Abstract

Description
Procedimiento de fabricación de un implante
ortopédico con una superficie metálica porosa.
La presente invención se refiere, generalmente,
a implantes ortopédicos, y, más específicamente, a un implante
ortopédico que tiene una capa superficial porosa de metal
biocompatible, o almohadilla.
Se conoce cómo proveer un implante ortopédico
cubierto por una capa porosa para favorecer el crecimiento del hueso
dentro de las aberturas de la misma, proporcionando así una fijación
a largo plazo del implante al hueso. Se conocen, también, varios
procedimientos para conseguir tal unión.
La unión por difusión es el procedimiento más
ampliamente conocido y aceptado para fijar una capa porosa a un
implante. La unión por difusión de una capa porosa a un sustrato que
tiene una superficie curvada, tal como una porción de un vástago de
cadera, sin embargo, no está exenta de dificultades. Por ejemplo, la
unión por difusión de un material poroso a un sustrato puede hacer
que se formen entallas en el sustrato, disminuyendo así la
resistencia del mismo. Los fabricantes de implantes ortopédicos han
compensado exitosamente el efecto de la unión por difusión, bien
limitando la cantidad de capa porosa fijada al sustrato, bien
ajustando el tamaño y forma del mismo para aumentar su
resistencia.
No obstante, la unión por difusión de una
almohadilla metálica porosa a un cuerpo de implante consume tiempo y
es cara desde el punto de vista de su fabricación. Por ejemplo, el
tiempo de subida en rampa y enfriamiento necesario para que un horno
efectúe un proceso de unión por difusión puede ser tan alto como 14
horas por ciclo. En algunas aplicaciones, tal unión por difusión de
una almohadilla metálica porosa a la superficie interior de
aplicación al hueso de una componente femoral de rodilla, puede
necesitar un mínimo de tres ciclos (42 horas) para completar la
operación de unión por difusión.
A fin de evitar algunos de estos problemas, el
solicitante ha desarrollado un proceso mejorado para unir una
almohadilla metálica porosa a la superficie de un implante
ortopédico que comprende la unión por difusión de una almohadilla
metálica fibrosa a una lámina metálica delgada, fijando entonces la
almohadilla metálica a un implante ortopédico usando un proceso de
soldadura láser. (Para los detalles de tal proceso de fijación, se
hace referencia por esta memoria a la patente de EE.UU. número
5.504.300 titulada "Orthopaedic Implant and Method of Making
Same", la patente de EE.UU. número 5.672.284 titulada "Method

número 5.773.789 titulada "Method of Making Orthopaedic Implant
Having a Metal Pad" y la patente de EE.UU. número 6.049.054
titulada "Method of Making an Orthopaedic Implant Having a Porous
Metal Pad", cada una de las cuales está cedida al cesionario de
la presente invención. En general, cada una de estas patentes
describe un procedimiento para fijar una superficie porosa a un
implante ortopédico, en el que una almohadilla metálica porosa, tal
como una almohadilla metálica fibrosa, está unida por difusión a una
lámina metálica delgada. La almohadilla metálica fibrosa está
configurada entonces para ser recibida dentro de un rebaje formado
en un cuerpo ortopédico. Los bordes de la lámina metálica delgada se
extienden al exterior del rebaje formado en el cuerpo de implante
ortopédico. Después de ello, se usa una soldadora láser para soldar
la lámina metálica delgada al cuerpo de implante ortopédico en
diversas posiciones, fijando así indirectamente la almohadilla
metálica fibrosa. La soldadura láser, sin embargo, aunque de algún
modo superior a la unión por difusión, o a la sinterización, de una
almohadilla metálica porosa directamente a un cuerpo de implante,
requiere dispositivos caros de soldadura láser.
Debido a las dificultades asociadas con los
medios conocidos para unir una superficie porosa al núcleo de un
implante ortopédico, se ha centrado la atención en el uso de
materiales no metálicos para construir todo o una porción de un
implante ortopédico y unir, entonces, la superficie metálica porosa
al mismo. Se han usado en tales dispositivos materiales tales como
plásticos, de tipo compuesto, cerámica y de tipo termoestable. Tales
materiales se usan con la esperanza de hacer concordar mejor la
rigidez a la flexión del hueso y ganar una mayor transferencia de
fuerzas a través del implante hasta el hueso biológico, a fin de
favorecer el crecimiento y la estabilidad del mismo. Las patentes
de EE.UU. números 5.236.457, a Devanathan; y 5.571.187, también a
Devanathan, ambas cedidas al solicitante, describen procedimientos
para unir un metal poroso a una capa polímera. En general, estas
patentes describen un procedimiento para producir un implante
ortopédico disponiendo un núcleo metálico rodeado por un polímero o
un cuerpo de implante completamente polímero; formando un inserto de
capa superficial porosa; situando ambos objetos en un molde; e
inyectando un polímero entre ellos, a fin de unir la superficie
porosa al cuerpo de implante. Después de ello, los polímeros amorfos
que se usan para formar la superficie porosa se eliminan del
implante usando un disolvente. Aunque este procedimiento produce un
producto exitoso y seguro, es deseable producir un proceso que no
requiera la extracción del polímero amorfo desde la superficie
exterior porosa, paliando así la necesidad de mantener disolventes
costosos y potencialmente peligrosos en las instalaciones de
fabricación. De modo similar, el documento US 5.326.354 describe un
procedimiento para producir un implante con una capa superficial
porosa, incrustando un separador patrón en la superficie del
implante y extrayéndolo, entonces, gracias a una solución ácida
acuosa. Existe una necesidad, por lo tanto, de un procedimiento
para impregnar una superficie interior de una almohadilla metálica
porosa con una película polímera, similar, en química, al polímero
usado en la etapa de unión. Esta mejora no requiere el uso de un
disolvente para eliminar un polímero amorfo de la superficie
exterior de la almohadilla impregnada. Existe una necesidad
adicional de unir tal almohadilla impregnada con un implante
ortopédico para formar un implante ortopédico que tenga una
superficie metálica porosa.
La invención comprende un procedimiento para
unir una superficie metálica porosa a un implante ortopédico que
tiene un cuerpo de plástico o un núcleo sustancialmente sólido
rodeado por un cuerpo de plástico. Además, la presente invención
proporciona un procedimiento para impregnar una almohadilla metálica
porosa con una película polímera, tal como una poliarilétercetona,
para facilitar la unión posterior de la superficie metálica porosa a
una superficie

El procedimiento para impregnar una almohadilla
metálica porosa comprende disponer una lámina de metal
biocompatible, preferiblemente, titanio comercialmente puro, una
lámina de película liberable resistente al calor de plástico,
generalmente una película de poliimida, tal como KAPTON^{TM}; una
lámina de película polímera semicristalina, tal como
poliéteretercetona, poliarilétercetona o sulfuro de polifenileno;
una almohadilla metálica porosa, una segunda lámina de película
liberable resistente al calor de plástico y una segunda lámina de
metal biocompatible. Estas láminas y la almohadilla están dispuestas
de manera que las láminas de metal biocompatible forman las capas
más exteriores y de manera que la lámina metálica porosa se apoya
contra la película polímera semicristalina. La disposición en capas
se sitúa posteriormente dentro de una prensa durante un tiempo
suficiente y bajo condiciones suficientes para permitir que la
película polímera semicristalina se interponga, a su vez, dentro del
metal poroso, produciendo así una almohadilla impregnada.
El procedimiento para formar un implante
ortopédico que tiene una superficie metálica porosa comprende
disponer una almohadilla metálica porosa de tamaño y forma deseadas
y adecuadas para unirse a un implante ortopédico, impregnar la
almohadilla metálica porosa, usando el procedimiento descrito
anteriormente, colocar la almohadilla metálica porosa impregnada
dentro de un dispositivo de moldeo, moldear entonces un polímero en
el dispositivo de moldeo, de manera que el polímero forme el cuerpo
de un implante ortopédico y se una al menos a una superficie de la
capa metálica porosa.
En una realización alternativa, se coloca un
núcleo sustancialmente sólido dentro del dispositivo de moldeo antes
de moldear un polímero. El núcleo puede comprender cualquier
material biocompatible, incluyendo titanio, o cerámica; sin embargo,
el núcleo comprende, preferiblemente, una aleación de cobalto, cromo
y molibdeno. Cuando el polímero está moldeado en el dispositivo,
rodea el núcleo y actúa como una capa intermedia entre un núcleo de
metal biocompatible y la superficie metálica porosa de un implante
ortopédico.
De esta manera, la presente invención
proporciona un procedimiento para realizar un implante ortopédico
que tiene una superficie porosa, sin las deficiencias de la
soldadura láser o de la unión por difusión. Además, el polímero
semicristalino reduce el tiempo de tratamiento requerido para
producir un implante.
Estas y otras ventajas de la presente invención
serán evidentes para los expertos en la técnica, tras leer la
presente memoria descriptiva y las reivindicaciones adjuntas.
La figura 1 es una vista en perspectiva de un
implante ilustrativo de hueso según la presente invención.
La figura 2 es una vista en alzado y en despiece
ordenado de la almohadilla metálica porosa impregnada.
La figura 3 es una vista en perspectiva de una
almohadilla metálica porosa impregnada.
La figura 4 es una vista en perspectiva y en
despiece ordenado de un molde ilustrativo de implante y un
implante.
La figura 5 es una vista en perspectiva de un
implante ilustrativo de hueso según la presente invención, que tiene
una superficie porosa, una capa intermedia polímera y un núcleo de
metal biocompatible (no mostrado).
Haciendo referencia a la figura 1, se representa
un implante protésico ilustrativo. Aunque la figura 1 muestra un
vástago femoral de cadera, los expertos en la técnica comprenden que
la presente invención se puede aplicar a otros implantes
endoprotésicos, para los que es deseable tener una superficie
metálica porosa, por ejemplo, la superficie de contacto con el hueso
de un implante condíleo femoral de rodilla. El vástago 1 de cadera
mostrado en la figura 1 incluye un cuerpo 110 y una superficie 120
porosa. Para producir un implante, se forma primero una almohadilla
240 porosa con un material adecuado, por cualquiera de una variedad
de procedimientos conocidos. Preferiblemente, incluye fibras de
metal biocompatible, tal como titanio y sus aleaciones o aleaciones
de cobalto-cromo. La almohadilla 240 se puede
producir también por perlas metálicas sinterizadas, cerámica
sinterizada u otro material poroso adecuado.
A continuación, se forma una almohadilla 300
porosa impregnada, como se muestra en la figura 3. Este inserto
llegará a ser, finalmente, la superficie porosa de un implante
ortopédico según la presente invención. Como se muestra en la figura
2, la superficie porosa se forma impregnando el interior de la
almohadilla 240 porosa con un polímero adecuado. Esto se consigue
disponiendo una primera lámina 210 de metal biocompatible,
preferiblemente titanio; una primera lámina de una película 220
liberable resistente al calor de poliimida; una lámina de una
película 230 polímera semicristalina; seleccionada a partir del
grupo que consiste en poliéteretercetona, poliacrilétercetona y
sulfuro de polifenileno; una almohadilla 240 metálica porosa; una
segunda lámina de una película liberable resistente al calor de
plástico y una segunda lámina 210 de metal biocompatible. Estas
capas están dispuestas de manera que las láminas 210 de metal
biocompatible forman las capas más exteriores de la disposición y la
almohadilla 240 se apoya contra la película 230 polímera
semicristalina en el interior de la disposición. Después de ello, un
modo preferido de introducir la película 230 polímera semicristalina
en la almohadilla 240 es apretando esta disposición en capas entre
sí durante un tiempo suficiente y a temperatura y presión
suficientes para permitir que la película 230 polímera
semicristalina avance hacia dentro de la almohadilla 240 de manera
controlable. Preferiblemente, la almohadilla 240, que se corta con
una forma deseada, impregnada con la película 230 polímera
semicristalina, toma entonces (si es necesario) su forma final. Sin
embargo, la almohadilla 240 porosa se puede impregnar con la
película 230 polímera semicristalina antes de tomar su forma final.
En cualquier caso, la almohadilla 300 impregnada está construida
para incluir unos agujeros 310 que ayudan a situar y asegurar el
inserto en un molde, tal como mediante pasadores que se extienden
desde el molde para aplicarse a los agujeros.
Al menos una almohadilla 300 impregnada está
colocada en una mitad 410 apropiada de molde, con alguno de los
poros sin llenar en comunicación con la cavidad de molde, como se
muestra en la figura 4. Este material formará finalmente el cuerpo
420 de implante, al que estará unida la almohadilla 300 impregnada.
Se pueden usar diversas técnicas de moldeo, incluyendo moldeo por
inyección, moldeo por compresión y colado. Es preferible, sin
embargo, usar moldeo por inyección. La almohadilla 300 porosa
impregnada está construida de manera que los materiales de
impregnación impiden que el material del cuerpo entre en los poros
de película de la almohadilla 300 impregnada, durante el moldeo,
pero permiten que se infiltren los poros desprotegidos para
establecer una fijación segura de la almohadilla 300 impregnada al
cuerpo 420. Preferiblemente, el material del cuerpo es un polímero
semicristalino, tal como poliarilétercetona, poliéteretercetona o
sulfuro de polifenileno. Son deseables los polímeros
semicristalinos, tales como los anteriores, debido a su
resistencia, su procesamiento, en general, con insolubilidad en los
disolventes comunes. Si se desea, otros agentes de refuerzo, tal
como fibra de carbono, se pueden incluir en el material del
cuerpo.
Aunque lo anterior ha descrito una realización
ilustrativa y preferida de la presente invención, son posibles
variaciones. Por ejemplo, los materiales y procedimientos descritos
en esta memoria se podrían usar para producir un implante con una
superficie porosa adecuada para cualquier implante endoprotésico.
Además, otras estructuras se podrían incluir en el molde antes de
moldear el cuerpo. Tales estructuras incluyen núcleos sólidos y
compuestos, pasadores de fijación e insertos huecos. En los dibujos
incluidos en esta memoria, se muestra un núcleo 415 sustancialmente
sólido en el molde entre dos almohadillas 300 impregnadas. El núcleo
415 está formado, preferiblemente, por un material biocompatible,
tal como una aleación de cobalto, cromo y molibdeno. Como se muestra
en la figura 3, el núcleo 415 tiene un cuello 416, un vástago 417 y
un botón 418. El cuello 416 y el botón 418 se aplican al molde 410
para formar sellados e impedir que el material inyectado revista sus
superficies. Además, el inserto de la almohadilla 300 impregnada se
puede fijar a un implante formado sustancialmente por completo
usando el procedimiento anteriormente descrito, en el que se llena
parcialmente el inserto y se introduce un material fluido entre la
almohadilla 300 impregnada y el cuerpo de implante, de manera que al
solidificar el material fluido, éste forme una superficie del cuerpo
en la que el inserto está embebido. Los expertos en la técnica
comprenden que la superficie porosa se puede usar para cubrir todo o
una porción del cuerpo de un implante, como se desee, para una
aplicación específica. Además, los expertos en la técnica comprenden
que las modificaciones antes mencionadas y numerosas variaciones
distintas se pueden hacer a la realización preferida presente en
esta memoria, sin salirse del alcance de la invención definida por
las reivindicaciones adjuntas.
Claims (14)
1. Un procedimiento para impregnar una
almohadilla metálica porosa con un polímero, caracterizado
porque el procedimiento comprende:
(a) proporcionar una primera lámina (210) de
metal biocompatible, una primera lámina de una película (220)
liberable resistente al calor de plástico, una lámina de una
película (230) polímera semicristalina, una almohadilla (240)
metálica porosa, una segunda lámina de la película (220) liberable
resistente al calor de plástico y una segunda lámina (210) de metal
biocompatible;
(b) crear una disposición de apoyo de las
láminas (210, 220, 230) y la almohadilla (240), de manera que las
láminas (210) de metal biocompatible formen las capas más exteriores
de la disposición; y
(c) colocar la disposición dentro de una prensa
durante un tiempo suficiente y a temperatura y presión suficientes
para permitir que la película (230) polímera semicristalina se
interponga, a su vez, dentro de la almohadilla (240) metálica
porosa, a fin de formar una almohadilla (300) metálica porosa
impregnada.
2. El procedimiento de la reivindicación 1,
caracterizado porque las láminas (210) de metal biocompatible
comprenden titanio comercialmente puro.
3. El procedimiento de las reivindicaciones 1 o
2, caracterizado porque la película (230) polímera
semicristalina se selecciona a partir del grupo constituido por
poliéteretercetona, poliarilétercetona y sulfuro de
polifenileno.
4. El procedimiento de cualquier reivindicación
precedente, caracterizado porque la película (220) liberable
resistente al calor de plástico es una película de poliimida.
5. El procedimiento de cualquier reivindicación
precedente, caracterizado porque las láminas y la almohadilla
están dispuestas de manera que cada una de ellas está adyacente al
menos a otra lámina o almohadilla, y la almohadilla metálica porosa
se apoya contra la lámina de película polímera semicristalina; y
porque el procedimiento comprende las etapas adicionales de colocar
la almohadilla impregnada dentro de un dispositivo de moldeo;
moldear un cuerpo de implante polímero, adyacente a la superficie
interior impregnada polímera de la almohadilla, de manera que se

4, caracterizado porque comprende las etapas iniciales
de:
(i) proporcionar un núcleo (415) sustancialmente
sólido; y
(ii) proporcionar una almohadilla (240) metálica
porosa, que tiene un tamaño deseado y una forma deseada que son
adecuados para unirse al núcleo; y porque el procedimiento comprende
las etapas adicionales, con posterioridad a la etapa (c), de;
(iii) colocar la disposición y el núcleo (415)
dentro de un dispositivo de moldeo por inyección; y
(iv) moldear por inyección una capa intermedia
polímera alrededor del núcleo (415) y entre el núcleo y la
superficie interior impregnada polímera de la almohadilla (300),
para formar un implante ortopédico que tiene un núcleo (415), una
capa intermedia polímera y una superficie metálica porosa.
10. El procedimiento de la reivindicación 9,
caracterizado porque el núcleo metálico comprende una
aleación de
cobalto-cromo-molibdeno.
11. El procedimiento de la reivindicación 9 o la
reivindicación 10, caracterizado porque la lámina de metal
biocompatible es titanio comercialmente puro.
12. El procedimiento de las reivindicaciones 10
u 11, caracterizado porque la película liberable resistente
al calor de plástico es una película de poliimida.
13. El procedimiento de la reivindicación 9,
caracterizado porque la capa intermedia polímera se
