- the present invention relates to minimally invasive surgery, particularly laparoscopic, thoracoscopic, arthroscopic surgery and NOTES surgery (Natural Orifice Trans Endoscopy Surgery). It relates more particularly trocars used for this purpose.
- Laparoscopic surgery allows many procedures to be performed. In general, this type of surgery is performed with the aid of several trocars which are arranged in such a way that the surgeon obtains a good triangulation between the various trocars in order to have in particular a lateral vision of the operating field with respect to the operating instruments.
- the fact of placing several trocars represents for the patient several scars which can be painful and which can be superinfected. On the other hand, many scars represent an aesthetic discomfort for the patient.
- technological advances currently make it possible to perform an intervention using a single trocar, trocar with several working channels.
- NOTES surgery natural pathways are used to enter the abdominal cavity, for example through the stomach, rectum or vaginal pouch. For this pathway, surgeons also use a single trocar with multiple working channels.
- Trocars for laparoscopic, thoracoscopic, arthroscopic surgery and NOTES surgery are well known to the surgical industry. These trocars consist of a main tube which may include one or more working channels inside the tube.
- the major disadvantage of trocars with several working channels is to have only an axial vision of the operating field, which does not allow the surgeon, as is the case using multiple trocars, to have good vision of the operating field. With axial vision, the surgeon does not have a depth of field.
- the present invention proposes to remedy in particular the aforementioned problems.
- a trocar comprising a main tube defined by a proximal end and a distal end.
- the main tube comprises a central conduit disposed between said ends, which is adapted to receive or a plurality of minimally invasive surgical instruments.
- the lateral face of the main tube comprises at least one orifice dimensioned so as to provide one or more other minimally invasive surgical instruments transversely to the main tube, in a direction that is angled with respect to its major axis, so that the distal end of said one or more other instruments is not situated in the extension of the main axis of said tube.
- the distal end of said one or more other instruments can emerge from the distal end of the main tube.
- the lateral face of the main tube comprises two orifices disposed on either side of the main tube, in a direction which forms an angle with respect to the main axis of said tube; said orifices being dimensioned so as to allow one or more other minimally invasive surgical instruments to be disposed transversely relative to the main tube, in said direction which forms an angle with respect to its main axis.
- the distal end of said one or more other instruments thus emerges at the side wall of the main tube.
- an optical instrument is disposed through the orifices. This configuration provides the surgeon with a lateral view of the operative field and a depth of field, which increases the safety of the surgical procedure.
