Definitions
- the invention relates to an instrument for removing a ceramic socket insert from a metal shell of a hip joint implant.
- a large part of all ceramic hip joint implants is anchored modularly via a conical clamping connection with a metal shell.
- the removal of ceramic hip joint implants is currently only possible for certain systems using expensive additional instruments. Also, the intraoperative approaches do not always allow to use the additional instruments (eg: minimally invasive access with little "work space" for the surgeon.) Overall, the revision of a ceramic hip joint implant is rarely necessary. Therefore, is often on the purchase of If there are no additional instruments in the respective hospital, the physician must destroy the ceramic insert with great effort and under high patient load.
- the invention has for its object to provide an instrumentation for removing a ceramic socket insert from a metal shell of a hip joint implant, with which a cost effective and effective removal is made possible intra-operatively. Furthermore, the instruments should create the possibility of a revision system for ceramic cup inserts.
- the instrumentation consists of a holding element, a fastening device connectable to the holding element, at least one spacer and a head, wherein the holding element has at one of its ends an adapter device for fixing the head, the head has an adaptation point for fastening the adaptation device, the Holding element at its other end has a coupling device for a tool for rotating the holding member about its longitudinal axis and the holding member on its peripheral surface has a set screw, wherein the fastening device is equipped with an internal thread and on the adjusting thread in the longitudinal axis of the holding member is rotatable and on the fastening device at least one spacer is arranged, which extends parallel to the longitudinal axis of the holding element in the direction of the adaptation device, is a cost effective and effective removal of a ceramic socket insert from e Iner metal shell of a hip joint implant intraoperatively possible. Furthermore, a revision system for ceramic cup inserts is created with this instrument
- the instrumentation according to the invention can be used for all metal shell sizes currently available on the market. Ideally, the instrumentation can be used for metal shells with an outer diameter between 40 mm and 70 mm.
- the instrument includes a head, preferably a ball head, which is glued into the ceramic socket insert of a hip joint implant. This head has an approximate shape for the socket insert. In a preferred embodiment, the head has a bore with a thread at its point of adaptation.
- the holding element according to the invention which has an adapter device for fastening the head at one of its ends, which usually consists of an adaptation thread, is screwed to the head.
- the head is also referred to as a spherical disposable.
- the head may also be secured to the support member with a bayonet lock or other means of connection.
- the instrument should then be able to press off with the help of spacers on the edge of the metal shell.
- spacers In order to ensure a uniform, axial and therefore patient-friendly method of removal, ideally three spacers each with 120 ° offset should ideally be used.
- the spacers are ideally height adjustable to compensate for a possible angular misalignment.
- the application can also be effected by a pulse, introduced via a shock weight, from the metal shell.
- the instrumentation should consist of a surgical steel that can be resterilized to ensure multiple use and avoid aging.
- the spacers between the spacer and the metal shell should be designed such that there is direct contact only between the metal shell and the instrumentation and will remain in place during use.
- Instrumentariums ideally applied an axial tensile force to remove the ceramic cup insert from the metal shell.
- the instrumentation may provide an adaptation to existing surgical instruments.
- the underlying method to be performed with the instrument will be described below. It should be first by targeted impacts on the edge of the metal shell, the interface strength between ceramic socket insert, also referred to as insert, and metal shell can be reduced.
- connection can be produced in a positive, cohesive or cohesive manner. It is advisable to create the connection by means of an adhesive.
- the connection surfaces should be adapted to the mode of operation of the adhesive used (eg cleaned, degreased, dried). At best, an acrylate-based adhesive (eg Histoacryl®) is used for the connection.
- the head should be made of a biocompatible material and provide a means of adapting an OP instrument (for example, a bone cement molded ball with an embedded surgical clamp).
- an OP instrument for example, a bone cement molded ball with an embedded surgical clamp.
- the shape of the head is modeled on the sliding partner of the ceramic cup insert (ball without cone but with thread) and consists of a steel or plastic which is possible for OP use and can be re-usable (resterilizable).
- the adaptation point of the head is therefore preferably a bore with a thread.
- the adaptation device on the threaded rod is preferably one Adapter thread on the outer circumference of the threaded rod, which fits to the thread of the adaptation point in the head.
- the pitch of the thread and possibly its multiple passes must be adapted to the requirements.
- the coupling device on the threaded rod are preferably structurings, such as flattenings, indentations or recesses on the outer circumference of the threaded rod. Preferred is an embodiment that is suitable for the use of a ring wrench.
- three spacers each with 120 ° offset are arranged on the holding element. However, it can be used as a spacer and a cup whose edge must be supported on the edge of the metal shell.
- a support ring with a support surface extending perpendicular to the longitudinal axis of the threaded rod is arranged at the end of the spacer or spacers, the support surface pointing in the direction of the head.
- This support surface must be supported on the edge of the metal shell.
- a head as part of an instrumentation according to the invention is characterized in that the head in a spherical recess of a ceramic socket insert of a hip joint implant is glued, wherein the socket insert is connected via a conical clamping with a metal shell.
- FIG. 1 shows, except for the head 6, an inventive instrumentation for removing a ceramic socket insert 1 from a metal shell 2 of a hip joint implant.
- the metal shell is implanted in a basin 17 (see FIG. 2).
- Figure 2 shows in section the basin 17 in which the metal shell 2 is implanted.
- a ceramic socket insert 1 is inserted and anchored via a conical clamping 15. It may happen that in one operation, the socket insert 1 must be removed again from the metal shell 2, for example to replace it.
- a head 6 is placed as part of an instrument according to the invention in the spherical recess 16 of the socket insert 1 and glued to the socket insert 1.
- the head 6 has at its protruding from the socket insert part 2 a bore 8 with a thread 12.
- the term head 6 is used because the socket insert 1 has a spherical recess 16. However, as shown in FIG. 2, the head 6 can also be hemispherical. The head 6 may also have a projection which rests on the edge of the metal shell 2. The decisive factor is that the head 6 is connected to the socket insert 6, however, preferably by gluing. In a preferred embodiment, the head 6 is a ball head.
- the holding element 3 may be a threaded rod.
- FIG. 1 shows the other parts of the instrumentation.
- the central unit of the instrumentation is a threaded rod 3, which has an adapter device 7 for attachment to the head 6 (see FIG. 2) at one end and has a coupling device 9 at its other end.
- This coupling device 9 is a hexagon in the embodiment shown here.
- a ring spanner is placed on the coupling device 9 and thus the holding element 3 is rotated about its longitudinal axis 10.
- the adaptation device 7 is an adaptation thread 13.
- the holding element 3 is screwed with its adaptation thread 13 into the bore 12 of the head 6.
- a setting thread 1 1 is arranged on the threaded rod 3.
- a holding element 4 with a thread which fits the adjusting thread 4.
- On the holding element 4 are located directly or as shown here via connecting elements 18 spacers 5, which extend in the direction of the adaptation device 7.
- the retaining element slides on the adjusting thread 1 1 until the spacers 5 rest on the metal shell 2 (see Figure 2).
- the holding force of the socket insert 1 in the metal shell 2 must be overcome. Once this has been done, the cup insert 1 can be lifted out of the metal shell 2 with the instruments described.
- the instrumentation is to be used for all metal shell sizes currently available on the market. Ideally, the instrumentation can be used for metal shells with an outer diameter between 40 mm and 70 mm.
- the instrumentation must have an adapter to which a conformal shape glued into the hip joint implant (at the adaptation site, Figure 2) can be attached. Ideally, the contoured form is connected to the instrumentation via a thread or a pin.
- the instrument should then be able to press off with the help of spacers on Metallpfannenrand.
- spacers on Metallpfannenrand.
- 3 spacers each with 120 ° offset should be used.
- the instrumentation should consist of a surgical steel that can be resterilized to ensure multiple use and avoid aging.
- the spacers between the spacer and the metal shell should be designed so that there is direct contact only between the metal cup and the instrumentation and will remain in place during use.
- An axial tensile force should be applied to the glued-in contoured shape with the aid of the instrumentation in order to remove the ceramic socket insert from the metal shell.
- the instruments should / can provide an adaptation to existing surgical instruments.
- the interface strength between the ceramic insert and the metal shell should be reduced by targeted impacts on the metal pan edge.
- a connection between the ceramic insert and a shape approximating the internal geometry of the ceramic cup insert should be created.
- the connection can be produced in a positive, cohesive or cohesive manner.
- the joint surfaces should be cleaned, degreased and dried. It is advisable to create the connection by means of an adhesive.
- an acrylate-based adhesive eg Histoacryl®
- the bonded form should be made of a biocompatible material and provide a means of adapting an OP instrument (for example, a clamp in bone cement or impactor ball with thread).
- the shape is modeled on the sliding partner of the ceramic socket insert (ball without cone but with thread), consists of a steel or plastic which is possible for OP use and is reusable (resterilizable).
