Description
Biomatriz para la regeneración de tejidos.
En el contexto de la reparación de tejido
esquelético, la terapia de regeneración de tejido es la aplicación
local de células autólogas (derivadas del huésped) para fomentar la
reconstrucción de los defectos del tejido causados por trauma,
enfermedad o intervenciones quirúrgicas. El objetivo del enfoque de
la terapia de regeneración del tejido es entregar altas densidades
de células competentes en la reparación (o células que pueden
volverse competentes cuando son influidas por el entorno local) al
sitio del defecto en un formato que optimiza tanto la mecánica de la
herida inicial como la producción eventual de neotejido. Para la
reparación de tejido blando, es probable que se necesite uno o más
vehículos de implante para 1) transportar y constreñir las células
autólogas en el sitio del defecto y 2) proporcionar estabilidad
mecánica inicial al sitio quirúrgico. En un sistema óptimo es
probable que el vehículo se degrade lentamente a una tasa comparable
con la producción de neotejido y el desarrollo de resistencia en el
tejido reparador (1).
El enfoque de la terapia de regeneración del
tejido contrasta de manera significativa con los enfoques más
pasivos para reparar heridas en los que no se intenta entregar ni
reclutar células reparadoras en el sitio del defecto. Por ejemplo,
en el caso de la reparación del ligamento cruzado anterior (ACL) con
injertos polímeros sintéticos (presumiblemente "inertes"), el
proceso de curación depende totalmente de las respuestas celulares
locales para iniciar y controlar la incorporación de un implante
permanente (2).
Recientemente, se ha ensayado dispositivos más
activos usando andamios de matriz diseñados para entregar y/o
dirigir procesos celulares. Estos últimos han influido, por ejemplo,
la reparación del tendón o ACL (3-7), reparación del
menisco (8-11) y reparación del cartílago articular
(12-15). Alternativamente, se ha estudiado también
el uso de factores peptídicos administrados localmente, destinados a
estimular el reclutamiento de células reparadoras y su fijación y/o
diferenciación (16-19).
En los, probablemente, mejor documentados
experimentos actuales de reparación del tendón, Silver, Dunn y sus
colegas, han descrito investigaciones extensivas del rendimiento de
las prótesis de fibra de colágeno para la reparación del tendón de
Aquiles (3-5) y ligamento cruzado anterior (ACL)
(6,7) en conejos. Informan de que a las 52 semanas después del
implante en el defecto del tendón de Aquiles, el tendón reconstruido
(prótesis + tejido de reparación) era aproximadamente un 66% tan
resistente como el tejido normal de todos los implantes ensayados,
incluido un injerto de tendón autólogo y composites de fibra de
colágeno reticulados con glutaraldehido o carbodiimida (5). Se
observó que tanto los implantes autólogos como las prótesis
reticuladas con carbodiimida se biodegradaban rápidamente,
recuperando entonces rápidamente resistencia al producirse nuevo
tejido. Las prótesis reticuladas con glutaraldehido se biodegradaron
mucho más lentamente en el modelo de tendón de Aquiles y quedaron
rodeadas con una gruesa cápsula que finalmente interrumpió el
proceso de degradación. Aunque el neotendón desarrollado en estos
estudios fue similar al normal, no era idéntico. Por ejemplo, el
ángulo de engarce del colágeno del neotendón era similar al tendón
normal en todos los implantes, pero la longitud del engarce de
neotendón era menor que aproximadamente el 30% del normal para los
dispositivos protésicos de colágeno. Además, los módulos de los
neotendones formados a partir de los implantes de degradación más
rápida (tendón autólogo y fibras de colágeno reticuladas con
carbodiimida) eran notablemente menores que para el tendón normal.
Finalmente, el neotendón observado no se ensambló con la
microarquitectura fascicular del tendón normal. Estos investigadores
llegaron a la conclusión de que la tasa de degradación de la
prótesis, y la consiguiente transferencia de carga al nuevo tejido,
puede ser tan importante como la resistencia a la tracción inicial
de la prótesis para determinar las propiedades finales del tejido de
reparación (5). Se observó una generación similar de neoligamento en
los implantes ACL al cabo de 20 semanas, aunque la recuperación de
resistencia del tejido puede ser algo más lenta en el entorno
sinovial avascular (7).
Basado en esta evidencia, está claro que por lo
menos en el animal sano, se puede reclutar células competentes en la
reparación a partir de los tejidos que rodean a defectos en tendones
y ligamentos, y que estas células iniciaran la producción de
neotejido. Estos investigadores no aclaran bien en qué grado las
células reclutadas representaron fenotipos diferenciados (por
ejemplo, fibroblastos del tendón), en oposición a las células cepa
pluripotentes no diferenciadas, ni si números incrementados de tales
células mejorarían la tasa de síntesis o la microarquitectura y las
propiedades mecánicas del neotejido producido.
Muchos procesos mediados por células que están
relacionados con la producción de tejido esquelético dependen del
número de células implicadas, tanto en la tasa como en la magnitud
del efecto. Por ejemplo, en la producción in vitro de tejido
conectivo, la tasa de contracción del gel de colágeno por
fibroblastos embebidos en el gel es dependiente del número de
células presentes en el cultivo (20). Una actividad de contracción
de gel similar ha sido también correlacionada con la secreción
dependiente de la densidad celular de un factor de promoción de
contracción por células endoteliales (21). Además, el grado de
orientación de los fibroblastos en cultivos realizados con geles de
colágeno está directamente relacionado con la densidad celular
inicial (22). Este efecto de orientación celular ha sido
correlacionado con la observación de "centros de organización"
en el cultivo, cuyo número se ha sugerido que es un indicador
directo de la capacidad morfogenética a los niveles molecular y
celular (23).
La diferenciación dependiente de la densidad
celular fue demostrada claramente en el cultivo de células de yema
de la pata del pollito (24). Cuando se cultivó a una densidad muy
baja (10^{6} células/platillo 35 mm), estas células no presentan
propiedades condrogénicas ni osteogénicas. A densidades
"intermedias" de cultivo celular (2 x 10^{6} células/platillo
35 mm), las células presentaron la máxima frecuencia de
osteogénesis, mientras que a una densidad todavía más alta (5 x
10^{6} células) se observó la máxima frecuencia de fenotipo
condrocitos.
En cada caso citado anteriormente, el número de
células inicialmente presente influye fuertemente en la naturaleza
de los procesos mediados por células que están implicados en la
formación del tejido esquelético, y la tasa a la que ocurren estos
procesos de desarrollo y fisiológicos. Por consiguiente, en los
procesos reparadores de los tejidos esqueléticos, Caplan y
colaboradores han planteado la hipótesis de que puede requerirse
cierto umbral mínimo de número celular en el sitio de reparación
antes de que pueda tener lugar la formación de neotejido
"normal" (25). Igualmente, en muchos casos, este umbral mínimo
puede rebasar el número de células reparadoras reclutables,
incluyendo menos células comprometidas que pueden diferenciar para
reparar fenotipos competentes; por lo tanto, el grado al que puede
ocurrir el proceso reparador puede limitarse por este único
parámetro.
Recientemente se han llevado a cabo
investigaciones preliminares del enfoque de la terapia de
regeneración de tejidos en un modelo de reparación de tendón en el
tendón de Aquiles del conejo (25). Había tres componentes en este
modelo: el defecto, las células y el vehículo para entregar las
células al sitio del defecto. El vehículo de entrega en este modelo
debe retener las células en el sitio del defecto, estabilizar la
mecánica del tejido, y luego biodegradarse lentamente a medida que
se produce el tejido nuevo.
US 5.061.283 describe un dispositivo apropiado
para usar en la reparación de un tendón lacerado o seccionado,
particularmente, un tendón flexor de la mano, que tiene un cuerpo de
banda plana con extremos opuestos del cuerpo destinados a anclar
suturas de conexión. Dicho dispositivo comprende una porción de
cuerpo alargada que tiene una estructura de banda plana, estando
adaptada la porción de cuerpo en extremos opuestos para conectarse a
por lo menos una sutura portadora de aguja. El cuerpo del
dispositivo y la sutura o suturas asociadas pueden cubrirse con uno
o más recubrimientos de gel tales como, por ejemplo, alginato
cálcico. Para reparar un tendón seccionado usando el dispositivo de
acuerdo con US 5.061.283 se han formado incisiones en cada extremo
del tendón seccionado, cuyas incisiones sirven para recibir parte
del dispositivo, respectivamente, que se sujeta entonces mediante
suturación del dispositivo en el tendón. US 5.061.283 no menciona
células vivas como parte de/o para producir el dispositivo
descrito.
US 5.226.914 en general describe varios procesos
y dispositivos para utilizar células mesenquimales derivadas de
médula aisladas y expandidas por cultivo (células cepa
mesenquimales) para tratar trastornos esqueléticos y otros
trastornos del tejido conectivo. Se expone en dicho documento que
las células cepa mesenquimales antes mencionadas puede incluirse
bajo ciertas condiciones para diferenciarse en tipos diferentes de
tejido conectivo deseado tales como células formadoras del hueso o
cartílago. Los factores que inducen diferenciación de las células
mesenquimales derivadas de la médula y expandidas por cultivo son
reputados como estímulos mecánicos, celulares y bioquímicos. Sin
embargo, la única diferenciación de células cepa mesenquimales
descritas en US 5.266.914 es la diferenciación al hueso que ocurrió
en cerámicas porosas de fosfato cálcico sin formación de
cartílago.
La presente invención se refiere a un implante
para reparar un defecto del tejido como se ha especificado en la
reivindicación 1.
Aunque un material preferido para la matriz de
gel empleada como en la reivindicación 1 se compone de fibrillas de
colágeno tipo I purificado, otros materiales que se puede usar
igualmente incluyen, por ejemplo, 1) geles de colágeno contraidos
por las células que contienen otros componentes de la matriz
extracelular, tales como proteoglicanos o glicosaminoglicanos,
glicoproteinas (fibronectina, laminina, etc), otros colágenos (por
ejemplo los tipos II o IV), elastina y/o factores de crecimiento de
péptidos bioactivos o citoquinas; 2) otros biopolímeros tales como
fibrina; 3) fibras sintéticas biodegradables fabricadas a partir de
polímeros tales como ácidos polilácticos o poliglicólicos,
policaprolactonas, o poliaminoácidos, o sus copolímeros, que
pudieran colarse o arrollarse en/o sobre la sutura; o 4) estructuras
compuestas tales como estructuras de colágeno/ácido poliláctico.
Además de las suturas sencillas de filamento
simple, se puede usar también dispositivos multifilamentos
producidos por trenzado, tisaje o tricotaje de materiales fibrosos
biodegradables incluidas las suturas o fibras de colágeno o
similares. Las células podrían fijarse en general a tales
dispositivos por procesos mediados por células tales como la
contracción de geles de colágeno, o por procesos físicos o químicos
no mediados por células tales como la gelatina de colada de gel, o
un arrollamiento de estructuras fibrosas o membranosas que contienen
células alrededor del dispositivo. Tales dispositivos de implante
podrían tener una o más agujas unidas a cada extremo del dispositivo
para facilitar la fijación a tejidos blandos o duros, y podrían ser
de una variedad de geometrías incluida la construcción planar,
cilíndrica o tubular, dependiendo del tejido específico a reparar,
el modo de fijación del implante y/o el método usado para unir la
combinación de biomatriz que contiene células al dispositivo de
implantación.
La presente invención se refiere a un implante
para usar en un método para la implantación de cualquier tipo de
células que efectúen la reparación del tejido. Aunque la invención
no se limita a ningún tipo de célula particular, una realización
particularmente preferida incluye células cepa mesenquimales humanas
(MSCs), o cualquiera de su progenie comprometida o diferenciada. Las
células se obtienen preferiblemente del material al que está
destinado el implante, y pueden ser expandidas por cultivo
preferiblemente antes del implante. El animal es preferiblemente un
ser humano.
Métodos han demostrado el cultivo de MSCs o
fibroblastos de tendón sobre suturas Dexon de dos agujas haciendo
que las células contraigan los geles de colágeno alrededor de la
región central de las suturas. El dispositivo compuesto de célula
autóloga/gel de colágeno/sutura puede implantarse directamente entre
los extremos libres de defectos del tendón de espesor total, tal
como para reparar el tendón de Aquiles humano, ligamento tal como
para reparar el ligamento cruzado anterior, o cartílago tal como
para reparar el cartílago articular, menisco, o el disco de la
articulación temporomandibular.
En la realización mostrada en la figura 1, el
implante 1 comprende un cordón de material de sutura 2 y una matriz
de gel 4 conteniendo células reparadoras 6 y que ha sido contraído
alrededor de la porción central 8 de la sutura 2. La sutura 2 tiene
extremos libres 10 y 12 que se usan para reunir el tejido T
adyacente al defecto. Como se muestra, los extremos libres 10 y 12
han sido cosidos en el cuerpo del tejido manteniendo no solamente
los extremos del tendón en su sitio sino manteniendo también la
matriz de gel 4 en posición en el defecto.
La figura 2 muestra un conjunto de molde 15 que
se puede usar para formar un implante de la invención. El conjunto
de molde 15 incluye el molde 16 en el que se ha formado la matriz de
gel que contiene células alrededor de la sutura 2 que se muestra
aquí con agujas 3 y 5 en sus extremos. El alambre de tensión 18 que
mantienen la sutura 2 bajo tensión en el molde 16 y el platillo de
incubación 20 en el que se incuba la preparación de matriz para
fraguar el gel. Una realización específica de esto es descrita en el
ejemplo que sigue.
Se usó un conjunto de molde para preparar un
implante destinado a reparar un defecto del tejido de acuerdo con la
invención. Pequeños cilindros de vidrio, 5 mm x 27 mm, cuyos
extremos habían sido cerrados por fusión, fueron cortados
longitudinalmente a través del centro para formar moldes de vidrio
en forma de canoa. Se curvaron alambres quirúrgicos rígidos para
formar pequeños alambres de tensión en forma de arcos con sus
extremos conformados para disponerse justamente a 2 mm de
profundidad dentro de los moldes de vidrio. El molde de vidrio fue
colocado dentro de un platillo de cultivo de 100 mm con una sutura
extendiéndose sobre el alambre de tensión situado en el centro del
molde como preparación para la suspensión de gel a verter.
Se suspendieron células cepa mesenquimales
autólogas (4 x 10^{6} células) en 0,5 ml de 2 X
DMEM-LG y se mezclaron perfectamente para crear una
suspensión de una sola célula. Después se añadieron 0,5 ml de
solución de colágeno tipo I esterilizado (Pancogene S^{TM},
Gattefossé SA, Lyon, Francia; 3 mg/ml; dializada en 0,001M HCl a la
suspensión de células y se pipetaron arriba y abajo para formar una
suspensión homogénea de células en el gel. Esta suspensión de gel
fue vertida inmediatamente dentro del molde de vidrio preparado en
el platillo de cultivo. Se colocó la tapa sobre el platillo y se
puso en la incubadora a 37ºC durante 15-20 minutos
para fraguar el gel. Después de completar la gelación, se inundó el
platillo con agente sin suero hasta que se cubrió el molde de vidrio
y se volvió a poner en la incubadora durante 4-6
horas. La contracción del gel por las células se produjo hasta el
punto de que el gel se despegó de las paredes del molde y disminuyó
en diámetro y longitud un 10% aproximadamente. Si se cultiva las
células en este aparato durante 20 horas aproximadamente el gel se
contrae aproximadamente al 60% de su dimensión radial original. Al
cabo de 4 horas, el gel estaba firmemente unido a la sutura central,
hasta el punto de que la sutura y el muelle de tensión podían
elevarse del agente, retirando el muelle de tensión, e implantando
el gel en el defecto quirúrgico como se ha descrito.
Los dispositivos de reparación del tejido
preparados por este procedimiento fueron implantados en el defecto
modelo del tendón de Aquiles del conejo con o sin una funda de
Vicrilo. Observaciones histológicas de estos implantes al cabo de 1,
3 y 8 semanas, indican que por este procedimiento se formaron
tejidos de neotendón ya en 1-3 semanas. Estos
tejidos precoces de neotendón son morfológicamente similares a los
tejidos producidos a partir de célula del tendón o implante MSC en
el modelo de reparación de funda de Vicrilo en tiempos
posteriores.

Claims (11)
1. Implante (1) para reparar un defecto de tejido
en un cuerpo humano o animal no humano, cuyo implante (1)
comprende:
- -
- un miembro (2) portador de carga fisiológicamente compatible que tiene medios para fijar bajo tensión tejido adyacente al defecto a reparar;
- -
- alrededor del cual se sitúa una matriz de gel contraída (4); y
- -
- células cepa mesenquimales (6) dentro de dicha matriz de gel contraída (4);
- -
- habiendo contraído las células (6) dicha matriz de gel (4); y
- -
- habiéndose contraído dicha matriz de gel (4) durante la aplicación de una fuerza de tracción al miembro fisiológico portador de carga (2).
2. Implante (1) según la reivindicación 1, en el
que la matriz de gel (4) es un gel de colágeno.
3. Implante (1) según cualquiera de las
reivindicaciones 1 a 2, en el que dicho implante (1) es apropiado
para el tratamiento de un defecto del tejido blando en un cuerpo
humano o animal no humano.
4. Implante (1) según la reivindicación 3, en el
que el defecto de tejido blando es un defecto del tendón, un defecto
del cartílago, o un defecto de cartílago articular o un defecto del
ligamento.
5. Implante (1) según la reivindicación 1, que
incluye un dispositivo de fijación fibroso, degradable como un
miembro portador de carga (2) contiguo con/y unido a dicha matriz de
gel (4) entre un primer y segundo emplazamientos espaciados para
extenderse a través de dicho defecto.
6. Implante (1) según la reivindicación 5, en el
que el miembro portador de carga (2) es alargado entre dichos primer
y segundo emplazamientos.
7. Implante (1) según la reivindicación 5 ó 6, en
el que el miembro portador de carga (2) es una sutura.
8. Implante (1) según la reivindicación 5 a 7, en
el que se selecciona el miembro portador de carga (2) del grupo
consistente en un filamento sencillo, filamentos múltiples,
materiales fibrosos biodegradables, filamentos tejidos, fibras de
colágeno y por lo menos un filamento flexible y plegable.
9. Implante (1) según la reivindicación 5 a 8, en
el que el miembro portador de carga (2) es por lo menos un filamento
con sus respectivos extremos (10, 12) extendidos más allá de dichos
primer y segundo emplazamientos.
10. Implante (1) según la reivindicación 1, en el
que el miembro portador de carga (2) presenta una carga de tracción
mientras es contraída dicha matriz de gel (4), aplicando dicha carga
al miembro portador de carga (2) en el primer y segundo extremos del
miembro portador de carga con dicho gel (4) localizado entre dichos
extremos.
11. Implante (1) según la reivindicación 5 a 10,
en el que el miembro portador de carga (2) es una sutura cuyos
extremos (10, 12) están destinados a ser unidos a dicho tejido T con
la sutura y matriz de gel (4) extendidas a través de dicho
defecto.
