Abstract
Die Erfindung ein Instrumentarium zum Entfernen eines keramischen Pfanneneinsatzes (1 ) aus einer Metallschale (2) eines Hüftgelenksimplantats. Zum kostengünstigen und effektiven Entfernen eines keramischen Pfanneneinsatzes (1 ) aus einer Metallschale (2) eines Hüftgelenksimplantats wird ein Instrumentarium vorgeschlagen, wobei das Instrumentarium aus einem Halteelement (3), einem mit dem Haltelement (3) verbindbaren Befestigungsvorrichtung (4), mindestens einem Abstandshalter (5) und einem Kopf (6) besteht, wobei das Halteelement (3) an einem seiner Enden eine Adaptionsvorrichtung (7) zur Befestigung des Kopfes (6) aufweist, der Kopf (6) eine Adaptionsstelle (8) zur Befesting der Adaptionsvorrichtung (7) aufweist, das Halteelement (3) an seinem anderen Ende eine Koppelvorrichtung (9) für ein Werkzeug zum Drehen des Halteelements (3) um seine Längsachse (10) aufweist und das Halteelement (3) auf seiner Umfangsfläche ein Stellgewinde (1 1 ) aufweist, wobei die Befestigungsvorrichtung (4) mit einem Innengewinde ausgerüstet ist und auf dem Stellgewinde (1 1 ) in Längsachse (10) des Halteelements (3) verdrehbar ist und an der Befestigungsvorrichtung (4) zumindest ein Abstandshalter (5) angeordnet ist, welcher sich parallel zur Längsachse (10) des Halteelements (3) in Richtung zur Adaptionsvorrichtung (7) erstreckt.
Description
INSTRUMENTARIUM ZUM TRENNEN MODULARER KERAMISCHER
HÜFTGELENKSIMPLANTATE
Die Erfindung betrifft ein Instrumentarium zum Entfernen eines keramischen Pfanneneinsatzes aus einer Metallschale eines Hüftgelenksimplantats.
Ein Großteil aller keramischen Hüftgelenksimplantate wird modular über eine konische Klemmverbindung mit einer Metallschale verankert.
Gründe für eine mögliche Explantation der keramischen Komponenten können sein:
• Verkippt eingesetzte Keramikimplantate
• Beschädigte Keramikimplantate (Chip-off oder Risse, die beispielsweise die Funktion des Gleitpartners beeinträchtigen können)
· Beschädigungen oder Defekte an der Metallschale, die eine Revision der Schale erforderlich machen und die nur mit ausgebauter Keramik durchgeführt werden kann.
Das Entfernen keramischer Hüftgelenksimplantate ist derzeit nur für bestimmte Systeme unter Verwendung teurer Zusatzinstrumentarien möglich. Auch die intraoperativen Zugänge erlauben es nicht in jedem Fall, die Zusatzinstrumente zu verwenden (Bsp: Minimal-invasiver Zugang mit wenig „Arbeitsraum" für den Chirurgen). Insgesamt ist die Revision eines keramischen Hüftgelenksimplantates nur selten notwendig. Daher wird oft auf die Anschaffung der teuren Revisionssysteme in Krankenhäusern verzichtet. Liegen keine Zusatzinstrumente im entsprechenden Krankenhaus vor, so muss der Arzt das keramische Insert mit hohem Aufwand und unter hoher Patientenbelastung zerstören.
Der Erfindung liegt die Aufgabe zugrunde, ein Instrumentarium zum Entfernen eines keramischen Pfanneneinsatzes aus einer Metallschale eines Hüftgelenksimplantats zu schaffen, mit dem ein kostengünstiges und effektives Entfernen intra-operativ ermöglicht wird. Weiterhin soll das Instrumentarium die Möglichkeit eines Revisionssystems für keramische Pfanneneinsätze schaffen.
Erfindungsgemäß wird diese Aufgabe mit einem Instrumentarium gemäß der Merkmale des Anspruchs 1 gelöst. Dadurch, dass das Instrumentarium aus einem Halteelement, einem mit dem Haltelement verbindbaren Befestigungsvorrichtung, mindestens einem Abstandshalter und einem Kopf besteht, wobei das Halteelement an einem seiner Enden eine Adaptionsvorrichtung zur Befestigung des Kopfes aufweist, der Kopf eine Adaptionsstelle zur Befesting der Adaptionsvorrichtung aufweist, das Halteelement an seinem anderen Ende eine Koppelvorrichtung für ein Werkzeug zum Drehen des Halteelements um seine Längsachse aufweist und das Halteelement auf seiner Umfangsfläche ein Stellgewinde aufweist, wobei die Befestigungsvorrichtung mit einem Innengewinde ausgerüstet ist und auf dem Stellgewinde in Längsachse des Halteelements verdrehbar ist und an der Befestigungsvorrichtung zumindest ein Abstandshalter angeordnet ist, welcher sich parallel zur Längsachse des Halteelements in Richtung zur Adaptionsvorrichtung erstreckt, ist ein kostengünstiges und effektives Entfernen eines keramischen Pfanneneinsatzes aus einer Metallschale eines Hüftgelenksimplantats intra-operativ möglich. Weiterhin ist mit diesem Instrumentarium ein Revisionssystem für keramische Pfanneneinsätze geschaffen.
Das erfindungsgemäße Instrumentarium ist für alle derzeit im Markt verfügbaren Metallschalengrößen zu verwenden. Idealerweise kann das Instrumentarium für Metallschalen mit einem Außendurchmesser zwischen 40 mm und 70 mm verwendet werden.
Das Instrumentariunn weist einen Kopf, bevorzugt einen Kugelkopf auf, der in den keramischen Pfanneneinsatz eines Hüftgelenksimplantats eingeklebt wird. Dieser Kopf weist eine konturnahe Form zum Pfanneneinsatz auf. Der Kopf weist in einer bevorzugten Ausführungsform an seiner Adaptionsstelle eine Bohrung mit einem Gewinde auf. Zum Entfernen des keramischen Pfanneneinsatzes aus einer Metallschale wird das erfindungsgemäße Halteelement, das an einem seiner Enden eine Adaptionsvorrichtung zum Befestigen des Kopfes aufweist, welche in der Regel aus einem Adaptionsgewinde besteht, mit dem Kopf verschraubt. Der Kopf wird auch als kugelförmiges Disposable bezeichnet. Der Kopf kann am Halteelement auch mit einem Bajonettverschluss oder anderen Vorrichtungen zum Verbinden befestigt sein.
Das Instrumentarium soll sich anschließend mit Hilfe von Abstandshaltern am Rand der Metallschale abdrücken können. Um ein gleichmäßiges, axiales und damit auch patientenschonendes Verfahren zum Entfernen zu gewährleisten, sollten idealerweise bevorzugt drei Abstandshalter mit je 120° Versatz verwendet werden. Die Abstandshalter sind idealerweise höhenverstellbar, um einen möglichen Winkelversatz auszugleichen. Alternativ kann das Ausbringen auch durch einen Impuls, eingebracht über ein Schlaggewicht, aus der Metallschale erfolgen.
Das Instrumentarium sollte aus einem chirurgischen Stahl bestehen, der resterilisierbar ist, um eine Mehrfachverwendung zu gewährleisten und Alterung zu vermeiden. Die Anschlussstellen zwischen Abstandshalter und Metallschale sollten so ausgelegt werden, dass ein direkter Kontakt nur zwischen Metallschale und Instrumentarium besteht und während der Anwendung bestehen bleibt.
Über die eingeklebte, konturnahe Form des Kopfes wird mit Hilfe des
Instrumentariums idealerweise eine axiale Zugkraft aufgebracht, um den keramischen Pfanneneinsatz aus der Metallschale zu entfernen. Das Instrumentarium kann eine Adaption für bestehende OP-lnstrumente bieten.
Das zugrunde liegende Verfahren, welches mit dem Instrumentarium ausgeführt werden soll, wird nachfolgend beschrieben. Es sollte zunächst durch gezielte Schläge auf den Rand der Metallschale die Schnittstellenfestigkeit zwischen keramischen Pfanneneinsatz, auch als Insert bezeichnet, und Metallschale reduziert werden.
Anschließend wird die Verbindung zwischen keramischem Pfanneneinsatz und einer der Innengeometrie des keramischen Pfanneneinsatzes angenäherten Form, d.h. dem Kopf, erzeugt. Die Verbindung kann form-, kraft- oder stoffschlüssig erzeugt werden. Es empfiehlt sich, die Verbindung stoffschlüssig mithilfe eines Klebers zu erzeugen. Die Verbindungsflächen sollten an die Funktionsweise des verwendeten Klebers angepasst werden (Bsp. gesäubert, entfettet, getrocknet). Bestenfalls wird ein Kleber auf Acrylatbasis (Bsp: Histoacryl®) für die Verbindung verwendet.
Der Kopf sollte aus einem biokompatiblen Werkstoff bestehen und eine Möglichkeit zur Adaption eines OP-lnstrumentes bieten (zum Beispiel eine aus Knochenzement geformte Kugel mit einer eingebetteten OP-Klemme). Idealerweise ist die Form des Kopfes dem Gleitpartner des keramischen Pfanneneinsatzes nachempfunden (Kugel ohne Konus aber mit Gewinde) und besteht aus einem für den OP Einsatz möglichen Stahl oder Kunststoff und kann wiederverwendbar (resterilisierbar) sein.
Zum Entfernen wird das Halteelement gedreht, bis die Abstandshalter auf dem Rand der Metallschale aufliegen. Anschließend wird das Halteelement weiter gedreht, wodurch eine axiale Zugkraft auf den Pfanneneinsatz aufgebaut wird, so lange, bis sich die Verbindung Kopf und Pfanneneinsatz löst.
Die Adaptionsstelle des Kopfes ist daher bevorzugt eine Bohrung mit einem Gewinde. Die Adaptionsvorrichtung am Gewindestab ist bevorzugt ein
Adaptionsgewinde auf dem Außenumfang des Gewindestabes, welches zum Gewinde der Adaptionsstelle im Kopf passt. Die Steigung des Gewindes und ggf. dessen Mehrgängigkeit muss den Anforderungen angepasst werden. Die Koppelvorrichtung am Gewindestab sind bevorzugt Strukturierungen, wie Abflachungen, Einkerbungen oder Ausnehmungen am Außenumfang des Gewindestabes. Bevorzugt ist eine Ausführungsform, die für den Einsatz eines Ringschlüssels passend ist. In einer vorteilhaften Ausführungsform sind am Halteelement drei Abstandshalter mit je 120° Versatz angeordnet. Es kann jedoch als Abstandshalter auch ein Becher verwendet werden, dessen Rand sich auf dem Rand der Metallschale abstützen muss. In einer Ausführungsform ist am Ende des oder der Abstandshalter ein Auflagering mit einer zur Längsachse des Gewindestabes senkrecht verlaufenden Auflagefläche angeordnet, wobei die Auflagefläche in Richtung zum Kopf zeigt. Diese Auflagefläche muss sich auf dem Rand der Metallschale abstützen. Ein erfindungsgemäßes Hüftgelenksimplantat mit einer äußeren Metallschale in der ein keramischer Pfanneneinsatz über eine konische Klemmung verankert ist und der Pfanneneinsatz eine sphärische Ausnehmung zur Aufnahme eines Kopfes aufweist, ist erfindungsgemäß dadurch gekennzeichnet, dass der Kopf in der sphärischen Ausnehmung des Pfanneneinsatzes verklebt ist und der Kopf eine Adaptionsstelle zur Befestigung eines Teils des erfindungsgemäßen Instrumentariums zum Entfernen des Pfanneneinsatzes aus der Metallschale aufweist.
Ein Kopf als Teil eines erfindungsgemäßen Instrumentariums ist dadurch gekennzeichnet, dass der Kopf in einer sphärischen Ausnehmung eines
keramischen Pfanneneinsatz eines Hüftgelenksimplantats verklebt ist, wobei der Pfanneneinsatz über eine konische Klemmung mit einer Metallschale verbunden ist.
Bevorzugt ist die Verwendung eines Klebers auf Acrylatbasis (Bsp: Histoacryl®) zum Verkleben eines oben beschriebenen Hüftgelenksimplantats oder eines oben beschriebenen Kopfes.
Nachfolgend wird die Erfindung anhand von zwei Figuren weiter beschrieben. Figur 1 zeigt bis auf den Kopf 6 ein erfindungsgemäßes Intrumentarium zum Entfernen eines keramischen Pfanneneinsatzes 1 aus einer Metallschale 2 eines Hüftgelenksimplantats. Die Metallschale ist in einem Becken 17 (siehe Figur 2) implantiert. Figur 2 zeigt im Schnitt das Becken 17 in dem die Metallschale 2 implantiert ist. In der Metallschale ist ein keramischer Pfanneneinsatz 1 eingesetzt und über eine konische Klemmung 15 verankert. Es kann vorkommen, dass in einer Operation der Pfanneneinsatz 1 wieder aus der Metallschale 2 entfernt werden muss, zum Beispiel um ihn zu ersetzen. Hierzu wird ein Kopf 6 als Teil eines erfindungsgemäßen Instrumentariums in die sphärische Ausnehmung 16 des Pfanneneinsatzes 1 gelegt und mit dem Pfanneneinsatz 1 verklebt. Der Kopf 6 weist an seinem aus dem Pfanneneinsatz 2 herausragenden Teil eine Bohrung 8 mit einem Gewinde 12 auf.
Der Begriff Kopf 6 wird verwendet, weil der Pfanneneinsatz 1 eine sphärische Ausnehmung 16 aufweist. Der Kopf 6 kann jedoch, wie Figur 2 zeigt, auch halbkugelförmig sein. Der Kopf 6 kann auch eine Auskragung aufweisen, die auf dem Rand der Metallschale 2 aufliegt. Ausschlaggebend ist, dass der Kopf 6 mit dem Pfanneneinsatz 6 verbunden wird, wie auch immer, bevorzugt durch Kleben.
In einer bevorzugten Ausführungsform ist der Kopf 6 ein Kugelkopf. Das Haltelement 3 kann ein Gewindestab sein.
Figur 1 zeigt die anderen Teile des Instrumentariums. Die zentrale Einheit des Instrumentariums ist ein Gewindestab 3, der an einem Ende eine Adaptionsvorrichtung 7 zur Befestigung mit dem Kopf 6 (siehe Figur 2) aufweist und an seinem anderen Ende eine Koppelvorrichtung 9 hat. Diese Koppelvorrichtung 9 ist in der hier gezeigten Ausführungsform ein Sechskant. Zum Verdrehen des Gewindestabes 3 wird zum Beispiel ein Ringschlüssel auf die Koppelvorrichtung 9 gesetzt und damit das Halteelement 3 um seine Längsachse 10 gedreht.
In der hier gezeigten Ausführungsform ist die Adaptionsvorrichtung 7 ein Adaptionsgewinde 13. das Halteelement 3 wird mit seinem Adaptionsgewinde 13 in die Bohrung 12 des Kopfes 6 eingedreht.
Zwischen der Adaptionsvorrichtung 7 und der Koppelvorrichtung 9 ist auf dem Gewindestab 3 ein Stellgewinde 1 1 angeordnet. Auf diesem Stellgewinde 1 1 befindet sich ein Halteelement 4 mit einem Gewinde, welches zum Stellgewinde 4 passt. Am Halteelement 4 befinden sich direkt oder wie hier gezeigt über Verbindungselemente 18 Abstandshalter 5, die sich in Richtung zu der Adaptionsvorrichtung 7 erstrecken. Beim Verdrehen des Gewindestabs 3 rutscht der Halteelement auf dem Stellgewinde 1 1 so lange, bis die Abstandshalter 5 auf der Metallschale 2 (siehe Figur 2) aufliegen. Dann muss durch Weiterdrehen des Gewindestabs 3 die Haltekraft des Pfanneneinsatzes 1 in der Metallschale 2 überwunden werden. Ist dies geschehen, lässt sich mit dem beschriebenen Instrumentarium der Pfanneneinsatz 1 aus der Metallschale 2 heben.
Das Instrumentarium ist für alle derzeit im Markt verfügbaren Metallschalengrößen zu verwenden. Idealerweise kann das Instrumentarium für Metallschalen mit einem Außendurchmesser zwischen 40 mm und 70 mm verwendet werden.
Das Instrumentarium muss einen Adapter besitzen, an den eine in das Hüftgelenksimplantat eingeklebte konturnahe Form (an der Adaptionsstelle, Bild 2) befestigt werden kann. Idealerweise wird die konturnahe Form über ein Gewinde oder einen Stift mit dem Instrumentarium verbunden.
Das Instrumentarium soll sich anschließend mit Hilfe von Abstandshaltern am Metallpfannenrand abdrücken können. Um ein gleichmäßiges, idealerweise axiales und damit auch patientenschonendes Verfahren zum Entfernen zu gewährleisten, sollten idealerweise 3 Abstandshalter mit je 120° Versatz verwendet werden.
Das Instrumentarium sollte aus einem chirurgischen Stahl bestehen, der resterilisierbar ist, um eine Mehrfachverwendung zu gewährleisten und Alterung zu vermeiden. Die Anschlussstellen zwischen Abstandshalter und Metallschale sollten so ausgelegt werden, dass ein direkter Kontakt nur zwischen Metallpfanne und Instrumentarium besteht und während der Anwendung bestehen bleibt.
Über die eingeklebte, konturnahe Form sollte mit Hilfe des Instrumentariums eine axiale Zugkraft aufgebracht werden, um den keramischen Pfanneneinsatz aus der Metallschale zu entfernen. Das Instrumentarium sollte/kann eine Adaption für bestehende OP-lnstrumente bieten.
Das zugrunde liegende Verfahren, welches mit dem erfindungsgemäßen Instrumentarium ausgeführt werden soll, wird nachfolgend beschrieben.
Es sollte zunächst durch gezielte Schläge auf den Metallpfannenrand die Schnittstellenfestigkeit zwischen keramischem Insert und Metallschale reduziert werden.
Anschließend sollte eine Verbindung zwischen keramischem Insert und einer der Innengeometrie des keramischen Pfanneneinsatzes angenäherten Form erzeugt werden. Die Verbindung kann form-, kraft- oder stoffschlüssig erzeugt werden. Zunächst sollten die Verbindungsflächen gesäubert, entfettet und getrocknet werden. Es empfiehlt sich, die Verbindung stoffschlüssig mithilfe eines Klebers zu erzeugen. Bestenfalls wird ein Kleber auf Acrylatbasis (Bsp: Histoacryl®) für die Verbindung verwendet. Die verbundene Form sollte aus einem biokompatiblen Werkstoff bestehen und eine Möglichkeit zur Adaption eines OP-lnstrumentes bieten (zum Beispiel eine Klemme in Knochenzement oder Impaktorkugel mit Gewinde). Idealerweise ist die Form dem Gleitpartner des keramischen Pfanneneinsatzes nachempfunden (Kugel ohne Konus aber mit Gewinde), besteht aus einem für den OP Einsatz möglichen Stahl oder Kunststoff und ist wiederverwendbar (resteril isierbar).

(2) eines Hüftgelenksimplantats, dadurch gekennzeichnet, dass das Instrumentarium aus einem Halteelement (3), einem mit dem Haltelement (3) verbindbaren Befestigungsvorrichtung (4), mindestens einem Abstandshalter (5) und einem Kopf (6) besteht, wobei das Halteelement (3) an einem seiner Enden eine Adaptionsvorrichtung (7) zur Befestigung des Kopfes (6) aufweist, der Kopf (6) eine Adaptionsstelle (8) zur Befesting der Adaptionsvorrichtung (7) aufweist, das Halteelement (3) an seinem anderen Ende eine Koppelvorrichtung (9) für ein Werkzeug zum Drehen des Halteelements (3) um seine Längsachse (10) aufweist und das Halteelement (3) auf seiner Umfangsfläche ein Stellgewinde (1 1 ) aufweist, wobei die Befestigungsvorrichtung (4) mit einem Innengewinde ausgerüstet ist und auf dem Stellgewinde (1 1 ) in Längsachse (10) des Halteelements
(3) verdrehbar ist und an der Befestigungsvorrichtung
(4) zumindest ein Abstandshalter (5) angeordnet ist, welcher sich parallel zur Längsachse (10) des Halteelements (3) in Richtung zur Adaptionsvorrichtung (7) erstreckt.
Instrumentarium nach Anspruch 1 , dadurch gekennzeichnet, dass die Adaptionsstelle (8) des Kopfes (6) eine Bohrung mit einem Gewinde (12) oder ein Bajonettverschluss ist.
Instrumentarium nach Anspruch 2, dadurch gekennzeichnet, dass die Adaptionsvorrichtung (7) ein Adaptionsgewinde (13) auf dem Außenumfang des Halteelements (3) ist, welches zum Gewinde (12) der Adaptionsstelle (8) im Kopf (6) passt.
Instrumentarium nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die Koppelvorrichtung (9) Strukturierungen, wie Abflachungen,
Einkerbungen oder Ausnehmungen am Außenumfang des Halteelements (3) sind.
5. Instrumentarium nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass an der Befestigungsvorrichtung (4) drei Abstandshalter (5) mit je 120° Versatz angeordnet sind.
6. Instrumentarium nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, dass am Ende des oder der Abstandshalter (5) ein Auflagering (14) mit einer zur Längsachse (10) des Halteelements (3) senkrecht verlaufenden Auflagefläche angeordnet ist, wobei die Auflagefläche in Richtung zum Kopf (6) zeigt.
7. Hüftgelenksimplantat mit einer äußeren Metallschale (2) in der ein keramischer Pfanneneinsatz (1 ) über eine konische Klemmung (15) verankert ist und der Pfanneneinsatz (1 ) eine sphärische Ausnehmung (16) zur Aufnahme eines Kopfes (6) aufweist, dadurch gekennzeichnet, dass der Kopf (6) in der sphärischen Ausnehmung (16) des Pfanneneinsatzes (1 ) verklebt ist und der Kopf (6) eine Adaptionsstelle (8) zur Befestigung eines Teils eines
Instrumentariums zum Entfernen des Pfanneneinsatzes (1 ) aus der Metallschale (2) nach einem der Ansprüche 1 bis 6 aufweist.
8. Kopf (6) als Teil eines Instrumentariums nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, dass der Kopf (6) in einer sphärischen Ausnehmung (16) eines keramischen Pfanneneinsatz (1 ) eines Hüftgelenksimplantats verklebt ist, wobei der Pfanneneinsatz (1 ) über eine konische Klemmung (15) mit einer Metallschale (2) verbunden ist.
9. Verwendung eines Klebers auf Acrylatbasis (Bsp: Histoacryl®) zum Verkleben eines Hüftgelenksimplantats nach Anspruch 7 oder eines Kopfes (6) nach Anspruch 8.
10. Instrumentarium nach einem der Ansprüche 1 bis 6, Hüftgelenksimplantat nach Anspruch 7 oder Kopf nach Anspruch 8, dadurch gekennzeichnet, dass der Kopf
(6) eine an die sphärische Ausnehmung (16) im Pfanneneinsatzes angepassten Form aufweist.
