Abstract
Die vorliegende Erfindung betrifft ein Verfahren zur Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache, wobei das Verfahren die gemeinsame Verabreichung einer therapeutisch wirksamen Menge eines ausgewählten CGRP-Antagonisten (A), eine physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und einer therapeutisch wirksamen Menge eines zweiten oder dritten aktiven Arzneistoffs gegen Migräne (B), insbesondere Sumatriptan, Zolmitriptan oder Dihydroergotamin oder eines physiologisch verträglichen Salzes davon, umfasst, sowie die entsprechenden pharmazeutischen Zusammensetzungen und deren Herstellung.

Verwendung ausgewählter CGRP-Antagonisten in Kombination mit anderen Arzneistoffen gegen Migräne für die Behandlung von Migräne
Hintergrund der Erfindung Migräne ist eine der häufigsten neurologischen Störungen und beinhaltet periodische Anfälle von Kopfschmerzen und Übelkeit sowie eine Vielzahl weiterer Symptome. Obwohl erhebliche Fortschritte gemacht worden sind, wird die Pathophysiologie von Migräne weiterhin nicht verstanden. Mehrere Beobachtungen deuten jedoch auf eine Beteiligung des Calcitonin-Gen-bezogenen Peptids (CGRP) hin. Migränekopfschmer- zen beinhalten die Aktivierung des trigeminalen Systems und die Erweiterung von kranialen Gefäßen. CGRP ist an Neuronen in trigeminalen Ganglia lokalisiert, und CGRP-Spiegel sind während eines Migräneanfalls erhöht, was vermutlich die beobachtete Vasodilation bewirkt. Dementsprechend ist denkbar, dass die Inhibition der von CGRP hervorgerufenen Erweiterung der kranialen Gefäße möglicherweise eine neue Behandlung von Migränekopfschmerzen erlaubt.
Breite Verwendung findende Arzneistoffe gegen Migräne sind sogenannte "Triptane", z.B. Sumatriptan und Zolmitriptan. Diese Verbindungen rufen ihre Wirkungen gegen Migräne aufgrund ihrer vasokonstriktiven Eigenschaften und vermutlich ihrer Inhibition der Freisetzung des Neuropeptids Calcitonin-Gen-bezogenes Peptid (CGRP) hervor (Ferrari, M. D., Saxena, P. R. (1995), 5-HT1 receptors in migraine pathophysiology and treatment, Eur. J. Neurology, 2, 5-21 ; Johnson, K. W., Phebus, L. A., Cohen, M. L. Ein völlig neuer Ansatz zur Behandlung von Migräne ist die Verwendung von CGRP-Antagonisten (Doods, H., Hallermayer, G., Wu, D., Entzeroth, M., Rudolf, K., Engel, W., Eberlein, W. deren physiologisch verträglichen Salzen und den Hydraten der Salze und einem 5-HTiB/iD-Agonisten oder einem Ergot-Alkaloid (B) zu einer besseren Wirkung im Vergleich mit der Wirkung nur eines Arzneistoffs führt.
Ausführliche Darstellung der Erfindung
Gemäß einem ersten Aspekt stellt die vorliegende Erfindung ein Verfahren zur Behandlung oder Prävention von Indikationen bereit, die aus der Gruppe ausgewählt sind, die aus Kopfschmerzen, Migräne und Cluster headache besteht, wobei das Verfahren die gemeinsame Verabreichung einer therapeutisch wirksamen Menge eines der erfindungsgemäß ausgewählten CGRP-Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und einer therapeutisch wirksamen Menge eines zweiten oder dritten aktiven Arzneistoffs gegen Migräne (B) an eine Person, die einer derartigen Behandlung bedarf, umfasst. Die Kombination mit zwei anderen aktiven Arzneistoffen ist z.B. dann sinnvoll, wenn ein CGRP-Antagonist (A) in Kombination mit einem Arzneistoff (B) synergistisch gegen Schmerz wirkt, zusätzlich aber auch eine antiemetische Wirkung erwünscht ist.
Der Arzneistoff (B) kann aus der Gruppe ausgewählt werden, die aus Angiotensin-Il- Antagonisten, α-Agonisten und α-Antagonisten, 5-HTiB/iD-Agonisten, AMPA- Antagonisten, Antidepressiva, Antiemetika, Antikonvulsiva, Antimuscarinika, ß-Blockern, Calciumantagonisten, Corticosteroiden, Ergot-Alkaloiden, Histamin- HI -Rezeptor-Antagonisten, schwachen Analgetika, Neurokinin-Antagonisten, Neuroleptika, nicht-steroidalen Antiphlogistika, NO-Synthase-Hemmern, Prokinetika und Serotonin-Wiederaufnahme-Hemmem besteht.
Ein nicht-steroidales Antiphlogistikum kann aus der Gruppe ausgewählt werden, die aus Acclofenac, Acemetacin, Acetylsalicylsäure, Acetaminophen (Paracetamol), Azathioprin, Diclofenac, Diflunisal, Fenbufen, Fenoprofen, Flurbiprofen, Ibuprofen, Indometacin, Ketoprofen, Leflunomid, Lomoxicam, Mefenaminsäure, Naproxen, Phenylbutazon, Piroxicam, Sulfasalazin, Tenoxicam, Zomepirac und deren physiologisch verträglichen Salzen, aus Meloxicam und anderen selektiven COX2- Inhibitoren, wie bespielsweise Celecoxib, Etoricoxib, Parecoxib, Rofecoxib und Valdecoxib, sowie aus Substanzen, die frühere oder spätere Schritte in der Prostaglandin-Synthese inhibieren, oder Prostaglandinrezeptor-Antagonisten, wie beispielsweise EP2-Rezeptor-Antagonisten und IP-Rezeptor-Antagonisten besteht.
Als Angiotensin-Il-Antagonisten können diejenigen Beispiele verwendet werden, die in der EP-A-253310, EP-A-323841 , EP-A-324377, EP-A-420237, EP-A-43983, EP-A- 459136, EP-A-475206, EP-A-502314, EP-A-504888, EP-A-514198, WO 91/14679, WO 93/20816, US 4,355,040 und US 4,880,804 beschrieben sind, oder die physiologisch verträglichen Salze davon. Bevorzugte Angiotensin Il-Antagonisten sind Sartane, wie Candesartan, Eprosartan, Irbesartan, Losartan, Olmesartan, Tasosartan, Telmisartan oder Valsartan.
Als 5-HTiB/iD-Agonisten können z.B. Almotriptan, Avitriptan, Donitriptan, Eletriptan, Frovatriptan, Naratriptan, Rizatriptan, Sumatriptan oder Zolmitriptan oder die physiologisch verträglichen Salze davon verwendet werden.
Geeignete Ergot-Alkaloide sind z.B. Ergotamin und Dihydroergotamin; und
als Serotonin-Wiederaufnahme-Hemmer können Citalopram, Duloxetin, Escitalo- pram, Fluoxetin, Fluvoxamin, Paroxetin, Sertralin, Trazodon oder die physiologisch verträglichen Salze davon verwendet werden.
Zusätzliche aktive Substanzen, die für die vorstehend genannten Kombinationen als Arzneistoffkomponente (B) in Betracht gezogen werden können, umfassen z.B. Metoclopramid, Domperidon, Diphenhydramin, Cyclizin, Promethazin, Chlorproma- zin, Vigabatrin, Timolol, Isomethepten, Pizotifen, Botox, Gabapentin, Pregabalin,

Topiramat, Riboflavin, Montelukast, Lisinopril, Micardis, Prochlorperazin, Dexametha- son, Flunarizin, Dextropropoxyphen, Meperidin, Metoprolol, Propranolol, Nadolol, Atenolol, Clonidin, Indoramin, Carbamazepin, Phenytoin, Valproat, Amitryptilin, Imipramine, Venlafaxine, Lidocain oder Diltiazem.
Als eine bevorzugte Ausführungsform des erfindungsgemäßen Verfahrens wird Arzneistoff (B) aus der Gruppe ausgewählt, die aus Ergot-Alkaloiden, und 5-HT1 B/iD-Agonisten besteht, wobei besonders bevorzugt Dihydroergotamin, Sumatriptan und Zolmitriptan sind und am meisten bevorzugt Sumatriptan oder die physiologisch verträglichen Salze davon sind.
Als eine weiter bevorzugte Ausführungsform des erfindungsgemäßen Verfahrens wird Arzneistoff (B) aus der Gruppe ausgewählt, die aus nicht-steroidalen Antiphlo- gistika besteht, wobei besonders bevorzugt Meloxicam oder dessen physiologisch verträgliche Salze sind.
Als eine noch weiter bevorzugte Ausführungsform des erfindungsgemäßen Verfahrens wird Arzneistoff (B) aus der Gruppe ausgewählt, die aus Serotonin- Wiederaufnahme-Hemmem besteht, wobei besonders bevorzugt Duloxetin oder dessen physiologisch verträgliche Salze sind.
Die Dosierung für den kombinierten Migränearzneistoff (B) ist ungefähr 1/50 der niedrigsten normalerweise empfohlenen Dosis bis zu 1/1 der normalerweise empfohlenen Dosis, und zwar auf oralem, nasalem, inhalativem, subkutanem oder intravenösem Weg. Als die normalerweise empfohlene Dosis für den kombinierten Migränearzneistoff (B) ist die Dosis zu verstehen, die in Rote Liste WinR 2001/1, Editio Cantor Verlag Aulendorf, beschrieben ist.
Erfindungsgemäß können die ausgewählten CGRP-Antagonisten (A), ein physiolo- gisch verträgliches Salz davon oder ein Hydrat des Salzes auf intravenösem oder subkutanem Weg in einer Dosierung von 0.0001 bis 3 mg/kg Körpergewicht verabreicht werden, auf oralem Weg in einer Dosierung von 0.1 bis 20 mg/kg Körpergewicht oder auf nasalem oder inhalativem Weg in einer Dosierung von 0.1

bis 10 mg/kg Körpergewicht einmal, zweimal oder dreimal täglich, und zwar in Kombination mit
Sumatriptan oder einem physiologisch verträglichen Salz davon, das auf oralem Weg in einer Dosierung von 0.03 bis 1.43 mg/kg Körpergewicht einmal, zweimal oder dreimal täglich oder
auf intravenösem oder subkutanem Weg in einer Dosierung von 0.002 bis 0.09 mg/kg Körpergewicht einmal oder zweimal täglich oder
auf rektalem Weg in einer Dosierung von 0.007 bis 0.36 mg/kg Körpergewicht einmal oder zweimal täglich oder

Zolmitriptan oder einem physiologisch verträglichen Salz davon, das auf oralem Weg in einer Dosierung von 0.0007 bis 0.036 mg/kg Körpergewicht einmal oder zweimal täglich verabreicht werden kann, oder
in Kombination mit Dihydroergotamin oder einem physiologisch verträglichen Salz davon, das auf oralem Weg in einer Dosierung von 0.001 bis 0.07 mg/kg Körpergewicht einmal oder zweimal täglich verabreicht werden kann, oder
in Kombination mit Meloxicam oder einem physiologisch verträglichen Salz davon, das auf oralem Weg in einer Dosierung von 0.004 bis 0.21 mg/kg Körpergewicht einmal täglich verabreicht werden kann oder
in Kombination mit Duloxetin oder einem physiologisch verträglichen Salz davon, der auf oralem Weg in einer Dosierung von 0.03 bis 1.43 mg/kg Körpergewicht einmal, zweimal oder dreimal täglich oder
auf intravenösem oder subkutanem Weg in einer Dosierung von 0.002 bis 0.09 mg/kg Körpergewicht einmal oder zweimal täglich oder
auf rektalem Weg in einer Dosierung von 0.007 bis 0.36 mg/kg Körpergewicht einmal oder zweimal täglich oder
auf nasalem Weg in einer Dosierung von 0.006 bis 0.29 mg/kg Körpergewicht einmal oder zweimal täglich verabreicht werden kann.
Die vorliegende Erfindung stellt gemäß einem zweiten Aspekt eine pharmazeutische Zusammensetzung zur Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache, die eine therapeutisch wirksame Menge eines ausgewählten CGRP-Antagonisten (A)1 eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und einen Arzneistoff gegen Migräne (B), der aus der Gruppe ausgewählt ist, die aus Sumatriptan, Zolmitriptan und Dihydroergotamin oder einem physiologisch verträglichen Salz davon besteht, als kombinierte Zubereitung für die gleichzeitige oder zeitlich aufeinanderfolgende Verabreichung bereit.
Eine erfindungsgemäße pharmazeutische Zusammensetzung kann eine einzelne Dosierungseinheit von 0.1 bis 1500 mg, vorzugsweise 0.3 bis 1000 mg, besonders bevorzugt 5 bis 750 mg, eines ausgewählten CGRP-Antagonisten (A), einer äquvalenten Menge eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und
eine einzelne Dosierungseinheit von 1 bis 100 mg Sumatriptan oder
eine einzelne Dosierungseinheit von 0.1 bis 2.5 mg Zolmitriptan oder
eine einzelne Dosierungseinheit von 0.1 bis 5 mg Dihydroergotamin oder
eine einzelne Dosierungseinheit von 7.5 bis 15 mg Meloxicam oder

eine einzelne Dosierungseinheit von 0.1 bis 150 mg, vorzugsweise 0.2 bis 100 mg, bespielsweise 10 bis 100 mg, besonders bevorzugt 10 bis 80 mg, insbesondere 40 bis 80 mg, Duloxetin enthalten.
Alle Dosen oder Dosierungseinheiten eines physiologisch verträglichen Salzes einer der vorstehend genannten aktiven Verbindungen sind als Dosis oder Dosierung der aktiven Verbindung selbst zu verstehen.
Ferner kann eine erfindungsgemäße pharmazeutische Zusammensetzung ein Kit von Teilen für die Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache sein, wobei das Kit umfasst:
(a) eine erste Umschließung enthaltend eine pharmazeutische Zusammensetzung umfassend eine therapeutisch wirksame Menge eines ausgewählten CGRP- Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines
Hydrats des Salzes und ein oder mehrere physiologisch verträgliche Verdünnungsmittel und/oder Trägerstoffe; und
(b) eine zweite Umschließung enthaltend eine pharmazeutische Zusammensetzung umfassend Sumatriptan, Zolmitriptan oder Dihydroergotamin oder ein physiologisch verträgliches Salz davon und einen oder mehrere physiologisch verträgliche Verdünnungsmittel und/oder Trägerstoffe.
Ein bevorzugtes Kit von Teilen umfasst Sumatriptan in der zweiten Umschließung.
Ein dritter Aspekt der vorliegenden Erfindung ist die Verwendung eines ausgewählten CGRP-Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes in Kombination mit einem zweiten oder dritten aktiven Arzneistoff gegen Migräne (B) für die Herstellung einer pharmazeutischen Zusammensetzung für die Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache. Arzneistoff (B) und bevorzugte Ausführungsformen davon sowie pharmazeutische Zusammensetzungen sind vorstehend unter dem ersten und zweiten Aspekt der Erfindung genannt. Am stärksten bevorzugt im

Hinblick auf alle Aspekte der Erfindung ist die Kombination eines ausgewählten CGRP-Antagonisten (A), einem physiologisch verträglichen Salz davon oder eines Hydrats des Salzes mit Sumatriptan oder von physiologisch verträglichen Salzen davon.
Mehrere der vorstehend genannten Arzneistoff (B)-Komponenten sind bereits auf dem Markt; z.B. wird Sumatriptan unter der Marke Imigran® vertrieben, Zolmitriptan wird unter der Marke Ascotop® vertrieben, Meloxicam wird unter der Marke Mobec® und Dihydroergotamin und die physiologisch verträglichen Salze davon unter der Marke Agit® vertrieben.
Die ausgewählten CGRP-Antagonisten (A) können in Kombination mit einem zweiten oder dritten weiteren aktiven Arzneistoff gegen Migräne (B1 und B2) z.B. unter Verwendung einer der folgenden pharmazeutischen Formulierungen verabreicht werden.
Um optimale Dosierungen und Compliance zu erreichen, werden die erfindungsgemäßen Zweier- oder Dreierkombinationen als fixe Kombinationen in verschiedenen Darreichungsformen wie in den folgenden Absätzen beschrieben verabreicht. Da bei Migräne rascher Wirkungseintritt vorteilhaft ist, wurden die oralen Formen auf rasche Wirkstofffreigabe optimiert. Falls bei ein oder mehreren Komponenten zusätzlich aber auch eine lang anhaltende Wirkung gewünscht wird (z.B. bei einigen nichtsteroidalen Antiphlogistika oder Antiemetika, die teilweise drei- bis viermal täglich verabreicht werden müssen) um eine mehrfache Anwendung täglich zu vermeiden, können ein oder mehrere Komponenten aber auch retardiert werden. Dies kann in Form langsam freigebender Matrixtabletten oder - vorzugsweise - um die intra- und interindividuelle Variabilität zu vermindern, in multipartikulären Systemen wie Pellets oder Extrudaten erfolgen.
Bevorzugte Darreichungsformen sind:

CGRP-Antagonist, Sumatriptanhydrogensuccinat und Lactose (fein) werden in einem geeigneten Mischer (z.B. Diosna P2) homogen gemischt; anschließend granuliert man die Mischung mit einer wässrigen Povidon-Lösung; Das Granulat wird mit einem Kressner-Sieb mit 1.6 mm gesiebt und 2 Stunden bei 40 0C getrocknet. Im Anschluss wird das Granulat in einer geeigneten Mühle, z.B. einer Comill bei 3000 U/Min mit Maschenweite 1.1 mm gesiebt. Daraufhin wird das Granulat 5 Minuten mit Crospovidon gemischt und anschließend noch 1 Minute mit Magnesiumstearat. Die so erhaltene Mischung wird in einer Tablettenpresse zu Tabletten mit einem geeigneten Durchmesser gepresst.

Pellets
Die erfindungsgemäßen Arzneistoffkombinationen können auch in Form kleiner Partikeln wie z.B. Pellets hergestellt werden. Hierbei können beide Wirkstoffe gemeinsam auf Neutralpellets, die aus Saccharose und Stärke oder mikrokristalliner
Cellulose bestehen, aufgetragen werden, oder es werden separate Pellets für jeden Wirkstoff hergestellt. Diese werden dann in den gewünschten Dosierungen in einer Kapsel gemischt. Eine Kombination unterschiedlicher Pellets ist insbesondere dann vorteilhaft, wenn die Wirkstoffe in verschiedenen Dosierungen verabreicht werden müssen um die optimale Wirkung am Patient zu erreichen: Wird Wirkstoff A in zwei, Wirkstoff B in drei Dosierungen verabreicht, so ergeben sich sechs fixe Arzneimittelkombinationen, die bei Tabletten sechs verschiedene Entwicklungen notwendig machen, bei Pellets müssen nur verschiedene Pelletmengen gemischt und in Kapseln abgefüllt werden. Falls aufgrund pH-abhängiger Löslichkeit eines Wirkstoffs saure oder basische Hilfsstoffe die Auflösung des Wirkstoffs verbessern, können auch saure oder basische Starterkerne anstelle von Neutralpellets verwendet werden. Falls bei Teilkomponenten eine Verlängerung der Wirkdauer angestrebt wird, können auch ein oder mehrere wirkstoffhaltige Pelletsorten mit Retardlacken überzogen werden. Hierzu können entweder pH-unabhängig freigebende Lacke wie z.B. Ethylcellulose mit Weichmachern/Porenbildnern wie Polyethylenglykol und Talkum als Schmiermittel verwendet werden oder es werden Polyacrylharze auf Basis von Coplolymerisaten von Methycralsäure und Methycralsäureestem mit Handelsnamen Eudragit eingesetzt, die dann eine pH abhängige Freigabe aufweisen.
Die Herstellung erfolgt in folgenden Schritten:
1. Auswahl bzw. Herstellung von Starterpellets
2. Aufbau der Wirkstoffschicht
Fakultativ: Überziehen von Pellets zur Stabilitätsverbesserung oder Geschmacks- korrektur oder - falls gewünscht - zur Retardierung einer oder mehrerer Wirkstoffe
Beispiel 4a
Herstellung basischer Starterkerne:

Povidone K25 3 Gewichtsteile
Mikrokrist. Cellulose 20 Gewichtsteile
Meglumin 77 Gewichtsteile
77 Gewichtsteile Meglumin, 20 Gewichtsteile mikrokristalline Cellulose und 3 Gewichtsteile Povidone K25 werden in einem Rhönradmischer 15 Minuten lang gemischt. Dann wird die Pulvermischung mit einer Geschwindigkeit von ca. 1 kg/h zusammen mit Wasser, das mit einer Dosierpumpe zugegeben wird, in einen Doppelschnecken-Extruder eingebracht. Die Wasserdosierung wird automatisch so geregelt, dass im Extruder ein Solldrehmoment von ca. 19% erzeugt wird. Die Extrusion erfolgt über eine Düsenplatte mit Bohrungen von 0.8 mm Durchmesser. Die Ausrundung der Extrusionsstränge zu Pellets erfolgt in einem Spheronizer, wobei ca. 3 Minuten mit ca. 850 RPM gerundet wird. Trocknung der Pellets bei 800C ca. 1.5 Stunden in einem Wirbelschichttrockner.
Das Kemmaterial wird über eine Taumelsiebmaschine mit unterschiedlichen Siebböden mit nominalen Maschenweiten von 0.71 bis 1.25 mm fraktioniert. Die jeweils geeigneten Gutfraktionen zwischen 0.71 und 0.90 bzw. 0.90 und 1.12 mm werden in den weiteren Prozessen verwendet.
Beispiel 4b
Herstellung einer Wirkstoffauftraqunq mit 100 Gewichtsteilen CGRP-Antaqonist und 70 mg Sumatriptan
Zusammensetzung: Kernmaterial 200 Gewichtsteile
Hydroxypropylcellulose 38 Gewichtsteile
Talkum 20 Gewichtsteile

In 250 Gewichtsteilen 2-Propanol wird Hydroxypropylcellulose unter Rühren gelöst und anschließend werden in dieser Lösung die Wirkstoffe und Talkum unter Rühren dispergiert.
In einer Wirbelschichtprozessanlage werden 200 Gewichtsteile Kernmaterial bei einer Zulufttemperatur von 20° bis 30° C mit der Wirkstoff haltigen Dispersion im Unterbettsprühverfahren besprüht. Die wirkstoffhaltigen Pellets werden anschließend im Umlufttrockenschrank bei 350C über 8 Stunden nachgetrocknet.
Zur Entfernung von Agglomeraten werden die wirkstoffhaltigen Pellets mit einem Sieb mit der nominalen Maschenweite 1.25 mm gesiebt. Die Gutfraktion (Korngröße < 1.25 mm) wird weiterverarbeitet.
Der Aufbau der Wirkstoffschicht erfolgt generell in immer gleicher Weise, wobei aber Wirkstoff art und Menge, Bindemittelart und Menge, Talkummenge und Wasser, Isopropanol bzw. Ethanolmenge variiert werden.

Anschließend werden in der Lösung unter Rühren 2 Gewichtsanteile Talkum dispergiert. In einer Wirbelschichtprozessanlage werden 23 Gewichtanteile wirkstoffhaltige
Pellets bei einer Zulufttemperatur von 35° bis 40° C mit der Gummi arabicum -
Talkum - Dispersion im Unterbettsprühverfahren besprüht.
Das isolierte Kernmaterial wird anschließend im Um lufttrockenschrank bei 40° C über
8 Stunden nachgetrocknet. Zur Entfernung von Agglomeraten werden die getrockneten wirkstoffhaltigen Pellets mit einem Sieb mit der nominalen Maschenweite 1.5 mm gesiebt. Die Gutfraktion
(Korngröße < 1.5 mm) wird weiterverarbeitet.

Zur Entfernung von Agglomeraten werden die getrockneten retardierten Pellets mit einem Sieb mit der nominalen Maschenweite 1.5 mm gesiebt. Die Gutfraktion
(Korngröße < 1.5 mm) wird weiterverarbeitet.

Extrudate
Die erfindungsgemäßen Arzneistoffkombinationen können auch in Form von Extrudaten hergestellt werden, die nach Schneiden/Spheronisieren direkt in Kapseln abgefüllt oder nach Mahlen zu Tabletten verarbeitet werden. Hierbei können beide Wirkstoffe gemeinsam extrudiert werden, oder es werden separate Extrudate für jeden Wirkstoff hergestellt, die dann in den gewünschten Dosierungen in einer Kapsel gemischt werden. Kombination unterschiedlicher Extrudate ist insbesondere
dann vorteilhaft, wenn die Wirkstoffe in verschiedenen Dosierungen verabreicht werden müssen um die optimale Wirkung am Patient zu erreichen: Wird Wirkstoff A in zwei, Wirkstoff B in drei Dosierungen verabreicht, so ergeben sich sechs fixe Arzneimittelkombinationen, die bei Tabletten sechs verschiedene Entwicklungen notwendig machen, bei Extrudaten müssen nur verschiedene Extrudatmengen gemischt und in Kapseln abgefüllt werden.
Die Herstellung erfolgt in folgenden Schritten: 1. Extrusion 2a. Schneiden/Spheronisieren

Herstellung von Feuchtextrudaten

Die Extrudate werden in einer geeigneten Mühle gemahlen, das entstehende Granulat wird mit üblichen Tablettierhilfsstoffen analog Beispiel 1 zu Tabletten weiterverarbeitet.
Pulverinhalativum
Herstellung sphärisch nanostrukturierter Mikropartikel der Wirkstoffe zur Herstellung eines Pulverinhalativums
Die Wirkstoffe werden zur Herstellung einer Wirkstofflösung von 4 Gew.-% entsprechend in einem Ethanol/Wasser (4:1)-Gemisch gelöst und die Wirkstofflösung so versprüht, dass ein Sprühnebel mit einer Tropfengröße mit dem Kennwert X50 (Medianwert = Teilchengröße/Tropfengröße, unterhalb derer 50% der Teilchenmenge liegt bezüglich der Volumenverteilung der einzelnen Teilchen/Tropfen) im Bereich von 1.5 bis 8 μm und Q(5.8) (Entspricht der Teilchenmenge, die bezogen auf die Volumenverteilung der Tröpfchen unterhalb von 5.8 μm liegt) zwischen 30 und 100% erzielt wird. Der so erhaltene Sprühnebel wird mit Hilfe eines Trocknungs- gases mit einer Eingangstemperatur zwischen 13O0C und 2000C und einer Ausgangstemperatur von 400C bis 1200C getrocknet Dabei beträgt der Volumenstrom des Sprühgases 1 Nm5Vh bis 15 Nm3/h und der Volumenstrom des Trock- nungsgases15 Nm3/h bis 150 Nm3/h. Der getrocknete Feststoffanteil wird mittels Massekraftabscheider und/oder Filtereinheit gesammelt.

Kapseln für die Pulverinhalation mit 0.5 mg CGRP-Antagonist und 0.35 mg Sumatriptan
Zusammensetzung:

(22) (S)-I -1 ,4'-Bipiperidinyl-1 '-yl-2-(3-chlor-4-hydroxy-5-trifluormethyl-benzyl)-4-f4- (2-oxo-i ,2,4,5-tetrahydro-i ,3-benzdiazepin-3-yl)-piperidin-1 -yl]-butan-1 ,4-dion, deren physiologisch verträglichen Salze und den Hydraten der Salze und einer therapeutisch wirksamen Menge eines oder zwei anderen aktiven Arzneistoffen (B) gegen Migräne an eine Person, die einer derartigen Behandlung bedarf, umfasst.
2. Verfahren nach Anspruch 1 , dadurch gekennzeichnet, dass der Arzneistoff (B) ausgewählt ist aus der Gruppe, die aus Angiotensin-Il-Antagonisten, α-Agonisten und α-Antagonisten, 5-HTiB/iD-Agonisten, AM PA-Antagonisten, Antidepressiva, Antiemetika, Antikonvulsiva, Antimuscarinika, ß-Blockem, Calciumantagonisten, Corticosteroiden, Ergot-Alkaloiden, Histamin-H1-Rezeptor-Antagonisten, schwachen Analgetika, Neurokinin-Antagonisten, Neuroleptika, nicht-steroidalen Antiphlogistika, NO-Synthase-Hemmern, Prokinetika und Serotonin-Wiederaufnahme-Hemmern besteht.
3. Verfahren nach Anspruch 2, dadurch gekennzeichnet, dass der Arzneistoff (B) aus der Gruppe ausgewählt ist, die aus Ergot-Alkaloiden und 5-HTiB/io-Agonisten besteht.
4. Verfahren nach Anspruch 3, dadurch gekennzeichnet, dass es sich bei dem Ergot-Alkaloid um Ergotamin oder Dihydroergotamin oder ein physiologisch ver- trägliches Salz davon handelt und dass es sich bei dem 5-HTiB/iD-Agonisten um Almotriptan, Avitriptan, Donitriptan, Eletriptan, Frovatriptan, Naratriptan, Rizatriptan, Sumatriptan oder Zolmitriptan oder die physiologisch verträgliches Salz davon handelt.
5. Verfahren nach Anspruch 4, dadurch gekennzeichnet, dass es sich bei dem Arzneistoff (B) um Sumatriptan, Zolmitriptan oder Dihydroergotamin oder ein physiologisch verträgliches Salz davon handelt.
6. Verfahren nach Anspruch 5, dadurch gekennzeichnet, dass der ausgewählte CGRP-Antagonist (A), ein physiologisch verträgliches Salz davon oder ein Hydrat des Salzes auf intravenösem oder subkutanem Weg in einer Dosierung von 0.0001 bis 3 mg/kg Körpergewicht, auf oralem Weg in einer Dosierung von 01 bis 20 mg/kg Körpergewicht oder auf nasalem oder inhalativem Weg in einer Dosierung von 0.1 bis 10 mg/kg Körpergewicht einmal, zweimal oder dreimal täglich verabreicht wird und
Sumatriptan oder ein physiologisch verträgliches Salz davon auf oralem Weg in einer Dosierung von 0.03 bis 1.43 mg/kg Körpergewicht einmal, zweimal oder dreimal täglich oder
auf intravenösem oder subkutanem Weg in einer Dosierung von 0.002 bis 0.09 mg/kg Körpergewicht einmal oder zweimal täglich oder
auf rektalem Weg in einer Dosierung von 0.007 bis 0.36 mg/kg Körpergewicht einmal oder zweimal täglich oder
auf nasalem Weg in einer Dosierung von 0.006 bis 0.29 mg/kg Körpergewicht einmal oder zweimal täglich verabreicht wird oder
Zolmitriptan oder ein physiologisch verträgliches Salz davon auf oralem Weg in einer Dosierung von 0.0007 bis 0.036 mg/kg Körpergewicht einmal oder zweimal täglich verabreicht wird oder
Dihydroergotamin oder ein physiologisch verträgliches Salz davon auf oralem Weg in einer Dosierung von 0.001 bis 0.07 mg/kg Körpergewicht einmal oder zweimal täglich verabreicht wird.
7. Verfahren nach Anspruch 2, dadurch gekennzeichnet, dass der Arzneistoff (B) aus der Gruppe ausgewählt ist, die aus Serotonin-Wiederaufnahme-Hemmern besteht.
8. Verfahren nach Anspruch 7, dadurch gekennzeichnet, dass es sich bei dem Serotonin-Wiederaufnahme-Hemmer um Citalopram, Duloxetin, Escitalopram, Fluo- xetin, Fluvoxamin, Paroxetin, Sertralin, Trazodon oder die physiologisch verträglichen Salze davon handelt.
9. Verfahren nach Anspruch 8, dadurch gekennzeichnet, dass es sich bei dem Arzneistoff (B) um Duloxetin oder ein physiologisch verträgliches Salz davon handelt.
10. Pharmazeutische Zusammensetzung für die Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache, umfassend eine therapeutisch wirksame Menge eines ausgewählten CGRP-Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und eines Arzneistoffs (B) gegen Migräne, der aus der Gruppe ausgewählt ist, die aus Sumatriptan, Zolmitriptan und Dihydroergotamin und einem physiologisch verträglichen Salz davon besteht, als kombinierte Zubereitung für die gleichzeitige oder zeitlich aufeinander folgende Verabreichung.
11. Pharmazeutische Zusammensetzung nach Anspruch 10, umfassend eine einzelne Dosierungseinheit von 0.1 bis 1500 mg eines ausgewählten CGRP- Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und
eine einzelne Dosierungseinheit von 1 bis 100 mg Sumatriptan oder
eine einzelne Dosierungseinheit von 0.1 bis 2.5 mg Zolmitriptan oder
eine einzelne Dosierungseinheit von 0.1 bis 5 mg Dihydroergotamin.
12. Kit von Teilen für die Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache, wobei das Kit umfasst:
(a) eine erste Umschließung enthaltend eine pharmazeutische Zusammensetzung umfassend eine therapeutisch wirksame Menge eines ausgewählten CGRP-Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes und ein oder mehrere physiologisch verträgliche Verdünnungsmittel und/oder Trägerstoffe; und - -
(b) eine zweite Umschließung enthaltend eine pharmazeutische Zusammensetzung umfassend Sumatriptan, Zolmitriptan oder Dihydroergotamin oder ein physiologisch verträgliches Salz davon und ein oder mehrere physiologisch verträgliche Verdünnungsmittel und/oder Trägerstoffe.
13. Kit von Teilen nach Anspruch 12, wobei das Kit Sumatriptan oder ein physiologisch verträgliches Salz davon in der zweiten Umschließung umfasst.
14. Verwendung eines ausgewählten CGRP-Antagonisten (A), eines physiolo- gisch verträglichen Salzes davon oder eines Hydrats des Salzes in Kombination mit einem oder zwei anderen aktiven Arzneistoffen (B) gegen Migräne für die Herstellung einer pharmazeutischen Zusammensetzung für die Behandlung oder Prävention von Kopfschmerzen, Migräne oder Cluster headache.
15. Verwendung gemäß Anspruch 14, dadurch gekennzeichnet, dass der CGRP- Antagonist (A) ausgewählt ist aus der Gruppe bestehend aus
(1) 4-(2-Oxo-1 ,2,4,5-tetrahydro-1 ,3-benzdiazepin-3-yl)-piperidin-1-carbonsäure-{(f?)- 1 -(4-amino-3-chlor-5-ethyl-benzyl)-2-[4-(4-methyl-piperazin-1 -yl)-piperidin-1 -yl]-2- oxo-ethyl}-amid,
(2) [1 '-((f?)-3-(4-Amino-3-chlor-5-trifluormethyl-phenyl)-2-{[4-(2-oxo-1 ,2,4,5-tetra- hydro-1 ,3-benzdiazepin-3-yl)-piperidin-1-carbonyl]-amino}-propionyl)-4,4'-bi- piperidinyl-1 -yl]-essigsäure,
(3) 3-{1 -[(R)-1 -(4-Amino-3,5-dibrom-benzyl)-2-[1 ,4']bipiperidinyl-1 '-yl-2-oxo-ethyl- carbamoyl]-piperidin-4-yl}-2-oxo-1 ,2,3,4-tetrahydro-chinazolin-7-carbonsäure,
(4) 4-(2-Oxo-1 ,2,4,5-tetrahydro-1 ,3-benzdiazepin-3-yl)-piperidin-1 -carbonsäure-ffl)- 1-(7-methyI-1 H-benztriazol-5-ylmethyl)-2-[4-(4-methyl-piperazin-1-yl)-piperidin-1- yl]-2-oxo-ethylester, n

Verwendung nach Anspruch 14 oder 15, dadurch gekennzeichnet, dass der Arzneistoff (B) ausgewählt ist aus der Gruppe, die aus Angiotensin-Il-Antagonisten, α-Agonisten und α-Antagonisten, 5-HT1B/iD-Agonisten, AMPA-Antagonisten, Antidepressiva, Antiemetika, Antikonvulsiva, Antimuscarinika, ß-Blockem, Calciumantagonisten, Corticosteroiden, Ergot-Alkaloiden, Histamin-H1-Rezeptor-Antagonisten, schwachen Analgetika, Neurokinin-Antagonisten, Neuroleptika, nicht-steroidalen Antiphlogistika, NO-Synthase-Hemmem, Prokinetika und Serotonin-Wiederauf- nahme-Hemmem besteht.
17. Verwendung nach Anspruch 16, dadurch gekennzeichnet, dass der Arzneistoff (B) aus der Gruppe ausgewählt ist, die aus 5-HTiB/iD-Agonisten und Ergot-Alkaloiden besteht.
18. Verwendung nach Anspruch 17, dadurch gekennzeichnet, dass es sich bei dem Ergot-Alkaloid um Ergotamin oder Dihydroergotamin oder ein physiologisch ver- trägliches Salz davon handelt und dass es sich bei dem 5-HT1B/iD-Agonisten um Almotriptan, Avitriptan, Donitriptan, Eletriptan, Frovatriptan, Naratriptan, Rizatriptan, Sumatriptan oder Zolmitriptan oder ein physiologisch verträgliches Salz davon handelt.
19. Verwendung nach Anspruch 18, dadurch gekennzeichnet, dass es sich bei dem Arzneistoff (B) um Sumatriptan, Zolmitriptan oder Dihydroergotamin oder ein physiologisch verträgliches Salz davon handelt.
20. Verwendung eines ausgewählten CGRP-Antagonisten (A), eines physiologisch verträglichen Salzes davon oder eines Hydrats des Salzes für die Herstellung einer pharmazeutischen Zusammensetzung oder eines Kits von Teilen nach einem der Ansprüche 10 bis 13.
