y 5 son vistas después de la colocación entre dos vértebras, respectivamente vista lateral y del lado dorsal, y
La figura 6 es una vista lateral del instrumento.
Las figuras 1 a 3 representan bajo diferentes ángulos una caja intervertebral 1, destinada, tal como lo muestran las figuras 4 y 5, a restablecer la distancia anatómica de dos vértebras 50 cuando el disco que relaciona estas vértebras 50 está hundido, y para producir una inmovilización relativa de estas vértebras 50 por crecimiento óseo en el espacio intervertebral.
La caja 1 presenta una forma alargada y delimita cuatro caras laterales 2, 3, a saber, dos caras laterales principales 2 y dos caras laterales secundarias 3.
Las caras 2 son planas y destinadas a ser colocadas paralelamente al eje de la columna vertebral cuando se coloca la caja 1. Definen entre ellas el espesor de la caja 1, que es relativamente reducido, a saber, que corresponde a alrededor del cuarto de la longitud de la caja 1. En particular, la caja 1 puede tener una longitud de 20 mm y un espesor del orden de los
5 mm.
Las caras 3 relacionan entre sí las caras 2 y están destinadas a contactar las vértebras 50. Como muestra la figura 4, estas caras 3 presentan, mientras se las ve en una dirección perpendicular a las caras 2, una curvatura de forma convexa correspondiente a la forma cóncava que presentan los platos vertebrales 51 en el plano antero-posterior. Estas caras 3, mientras son vistas en la dirección longitudinal de la caja (figura 3) tienen además una forma curva, donde la caja 1 comprende dos ranuras longitudinales 4, con sección en forma de "V", que desemboca en la zona mediana de cada una de ellas.
La caja 1 presenta cuatro series de entallas 5 dispuestas en sus paredes delimitadas por las caras 3. Estas entallas 5 tienen una forma de "V" con un ángulo de 45º, y están configuradas para delimitar unos dientes romos que favorecen una ligera inserción de la caja 1 en los platos 51.
La caja 1 está por otra parte perforada longitudinalmente por un orificio 10 que desemboca en la cara del extremo proximal. Comprende cuatro acanaladuras proximales 11 dispuestas en la pared que delimita este orificio 10, estas acanaladuras definiendo los ángulos de un cuadrado como se muestra en la figura 3. Las acanaladuras 11 permiten el calado de la caja 1 sobre el extremo del instrumento de instalación como se describe posteriormente. La caja comprende del mismo modo un orificio terrajado 12 en el fondo del orificio 10, coaxial a este último.
La caja 1 presenta por otra parte unos orificios 15 que atraviesan sus paredes laterales 2, 3 y colocan el orificio 10 en comunicación con el exterior.
Las dimensiones de la caja 1 aparte de las indicadas anteriormente pueden en particular ser las siguientes: altura: 9, 11 o 13 mm (se prevén tres modelos para adaptar la altura de la caja a las posibles diferentes alturas de espacio intervertebral), diámetro generando la curvatura de las caras 3 en el sentido transversal de la caja: 12,91 mm, diámetro del orificio 10: 3,5 mm, diámetro de los orificios 15: 2 mm.
El instrumento 20 de colocación de la caja 1 comprende dos piezas 25, 26, a saber:
- una pieza tubular 25, cuyo extremo presenta una zona en forma cuadrada 27 cuya forma corresponde al espacio cuadrado delimitado por las acanaladuras 11, y
- un vástago 26 encajable y pudiendo ser deslizado en esta pieza tubular 25, cuyo extremo 28 comprende un fileteado permitiendo su encaje en el taladrado terrajado 12.
En la práctica, las dos cajas 1 están normalmente colocadas a nivel de un mismo disco intervertebral, a una y otra parte de la columna vertebral. Para realizarlo, como se deduce de la figura 5, se disponen dos alojamientos de recepción a través del disco intervertebral, sin resección de los platos vertebrales 51. Las vértebras 50 son inmediatamente apartadas por medio de una calza 60 insertada en uno de los alojamientos para insertar después una caja 1 en el otro alojamiento por medio del instrumento 20.
Después de la colocación de la primera caja 1, se retira la calza 60; se coloca entonces la segunda caja 1 en el segundo alojamiento con relleno de cortes de virutas de hueso esponjoso a su alrededor.
Como se ha indicado anteriormente, la invención proporciona una caja intervertebral que presenta, en relación a las cajas homólogas de la técnica anterior, la ventaja esencial de ser al mismo tiempo poco invasiva, es decir, de limitar las operaciones a realizar para la disposición del alojamiento de recepción de esta caja, presentando un riesgo limitado de inserción en los platos vertebrales y garantizar la obtención de una fusión perfectamente sólida de las vértebras.
Se afirma que la invención no está limitada a la forma de realización descrita anteriormente a título de ejemplo sino que contrariamente abarca todas las variantes de realización apuntadas por las reivindicaciones que se anexan.

Claims (6)

1. Caja intervertebral, presentando una forma alargada y delimitando cuatro caras laterales (2,3), es decir dos caras laterales principales (2), colocadas paralelamente al eje de la columna vertebral cuando se coloca la caja (1) y definiendo entre ellas el espesor de la caja, y dos caras laterales secundarias (3), relacionando entre sí las dos caras principales (2), y destinadas a contactar con las vértebras (50); dichas caras secundarias (3) presentando, vistas según una dirección perpendicular a dichas caras principales (2), una curvatura de forma convexa correspondiente a la forma cóncava que presentan los platos vertebrales (51) en el plano antero-posterior, siendo el espesor de la caja (1) reducido, es decir, entre 3,5 y 7 mm, la caja presentando, a nivel de al menos una de dichas caras secundarias (3), una ranura longitudinal (4),
caja caracterizada en que la ranura longitudinal (4) es única y está dispuesta en la zona mediana de dicha cara secundaria (3), y en que la caja tiene una forma convexa a nivel de esta misma cara secundaria (3), vista en la dirección longitudinal de la
caja (1).
2. Caja intervertebral según la reivindicación 1, caracterizada en que presenta dicha ranura longitudinal (4) y dicha forma curva a nivel de sus dos caras secundarias (3).
3. Caja intervertebral según la reivindicación 1 ó la reivindicación 2, caracterizada en que presenta al menos una serie de entallas (5) a nivel de al menos una de sus caras secundarias (3), estas entallas (5) estando configuradas para delimitar unos dientes que favorecen una ligera inserción de la caja (1) en los platos vertebrales (51).
4. Caja intervertebral según una de las reivindicaciones 1 a 3, caracterizada en que al menos una de sus paredes principales y/o secundarias comprende al menos un orificio (15) destinado a ser invadido por las células óseas durante su crecimiento.
5. Caja intervertebral según una de las reivindicaciones 1 a 4, caracterizada en que está perforada longitudinalmente por un orificio (10) que desemboca en una de sus caras extremas, y en que comprende dos acanaladuras (11) dispuestas en la pared que delimita este orificio (10).
6. Caja intervertebral según la reivindicación 5, caracterizada en que presenta un orificio roscado (12) en el fondo de dicho orificio (10), coaxial a este orificio (10).
