Abstract
Aparato para la hemostasis de una arteria (112) que tiene una punción después de un cateterismo arterial, comprendiendo el aparato: un catéter (120) de hemostasis que incluye un anclaje inflable (134) en un extremo delantero del mismo y un globo (132) adyacente a dicho extremo delantero y espaciado de dicho anclaje inflable; un inflador seleccionable para inflar el anclaje inflable dentro de dicha arteria; donde el borde delantero del catéter de hemostasis se extiende hacia delante menos que la extensión del globo cuando es inflado, con lo cual el inflado del globo (132) adyacente al extremo delantero del catéter de hemostasis en encaje con la superficie exterior de la pared de la arteria causa que se retire completamente el extremo delantero del catéter de hemostasis de la pared de la arteria y que se aparte de la misma.
Description
Aparato para hemostasis posterior a un
cateterismo arterial.
La presente invención se refiere a un aparato
para hemostasis posterior a un cateterismo arterial.
Se conocen diversas técnicas para el cateterismo
arterial. Después del cateterismo arterial, es necesario originar la
hemostasis rápidamente y sin molestia indebida para el paciente.
La patente de EEUU 5.728.134 y la solicitud de
patente PCT WO 98/11830 publicada del solicitante describen un
método y un aparato para la hemostasis que simplifican en gran
medida la hemostasis y por tanto reduce grandemente la incomodidad
del paciente siguiente a un cateterismo arterial. Se considera que
la técnica anterior a la que se hace referencia en la solicitud de
patente PCT WO 98/11830 publicada y en la patente de EEUU 5.728.134
del solicitante representan el estado actual de la técnica.
La patente de EEUU 5.486.195 describe un aparato
para cerrar un emplazamiento de arteriotomía en un paciente, que
comprende un primer catéter a introducir en la arteria, un segundo
catéter a través del cual se puede introducir el primer catéter,
teniendo ambos catéteres globos distales respectivos a inflar, y
medios para suministrar un producto a fin de cerrar el emplazamiento
de la arteriotomía. Finalmente se inyecta el producto hemostático a
través de un pequeño orificio próximo al extremo delantero del
catéter.
La presente invención busca proporcionar un
aparato mejorado para la hemostasis.
Según la presente invención, se proporciona un
aparato para la hemostasis de una arteria que tenga una punción
después de un cateterismo arterial como el de la reivindicación.
Adicionalmente, según una realización preferida
de la presente invención, el aparato incluye además un inyector para
inyectar un agente hemostático a través del introductor del catéter
en un emplazamiento externo de la arteria.
Preferiblemente, el borde delantero del
introductor del catéter se extiende menos de 4 mm más allá del borde
delantero del globo cuando no se encuentra inflado.
Se entenderá y se apreciará más plenamente la
presente invención a partir de la descripción detallada siguiente,
considerada conjuntamente con los dibujos, en los cuales:
Las Figs. 1-12 son ilustraciones
simplificadas de un aparato para la hemostasis posterior a un
cateterismo arterial, de acuerdo con la presente invención que
emplea un catéter novedoso, y su método de uso, en el cual:
la Fig. 1 es una ilustración de inserción de un
introductor de catéter en una arteria, como se conoce en la técnica
anterior;
la Fig. 2 es una ilustración del introductor de
catéter y un catéter asociado durante el cateterismo arterial, como
se conoce en la técnica anterior;
la Fig. 3 es una ilustración del introductor de
catéter después de retirar el catéter usado durante el cateterismo
arterial, como se conoce en la técnica anterior;
la Fig. 4 es una ilustración de la introducción
de un catéter de hemostasis en la arteria;
la Fig. 5 es una ilustración del inflado de un
anclaje inflable en el catéter de hemostasis;
la Fig. 6 es una ilustración de la retracción
del catéter de hemostasis en encaje con el extremo delantero del
introductor de catéter;
la Fig. 7 es una ilustración de la retracción
del catéter de hemostasis de tal manera que el anclaje inflable
inflado se sitúa contra la superficie interior de una pared de la
arteria;
la Fig. 8 es una ilustración de la retracción
del introductor de catéter desde la vecindad de la arteria;
la Fig. 9 es una ilustración del inflado parcial
de un globo adyacente al extremo delantero del catéter de hemostasis
de tal manera que se impide el flujo de sangre que sale de la
arteria por el globo inflado;
la Fig. 10 es una ilustración del desinflado del
anclaje inflable después de la etapa de la Fig. 9;
la Fig. 11 es una ilustración del inflado
adicional del globo adyacente al extremo delantero del catéter de
hemostasis de tal manera que se retrae el extremo delantero del
catéter de hemostasis y se separa de una superficie exterior de la
pared de la arteria y se impide el flujo de sangre que sale de la
arteria por el globo inflado; y
la Fig. 12 es una ilustración de la inyección de
un agente hemostático después de la etapa ilustrada en la Fig.
11.
Se hace referencia a continuación a las Figs.
1-12, las cuales son ilustraciones simplificadas de
un aparato para la hemostasis posterior a un cateterismo arterial,
de acuerdo con la presente invención que emplea un catéter de
hemostasis novedoso, y de su uso.
El método y el aparato de las Figs.
1-12 emplea un introductor 110 de catéter
convencional que tiene un extremo delantero 111, el cual es
introducido en una arteria 112 de una manera totalmente
convencional, como se conoce bien en la técnica anterior, y se
muestra en la Fig. 1. El cateterismo arterial se realiza también de
una manera totalmente convencional usando un catéter convencional
114, como se muestra en la Fig. 2. Después de retirar el catéter
114, el introductor 110 de catéter convencional permanece en la
arteria 112, como se ilustra en la Fig. 3 y se conoce bien en la
técnica anterior.
En esta etapa, como se muestra en la Fig. 4, un
catéter 120 de hemostasis, construido y operativo según la presente
invención, se introduce en la arteria 112 a través del introductor
110 de catéter. El catéter 120 de hemostasis comprende una funda 122
que tiene un primer y un segundo lúmenes 123 y 124 que tienen un par
de cabezas 125 y 126, asociadas a las válvulas respectivas 128 y
130. Adyacente al extremo delantero 131 del mismo, se proporciona un
globo periférico 132 y extendiéndose hacia delante del extremo
delantero 131 se proporciona un globo de anclaje 134.
Siguiendo a la introducción del catéter 120 de
hemostasis, se infla el globo 134 de anclaje dentro de la arteria
112, como se ilustra en la Fig. 5, típicamente por medio de una
jeringa 140 acoplada a través de la válvula 128, la cabeza 125 y el
lumen 123.
Siguiendo al inflado del globo 134 de anclaje,
se retrae preferiblemente la totalidad del catéter 120 de hemostasis
de tal manera que el globo 134 de anclaje inflado se pone a tope
contra el extremo delantero 111 del introductor 110 de catéter, como
se ve en la Fig. 6.
Se retrae adicionalmente el catéter 120 de
hemostasis, como se muestra en la Fig. 7, retrayendo también así el
introductor 110 de catéter, debido al encaje del globo 134 de
anclaje inflado contra el extremo delantero 111 del introductor 110
de catéter. Esta retracción preferiblemente continúa hasta que el
globo 134 de anclaje inflado se apoya contra una superficie 160 de
una pared 162 de la arteria 112. En esta orientación, el globo 134
de anclaje inflado impide el flujo de sangre a través de la abertura
166 de la pared 162.
Como se ve en la Fig. 8, el introductor 110 de
catéter se retrae ligeramente, como se indica mediante la flecha
164, suficientemente para permitir que se infle el globo periférico
132, y para poner el extremo delantero 110 del introductor de
catéter fuera de encaje con la pared 162 de la arteria, aunque
reteniendo el extremo delantero 110 dentro del tejido y en la
proximidad general de la abertura 166 de la pared 162 de la
arteria.
Siguiendo a la retracción del introductor 11 de
catéter ilustrada en la Fig. 8, se infla parcialmente el globo
periférico 132, como se ilustra en la Fig. 9. Este inflado se
efectúa preferiblemente por medio de una jeringa 170 acoplada a
través de una válvula 130, una cabeza 126 y un lumen 124. El inflado
parcial es preferiblemente tal que se impide el flujo de sangre que
sale de la arteria por el globo 132 inflado.
Después del inflado parcial ilustrado en la Fig.
9, se desinfla el globo 134, como se muestra en la Fig. 10.
Siguiendo el desinflado del globo 134 de anclaje, se infla
adicionalmente el globo periférico 132 de tal manera que se retrae
el extremo delantero 131 del catéter de hemostasis y se separa de la
superficie exterior de la pared de la arteria y se impide el flujo
de sangre que sale de la arteria por el globo 132 inflado.
Una característica particular de la presente
invención es que la colocación del globo periférico 132 con respecto
al extremo delantero 131 del catéter de hemostasis es tal que cuando
el globo 132 está completamente inflado, como se muestra en la Fig.
11, la superficie exterior 168 orientada hacia delante del globo 132
se sitúa por delante del extremo delantero 131 del catéter 120 de
hemostasis, y por tanto, debido al encaje de la superficie 168 con
una superficie exterior 170 de la pared de la arteria 112, retrae el
extremo delantero 131 del catéter de hemostasis sacándolo del encaje
con la pared 162 de la arteria 112, mientras que la superficie
exterior 170 de la pared de la arteria está al mismo tiempo
firmemente encajada por la superficie exterior 168 orientada hacia
delante del globo 132.
La disposición de cierre resultante permite que
se produzca la hemostasis en la abertura 164 formada en la pared 162
de la arteria 112. Siguiendo a una hemostasis suficiente, se
desinfla el globo periférico 132 y se retira el catéter 120 de
hemostasis.
Opcionalmente, como se ilustra en la Fig. 12, se
puede inyectar un agente hemostático 176 a través del introductor
110 de catéter hasta un emplazamiento adyacente a la abertura 174
pero fuera de la arteria 112. Esta inyección puede emplear una
jeringa 178, la cual se puede acoplar a través de un manómetro
118.
Claims (3)
1. Aparato para la hemostasis de una arteria
(112) que tiene una punción después de un cateterismo arterial,
comprendiendo el aparato:
- un catéter (120) de hemostasis que incluye un anclaje inflable (134) en un extremo delantero del mismo y un globo (132) adyacente a dicho extremo delantero y espaciado de dicho anclaje inflable;
- un inflador seleccionable para inflar el anclaje inflable dentro de dicha arteria;
- donde el borde delantero del catéter de hemostasis se extiende hacia delante menos que la extensión del globo cuando es inflado, con lo cual el inflado del globo (132) adyacente al extremo delantero del catéter de hemostasis en encaje con la superficie exterior de la pared de la arteria causa que se retire completamente el extremo delantero del catéter de hemostasis de la pared de la arteria y que se aparte de la misma.
2. Aparato para la hemostasis de una arteria
según cualquiera de las reivindicaciones precedentes y que
comprende también un inyector para inyectar un agente hemostático en
un emplazamiento externo de la arteria.
3. Aparato para la hemostasis según cualquiera
de las reivindicaciones precedentes y donde dicho extremo delantero
se extiende menos de 4 mm más allá del borde delantero del globo
cuando no se encuentra inflado.
