Description
Tubo endotraqueal con sensor de imágenes.
La presente invención se refiere en general a
medios médicos para monitorizar a pacientes en estado crítico y
anestesiados, incluyendo la monitorización de pacientes ventilados.
Más específicamente, la invención es un dispositivo para
monitorizar los órganos y las cavidades del paciente.
La inserción de tubos en los órganos, cavidades
y conductos del cuerpo del paciente es un procedimiento común en
medicina de emergencia e intensiva. Puede insertarse un tubo
endotraqueal en la tráquea de un paciente que padezca insuficiencia
respiratoria aguda o se someta a anestesia general. El tubo
endotraqueal, del cual se muestra un ejemplo típico en el documento
YS 6,164,277, debe colocarse rápida y precisamente, y posicionarse
con la punta en la porción media de la tráquea del paciente para
prevenir el deslizamiento accidental y para proporcionar la
obturación y ventilación correctas de ambos pulmones. De modo
similar, comúnmente se inserta una sonda nasogástrica por la nariz
o la boca en el estómago de pacientes que necesitan alimentación o
evacuación artificial del contenido del estómago. Otro tubo que se
inserta frecuentemente en el cuerpo durante un tratamiento de
urgencia es la sonda urinaria. Esta sonda se pasa a través de la
uretra hacia la vejiga urinaria. La correcta ubicación de estos
tubos y sondas durante su uso es críticamente importante.
Muchos pacientes en estado crítico o que se
someten a anestesia general necesitan ventilación artificial.
Durante más de 40 años, el método más común para proveer
ventilación artificial ha sido bombear aire comprimido a los
pulmones del paciente a través de un tubo endotraqueal. Este tubo
se inserta en la boca o la nariz del paciente y se pasa entre las
cuerdas vocales hacia la tráquea. Alternativamente, puede
insertarse un tubo en la tráquea a través de una incisión
quirúrgica de traqueotomía.
Para la intubación oral, el operador usualmente
utiliza un laringoscopio, que consiste en una empuñadura y una
varilla. El operador inserta una varilla en la boca del paciente y
la empuja hasta que la punta se apoya en la faringe más allá de la
raíz de la lengua. Se usa entonces la empuñadura para manipular la
varilla y empujar la lengua hacia afuera de the
Nay^{1} hasta que pueden verse la epiglotis
y las cuerdas vocales inferiores. La punta del tubo endotraqueal
puede luego dirigirse y empujarse entre las cuerdas vocales
inferiores hacia la tráquea. Este método de inserción se utiliza en
la mayoría de las intubaciones, pero requiere destreza,
capacitación y experiencia, y solo lo ponen en práctica médicos
especialistas y paramédicos autorizados.
Un método alternativo que se usa con frecuencia
cuando se anticipa una intubación difícil es el uso de un
broncoscopio de fibra óptica. Primero, el broncoscopio se conecta a
una fuente de luz para proveer la iluminación necesaria del campo
mirando hacia la punta. El eje del broncoscopio se inserta luego a
través del tubo endotraqueal y se inserta lo más internamente
posible. La punta del broncoscopio en la vía respiratoria del
paciente se empuja bajo la visualización a través del ocular del
broncoscopio o una pantalla de vídeo entre las cuerdas vocales
inferiores y la tráquea. El tubo endotraqueal puede ahora empujarse
hacia el eje del broncoscopio y moverse entre las cuerdas vocales
inferiores hacia la tráquea. El tubo endotraqueal puede ahora
asegurarse, y se puede retirar el broncoscopio para liberar el
lumen del tubo endotraqueal. Si bien el método es más seguro con el
laringoscopio, el equipo necesario es costoso, delicado y más
engorroso, y es raro encontrarlo en el campo o en vehículos médicos
de urgencia.
Asegurar el tubo endotraqueal y prevenir su
movimiento accidental durante el uso es crítico para la prevención
de accidentes fatales. Inflar un manguito que rodea el tubo cerca
de su punta ocluye el espacio entre la pared externa del tubo y la
pared interna de la tráquea para proveer una obturación hermética.
El manguito se conecta al extremo externo del tubo endotraqueal a
través de un canal delgado en la pared del tubo. El canal se
conecta a una válvula unidireccional a través de la cual se puede
inyectar aire para inflar el manguito hasta la presión y el volumen
deseados. El manguito es también útil para asegurar el tubo en su
sitio, pero por lo general se aplican sujetadores adicionales
alrededor de la cabeza para prevenir que el tubo se deslice o se
suelte.
Una vez que el tubo ha sido insertado, es
obligatorio verificar su posición correcta. La inserción accidental
del tubo en el esófago o su colocación muy profunda dentro de las
vías respiratorias, de modo que la punta se suelte en uno de los
bronquios del tallo principal en lugar de la tráquea puede conducir
a consecuencias catastróficas y asfixia. Existen muchos métodos
para verificar la colocación del tubo endotraqueal. Por lo general
se realiza la auscultación de ambos lados del pecho para verificar
la entrada simétrica de aire a ambos pulmones. Otro método
comprobado para verificar la colocación del tubo es una radiografía
de tórax. La radiografía revela la relación entre la punta del tubo
endo-
traqueal y la primera bifurcación traqueal (carina). Se pueden y se deben tomar radiografías cada vez que se coloque o

Además, la colocación del tubo puede verificarse a través de un
broncoscopio de fibra óptica, mediante un bulbo de succión o
enviando y recibiendo una señal acústica. Estos métodos se usan
para verificar la colocación inicial del tubo endotraqueal. No
existen actualmente métodos para monitorización continua de la
colocación real del
tubo.
Las ventajas de la visualización con fibra
óptica se combinaron con el simple diseño del laringoscopio según
se describe en varias patentes y documentos científicos. Además, se
ha descrito el uso de dispositivos de visualización que incluyen
métodos para ver las vías respiratorias durante la inserción de un
tubo endotraqueal. No obstante, no hay métodos conocidos para
incorporar los métodos de visualización permanentemente en la cara
anterior del tubo endotraqueal de modo que pueda lograrse la
visualización de las vías respiratorias durante la inserción y
continuamente de allí en más.
Los endoscopios con sensores de imágenes en su
superficie anterior para inserción en el tubo gastrointestinal se
conocen del documento W094/28784.
La presente invención provee un tubo
endotraqueal multifunción como se expone en lo sucesivo en la
reivindicación 1 de las reivindicaciones anejas.
La Fig. 1 es un esquema isométrico del tubo de
la invención que incorpora tres tipos de conductos; la Fig. 2 es
una descripción isométrica esquemática de una porción de la cara
anterior del tubo de la invención en la cual se incorpora una
cámara de vídeo en miniatura; la Fig. 3 es una descripción
esquemática de los artículos que viajan a lo largo del tubo de la
invención, en relación con el desempeño de las tareas de
inspección.
En la realización ilustrada dé la invención, se
provee un tubo de inspección multifuncional para recoger
información sobre las cavidades y espacios internos del cuerpo de
un paciente o un animal en relación con la inserción de un tubo de
inspección en el cuerpo. El tubo de inspección multifuncional es un
tubo endotraqueal modificado. Según la presente invención, el tubo
está equipado con medios para examinar tanto el posicionamiento del
tubo de inspección con respecto a los órganos del cuerpo como los
aspectos funcionales del cuerpo durante y después de la inserción.
Por lo tanto, el tubo de la invención puede usarse no solo para
tareas de tratamiento médico habituales, sino también para tareas
de inspección que examinan la reacción a dicho tratamiento y la
condición de los órganos penetrados. El tubo de inspección
multifuncional incorpora un medio para recibir señales relacionadas
con la condición de los órganos penetrados, como señales visuales y
de audio, empleando sensores adecuados incorporados en la cara
anterior del tubo o cerca de ésta. Las señales producidas por los
sensores se transmiten vía cables o fibras de comunicación que
pasan a lo largo del tubo hacia un conector o un transmisor
inalámbrico ubicado en la porción posterior del tubo cerca de su
conector estándar hacia la fuente de ventilación. Las señales se
reciben con un receptor que contiene un medio adecuado de
acondicionamiento de la señal para posterior procesamiento,
exhibición, grabación y/o monitorización. El concepto estructural
de la invención se explica mejor con referencia a la Fig. 1. Se
muestra una porción 20 de un tubo de inspección multifuncional de
la invención, que incluye una cara anterior 22. Se asocian canales
y un conductor con la pared del tubo. Un canal totalmente
incorporado 24 pasa por el tubo de inspección dentro de su pared, a
lo largo de un canal ranurado 26. Un elemento conductor 28 pasa a
lo largo del tubo sin estar incorporado a la pared del tubo, sino
que unido a la pared del tubo, y ocupa un espacio en el lumen del
tubo. El conducto puede estar parcialmente incorporado al tubo o
puede estar insertado dentro de una ranura o puede pasar dentro de
un canal totalmente incorporado sin unirse al tubo. Incluso en
realizaciones en las que el lumen contiene un elemento conductor
unido a al pared, como se describió anteriormente, el lumen del
tubo está en gran medida libre para transferir gases en ambas
direcciones. En este sentido, puede ser preferible la realización
en la que los canales y los elementos conductores están
incorporados o totalmente insertados en la pared.
Una característica típica del tubo de inspección
multifuncional es la adquisición de imágenes internas del cuerpo.
Para adquirir las imágenes, se puede emplear un sensor de imágenes,
como una cámara electrónica en miniatura que emplea un chip CCD o
CMOS incorporado en la cara anterior del tubo de inspección. En una
realización, la cámara se incorpora en una ranura en la pared del
tubo como se describe en la Fig. 2 a la que se hace ahora
referencia. Se muestra una porción de un tubo de inspección, dentro
del lado interno 40 en el cual se inserta una cámara 42 en un canal
ranurado, que sobresale de la cara anterior 44 del tubo. La señal
de la cámara es transmitida por un elemento conductor 46,
típicamente un alambre de cobre o una fibra óptica. La lente de la
cámara mira hacia afuera del tubo. Las señales que llegan desde la
cámara son posteriormente alimentadas a un receptor y pueden
exhibirse luego en una pantalla, que puede ser una minipantalla
independiente, una pantalla de vídeo común o una porción de la
pantalla comúnmente utilizada para monitorizar los parámetros
fisiológicos y de bienestar del paciente. En algunas realizaciones
de la invención, se utiliza un elemento de fibra óptica que pasa a
lo largo del tubo para transmitir luz a fin de iluminar el campo
visual de la cara anterior del tubo de inspección.
Alternativamente, puede asociarse una fuente de luz con la cara
proximal del tubo. Los ejemplos de recursos de luz son lámparas
halógenas en miniatura, diodos de emisión de luz (LED), láser o
cualquier otra fuente de emisión de luz de tamaño adecuado. Un
método alternativo de iluminación consiste en construir el tubo de
inspección de material conductor de luz. En algunas realizaciones
de la invención, se emplean métodos para mantener la lente de la
cámara y/u otros sensores limpios y transparentes. Las vías
respiratorias de un paciente enfermo por lo general se llenan de
secreciones que pueden ser espesas y viscosas. Por lo tanto, es
bastante posible que las secreciones se acumulen en la lente y
obscurezcan su campo visual, o en otros sensores, modificando así
su grado de respuesta. Para superar dicho obstáculo, se bombea un
flujo de aire o flujo fluido fisiológico constante o intermitente a
través de un canal en la pared del tubo, por el cual la descarga
del canal se dirige directamente hacia la lente o la superficie
activa del sensor. Este flujo puede ser generado por una sola
fuente de flujo o por un dispositivo que se activa para emitir
flujo tras el comando de un cuidador humano, un cronómetro o un
programa de software que monitoriza la señal y determina cuándo se
requiere una acción de limpieza.
En general, el tubo de inspección se usa como
plataforma de transporte bidireccional para diversos elementos
requeridos para el cumplimiento de sus tareas de inspección. Esto
se describe esquemáticamente en la Fig. 3 a la que se hace ahora
referencia. El tubo 52 recibe energía de activación 54 de uno o
varios tipos en su extremo posterior, y descarga información 56, en
bruto o procesada del mismo extremo. En el extremo anterior 58, el
tubo recibe señales 60 de uno o varios tipos, y consume energía 62
que luego será elaborada.
En algunas realizaciones de la invención, se
emplea un micrófono en el tubo. Dicho micrófono puede incorporarse
en la pared del tubo. Dicho micrófono recibe señales acústicas de
por lo menos la proximidad de la parte anterior del tubo, y
transfiere las señales, en bruto o procesadas, a la parte posterior
del tubo para descarga y procesamiento adicionales.
Se pueden emplear eficazmente una pluralidad de
sensores en la cara anterior del tubo de la invención, la lista no
limitativa incluye cámaras, videocámaras, micrófonos, transductores
de presión y sensores térmicos. Los sensores de gas, por ejemplo
sensores para gases particulares tales como oxígeno y dióxido de
carbono, pueden también emplearse. La energía requerida para
activar dichos sensores es provista por conductos de energía tales
como cables eléctricos incorporados al tubo. Además, se puede
suministrar energía auxiliar a la proximidad de la cara anterior
del tubo para fines de limpiar y aclarar las facetas activas de los
sensores, limpiándolos con medios de limpieza tales como gases, aire
u oxígeno humidificado, o líquidos, típicamente una solución
fisiológica, a través de canales en la pared del tubo. Los líquidos
o gases para limpiar se energizan y conducen típicamente vía un
canal totalmente incorporado. El tubo de inspección de la invención
puede utilizarse solo o en combinación con otras sondas y tubos
que se insertan ordinariamente en las vías respiratorias de los
pulmones. La tecnología actual puede aplicarse para la ventilación
artificial del pulmón.
Los sensores del tubo transmiten uno o más tipos
de señales, que son preprocesadas en el sensor, por ejemplo en el
chip CCD, y pueden enviarse en bruto, para ser procesadas
adicionalmente por circuitos análogos o digitales a fin de producir
información relacionada con el estado del órgano o la cavidad del
cuerpo inspeccionada. Los dispositivos receptores o procesadores
tales como monitores, visores, medios de almacenamiento,
analizadores, procesadores DSP, ordenadores y generadores de
señales de alarma típicamente se conectan con una o una pluralidad
de conectores del tubo. El tubo de la invención puede usarse para
inserción a través de orificios tales como la nariz o la boca.
La transmisión de señales en bruto o procesadas
se realiza a través de conductores a lo largo del tubo tales como
cables o fibras ópticas, que se conectan a un conector en la parte
posterior del tubo. Puede aplicarse un transmisor inalámbrico o
transmisor-receptor en cualquier parte adecuada del
tubo, típicamente en la parte posterior, para comunicar con una
consola que contiene un receptor y un procesador y/o un módulo de
control.
El tubo de inspección de la invención puede
también detectar cambios en las indicaciones de las funciones
vitales de un paciente. Por consiguiente, la imagen y la señal
acústica se detectan, procesan y comparan con una estructura de
imagen o sonido base de referencia. Suena una alarma ni bien
ciertos cambios en la indicación pasan un umbral predeterminado.
Por ejemplo, la acumulación de secreciones o el desarrollo de
ruidos excesivos o insuficientes anormales en los pulmones.

Claims (14)
1. Un tubo endotraqueal multifuncional (52) que
tiene un lumen sustancialmente libre para transportar diversos
elementos y gases hacia adentro y hacia afuera de los pulmones y
pasajes de aire penetrados por dicho tubo (52), estando el tubo
caracterizado por:
un sensor de imágenes (42) incorporado en la
cara anterior (22,44) de dicho tubo, por lo menos un conducto de
energía (54) para activar dicho sensor de imágenes (42) está
asociado con la pared de dicho tubo (52);
por lo menos un elemento conductor (28) para
transmitir señales de imágenes de dicho sensor de imágenes al
extremo posterior de dicho tubo, y por lo menos un receptor para
dichas señales de imágenes.
2. Un tubo multifunción (52) según la
reivindicación 1, y donde la pared de dicho tubo (52) comprende un
canal (24, 26).
3. Un tubo multifunción (52) según la
reivindicación 2, y donde la pared (20) de dicho tubo comprende un
canal totalmente incorporado (24).
4. Un tubo multifunción según la reivindicación
2, y donde la pared de dicho tubo comprende un canal ranurado
(26).
5. Un tubo multifunción según la reivindicación
1, y donde un elemento de iluminación se asocia con dicha cara
anterior de dicho tubo.
6. Un tubo multifunción según la reivindicación
5, y donde dicho elemento de iluminación es una fibra óptica que
pasa a lo largo de la pared de dicho tubo.
7. Un tubo multifunción según la reivindicación
5, y donde dicho elemento de iluminación es una fuente de emisión
de luz.
8. Un tubo multifuncional (52) según la
reivindicación 1, que además comprende medios de limpieza para
mantener limpio dicho sensor de imágenes.
9. Un tubo multifuncional (52) según la
reivindicación 8, en el que dichos medios de limpieza se activan
con por lo menos un software o un cronómetro.
10. Un tubo multifunción (52) según cualquiera
de las reivindicaciones precedentes, que además incluye un
micrófono para recibir señales acústicas de la proximidad de la
parte anterior del tubo.
11. Un sistema que comprende un tubo
multifuncional (52) según cualquiera de las reivindicaciones
precedentes, en combinación con por lo menos un circuito para
procesar señales provistas por dicho sensor (42) como para producir
información relativa al estado de los pulmones y los pasajes de
aire que estén siendo inspeccionados por dicho tubo (52).
12. Un sistema según la reivindicación 11, que
además comprende medios para detectar cambios en la indicación de
las funciones vitales, que comprenden medios para procesar y
comparar las señales de imágenes con una imagen base de referencia,
una alarma que se activará ni bien ciertos cambios en la indicación
pasen un umbral predeterminado.
13. Un sistema según la reivindicación 11 ó 12,
donde dicho circuito se comunica con dicho sensor a través de
comunicación alámbrica o inalámbrica.
14. Un tubo multifuncional según cualquiera de
las reivindicaciones 1 a 10, en donde el sensor (42) se adapta para
enviar señales a dispositivos de exhibición y monitorización como
para producir información relacionada con el estado de los pulmones
y los pasajes de aire que estén siendo inspeccionados por dicho
tubo (52).
