EN LA PRESENTE INVENCION, SE ADAPTA UN INSTRUMENTO ELECTROQUIRURGICO SIN CABLES CON ACOPLAMIENTO CAPACITIVO PARA RECIBIR ENERGIA ELECTROQUIRURGICA DE UN TROCAR O ADAPTADOR DE TROCAR ESPECIALMENTE DISEÑADO. EN UNA REALIZACION DE LA PRESENTE INVENCION, UN INSTRUMENTO ELECTROQUIRURGICO CAPACITIVAMENTE ACOPLADO INCLUYE UNA EMPUÑADURA, UN TUBO ELONGADO Y UN EFECTOR TERMINAL ELECTROQUIRURGICO. Description
Instrumento quirúrgico inalámbrico con
acoplamiento capacitivo.
La Presente invención se refiere, en general, a
un instrumento electroquirúrgico perfeccionado y a un método de uso
y, más en particular, a un instrumento electroquirúrgico
inalámbrico acoplado capacitivamente, que está adaptado para
recibir energía electroquirúrgica desde un trócar electroquirúrgico
especialmente adaptado.
El trócar quirúrgico se ha convertido en el
soporte principal en el desarrollo y aceptación de procedimientos
quirúrgicos endoscópicos. La cirugía endoscopio incluye la
realización de cirugía a través de un número de aberturas que
tienen un diámetro relativamente pequeño. Estas aberturas se hacen
con el trócar, el cual incluye típicamente un obturador de trócar y
una cánula de trócar. El obturador constituye el implemento
perforador que punciona la pared del cuerpo para realizar la
abertura. Una vez que se ha realizado la punción, se retira el
obturador de la cánula. La cánula proporciona entonces un paso de
pequeño diámetro hacia, y a través del cuerpo, para proporcionar un
acceso para instrumentación quirúrgica adicional hasta el lugar
quirúrgico. La función, estructura y operación de un trócar típico,
han sido descritas con detalle en la Patente U.S. núm.
5.387.197.
Tales instrumentos quirúrgicos adicionales pueden
incluir, por ejemplo, instrumentos electroquirúrgicos bipolares o
monopolares, los cuales utilizan energía quirúrgica de
radiofrecuencia. Los instrumentos electroquirúrgicos conocidos
incluyen, por ejemplo, los fórceps bipolares, las tijeras bipolares,
los ganchos monopolares, las tijeras monopolares, y los
endocortadores bipolares. Cada uno de esos instrumentos tiene un
efector electroquirúrgico extremo, el cual está adaptado para
tratar tejido mediante la aplicación de energía electroquirúrgica
(por ejemplo, radiofrecuencia o RF) al tejido, el cual se pone en
contacto con el efector electroquirúrgico extremo. Los instrumentos
electroquirúrgicos más conocidos están conectados por medio de
cables eléctricos a los generadores electroquirúrgicos. La
estructura y actuación de un cortador / grapador mecánico típico,
se encuentran descritas en la Patente U.S. núm. 5.597.107. La
estructura y actuación de un cortador / grapador bipolar típico
("endocortador bipolar"), han sido descritas en la Patente U.S.
núm. 5.403.312, la cual muestra un dispositivo que comprende las
características definidas en la parte
pre-caracterizadora de la reivindicación 1.
Los generadores electroquirúrgicos, tal como el
generador Force II (el cual está disponible en Valleylab de Bolder,
Colorado), proporcionan energía quirúrgica a los instrumentos
electroquirúrgicos mediante cables eléctricos. Los cables
eléctricos, al estar sujetos directamente al instrumento
electroquirúrgico, pueden hacer que el instrumento electroquirúrgico
resulte dificultoso de usar. Alternativamente, los cables
eléctricos pueden provocar retrasos indeseables debido a que un
instrumento electroquirúrgico se encuentre desenchufado del
generador y que se encuentre otro conectado al mismo. De este modo,
resultaría ventajoso diseñar un instrumento electroquirúrgico
inalámbrico, tal como un instrumento electroquirúrgico inalámbrico
en el que la energía electroquirúrgica esté acoplada
capacitivamente al instrumento. Sin embargo, un instrumento
electroquirúrgico inalámbrico de este tipo tendría que ser conectado
al generador electroquirúrgico a través de alguna disposición
alterna. Por lo tanto, también sería ventajoso diseñar un trócar, o
un adaptador de trócar, que esté adaptado para acoplar
capacitivamente energía electroquirúrgica a instrumentos
electroquirúrgicos inalámbricos diseñados especialmente. Además,
sería ventajoso diseñar un instrumento electroquirúrgico inalámbrico
y un trócar electroquirúrgico, o adaptador de trócar, en el que la
energía electroquirúrgica se acopla capacitivamente desde el trócar
electroquirúrgico hasta el instrumento electroquirúrgico
inalámbrico cuando se aplica energía electroquirúrgica al trócar
electroquirúrgico o al adaptador de trócar.
En la presente invención, según se define en la
reivindicación 1, un instrumento electroquirúrgico inalámbrico
acoplado capacitivamente, está adaptado para recibir energía
electroquirúrgica desde un trócar o adaptador de trócar,
especialmente diseñado. El instrumento electroquirúrgico acoplado
capacitivamente incluye un mango, un tubo alargado, y un efector
electroquirúrgico extremo. El mango puede incluir un actuador, tal
como un gatillo, que está conectado operativamente al efector
extremo a través del tubo alargado. El tubo alargado puede ser un
tubo de cierre que se ha adaptado para cerrar el efector extremo
cuando se mueve el actuador de mango. Alternativamente, el tubo de
cierre puede incluir un mecanismo que conecta el actuador de mango
con el efector extremo, cuyo mecanismo actúa para cerrar el efector
extremo cuando se mueve el actuador de mango. El efector
electroquirúrgico extremo está adaptado para aplicar energía
electroquirúrgica a tejido biológico con el fin de efectuar el
tratamiento del tejido. El tubo de cierre alargado incluye una o
más placas de condensador, adaptadas para acoplar energía
electroquirúrgica al efector extremo. Las placas de condensador se
han conectado al efector extremo por medio de uno o más conductores
eléctricos. El tubo alargado está construido en un material
dieléctrico, eléctricamente aislante.
En una realización de la presente invención, el
tubo de cierre alargado está hecho de un material dieléctrico, y al
menos una de las placas de condensador del instrumento, está
situada en, y se extiende a lo largo del, interior del tubo de
cierre alargado. En una realización adicional de la invención,
ambas placas de condensador están dispuestas en, y se extienden
axialmente a lo largo del, interior del tubo alargado. En una
realización adicional de la presente invención, la primera y la
segunda placas de condensador están conectadas eléctricamente a un
primer y un segundo electrodos, ubicados en el efector extremo.
En una realización adicional de la presente
invención, el instrumento electroquirúrgico conforme a la presente
invención está adaptado para acoplarse a través de un trócar que
incluye un adaptador electroquirúrgico con al menos una primera
placa de condensador dispuesta alrededor de, y extendiéndose
axialmente a lo largo de, la abertura alargada, al menos un primer
conductor eléctrico, al menos un primer conductor externo, un
mecanismo de compresión, un alojamiento externo y un cable
eléctrico.
En una realización adicional de la presente
invención, cada una de las placas de condensador del instrumento
electroquirúrgico comprende una placa eléctricamente conductora,
cubierta por una capa de material altamente dieléctrico. En una
realización adicional de la presente invención, la capa de material
altamente dieléctrico comprende, al menos en parte, una porción del
tubo de cierre. El material altamente dieléctrico puede estar
compuesto, al menos en parte, por un material duradero altamente
dieléctrico, tal como el Titilato de Bario (BaTiO_{3}) u otro
material adecuado.

La Figura 1 es una vista en perspectiva de un
trócar 11 electroquirúrgico capacitivo. La Figura 1A es una vista
en perspectiva de un trócar 11 electroquirúrgico capacitivo que
incluye una porción de tubo de cierre 50 del instrumento 16
electroquirúrgico capacitivo de acuerdo con la presente invención.
El trócar 11 electroquirúrgico capacitivo incluye una cánula 8 de
trócar y un adaptador 14 electroquirúrgico capacitivo. El trócar 11

conjunto, el aislador 134 de trócar. El aislador 30 superior y el
aislador 31 inferior están situados contra la pestaña 25 frontal y
la pestaña 24 de base. El miembro 32 de compresión es, en la
presente realización, un anillo en "o" que se sitúa por fuera
del aislador 30 superior y del aislador 31 inferior para empujar el

adaptador 14 electroquirúrgico capacitivo incluye el alojamiento 15

bipolar. El cable eléctrico 18 está conectado eléctricamente al
conductor 36 superior y al conductor 38 inferior. El conductor 36
superior está conectado eléctricamente a la lengüeta 26 de estátor
superior, la cual está conectada eléctricamente a la primera placa
128 estátor de condensador proximal. El conductor 136 conecta
eléctricamente la lengüeta 26 de estátor superior con la lengüeta
27 de estátor inferior, la cual está conectada eléctricamente a la
segunda placa 130 estátor de condensador proximal. El conductor 38
inferior está conectado eléctricamente a la lengüeta 127 de estátor
inferior, la cual está conectada eléctricamente a la segunda placa
131 estátor de condensador distal. El conductor 138 conecta
eléctricamente la lengüeta 127 de estátor inferior con la lengüeta
126 de estátor superior, la cual está conectada eléctricamente con
la primera placa 129 estátor de condensador distal. De este modo, la
energía electroquirúrgica puede ser acoplada desde el enchufe 64
electroquirúrgico bipolar, con cada placa 28 de condensador
proximal y cada placa 29 de condensador distal. La placa 28 de
condensador proximal y la placa 29 de condensador distal, están
situadas en, y aisladas eléctricamente una de otra por, el aislador
134 de trócar. En particular, la primera placa 128 estátor de
condensador proximal y la primera placa 129 estátor de condensador
distal, están situadas en el aislador 30 superior, el cual aísla
también la primera placa 128 estátor de condensador proximal de la
primera placa 129 estátor de condensador distal. Además, se ha
dispuesto una segunda placa 130 estátor de condensador proximal y
una segunda placa 131 estátor de condensador distal, en el aislador
131 inferior, el cual aísla también la segunda placa 130 estátor de
condensador proximal de la segunda placa 131 estátor de condensador
distal. El miembro 32 de compresión rodea el aislador 30 superior y
el aislador 31 inferior. La primera región 151 dieléctrica proximal
comprende la porción de aislador 30 superior dispuesta entre la
primera placa 128 estátor de condensador proximal, y la abertura 19
central. La segunda región 152 dieléctrica proximal comprende la
porción de aislador 31 inferior situada entre la segunda placa 130

La Figura 4 es una vista en sección del adaptador
14 electroquirúrgico capacitivo, tomada a lo largo de la línea
4-4 de la Figura 2. La Figura 4A es una vista en
sección del adaptador 14 electroquirúrgico capacitivo, tomada a lo
largo de la línea 4A-4A de la Figura 2A. Haciendo
ahora referencia a las Figuras 2-4, y en particular
a las Figuras 4 y 4A, la abertura 19 central está definida por la
abertura de la pared 21 interior. La porción de pared 21 interior
abierta, visible en la Figura 4, está formada, al menos en parte,
por una primera superficie 60 aisladora del aislador 30 superior, y
por la superficie 61 aisladora del aislador 31 inferior. El miembro
32 de compresión, el cual comprende dos anillos en "o" en la
realización de las Figuras 2-4, empuja el aislador
30 superior y el aislador 31 inferior hacia el centro de la abertura

conductor 136. El cable eléctrico 18 está conectado a la segunda
placa 131 estátor de condensador distal de la placa 29 de
condensador distal por medio del conductor 38 inferior y de la
lengüeta 27 de estátor inferior. Según se ha ilustrado, en
particular, en las Figuras 2 y 3, la lengüeta 126 de estátor
superior está conectada a la lengüeta 127 de estátor inferior por
medio del conductor 138. La pestaña 24 de base y la pestaña 25
frontal, las cuales son parte del alojamiento 15 de adaptador,
mantienen el aislador 30 superior y el aislador 31 inferior en su
lugar, posicionando de este modo la placa 28 de condensador
proximal y la placa 29 de condensador distal alrededor de la
abertura 19 central. El protector contra tirones 23, protege el
cable eléctrico 18 según pasa a través del alojamiento 15 de
adaptador. Aunque la placa 28 de condensador proximal ha sido
ilustrada de forma visible en las Figuras 4 y 4A, es evidente que
la placa 28 de condensador proximal se ha representado de forma
visible por motivos de conveniencia para la descripción del trócar,
y realmente estará oculta.
La Figura 5 es una vista en perspectiva de un
instrumento 16 electroquirúrgico inalámbrico capacitivo que puede
ser, por ejemplo, un cortador / grapador bipolar. En la Figura 5,
el instrumento 16 electroquirúgico capacitivo incluye el mango 72,
el tubo 50 de cierre, y el efector 57 extremo, el cual, en la
realización de la invención que se ilustra en la Figura 4, es un
cortador / grapador bipolar. Los efectores extremos
electroquirúrgicos alternativos pueden incluir: un fórceps bipolar,
tal como el fórceps ilustrado en el documento 5.540.684, un
instrumento bipolar de corte y coagulación, tal como el fórceps de
corte de tejido ilustrado en la Patente U.S. núm. 5.445.638; unas
tijeras bipolares, tal como las cizallas ilustradas en la patente
U.S. núm. 5.352.222; o una sonda bipolar tal como la sonda
ilustrada en la Patente U.S. núm. 5.342.357. El tubo 50 de cierre
es alargado para facilitar la inserción del efector 57 extremo a
través de la cánula de trócar, facilitando con ello el uso del
instrumento 16 electroquirúrgico capacitivo en procedimientos
quirúrgicos endoscópicos o laparoscópicos. El tubo 50 de cierre
puede ser de cualquier forma apropiada, incluyendo, por ejemplo, un
tubo alargado cuadrado o triangular. El mango 72, que está situado
en el extremo proximal del instrumento 16 electroquirúrgico
capacitivo, incluye el gatillo 74 de agarre manual, el gatillo 76
disparador, y el gatillo 78 liberador. El tubo 50 de cierre, el cual
conecta el mango 72 con el efector 57 extremo, incluye el botón 70
de giro. El efector 57 extremo, el cual está situado en el extremo
distal del tubo 50 de cierre, incluye el yunque 58, el canal 88
para cartucho, y el cartucho 68 de grapas. El instrumento 16
electroquirúrgico capacitivo es similar, en cuanto a estructura y
funcionamiento, al instrumento de corte y grapado lineal de
electrocauterización endoscópico bipolar ilustrado y descrito en la
Patente U.S. núm. 5.403.312. Sin embargo, el instrumento 16
electroquirúrgico capacitivo es inalámbrico, y la energía
electroquirúrgica está acoplada capacitivamente al instrumento 16
electroquirúrgico. En el elemento 16 electroquirúrgico capacitivo,
la energía electroquirúrgica se alimenta al efector 57 extremo del
instrumento 16 a través de una o más placas capacitivas, las cuales
pueden estar ubicadas en el tubo 50 de cierre.
La Figura 5 es una vista en perspectiva,
despiezada, del extremo distal de un instrumento electroquirúrgico
de acuerdo con la presente invención, tal como el instrumento 16
electroquirúrgico. En los instrumentos ilustrados en las Figuras 4
y 5, el tubo 50 de cierre es eléctricamente aislante, y puede estar
construido, con preferencia, con un material que tenga una alta
constante dieléctrica. El contacto 42 de placa de condensador
proximal, está situado en el tubo 50 exterior, y está conectado
eléctricamente al primer conductor 48 de electrodo. El aislador 44
del tubo de cierre separa la placa 42 de condensador proximal de
instrumento, de la placa 43 de condensador distal de instrumento. La
placa 43 de condensador distal de instrumento se encuentra situada
en el tubo 50 exterior, y está conectada eléctricamente al canal
88, el cual contiene el cartucho 68 de grapas. La varilla 84 de
disparo pasa a través del tubo 50 externo, y está conectada
mecánicamente al conjunto 82 de bloque en forma de cuña. En un
instrumento electroquirúrgico alternativo, la placa 43 de
condensador distal de instrumento podría estar conectada
eléctricamente a un segundo electrodo, o electrodo de retorno (no
representado), en el efector 57 extremo, por medio de un cable
aislado (no representado), que discurre a través del tubo 50 de
cierre y, en un instrumento de este tipo, el canal 88 puede estar
construido con un material aislante. En la Figura 5, el conductor 48
pasa a través del tubo 50 de cierre desde la placa 42 de
condensador proximal del instrumento hasta el conjunto 52 de
electrodo, conectando eléctricamente la placa 42 de condensador
proximal de instrumento con el conjunto 52 de electrodo. El
conjunto 52 de electrodo está situado en el yunque 58. El conjunto
52 de electrodo puede estar aislado eléctricamente del yunque 58 y
del tubo 50 de cierre, para evitar que el conjunto 52 de electrodo
entre en cortocircuito con el yunque 58 o con el tubo 50 de cierre.
El conductor 48 puede estar aislado con el fin de impedir que entre
en cortocircuito con el tubo 50 de cierre o con cualquiera de los
mecanismos del tubo 50 de cierre.
La Figura 6 es una vista en corte del instrumento
16 electroquirúrgico ilustrado en la Figura 4. En la Figura 6, el
mango 72 incluye el asidero 62, el gatillo 74 de agarre manual, y
el gatillo 76 de disparo. El movimiento pivotante del gatillo 74 de
agarre manual da como resultado el movimiento distal del yugo 63 y
del tubo 50 de cierre, cerrando el yunque 58 contra el cartucho 68
de grapas que está situado en el canal 88 de cartucho. El
movimiento pivotante del gatillo 74 de agarre manual, libera además
la varilla 84 de disparo, y posiciona el gatillo 76 de disparo para
enganchar el miembro 67 actuador. El movimiento pivotante adicional
del gatillo 76 de disparo hacia el asidero 62, da como resultado el
movimiento distal del miembro actuador 67, el cual hace girar el
multiplicador 66 en dirección contraria a las agujas del reloj. La
rotación en contra de las agujas del reloj del multiplicador 66, da
como resultado el movimiento distal de la varilla 84 de disparo, la
cual, en la realización de la invención que aquí se ilustra,
dispara las grapas del cartucho 68 de grapas.
La Figura 8A es una vista en corte de un efector
57 de instrumento 16 electroquirúrgico inalámbrico capacitivo. La
Figura 8B es una vista en corte de una porción de tubo 50 de cierre
del instrumento 16 electroquirúrgico inalámbrico capacitivo. La
Figura 9 es una vista en corte de una realización alternativa de
una porción de tubo 50 de cierre del instrumento 16
electroquirúrgico inalámbrico capacitivo. En la realización del
instrumento 16 electroquirúrgico que se ha ilustrado en las Figuras
8A, 8B y 9, la base de yunque 73 del yunque 58, soporta el conjunto
52 de electrodo, e incluye la guía de yunque 65 y ranuras (no
representadas) de formación de grapas. El conjunto 52 de electrodo
está acoplado eléctricamente al primer conductor 48 de electrodo y
a los electrodos 55 de yunque. La base 73 de yunque está aislada del
conjunto 52 de electrodo por medio del aislador 59 de yunque. El
primer conductor 48 de electrodo está conectado eléctricamente a la
placa 42 de condensador proximal de instrumento. La placa 42 de
condensador proximal de instrumento está situada en la porción
proximal del tubo 50 de cierre. El canal 88 del efector 57 extremo
soporta el cartucho 68 de grapas, la guía 80 de cuña, y el conjunto
82 de bloque en forma de cuña. El canal 88 construido con material
eléctricamente conductor, se extiende por, y está acoplado
eléctricamente a, la placa 43 de condensador distal de instrumento,
la cual está situada en la porción distal del tubo 50 de cierre. De
este modo, el canal 88 puede proporcionar una trayectoria de retorno
para la energía eléctrica acoplada a los electrodos 55 de yunque
del efector 57 extremo cuando se utiliza el efector 57 extremo para
agarrar el tejido u otro material eléctricamente conductor, y ese
material eléctricamente conductor toca tanto el canal 88 como los
electrodos 55 de yunque. La energía electroquirúrgica acoplada al
canal 88, puede ser acoplada de nuevo al trócar 11 electroquirúrgico
a través de la placa 43 de condensador distal de instrumento. La
placa 42 de condensador proximal de instrumento está aislada
eléctricamente de la placa 43 de condensador distal de instrumento
por medio del aislador 44 del tubo de cierre. El tubo 50 de cierre
también soporta y encierra el extremo proximal del yunque 58, el
extremo proximal del canal 88, la varilla 84 de disparo, el extremo
proximal de la cuchilla 90, el retenedor 86 de canal, y al menos una
porción del conjunto 82 de bloque en forma de cuña y la guía 80 de
cuña. El tubo 50 de cierre puede estar construido, con preferencia,
con un material aislante duradero altamente dieléctrico, tal como,
por ejemplo, Titilato de Bario (BaTiO_{3}). El yunque 58 abre y
cierra mediante, por ejemplo, pivotamiento alrededor de uno o más
pernos 85 de pivotamiento. En la realización ilustrada en la Figura
9, el inductor 49 de adaptación puede ser utilizado para
perfeccionar la eficacia de la transferencia de energía al tejido
agarrado por el efector 57 extremo. La cuchilla 90 está conectada
al conjunto 82 en forma de cuña, y el conjunto 82 en forma de cuña
se ha conectado a la varilla 84 de disparo, la cual, a su vez, está
conectada operativamente al gatillo 76 de disparo. El tubo 50 de
cierre está conectado operativamente al botón 70 de rotación, al
gatillo 74 de agarre manual, y al gatillo 78 de liberación. La guía
80 de cuña se encuentra acoplada al conjunto 82 de bloque en forma
de cuña para guiar el conjunto 82 de bloque en forma de cuña según
mueve la varilla 84 de disparo al conjunto 82 de bloque en forma de
cuña. El canal 88 está acoplado al, y se mantiene en su lugar por
medio del, extremo distal del retenedor 86 de canal. La estructura
y operación de las características mecánicas del instrumento 16
electroquirúrgico, pueden ser mejor comprendidas con referencia al
instrumento mecánico de corte y grapado que se encuentra ilustrado
y descrito en la Patente U.S. núm. 5.597.107.
La Figura 10 es un diagrama esquemático que
ilustra gráficamente el acoplamiento capacitivo entre el trócar 11
electroquirúrgico capacitivo y el instrumento 16 electroquirúrgico
capacitivo. En la Figura 10, el condensador 142 proximal comprende
la placa 28 de condensador proximal, el aislador 134 de trócar, el
tubo 50 de cierre, y la placa 42 de condensador proximal de
instrumento. Más en particular, el condensador 142 proximal
comprende una primera placa 128 estátor de condensador proximal,
una primera zona 151 dieléctrica proximal, una segunda placa 130
estátor de condensador proximal, una segunda zona 152 dieléctrica
proximal, una porción del extremo proximal del tubo 50 de cierre, y
la placa 42 de condensador proximal de instrumento. El condensador
143 distal comprende la placa 29 de condensador distal, el aislador
134 de trócar, el tubo 50 de cierre y la placa 43 de condensador
distal de instrumento. Más en particular, el condensador 143 distal
comprende una primera placa 129 estátor de condensador distal, una
primera zona 153 dieléctrica distal, una segunda placa 131 de
condensador distal, una segunda zona 154 dieléctrica distal, una
porción del extremo distal del tubo 50 de cierre, y la placa 43 de
condensador distal de instrumento.
En las Figuras 10 y 11, la primera salida del
generador 5 electroquirúrgico se ha conectado a la placa 28 de
condensador proximal del condensador proximal 142 a través del
cable 18 y del conductor 36 superior. La segunda salida 7 del
generador 5 electroquirúrgico se ha conectado a la placa 29 de
condensador distal del condensador 143 distal a través del cable 18
y del conductor 38 inferior. Cuando el efector 57 extremo está
cerrado alrededor de material eléctricamente conductor, tal como
tejido biológico, el circuito eléctrico de la placa 42 de
condensador proximal de instrumento del condensador 142 proximal con
la placa 43 de condensador distal de instrumento del condensador
143, está completo. De este modo, con el efector 57 extremo cerrado
alrededor del material conductor y con el generador 5
electroquirúrgico encendido, la energía electroquirúrgica, tal como
corriente eléctrica a una potencia y una frecuencia de salida
predeterminadas, pasa desde el generador 5 electroquirúrgico, a
través del condensador 142 proximal, hasta el efector 57 extremo, y
vuelve a través del condensador 143 distal, y de nuevo hasta la
segunda salida 7 del generador 5 electroquirúrgico.
Según ilustran esquemáticamente las Figuras 10 y
11, la placa 42 de condensador proximal de instrumento y la placa
43 de condensador distal de instrumento, son alargadas, de modo que
el movimiento del instrumento 16 electroquirúrgico no da como
resultado la pérdida de acoplamiento capacitivo en los
condensadores 142 y 143. De este modo, incluso según se mueve el
instrumento por el interior del trócar 11 para facilitar el
tratamiento del paciente, se puede mantener el acoplamiento
capacitivo. El circuito ilustrado en la Figura 11 incluye un
inductor 49 de adaptación que puede ser utilizado para adaptar
eléctricamente el instrumento 16 electroquirúrgico capacitivo al
trócar 11 electroquirúrgico capacitivo con el fin de incrementar la
potencia acoplada al tejido agarrado por el efector 57 extremo. En
particular, el inductor 49 podría ser elegido de modo que la carga
representada por el trócar, el instrumento y el tejido, se presente
como sustancialmente resistiva a la frecuencia que interesa.
En operación, la cánula 8 de trócar se utiliza
con un orbitor de trócar convencional (no representado), para
penetrar la pared de la cavidad de un cuerpo, tal como, por
ejemplo, la pared abdominal de un ser humano. Una vez que la pared
del cuerpo ha sido penetrada, el conjunto obturador se retira de la
cánula 8 de trócar, y la cánula se utiliza como portal de acceso
para el paso de diversos instrumentos endoscópicos, con el fin de
proporcionar, por ejemplo, acceso a los órganos internos de un ser
humano. Cuando el instrumento endoscópico que se va a utilizar, es
un instrumento electroquirúrgico capacitivo inalámbrico como el
instrumento 16 electroquirúrgico, el adaptador 14 electroquirúrgico
capacitivo puede ser sujetado a la cánula 8 de trócar. Una vez que
se ha sujetado el adaptador 14 electroquirúrgico capacitivo a la
cánula 8 de trócar, y el cable 18 eléctrico se ha unido a un
generador electroquirúrgico adecuado (tal como el generador 5 de la
Figura 10), se puede usar el trócar 11 electroquirúrgico capacitivo
para proporcionar energía electroquirúrgica a instrumentos
electroquirúrgicos capacitivos inalámbricos tales como el
instrumento 16 electroquirúrgico. Cuando un instrumento
electroquirúrgico capacitivo inalámbrico, tal como el instrumento
16 electroquirúrgico, se inserta en una cavidad corporal a través
de, por ejemplo, el trócar 11 electroquirúrgico capacitivo, el
efector 57 extremo pasa a través de la cánula 8 de trócar, y a
través de la cavidad, mientras la mayor parte del tubo 50 de cierre
permanece en el trócar 11. El mango 72, que está fuera del trócar
11 electroquirúrgico capacitivo, puede ser manipulado por el
cirujano para controlar la posición del efector 57 extremo.
Un instrumento electroquirúrgico bipolar
capacitivo inalámbrico de acuerdo con la presente invención, tal
como el instrumento 16 electroquirúrgico de la Figura 5, puede ser
utilizado insertando el instrumento inalámbrico en un trócar
electroquirúrgico capacitivo apropiado, tal como el trócar
electroquirúrgico ilustrado en la Figura 1. En el trócar
electroquirúrgico capacitivo que se ha ilustrado en la Figura 1, la
energía electroquirúrgica se suministra al instrumento 16 mediante,
por ejemplo, el acoplamiento capacitivo entre la placa 28 de
condensador proximal del trócar 11, y la placa 42 de condensador
proximal de instrumento del instrumento 16. Se ha previsto una
trayectoria de retorno eléctrico mediante, por ejemplo, el
acoplamiento capacitivo entre la placa 29 de condensador distal del
trócar 11 y la placa 43 de condensador distal de instrumento del
instrumento 16. El diámetro de la abertura 19 central corresponde,
en general, al diámetro externo del tubo 50 de cierre, de modo que
el tubo 50 de cierre desliza a través de la abertura 19 central y
por el interior del tubo 10 de cánula. El acoplamiento capacitivo
eficaz podrá ser mantenido mientras al menos una porción de las
placas 42 y 43 de condensador estén situadas en la abertura 19
central, opuestas a al menos una porción de las placas 28 y 29 de
condensador, para formar condensadores 142 y 143. El aislador
superior 30 y el aislador inferior 31, forman el aislador 134 de
trócar. El tubo 50 de cierre y el aislador 134 de trócar, al estar
preferentemente formados por un material que tiene una constante
dieléctrica, actúan como dieléctrico para el condensador 142
proximal y para el condensador 143 distal, los cuales se han
ilustrado esquemáticamente en la Figura 10. El miembro 32 de
compresión ayuda a asegurar que el aislador 134 de trócar y el tubo
50 de cierre mantienen un buen contacto físico, minimizando
cualquier separación entre las placas de condensador proximal 142 y
las placas de condensador distal 143. El acoplamiento eléctrico
capacitivo puede ser incrementado con la utilización de múltiples
condensadores en el trócar 11 electroquirúrgico capacitivo. Con las
placas 42 y 43 de condensador de instrumento situadas en posiciones
opuestas a las placas 28 y 29 de condensador, se puede suministrar
energía electroquirúrgica al instrumento 16 a través del cable
eléctrico 18 y del trócar 11 electroquirúrgico capacitivo. La
energía electroquirúrgica suministrada al trócar 11 por medio del
cable 18, pasa a través de los conductores 36, 38, 136 y 138 hasta
las lengüetas 26, 127, 27 y 127 de estátor, y de las placas 28 y 29
de condensador hacia el instrumento 16 electroquirúrgico por medio
de las placas 42 y 43 de condensador de instrumento. La energía
electroquirúrgica suministrada al instrumento 16 electroquirúrgico
por medio de las placas 42 y 43 de condensador de instrumento,
puede ser alimentada al efector 57 extremo por medio del circuito
formado por la placa 42 de condensador proximal de instrumento, el
conductor 48, el conjunto 52 de electrodo, el canal 88 de cartucho,
y la placa 43 de condensador distal de instrumento. Este circuito
está completo cuando el tejido biológico, u otro material
conductor, es agarrado por el efector 57 extremo, proporcionando una
trayectoria desde el conjunto 52 de electrodo hasta el canal 88 de
cartucho. En el instrumento 16 electroquirúrgico, el canal 88 de
cartucho y el electrodo 55 de yunque son eléctricamente
conductores. De este modo, cuando el conjunto 52 de electrodo actúa
como electrodo primario, el canal 88 de cartucho actúa como
electrodo secundario o de retorno. Cuando el tejido eléctricamente
conductor es agarrado por el efector 57 extremo, tocando tanto el
conjunto 52 de electrodo como el canal 88 de cartucho, y se conecta
un generador electroquirúrgico a la primera placa 42 de condensador
proximal de instrumento y a la segunda placa 43 de condensador
distal de instrumento, fluirá energía electroquirúrgica a través
del tejido agarrado, coagulando el tejido agarrado siempre que el
instrumento 16 electroquirúrgico capacitivo esté situado en el
trócar 11 de la manera que aquí se describe. También puede resultar
ventajoso proporcionar uno o más conmutadores (no representados)
para controlar el flujo de corriente eléctrica hasta el trócar 11 o
hasta el efector 57 extremo del instrumento 16.
Mientras que se han mostrado y descrito en lo que
antecede realizaciones preferidas de la presente invención,
resultará obvio para los expertos en la materia que tales
realizaciones se proporcionan solamente a título de ejemplo. Los
expertos en la materia podrán concebir numerosas variaciones,
cambios y sustituciones sin apartarse por ello de la invención. En
consecuencia, se prevé que la invención esté limitada solamente por
el alcance de las reivindicaciones anexas.
Claims (10)
1. Un instrumento (16) electroquirúrgico, en el
que dicho instrumento (16) electroquirúrgico comprende:
a) un mango (72);
b) un efector (57) extremo, conectado
operativamente a dicho mango (72), y
c) un tubo (50) alargado que conecta dicho
efector (57) extremo con el citado mango (72);
caracterizado porque el instrumento (16)
es inalámbrico, y comprende además:
d) una primera placa (42) de condensador,
conectada eléctricamente a dicho efector (57) extremo.
2. El instrumento (16) electroquirúrgico de la
reivindicación 1, que comprende además una segunda placa (43) de
condensador posicionada en el citado tubo (50) alargado, en el que
dicha segunda placa (43) de condensador está conectada
eléctricamente con el citado efector (57) extremo.
3. El instrumento (16) electroquirúrgico de la
reivindicación 1 o la reivindicación 2, en el que dicho instrumento
(16) electroquirúrgico es endoscópico; el efector (57) extremo
incluye, al menos, un primer electrodo; y la primera placa (42) de
condensador está situada en el citado tubo (50) alargado, en el que
dicha primera placa (42) de condensador está conectada
eléctricamente con el citado primer electrodo por medio de un
conductor eléctrico, estando posicionado dicho conductor eléctrico,
al menos parcialmente, en el interior de dicho tubo (50)
alargado.
4. El instrumento (16) electroquirúrgico de
acuerdo con la reivindicación 3, que comprende además:
a) una segunda placa (43) de condensador,
posicionada en el citado tubo (50) alargado, y
b) un segundo electrodo en el citado efector (57)
extremo, en el que dicha segunda placa (43) de condensador está
conectada eléctricamente a dicho segundo electrodo.
5. El instrumento (16) electroquirúrgico de la
reivindicación 1, incluyendo dicho mango (72) un actuador;
incluyendo dicho efector (57) extremo un primer y un segundo
electrodos, en el que dicho efector (57) extremo comprende un
primer elemento de agarre manual y un segundo elemento de agarre
manual; dicho tubo (50) alargado incluye un mecanismo que conecta
operativamente el citado efector (57) extremo con el citado
actuador; estando la citada primera placa (42) de condensador
posicionada al menos parcialmente en dicho tubo, y extendiéndose a
lo largo de al menos una porción de dicho tubo, en el que dicha
primera placa (42) de condensador está conectada eléctricamente a
dicho primer electrodo por medio de un cable aislado, situado en el
citado tubo (50) alargado; y estando una segunda placa (43) de
condensador situada en, y extendiéndose a lo largo de, dicho tubo
distal hasta la citada primera placa (42) de condensador, en el que
dicha segunda placa (43) de condensador está conectada
eléctricamente al citado segundo electrodo.
6. El instrumento (16) quirúrgico de la
reivindicación 1, estando destinado el citado mango (72) a sujetar
a dicho instrumento (16); destinándose dicho efector (57) extremo
para tratar el tejido; estando destinado el citado tubo (50)
alargado a conectar dicho medio de efector (57) extremo con el
citado medio de mango (72); y estando la citada placa de
condensador destinada a acoplar capacitivamente energía
electroquirúrgica, a través de dicho medio de tubo (50) alargado,
con el citado instrumento (16).
7. El instrumento (16) electroquirúrgico de la
reivindicación 6, que comprende además una segunda placa (43) de
condensador para acoplar energía electroquirúrgica a través de
dicho tubo (50) alargado, en el que dicha placa (43) de condensador
está conectada eléctricamente con el citado efector (57)
extremo.
8. El instrumento (16) electroquirúrgico de la
reivindicación 1, siendo dicho instrumento (16) endoscópico,
estando destinado el citado mango (72) para sujetar el citado
instrumento (16); estando el citado efector (57) extremo destinado
a tratar tejido, e incluyendo al menos un primer medio de electrodo
para transmitir energía eléctrica al tejido contactado por dicho
efector (57) extremo; estando el citado tubo (50) extremo destinado
a conectar dicho efector (57) extremo con dicho mango (72); estando
la citada primera placa (42) de condensador destinada a acoplar
capacitivamente energía eléctrica con dicho primer electrodo, en el
que dicha primera placa (42) de condensador está posicionada en el
citado tubo (50) alargado; y comprendiendo un medio conductor
eléctrico, para conducir electricidad desde la citada primera placa
(42) de condensador hasta el citado medio de electrodo, estando
posicionado dicho conductor eléctrico, al menos parcialmente, en el
interior del citado tubo (50) alargado.
9. El instrumento (16) electroquirúrgico
electroscópico de la reivindicación 8, que comprende además una
segunda placa (43) de condensador para acoplar capacitivamente
energía eléctrica con el segundo electrodo, en el que dicha segunda
placa (43) de condensador está situada en el citado tubo (50)
alargado.
10. El instrumento (16) electroquirúrgico de la
reivindicación 1, destinándose dicho mango (72) a sujetar el
instrumento (16);
que comprende:
un medio actuador sujeto a dicho mango (72), para
abrir y cerrar el citado efector (57) extremo;
sirviendo dicho efector (57) extremo para
manipular tejido, incluyendo dicho efector (57) extremo un primer y
un segundo electrodos;
siendo dicho tubo un tubo dieléctrico alargado
para conectar operativamente dicho mango (72) con el citado efector
(57) extremo, e incluyendo un mecanismo que conecta operativamente
el citado efector (57) extremo con el citado medio actuador;
estando la citada primera placa (42) de
condensador situada, al menos parcialmente, en el citado tubo (50)
alargado, en el que dicha primera placa (42) de condensador está
conectada eléctricamente a dicho primer electrodo por medio de un
cable aislado posicionado en el citado tubo (50) alargado, y
comprendiendo además una segunda placa (43) de
condensador posicionada en, y extendiéndose a lo largo de, dicho
tubo (50) alargado, en el que dicha segunda placa (43) de
condensador está conectada eléctricamente con el citado segundo
electrodo.
